1.Effects of two sputum aspirations on elderly critical patients

Objective To investigate the effects of two sputum aspirations on old critical patients.Methods Sixty elderly critical patients were divided into the experiment group and control group in equal number.The former group received sputum aspiration by oropharynx airpipe and the control group by oronasal airpipe.The two groups were compared in terms of sputum aspiration effect and saturation of blood oxygen.Result The observation group was significantly better than the control group in terms of sputum aspiration effect and saturation of blood oxygen(both P<0.05).Conclusion Sputum aspiration by oropharynx airpipe is effective in the aspiration of secretions from respiratory tract and the reduced effect of saturation of blood oxygen.
2.Clinical effect of salvia miltiorrhiza injection combined with 3D-radiotherapy for esophageal cancer therapy

ObjectiveTo evaluate the efficacy and toxicity of 3D-radiotherapy combined with salvia miltiorrhiza injection to treat esophageal cancer. Methods62 patients were randomly divided into group A and B.Group A were treated with 3D-radiotherapy (60-70 Gy) combined with salvia miltiorrhiza injection (20 ml/d).Group B were treated with 3D-radiotherapy (60-70 Gy) combined with cisplatin (30 mg/m2,d1-d3/w).ResultsThe 71.0 %(22/31 ) recent effective rate of group A was obtained[74.2 %(22/31) of group B],the 2 to 3 years survival rates were 66.7 % and 43.3 % respectively(55.3 % and 16.7 % of group B). Conclusion The effect is good using 3D-radiotherapy combined with salvia miltiorrhiza injection to treat esophageal cancer.
3.Image quality evaluation of virtual monoenergetic spectral images obtained in two types of dual energy CT based on noise level

Objective To investigate the image quality difference of the virtual monochromatic spectral(VMS) images synthesized from fast kilovoltage switching and dual-source dual-energy CT for a given radiation dose. Methods A plurality of disposable syringes containing 15 mg/ml iodine contrast agent and saline were placed on the surface of the male anthropomorphic phantom consisting ofhead neck and torso, GE HD750 gemstone spectral imaging and Siemens Somatom Definition Flash dual energy CT protocols were performed on the phantom for a same dose(volume CT dose index 6.52 mGy), and VMS images (40, 60, 70, 80, 100, 120, 140 keV) were reconstructed and obtained(VMSkV switching and VMSdual-source image), respectively. The objective image noise, iodine signal noise ratio(SNR), iodine contrast noise ratio (CNR) and CT values were measured. The results were analyzed using the paired t test and ANOVA. Results All VMS iodine CT values were gradually decreased with the increasing of keV , and iodine CT values on VMSdual-source images were greater than on VMSkV switching images(P<0.05), VMSkV switching iodine CT values in descending order from the head , chest, abdomen at less than 100 keV,while most VMSdual-source iodine CT values were highest on chest(P<0.05). VMSdual-source and VMSkV switching image noise were highest at 40 keV and successively raised from the head, chest , abdomen. VMSkV switching image noise gradually decreased with the increase of keV in the range of 40 to 70 keV , gradually increased up to the trough after 70 keV from 80 keV gradually decreased, while VMSdual-source image noise was lowest at 70 keV or 80 keV , and then with the keV increased gradually increased. Image noise on VMSkV switching images in the range of 40 to 100 keV were higher than that on VMSdual-source images and lower in the range of 100 to 140 keV at most part(P<0.05). VMSdual-source and VMSkV switching iodine SNR were highest at 40 keV or 60 keV,SNR on VMSdual-source images are greater than on VMSkV switching images except 40 keV images on head and abdomen(P<0.05) . VMSdual-source and VMSkV switching iodine CNR were highest at 60 keV,CNR on VMSdual-source images are greater than on VMSkV switching images except 40 keV images on head 100 keV and 120 keV on abdomen(P<0.05).SNR and CNR decreased from the head, chest, abdomen (P<0.05). Conclusion VMS images synthesized from fast kilovoltage switching and dual-source dual-energy CT have different image quality performance at most keV and body parts, but VMS imaging at approximately 70 keV yielded lower image noise, and at approximately 60 keV yielded highest SNR and CNR.
4.Effects of different types of anesthesia on lower respiratory tract infections in elderly patients of orthopedic surgery

