1.Mycoplasma pneumoniae infection and its relationship with asthma

Mycoplasma pneumoniae(MP) is one of the most important pathogens for respiratory tract infections in older children and adults. And it also has a close relationship with the development, acute exacerbation and persistence of chronic difficult-to-control symptoms of asthma.But so far,the mechanisms inducing or causing asthma remain unclear.Possible mechanisms include direct damage of bronchial mucosa, IgE-mediatad immune response, a variety of cytokines and inflammatory mediators releasing and the propensity of host.
2.Effect of atopic status on interleukin-4, interferon-γ and interleukin-5 levels in serum of children with mycoplasma pneumoniae infection

ObjectiveTo explore the effect of atopic status on interleukin-4 (IL-4),interferon-γ (IFN-γ) and interleukin-5 (IL-5) levels in serum of children wih mycoplasma pneumoniae (MP) infection,and to investigate the relation between these cytokines and disease severity.MethodsNinety-five children with MP infection were enrolled in our study,which were divided into atopic group and non-atopic group,mild group and severe group,normal-TIgE group and high-TlgE group.Meanwhile,38 normal children were enrolled as normal control group.The serum levels of IL-4,IFN-γ and IL-5 were detected by ELISA method.Results(1) Thelevels of serum IL-4 (pg/ml),IFN-γ (pg/ml) and IL-5 (pg/ml) in MP infection group[2.82(2.12,7.13 ),39.70(18.82,181.42),16.12( 10.35,28.07 ) ] were higher than those in normal control group [ 1.17(1.05,1.60),13.25(8.77,22.56),9.72(6.11,12.39) ] ( P<0.05 ).(2) Serum IL-5 levels in atopic group [ 15.55 (10.35,22.61 ) ],severe group [ 15.98 ( 11.95,26.58 ) ] and high-TlgE group [ 16.55( 10.35,23.65 ) ] were much higher than those in normal control group( P<0.05 ).Rather than the serum IL-5 levels of atopic group,mild group and normal TIgE group showed no significant difference compared with the normal control group( P>0.05 ).(3) No significant differences of serum IL-4,IFN-γ,andIL-4/IFN-γ were found between atopic group and non-atopic group,mild group and severe group,normalTIgE group and high-TIgE ( P>0.05 ).ConclusionIL-4,IFN-γand IL-5 are involved in the pathogenesis of MP infection,furthermore,atopic status of the host may lead to the increase of serum IL-5,which may enhance the progress of the disease.
3.Prognostic factors of patients with hepatocellular carcinoma after hepatectomy

Objective To study the prognostic factors in patients of HCC after hepatectomy by Cox proportional hazard.Methods 145 patients operated on for HCC from 1986 to 1996 were followed up to the end of 1999. Seventeen possible factors associated with long survival were analyzed by Kaplain Meier Log rank estimation. A multivariative survival analysis of these individual variables was undertaken using the cumulative survival rate by the computer′s Cox proportional hazard. Results The overall cumulative survival rate at 1?3?5?7?10?12 years was 75 0%?44 4%?29 5%?23 5%?21 2%? and 16 9%, respectively. Results showed that the way by which a tumor was found?tumor size?portal thrombi?satellite nodule?UICC TNM stage?cutting margin?radical resection were the prognostic factors by individual variable analysis;A multivariative analysis showed that tumor finding mode?UICC stage?cutting margin?recurrence?radical resection were associated with prognosis.Conclusion The prognostic factors of HCC focused on two aspects: early diagnosis and treatment method. UICC TNM stage is most predictive for prognosis. Cutting margin of more than 1 cm is needed for long term survival.
4.Postoperative progression and its analysis of ossification of the posterior longitudinal ligament on cervical spine

