Objective To understand the main and multiple kinds of diseases and probe into key points of diseases for hospice care in geriatric nursing organization in Beijing, Tianjin and Shanghai city. Methods Pareto chart wag used to analyze situation on disease component of deathbed-patients discharged from 22 geriatric nursing organizations from years 2005 to 2007 in Beijing, Tianjin and Shanghai. Results Components of cardiovascular disease, cerebrovascular disease, respiratory system disease, finale malignancy and senile decrepitude were accounted more than 95%.Conclusions Kinds of diseases in deathbed-patients discharged from nursing organizations in city were relatively concentrated, especially cardiovascular disease, cerebrovascular disease, respiratory system disease, finale malignancy and senile decrepitude, etc should be focal point of hospice care service in geriatric nursing organizations.

4.Relationship between colorectal carcinoma and lymph node metastasis

Objective To provide the theoretic basis for the clearance of the metastatic lymph nodes in radical operation of colorectal carcinoma(CRC) by means of exploring the relationship between mass size of both tumor and lymph nodes and the rate of lymph node metastasis. Methods A total of 702 lymph nodes from 57 cases of CRC radical operations were divided into 5 groups and each of the lymph nodes was examined. The size of lymph nodes, Dukes' phases, the pathological types of CRC and their metastasis rate were analyzed. Results ① High metastasis rate was found at Dukes' C and D phases of CRC, being 29.93% and 48.12%, respectively. ② The metastasis rates of mucous adenocarcinoma and signet ring cell carcinoma were the highest of all pathological types, being 59 52% and 59.26%, respectively. ③ Metastasis rate of lymph nodes was not correlated with the mass size of tumor, but correlated with Dukes' phase. Conclusion ① Metastasis rate of lymph nodes increases with the growth and infiltration of tumor. ② Metastasis rates of mucous adenocarcinoma and signet ring cell carcinoma are higher than those of well and mid differentiated carcinomas. ③ Metastasis may be suggested for lymph node of CRC more than 5 mm in diameter during operation. However, clearance of lymph node of CRC less than 5 mm in diameter needs further study.
5.THE STUDY OF OCCUPATIONAL HAZARDS OF VDT

Both monitoring of occupational hazards and assessment of healthy effects on VDT operators were made. The results showed that there were several occupational hazards in VDT,including insalubrious microclimate and air ion condition, lower illuminance, higher noise, some doses of x-radiation, and unfitting ergonomics. The healthy effects of the hazards of the VDT mainly involved the operator's nervous system, psychology and behavior, eyes, musculoskeletal system as well as the woman operator's menses and pregnancy, especially in the 18 to 25 age bracket who had more VDT work hours than that of the others, it suggested that the various preventive measures against the hazards of the VDT must be strengthened.
6.VDT OPERATORS' OCCUPATIONAL STRESS AND PSYCHOPHYSICAL STATE

VDT operators' occupational stress and psy-chophysical state were studied. The resultsshowed that the nature, environment and require-ments of VDT operation shaped the major occupa-tional stress in VDT operation. The longer VDTworking hours and more workloads gave the opera-tors more occupational strains. VDT operators hada lower satisfactory level to the job and lower psy-chological health level than contral group, espe-cially on obsession and depression. VDT operationperhaps gave the operators some kinds of psycho-physiological troubles. We did not find there wereharmful effects of VDT on the operators' autonom-ic nervous functions.
7.PROTECTIVE EFFECT OF MELATONIN ON KIDNEY IN DIABETIC RATS

To investigate the protective effect of melatonin on the kidneys of STZ-induced diabetic rats, experimental diabetes was induced by intraperitoneal injection of streptozotocin 50 mg/kg in male Sprague-Dawley rats (150~200g). Three days after streptozotocin injection, the diabetic rats were assigned to one of the following groups: (1) DM, untreated; (2) DM+Mel 1, melatonin supplement at 0.2mg?kg -1?d -1, by gavage; (3) DM+Mel 2, melatonin supplement at 5mg?kg -1?d -1, by gavage. The treatment continued for 8 weeks. Periodically, body weight, blood glucose, plasma triglyceride and cholesterol levels were measured as clinical and biochemical parameters; plasma creatinine, creatinine clearance( Ccr), urinary protein excretion were detected as renal function indexes; kidney weight, glomerular area, PAS positive area, and cell counts were evaluated as histological damage indexes. Melatonin treatment had no significant effects on blood glucose and plasma cholesteral, but lowered plasma triglyceride level in diabetic rats. Melatonin treatment reduced urinary protein excretion. Until the end of the experiment, there were no significant changes in plasma creatinine level, Ccr in diabetic rats. Melatonin treatment decreased kidney weight/body weight ratio, reduced PAS positive area, glomerular cellular number and glomerular area which were increased in DM rats. The protective effects of melatonin were more obviose in high dosage groups. Melatonin treatment can aggravate glomerular hypertrophy and mesangial cell proliferation, delay the progression of glomerulosclerosis, and improve renal function of diabetic rats.

