1.An analysis of LIF spectrum for detecting colorectal carcinoma in vivo and in vitro

Objective To study the correlativity of laser-induced fluorescence (LIF) spectrum on the same specimen of colorectal carcinoma both in vivo and in vitro and to obtain the best diagnostic parameter in detecting colorectal carcinoma. Methods LIF spectra were analyzed from 10 patients with colorectal carcinoma during colonoscopy, and then analyzed again after resection. The cancer foci and normal mucosa more than 5 cm apart from the foci were excited by a nitrogen laser (337 nm)using a quartz optical fiber in gentle contact with the area of interest, and then the fluorescence emission was collected by the same quartz optical fiber. The LIF spectrum was analyzed using an optical multichannel analyzer (OMAⅢ). Results (1)The LIF spectrum from cancer tissues was same as that of normal tissues, both showed the main and the second peak respectively located at about 460 nm and 390 nm in vivo and in vitro. The main peak intensities in vivo from both cancer and normal tissue were significantly lower than these in vitro. The main peak intensities in vivo and in vitro from cancer tissue were decreased about 1 time to those from normal tissue.(2) The integrated spectral intensity in vivo and in vitro of cancer tissue from each patient was significantly lower than that from the corresponding normal tissue respectively, P
2.The relationship between the fracture of the clavicle which was fixed by torsional-shaped plate and nonunion

Objective To discuss the relationship between the fracture of the clavicle which was fixed by tor-sional-shaped plate and nonunion after operation.Methods Retrospective analysis was conducted in 426 patients with midshaft clavicle fractures.Among the patients,the A group used torsional-shaped plate to meet the anatomical structure of the clavicle:the medial plate matched under the front face of the clavicle,and the lateral plate matched the up face of the clavicle.And the traditional method was used in the B group.Patients were followed up for at least 12 months,the patients were judged by X-ray criteria for nonunion.Results A total of 28 patients with nonunion, including 4 cases in A group,24 cases in B group.After statistics processing,the difference between A group and B group was significant (χ2 =6.679,P=0.010).Conclusion We find that the treatment of the fracture of the middle part of the clavicle with torsional-shaped plate can effectively reduce the incidence of nonunion.
3.Influence of cervical anterior plate on restoring physiological curvature of cervical vertebra after cervical spondylotic myelopathy: A one-year outcome follow-up

BACKGROUND: It is necessary to keep a good cervical curvature for improving nerve symptom in patients with cervical spondylotic myelopathy (CSM). It is verified that cervical anterior plate internal fixation implant is effective to restore the physiological curvature of cervical vertebra.OBJECTIVE: To assess the physiological lordosis of cervical vertebra in CSM patients after cervical anterior plate internal fixation implantation.DESIGN: Observational study.SETTING: First Affiliated Hospital of Nanchang University.PARTICIpANTS: Totally 72 CSM patients were enrolled at Department of Orthopaedics, First Affiliated Hospital,Nanchang University from February 2003 to January 2006, including 46 males and 26 females, aged 45-70 years,averagely 54 years, with the history of 2-6 years. Inclusive criteria included ①cervical spinal cord compression shown in MRI, and ②using autogenous iliac bone. The experiment was approved by Hospital Ethics Committee, and all patients signed the informed consent. Cervical anterior plate was Orion locking plate offered by Shufamo Company. There were single segmental discectomy group (n =32) and two-level segmental discectomy group (n =40) according to experimental requirement.METHODS: ①Anterior decompression plus autogenous iliac bone implant plus titanium plate internal fixation were utilized.The patients were in supine position; shoulder and back were blocked up; neck backward, and endotracheal tube was done under general anesthesia. Transverse or oblique incision was made at right anterior neck. Discectomy decompression was performed with Caspar vertebra ecarteur. Three-dimensional autogenous iliac bone was implanted in decompressed intervertebral space. Immediate stability was obtained after Orion locking titanium plate was fixed, with placement drainage, and then the incision was sutured. ②Postoperative daily activities were conducted with cervical gear, which was removed 8 weeks later. ③Lateral cervical vertebrae X-rays were taken before operation, immediately after operation, 6 and 12 months after operation. Lordosis of the fusion segment was measured with Cobb's angle, and lordosis of cervical vertebra was assessed with D value. These data were analysed by statistic software. ④Bone fusion of the patients was followed up for 12 months after operation. Vertebral fusion standards included no abnormity between two spinous processes of the fusion segment, no lucency between implant and vertebra, as well as bone trabecula across the interface between implant bone and vertebra. ⑤Biocompatibility between material and host was observed during follow-up.MAIN OUTCOME MEASURES: ①Changes of Cobb's angle and D value at different time points before and after operation,and ②Bone fusion and rejection were followed up.RESULTS: Totally 72 patients were involved in the result analysis. ①Cobb's angle was larger and D value was higher immediately after operation, 6 and 12 months after operation than those before operation in the single segmental discectomy group and the two-level segmental discectomy group (P<0.05). ②Findings of 12-month follow-up showed that bone fusion appeared in all patients of the two groups. ③No significant rejection was found after titanium plate internal fixation implantation. It was found that material and host had a good biocompatibility.CONCLUSION: It is satisfactory to restore physiological lordosis of cervical spine by using cervical anterior plate internal fixation plus autogenous iliac bone for CSM.
4.Analysis on the detection and antibiotic resistance of extended-spectrum β-lactamases-producing Klebsiella pneumoniae and Escherichia coliisolated from patients in Intensive Care Unit

