1.Application of blood flow resistance index plus the risk of malignant index in preoperative assessment of ovarian neoplasm

Objective To investigate the possibility of blood flow resistance index (RI),pulse index (PI) plus the risk of malignant index (RMI) in preoperative diagnosis of ovarian neoplasm.Methods Four hundred and sixty-eight women with ovarian tumor,30 years or older,were enrolled in current study from January 2007 to December 2012 in the People's Hospital of Tianjin.Among them,127 were with malignant ovarian tumors and 341 with benign ovarian tumors.They were retrospectively prospective assessed by RI + PI,RMI,and RI + PI combine with RMI.Results The value of RI,PI of malignant ovarian tumors group were lower than those of benign ovarian tumors group ((0.41 ± 0.08) vs (0.68 ± 0.15),t =28.83,P < 0.01 ; (0.82 ± 0.24) vs (1.67 ±0.71),t =22.26,P <0.01).RMI of malignant ovarian tumors group was (269.7 ±41.2),higher than that of benign ovarian tumors group(25.6 ± 11.4,t =107.55,P <0.01).The indices of RI <0.40 and/or PI <1.00,RMI > 200 were served as the critical biomarker respectively.Histopathological examination result was conducted and considered as gold standard for identifying patients with ovarian cancer pelvic masses.The sensitivity of RI,PI plus RMI was 92.57%,higher than that used by RI and PI (81.02%) or RMI only (84.21%),and the difference was significant (P < 0.05).The sensitivity,specificity,positive and negative predictive values of the combined index of RI,PI adding to RMI were all higher than 90%.Conclusion The combined index of RI,PI plus RMI was proved to enhance the diagnostic accuracy regarding of the preoperative diagnosis of benign or malignant ovarian tumors.
2.Clinical analysis on the relationship between gynecologic malignant neoplasms and metabolic syndrome

Objective To investigate the relationship between gynecologic malignant neoplasms and metabolic syndrome (MS),in order to give a direction to the prevention and treatment of gynecologic malignant neoplasms.Methods A total of 359 cases of gynecologic malignant neoplasms (including 142cases of ovarian cancer,131 cases of endometrial cancer,86 cases of cervix cancer) were enrolled as study group,and 400 cases of gynecologic benign neoplasms were enrolled as control group.The general conditions,physical examination,fasting blood glucose,fasting insulin,homeostatic model assessment insulin resistance index (HOMA-IR),blood fat were compared between two groups.Clinical characteristics between two groups were compared.The occurrence of MS was counted.Results There was no significant difference in menarche age,onset age,gravidity and parity between two groups (P >0.05).The level of systolic pressure,diastolic pressure,body mass index,abdominal circumference,fasting blood glucose,fasting insulin,HOMA-IR,triacylglycerol in study group was higher than that in control group [(129 ± 13)mmHg (1mmHg=0.133kPa)vs.(112±14)mmHg,(87±11)mmHgvs.(75±8) mmHg,(26.7±2.8) kg/m2 vs.(22.2 ±2.1) kg/m2,(88 ±7) cm vs.(76 ±9) cm,(6.5 ±2.9) mmol/L vs.(4.7 ±0.9)mmol/L,(9.2 ± 4.7) mU/L vs.(5.2 ± 3.0) mU/L,3.9 ± 0.8 vs.3.1 ± 0.6,(3.21 ± 1.96) mmol/L vs.(1.56 ±1.22) mmol/L],and the level of high density lipoprotein cholesferol was lower than that in control group [(1.25 ± 0.51) mmol/L vs.(1.65 ± 0.47) mmol/L],and there were significant difference between two groups (P < 0.05).The occurrence of MS in study group was 37.60% (135/359),among of them,37.32% (53/142)in ovarian cancer,43.51%(57/131) in endometrial cancer,29.07% (25/86) in cervix cancer,which was higher than that in control group [12.25% (49/400)],and there was significant difference (P <0.05).Conclusions The occurrence of MS in gynecologic malignant neoplasms is higher than that in gynecologic benign neoplasms,and there is a mutual promotion and influence of the relationship between gynecologic malignant neoplasms and MS.It suggests that there is a possible role of MS in the occurrence and development of gynecologic neoplasms,gynecologist should pay attention to it and control MS actively,those measures could make important significance of prevention and treatment for gynecologic malignant neoplasms.
3.Fracture load and fracture modes of prefabricated fiber post restoration in different tilted mandible premolars:A pilot study