9.Importance of aneuploidy screening in preimplantation genetic diagnosis for the couples of chromosome translocation carriers

Objective To determine the importance of aneuploidy screening in preimplantation genetic diagnosis for the couples of chromosome translocation carriers. Methods To perform 11 prenatal genetic disgnosis (PGD) cycles for 7 couples of chromosome translocation carriers from January 2006 to March 2009 in the Reproductive Medical Center, First Affiliated Hospital of Zhengzhou University. To re-analyze the well-fixed, non-multinuclear and non-debris nuclei using the probes of LSI 13, 18, 21,CEPX, CEPY to detect the aneuploidy rate which come from the PGD cycles of the couples of chromosome translocation carriers. The euploid embryo was defined as two fluorescence in situ hybridization (FISH)signals of LSI 13, 18, 21 respectively and two signals of CEPX, or one signal of CEPX and one signal of CEPY. The other abnormal signals were defined as aneuploid embryo. Results (1) A tolal of 130 nuclei from 11 PGD cycles got the integrated re-FISH signals. Nine hundred and thirty-seven FISH signals were analysized, including 304 signals from 38 euploid nuclei and the others from 92 aneuploid nuclei. (2) The number of the aneuploid nuclei from grade Ⅰ , Ⅱ and Ⅲ embryo was 20 (22%), 36(39%), and 36(39%). The number of the euploid nuclei from grade Ⅰ , Ⅱ and Ⅲ embryo was 13(34%), 17(45%),and 8(21%). There was no significant difference of aneupioidy rate between the embryos form different grades (P>0.05). However, the rate of aneuploid nucleus from good quality embryos (grade Ⅰ + grade Ⅱ) was 60% (59/92). (3) The euploidy rate was 71.4% (30/42) from balanced embryos, while 9.1%(8/88)from unbalanced embryos. There was significant difference between them (x2=53.4, P<0.05).The rate of aneuploidy from balanced embryos was lower than those from unbalanced embryos (P<0.05).Conclusions Since higher rate of aneuploidy was detected in embryos of the couples of chromosome translocation carriers. It is advisable to recommend the FISH re-analysis for aneuploidy screening to preimplantation genetic diagnosis for the couples of chromosome translocation carriers.
10.Analysis on the detection and antibiotic resistance of extended-spectrum β-lactamases-producing Klebsiella pneumoniae and Escherichia coliisolated from patients in Intensive Care Unit

Objective To analyse the detection rates and antibiotic resistance of extended-spectrum β-lactamases (ESBLs) producing Klebsiella pneumonia and Escherichia coli in Intensive Care Unit (ICU) and to guide the clinical administration of treatment Methods Klebsiella pneumonia and Escherichia coli collected from clinical samples from January 2008 to December 2010 were tested by Phenotypic Confirmatory Test and confirmed by the method advised by NCCLs and drug-sensitivity was tested with K-B. Results Among the isolated 90 samples,49 strains were considered ESBLs-producing bacteria (54.4%) .with 52. 5% (31/59)of Klebsiella pneumonia and 58. 1% (18/31) of Escherichia coli respectively; with the specimens of respiratory system having the highest rate of 75. 5% (37/49). ESBLs producing bacteria were highly resistant to penicillins and cephalosporins, multi drug resistant to aminoglycosides and quinolones; low to piperacillin/tazobactam,cefoperazone/sulbactam,cefoxitin and amikacin; and all sensitive to imipenem. When compared to non-ESBLs producing strains, the rates of antibiotic resistance of the producing ESBLs strains were significantly higher. Conclusion The test results showed that the isolation rates of ESBLs-producing Klebsiella pneumoniae and Escherichia coli in ICU were high,which had high resistance to most antimicrobial agents,and the resistance was multiple. Imipenem could be the best choice to control the infection due to ESBLs-producing organisms. Timely detection of ESBLs producing bacteria and drug resistance is essential to guide clinical antibiotic using in ICU.