Objective To analyse the detection rates and antibiotic resistance of extended-spectrum β-lactamases (ESBLs) producing Klebsiella pneumonia and Escherichia coli in Intensive Care Unit (ICU) and to guide the clinical administration of treatment Methods Klebsiella pneumonia and Escherichia coli collected from clinical samples from January 2008 to December 2010 were tested by Phenotypic Confirmatory Test and confirmed by the method advised by NCCLs and drug-sensitivity was tested with K-B. Results Among the isolated 90 samples,49 strains were considered ESBLs-producing bacteria (54.4%) .with 52. 5% (31/59)of Klebsiella pneumonia and 58. 1% (18/31) of Escherichia coli respectively; with the specimens of respiratory system having the highest rate of 75. 5% (37/49). ESBLs producing bacteria were highly resistant to penicillins and cephalosporins, multi drug resistant to aminoglycosides and quinolones; low to piperacillin/tazobactam,cefoperazone/sulbactam,cefoxitin and amikacin; and all sensitive to imipenem. When compared to non-ESBLs producing strains, the rates of antibiotic resistance of the producing ESBLs strains were significantly higher. Conclusion The test results showed that the isolation rates of ESBLs-producing Klebsiella pneumoniae and Escherichia coli in ICU were high,which had high resistance to most antimicrobial agents,and the resistance was multiple. Imipenem could be the best choice to control the infection due to ESBLs-producing organisms. Timely detection of ESBLs producing bacteria and drug resistance is essential to guide clinical antibiotic using in ICU.
5.Microfracture technique and osteochondral transplantation in the treatment of articular cartilage defects

BACKGROUND:Microfracture technique and osteochondral transplantation are one of the methods for the treatment of articular cartilage defects with broad application prospects. OBJECTIVE:To investigate the effect of microfracture technique and autologous and al ogeneic osteochondral transplantation in the treatment of knee osteoarthritis complicated with articular cartilage defects. METHODS:The articular cartilage defects were treated with arthroscopic debridement combined with microfracture surgery in the cartilage defect region, and then the clinical effect, clinical symptoms and Tenger sport score were detected. The patients were fol owed-up for 3-24 months. The articular cartilage defects patients treated with autologous osteochondral transplantation were fol owed-up. The effect of autologous osteochondral transplantation was evaluated through assessing the range of motion after transplantation, improvement of clinical symptoms and joint imaging examination. The animal experiment of al ogeneic osteochondral transplantation for the treatment of articular cartilage defects was conducted to evaluate the effect of al ogeneic osteochondral transplantation through general observation of transplantation site, histological and immunohistochemical staining. RESULTS AND CONCLUSION:For the patients treated with arthroscopic microfracture technique, the total efficiency of joint debridement combined with microfracture surgery in the cartilage defect region was 89.7%. For the articular cartilage defects patients treated with autologous osteochondral transplantation, the symptoms of joint pain and swel ing were improved, and the range of motion was normal with rest pain and slight pain after movement occasional y. Radiographic examination showed the transplanted osteochondral was in good position with good healing. In the experiment animals with articular cartilage defects after treated with al ogeneic osteochondral transplantation, the range of motion was normal, transplanted articular surface was finishing, the articular cartilage was covered with hyaline cartilage, and the cells were arranged in order;there was cartilage matrix secretion, and the col agen type II in the repaired articular cartilage was strongly positive with immunohistochemical staining.

OBJECTIVE To investigate the problems and countermeasures in the chemical disinfectants application.METHODS To analyze the existing problems in the chemical disinfectants application from 8 hospitals of different care levels at the same city and provide corresponding measures.RESULTS There were many problems in the use of chemical disinfectants,therefore the key points were enhencing the leadership and management standards,staff training,of corresponding knowledge and the correct methods,and strengthening the guidance,inspection and supervision by functional departments.CONCLUSIONS Medical personnel must properly use the chemical disinfectants to ensure the medical safety and prevent hospital infection effectively.

The general system of clinical engineering professional certification in Germany was introduced from the aspects of history, certification authority, requirement of certification, skills and knowledge points, methods of implementation and so on. Through comparative study, some suggestions were proposed to promote clinical engineering staff technology level assessment and certification, which might provide some references for the further improvement of clinical engineering certification system in China.