Objective To investigate and analyze the postoperative progression of ossification of the patients with ossification of the posterior longitudinal ligament (OPLL) on cervical spine. Methods From Jaunary 2001 to December 2007, 95 postoperative patients with cervical OPLL were followed and analyzed retrospectively. There are 72 males, 23 females, with the average age of 56.3 years (range, 40-73years). The follow-up time was from 1 to 6 years, average 3.1 years. Among them 36 patients were performed with anterior cervical corpectomy, fusion with titanium mesh and fixed with cervical plates, others treated with posterior cervical laminectomy and fixation. 2 of the 95 cases were performed anterior and posterior combined operation. Clinical data, X-rays, CT and MR images and progression of ossification, were measured and analyzed in details .The relationships between the progression of ossification and relative factors, as gender, age, C3 ossified involved, T-OPLL, OPLL-type, time of follow-up, surgical approach, Japanese Orthopaedic Association (JOA) scores and improvement rate of JOA scores, were analyzed. Results Progression of ossification in 39 cases among the 95 followed postoperative OPLL patients, 28 men and 11 women, average age 55.9 years, range 41-71 years. The age of progressed patients included 12 cases of ≤49 years, 12of 50-59 years, 12 of 60-69 years and 3 of ≥70 years. 35 patients were operated by posterior approach and only 4 treated with anterior operation. According to the standard of the progression of ossification that 2 mm in the length or/and thickness, there are 4 cases progressed only in length, 2 only in thickness, other 33 patients both the length and thickness. Progression of length is from 2mm to 20mm (average 7.74±4.71). But thickness is progressed from 2 mm to 6 mm (average 2.67±1.51). From 1 to 3 years follow-up time it appeard as a downtrend about the progression of ossification. But it may appear an uptrend from the 4th year. JOA score and improvement rate of the JOA score were almost improved to the high-point in three years. And according to the statistic data there are obvious relationship between progression of ossification with age, surgical approach and C3 ossified involved. Conclusion There is a high rate of postoperative ossification progression in cervical OPLL patients. Cervical OPLL patients with C3 ossification involved, performed with posterior laminectomy and those young at surgery may have higher rate of progression of the ossification. The JOA score and improvement rate of the JOA score were little influenced by the progression of the OPLL during the short and intermediate-term follow-up.
5.Effect of Transverse Position and Numbers on the Stability of the Spinal Pedicle Screw Fixation during the Pedicle Cortex Perforation.

Objective To evaluate the effect of transverse position and numbers on the stability of the spinal pedicle screw fixation during the pedicle cortex perforation. Methods The vertebral compression fracture was performed in the L1 vertebral body using the Chiba's method from 60 fresh thoracic and lumbar vertebrae samples of sheep(T13-L3),which were randomly divided into 6 groups(A,B,C,D,E,and F)using a lottery method. Bilateral pedicles of vertebral arch of T14 and L2 were inserted pedicle screws,connecting titanium rods to fix T14-L2 segments. Then the samples of groups B,C,D,E,and F were removed a quarter of right side of lateral T14 thoracic pedicle cortical,which were considered the pedicle cortex perforation model. Finally,each group was fixed on different numbers of crosslinks:group A(0 crosslink,Intact),group B(0 crosslink,NCL),group C(1 crosslink,1/2 of the rods,MCL),group D(1 crosslink,1/3 of the rods close to T14,PCL),group E(1 crosslink,2/3 of the rods away from T14,DCL)and group F(2 crosslinks,1/3 and 2/3 of the rods respectively,TCL). After all samples were subject to 10 000 times of fatigue test with biomechanics test machines,the axial compressive stiffness,range of the motion(ROM)of the 6 directions(flexion,extension,lateral bending,and axial rotation),and the maximum pullout of the screws of the T14 pedicle cortex perforation were measured and compared among these 6 groups. Results The axial compressive stiffness in groups A,C,D,E,and F was significantly higher than that in group B(all P<0.05),and group A was significantly higher than group F(P<0.05) . The maximum pullout in groups A,C,D,E,and F were significantly higher than that in group B(all P<0.05),and group A was significantly higher than group F and groups C,D,and E were significantly lower than group F(all P<0.05). ROM of flexion,extension,lateral bending,and axial rotation in groups A,B,C,D,E,and F were significantly lower than that in group B(P=0.000),and ROM of left and right axial rotation in groups C,D,and E were significantly higher than in that group F(P=0.000). Conclusions During the pedicle cortex perforation,adding of one crosslink can improve the stability of the pedicle cortex perforation,and adding of two crosslinks can approximately achieve the same stability as the pedicle screw fixation with no pedicle cortex perforation. The location of the crosslink has no obvious effect on the short segment of spinal fixation.
6.A Quantitative Method for Simultaneous Determination of Four Anthraquinones with One Marker in Rhei Radix et Rhizoma

ObjectiveTo develop a quantitative method for simultaneously determining multi-components in Rhei Radix et Rhizoma using one chemical reference substance.MethodsThe contents of multi-components were calculated by the UV relative correction factors (RCFs) of chrysophanol,physcion,and rhein to emodin.ResultsThe values of RCFs at 274 nm for rhein,chrysophanol,and physcion to emodin were 0.712,0.674,and 1.051.The calibration curves were linear over the ranges of 0.02-4.08,0.02-4.12,0.07- 12.92,and 0.02-3.68 μg/mL for rhein,emodin,chrysophanol,and physcion,respectively.The contents of emodin in 18 samples were determined by the extemal standard method,and the contents of the other three anthraquinone aglycones were calculated according to their RCFs.ConclusionNo significant difference is found in comparison with the classical method,indicating that the RCFs have high reliability within their linear ranges and could be used in quality control of Rhei Radix et Rhizoma.The quantitative analysis of multi-component with a single marker is especially suitable for herbal medicines containing unstable or hard to be purified components as quality control markers.