Objective To investigate the effects of different types of anesthesia on the incidence rates of low-er respiratory tract infections of elderly patients in orthopedic department.Methods Clinical data from 445 elderly patients in orthopedic department were collected and retrospectively analyzed.We also investigated the relationship be-tween the types of anesthesia and the lower respiratory tract infections.Results The incidence rates of lower respira-tory tract infections were 23.08%in 169 cases,12.14%in 173 cases,and 4.85%in 103 cases after general anesthe-sia with tracheal intubation,intraspinal anesthesia,and nerve blocking anesthesia,respectively.There was statistically significant difference in the infection rates among the three groups (χ2 =13.784,P=0.001).Conclusion Different ways of anesthesia is critically correlated with the incidence of the lower respiratory tract infections in elderly patients.
5.Level of Serum Creatinine in Patients with Spinal Cord Injury

ObjectiveTo investigate the value of serum creatinine(SCr) to monitor the renal function of patients with spinal cord injury(SCI).Methods1298 SCI patients were investigated in Beijing Charity Hospital from January 2004 to May 2009. 249 healthy people were involved as control. The data of SCr were analysed.ResultsThe level of SCr for the SCI patients was (56.81±14.33) μmol/L with normal distribution, 95% CI was 28.73~84.89 μmol/L; The level of SCr for the healthy people was (75.98±11.34) μmol/L, 95% CI was 57.36~101.80 μmol/L. The level of SCr was significantly lower in the SCI patients comparing to the healthy people (P<0.01). Among the SCI patients, the level of SCr was (59.59±13.76) μmol/L for male while (46.41±11.31) μmol/L for female(P<0.05). There was no difference in level of SCr among different ages of the SCI patients(P>0.05). The characteristics were likely in the two groups. There was no difference in level of SCr between paraplegic (55.54±14.96) μmol/L and quadriplegic (57.67±13.83) μmol/L (P>0.05).ConclusionThe characteristics of level of SCr were likely in SCI patients and healthy people with normal distribution. The level of SCr was lower in SCI patients comparing to healthy people, which was higher for male SCI patients comparing to female SCI patients.
6.Evaluation on Ability to Detect the Intracranial Hematoma with Different Density Using C-Arm Cone-beam Computed Tomography Based on Animal Model.

This study aims to evaluate the ability of C-arm cone-beam CT to detect intracranial hematomas in canine models. Twenty one healthy canines were divided into seven groups and each group had three animals. Autologous blood and contrast agent (3 mL) were slowly injected into the left/right frontal lobes of each animal. Canines in the first group, the control group, were only injected with autologous blood without contrast agent. Each animal in all the 7 groups was scanned with C-arm cone-beam CT and multislice computed tomography (MSCT) after 5 minutes. The attenuation values and their standard deviations of the hematoma and uniformed brain tissues were measured to calculate the image noise, signal to noise ratio (SNR) and contrast to noise ratio (CNR). A scale with scores 1-3 was used to rate the quality of the reconstructed image of different hematoma as a subjective evaluation, and all the experimental data were processed with statistical treatment. The results revealed that when the density of hematoma was less than 65 HU, hematomata were not very clear on C-arm CT images, and when the density of hematoma was more than 65 HU, hematomata showed clearly on both C-arm CT and MSCT images and the scores of them were close. The coherence between the two physicians was very reliable. The same results were obtained with C-arm cone-beam CT and MSCT grades in measuring SD value, SNR, and CNR. The reasonable choice of density detection range of intracranial hematoma with C-arm cone-beam CT could be effectively applied to monitoring the intracranial hemorrhage during interventional diagnosis and treatment.

7.A prospective randomized study on digestive tract cleaning before ERCP for the prevention of postprocedure cholangitis

Objective To evaluate the effectiveness of mouth care combined with intestinal and endoscopic working channel washing for ERCP related cholangitis.Methods A total of 573 patients diagnosed as having obstructive jaundice were randomized into three groups,190 cases in the control group,192 in the saline group and 191 in the amikacin group.Clinical and laboratory data were collected before ERCP and ERCP related cholangitis were recorded.Results There were no significant differences among the three groups in sex,age,the level of obstruction,the category of obstruction,total bilirubin or WBC counting.The incidences of ERCP related cholangitis were 21.1% (40/190),13.5% (26/192) and 4.7% (9/191)in control group,saline group and amikacin group respectively,which was significantly different (x2 =22.409,P =0.000 ).The incidences of ERCP related cholangitis were 19.5% ( 65/333 ) and 4.2%(10/240) in patients diagnosed as having hilar duct obstruction and low positioned biliary obstruction respectively (x2 =27.175,P =0.000).There was no significant difference in ERCP related cholangitis between benign and malignant biliary obstruction.Subgroup of hilar duct obstruction showed the incidences of ERCP related cholangitis were 29.7% (33/111 ),20.5% (24/117)and 7.6% (8/105)in the control group,the saline group and the amikacin group,respectively (x2 =16.905,P =0.000).Conclusion The incidence of ERCP related cholangitis is relatively higher in patients with hilar duct obstruction.Mouth care combined with intestinal and endoscopic working channel washing can effectively reduce the incidence of ERCP related cholangitis,especially in the amikacin group.
8.Characteristics of amplitude integrated EEG in neonates with hypoglycemic brain damage