80 extracted human mandible premolars were divided into 10 groups(n =8)based on the post with different tilted angles(0°, buccal 15°and 30°,ligual 15°and 30°).The samples in experiment group were restored with fiber post while in the control group with cast post.Then all the teeth were restored with cast crown and bonded by glass ionomer.Compressive load with the speed of 1.0 mm/min was ap-plied to the restored teeth with a universal material testing machine until failure and the fracture modes were observed.The fracture load of fi-ber post and cast post restoration with the same tilted angles was similar(P >0.05).The main fracture mode of the teeth in all groups was unfavorable tooth fracture.No significant interaction was observed(P =0.217)between the 2 kinds of post and angulation of tilted teeth.
4.Chondrogenic differentiation and osteogenesis gene expression following a short-time induction of adipose-derived stem cells with bone morphogenetic proteins

OBJECTIVE:To investigate osteogenic and chondrogenic differentiation of adipose-derived stem cells following a short time induction with bone morphogenetic protein 2(BMP-2) or BMP-7.METHODS:The subcutaneous adipose tissue was obtained from adult New Zealand rabbits under aseptic condition,and cultured in vitro with collagenase digestion.All cells were divided into 3 groups:in the BMP-2 group,cells were cultured with medium containing 0.1 g/L vitamin C,10 mmol/L?-sodium glycerophosphate and 10?g/L BMP-2 for 10 minutes,followed by 4-14 days inoculation with density of 18?104 cells per pore.In the BMP-7 group,cells were cultured with BMP-7 with the same methods as BMP-2 group.The cells were cultured with simple culture medium in the control group.RESULTS:Compared to the control group,the number of adipose-derived stem cells,protein level and ALalkaline phosphatase(ALP) activity of BMP-2 and BMP-7 groups was up-regulated 1.78-1.79,1.15-1.95,and 32-40 times,respectively.At days 4 after a 15 minutes induction,the runx-2 gene expression,osteopontin gene,and biglycan gene were increased 1.9,2.2 and 1.3-1.7 folds than that of the control group.Meantime,only the biglycan gene expression was increased 1.3-1.7 folds in the BMP-7 group,the runx-2 gene expression,osteopontin gene was not changed.At day 14 after a 15 minutes induction,there was no alter of runx-2,osteopontin,biglycan,as well as aggrecan gene expression in the BMP-2 group;while down-regulated runx-2,osteopontin,biglycan 1.8,5.0 and 1.7 folds,with increased aggrecan gene in the BMP-7 group.CONCLUSION:Following a short time induction,BMP-2 can stimulate runx-2 and osteogenic expression at 4 days after re-culture,whereas BMP-7 down-regulate genes expression at days 14,yet down-regulate aggrecan mRNA expression.

AIM: To investigate the effect of ethanol and chemical carcinogen N,N'-dinitrosopiperazine(DNP) on the exogenous expression of CYP2E1.METHODS: Exogenous hCYP2E1 was introduced into NIH3T3 mediated by lipofectamine. Then the integration of exogenous gene was showed by Southemrn blot. After treated with different concentration of ethanol and DNP,RT-PCR and Western blotting were used to analyze the expression change of hCYP2E1 in NIH 3T3. RESULTS: Two cell clones with integration and stable expression of exogenous hCYP2E1 were obtained. The RT-PCR and Western blotting showed that human CYP2E1 mRNA and protein expression was enhanced with increase in ethanol and DNP concentration. CONCLUSION: Exogenous expression of hCYP2E1 was steadily induced by ethanol and DNP. The mechanism may be due to the activation of its transcription. The DNP carcinogenesis might be related to its in situ activation by CYP2E1.
7.CT diagnosis of high altitude pulmonary edema

Objective To explore the value of CT diagnosis of high altitude pulmonary edema (HAPE). Methods The CT findings in 16 patients unfit to high altitude were analyzed. Results The findings on CT were as follows: (1) The early stage of HAPE showed ground glass opacity, most of which located at the superior segment and posterior basis segment of inferior lobes, with the right lung to occur earlier than that of the left lung. (2)The advanced stage showed shaggy opacity. (3) The late stage lesions developed to posterior and apical segment of the superior lobes, air bronchus sign could be seen on involved segments. (4)Right lung was more serious than left lung. Conclusion CT was an ideal method to find HAPE. The accuracy of CT diagnosis in HAPE was 100%.