Alveolar ridge preservation (ARP) can prevent alveolar bone resorption,promote new bone formation and maintain the hard and soft tissue morphologies of tooth socket after tooth extraction,which plays an important role in the field of oral implantology.ARP has become a hot topic in dental research with the rapid development of bone substitute materials.This review focuses on recent development of various bone filling materials as well as future perspective of biomedical materials for ARP.
8.Chemical Disinfectant Application:Problems and Countermeasures

OBJECTIVE To investigate the problems and countermeasures in the chemical disinfectants application.METHODS To analyze the existing problems in the chemical disinfectants application from 8 hospitals of different care levels at the same city and provide corresponding measures.RESULTS There were many problems in the use of chemical disinfectants,therefore the key points were enhencing the leadership and management standards,staff training,of corresponding knowledge and the correct methods,and strengthening the guidance,inspection and supervision by functional departments.CONCLUSIONS Medical personnel must properly use the chemical disinfectants to ensure the medical safety and prevent hospital infection effectively.
9.Value of multi-slice spiral CT image texture analysis in diagnosing lymphatic metastasis of rectal cancer

Objective To investigate the value of multi-slice spiral CT image texture analysis in differentiating metastatic (MLN) from non-metastatic lymph nodes (NLN) in patients with rectal cancer.Methods Thirty five patiets with rectal cancer who were pathologically confirmed by total mesorectum excision were included retrospectively,with regional lymph nodes (short-axis diameter of larger than 3 mm)found in preoperative CT images.All the patients underwent preoperative abdominal and pelvic dynamic contrast-enhanced CT scan.Regional lymph nodes were identified according to pathological findings,and were divided into MLN and NLN groups.The short-axis diameter,short-to long-axis diameter ratio of lymph nodes were manually measured and calculated,and the texture features,including skewness,kurtosis,variance,entropy and inverse difference moment,were analyzed.The above parameters between MLN and NLN groups were compared using independent sample t test or Mann-Whitney U test.ROC curve analysis was performed regarding the statistically significant parameters and the areas under curve (AUC) were calculated.Multivariate Logistic regression analysis was accomplished to obtain the independent predictive factor of diagnosing regional lymph nodes.Results A total of 68 regional lymph nodes were obtained and consisted of 31 MLNs and 37 NLNs.The short-axis diameter,kurtosis,and entropy of the MLN group were significantly higher than those of the NLN group (all P<0.05).Whereas,the short-to long-axis diameter ratio,skewness,variance,and inverse difference moment did not differ significantly between the two groups (all P>0.05).The AUC for distinguishing MLN from NLN of the short-axis diameter,kurtosis and entropy were 0.79,0.67,and 0.85,respectively.Multivariate logistic regression analysis showed that only entropy (odds ratio=8.48,95% confidence interval was 3.01 to 23.92,P<0.01) was screened out as the independent variable,which suggested that the entropy was the unique predictor for characterizing regional lymph nodes of rectal cancer.Conclusion Multi-slice spiral CT images texture analysis can facilitate the accurate differentiation between MLN and NLN in patients with rectal cancer,and especially the entropy has the optimal reference significance.
10.Expression of nerve growth factor in spinal dorsal horn following crushed spinal cord injury

AIM: The aim of this study was to explore the expression of nerve growth factor(NGF) in spinal dorsal horn following crushed spinal cord injury. METHODS: The adult Srague-Dawley rat model of crushed spinal cord injury was established by the method in our laboratory, and intact spinal cord was used as control. The rats were sacrificed respectively after 24 hours, 7 days, and 21 days of operation, and the L3 spinal segments were removed out and fixed in 4% polyformaldehyde. The segments were sectioned into sections of 20 μm in thickness. The sections were stained with anti-NGF antibody by ABC method of immunohistochemistry technique. The immunoreactive intensity of NGF and the number of positive neurons as well as glial cells in dorsal horn were observed and counted under light microscope. RESULTS: The number of positive cells and immunoreactive intensity of NGF increased gradually in the dorsal horn at 24 hours, 7 days and 21 days following crushed spinal cord injury compared with control group (P<0.01). CONCLUSION: These results indicated that NGF plays an important role in the postoperative reaction during the early period of the crushed spinal cord injury.