Objectives To investigate the clinical characteristics of hypoglycemic brain damage, and to assess the ifndings of amplitude-integrated electroencephalography (aEEG) and its predictive value in hypoglycemic brain damage. Methods Twenty-four neonates diagnosed with hypoglycaemia were selected. 12-hour continuous aEEG recordings were performed on the day when hypoglycaemia was diagnosed and second aEEG tracings was performed on the same day or the day after. The variability of aEEG background, appearance of sleep-wake cycling, bandwidth span and amplitude of lower border were analysed and compared with the results of brain MRI. Results Different degrees of epileptic seizures were found in neonates with severe hypoglycemic brain damage and were persisted after the blood sugar was corrected. aEEG in hypoglycemic brain damage was characterized by calyptriform or jagged epileptiform activity, disappearance of the sleep-wake cycle, but little impact on amplitude of lower border and bandwidth span. The recovery of sleep-wake cycle was a sign of brain function recovery. The aEEG and MRI had a good consistency in monitoring the hypoglycemic brain damage. Conclusions aEEG have signiifcant changes in hypoglycemic brain damage and can be used to monitor dynamically hypo-glycemic brain damage.
9.Change and significance of serum brain-derived neurotrophic factor level in neonatal hyperbilirubinemia

Objective To explore the clinical significance of the serum brain?derived neurotrophic factor( BDNF) level in severe neonatal hyperbilirubinemia. Methods One hundred and twenty term and birth weight>2500 g infants admitted to the Neonatal Intensive Care Unit of Bethune nternational Peace Hospital of People Liberation Army were divided into severe hyperbilirubinemia group and control group according to their total bilirubin concentration. Total bilirubin( TBIL) concentration,BDNF and albumin in serum were determined in two groups. In addition, craniocerebral MRI was performed in severe neonatal hyperbilirubinemia before discharge. The correlation of the BDNF, TBIL, B/A, MRI results between severe hyperbilirubinemia group and control group were compared. Results The serum BDNF levels in severe hyperbilirubinemia group was ( 8. 84 ±3. 26) μg/L,significantly higher than that in control group((6. 24±1. 71) μg/L,t=3. 88,P<0. 05). In severe hyperbilirubinemia group,BDNF level was positively correlated with B/A level(r=0. 429,P<0. 05),but there was no correlation between BDNF and total bilirubin level(r=0. 278,P>0. 05). The serum BDNF level with craniocerebral MRI abnormal was ( 9. 53 ± 2. 77 ) μg/L, higher than that with craniocerebral MRI abnormal ((7. 81±3. 76) μg/L),but there was no statistical difference between them(t=1. 439,P>0. 05). Conclusion In severe neonatal hyperbilirubinemia, the body can secrete BDNF increasely. BDNF level is positivelycorrelated with B/ A level. As a marker of brain damage,BDNF is sensitive than craniocerebral MRI.
10.Risk factors for perioperative acute kidney injury in adult cardiac valve surgery with cardiopulmonary bypass

Objective To investigate the risk factors of acute kidney injury (AKI) in patients undergoing cardiac valve surgery with cardiopulmonary bypass (CPB).Methods A retrospective cohort database study was conducted, involving 1 349 patients undergoing heart valve surgery with CPB technique.Logistic regression was used to screen out the risk factors of AKI after the surgery.Results Of the 1 349 patients, the incidence of AKI in valve surgery was 28.4%.One year older (OR=1.05, 95%CI 1.03-1.06, P<0.001), diabetes (OR=2.11, 95%CI 1.22-3.68, P=0.008), anemia (OR=1.50, 95%CI 1.05-2.21, P=0.026), each additional basic serum creatinine of 1 mg/dl (OR=1.01, 95%CI 1.01-1.02, P=0.001), each additional operation time of 1 hour (OR=1.28, 95%CI 1.15-1.41, P<0.001), plasma transfusion during surgery (OR=1.50, 95%CI 1.14-1.97, P=0.004) were the independent risk factors for AKI in multivariate logistic regression model.Conclusion AKI is a common and serious complication following cardiac valve surgery.More attention should be paid to the patients with elder age, anemia, prolonged operation time, diabetes, increased basic serum creatinine and requirement of plasma transfusion during surgery.