Objective To evaluate the prognostic significance of MMP-2 , MMP-9 and Cathepsin-D expression in lymph node-negative human breast carcinoma. Methods Specimens from the primary breast tumors of 270 axillary lymph node negative breast cancer patients were studied for MMP-2 , MMP-9 and Cathepsin-D protein expression by immunohistochemical staining. Results The positive expression of MMP-2, MMP-9 and Cathepsin-D was 56.7%(153/270),59.6%(161/270) and 55.6%(150/270), respectively. In univariate analysis, MMP-2, MMP-9 and the co-expression of MMP-2 and MMP-9 were significantly associated with tumor free survival. 9.Slow anti-epileptic drug taper protocol in video-EEG monitoring for presurgical evaluation of epilepsy.

OBJECTIVETo evaluate the efficacy and safety of slow anti-epileptic drug (AED) taper protocol and a rescue benzodiazepine protocol in video-electroencephalography (video-EEG) monitoring for presurgical evaluation of epilepsy.
METHODSSixty-two of 109 patients with refractory focal epilepsy underwent pre-surgical video-EEG monitoring with a slow AEDs taper protocol and a rescue benzodiazepine protocol. Seizures were recorded by video-EEG in 56 patients. The time to the first seizure, duration of monitoring, incidence of 4-h and 24-h seizure clustering, secondarily generalized tonic-clonic seizures (sGTCS), status epilepticus, falls and cardiac asystole were evaluated.
RESULTSA total of 191 seizures were recorded in the 56 cases, and the diagnostic efficiency of video-EEG was 90.3%. The mean time to the first seizure was 2.4 days and the time to conclude video-EEG monitoring averaged 6.8 days. Eight (12.9%) patients had 4-h clusters and 24 (38.7%) had 24-h clusters. Seizure clusters were more frequent in extra temporal epilepsy than in temporal lobe epilepsy. While 19 sGTCS were recorded in 15 patients (26.8%), status epilepticus did not occur and no seizure was complicated by cardiac asystole. Epileptic falls with no significant injuries occurred in 4 patients.
CONCLUSIONSeizure clustering is common during presurgical video-EEG monitoring, but serious adverse events are rare with a slow AED tapering and a rescue benzodiazepine protocols. These two protocols are effective and save in presurgical video-EEG monitoring for refractory focal epilepsy.

Background and purpose:Rabs are members of Ras-related small GTPase superfamily. Rab27A is a unique member in the Rab family and has specific implications in human genetic diseases. We studied the potential role of Rab27A in proliferation, distribution of cell cycle, apoptosis and invasion of breast cancer cells and its mechanism(s). Methods:The eukaryotic expression vector containing Rab27A open reading frame (ORF) pcDNA3.1(+) - Rab27A was constructed and transfected into MDA-MB-231 breast cancer cells. Then we detected the changes in terms of cell growth, cell cycle distribution, apoptosis and in vitro invasion capability before and after transfection. We also applied RT-PCR to investigate the molecular basis.Results:① The expression of Rab27A was increased as invasive and metastatic ability increased in four human breast cancer cell lines. ② Overexpression of Rab27A can promote breast cancer cells to grow faster, increase the proportion of S phase cells, avoid apoptosis and invade in vitro. ③ Rab27A transfectants constitutively enhanced the expression of Cyclin D1, MMP-7 and MMP-9 in MDA-MB-231 cell lines, on the contrary, that of p16 were down-regulated constitutively. Reduced Rab27A expression by RNAi down-regulated the expression of Cyclin D1, MMP-7 and MMP-9, and up-regulated p16 expression.Conclusions:Rab27A can stimulate breast cancer cells to proliferate, increase the proportion of cells in S phase,avoid apoptosis and invade in vitro by regulating the expression of Cyclin D1, MMP-7, MMP-9 and p16.
