Objective: To observe left and right ventricular mechanical sequence in systole and diastole by dual Doppler Echocardiography in healthy subjects. Methods: Dual Doppler echocardiography was performed in 100 normal subjects with dual-outflow-tract-view and apical four chamber view to simultaneously record the spectrum of left/right ventricular outlfow tract (LVOT)/(RVOT) and left/right ventricular inlfow tract (LVIT)/(RVIT) at the same cardiac cycle. The time cycles of blood lfow spectrum from the peak of QRS complex to aorta, pulmonary artery, mitral valve (MV) and tricuspid valve (TV) were measured, the starting time differences for blood lfow spectrum of aorta and pulmonary artery, MV and TV were calculated to assess the mechanical sequence of left and right ventricle in systole and diastole respectively. Results: There were 48/100 (48%) subjects having LV ejection preceded than RV (95% CI 0.38-0.58), 46 (46%) having RV ejection preceded than LV (95% CI 0.36-0.56) and 6 (6%) having LV, RV simultaneous ejection (95% CI 0.01-0.11). There were 96/100 (96%) subjects having RV iflling prior to LV (95% CI 0.90-0.99), 3 (3%) having LV iflling prior to RV (95% CI 0.01-0.08) and 1 (1%) having LV, RV simultaneous iflling (95% CI 0.00-0.05). Conclusion: There is a regular pattern for mechanical sequence of LV and RV in systole and diastole in healthy subject; in systole, the sequence of LV varies from person to person, while in diastole, RV is always preceded. Based on normal ventricular mechanical sequence, optimizing programming parameter of pace maker and choosing cardiac sequence in diastole might be the ifnal criteria for cardiac resynchronization therapy in the future.

OBJECTIVE To determine the distribution of allergens in patients with allergic rhinitis in Wenzhou area. METHODS Patients with AR symptoms from January 2013 to December 2014 were given skin prick test (SPT). The clinical data about SPT was retrospectively colleceted to analyze SPT results. RESULTS 1. Among 2991 individuals, the total positive rate of SPT was 82.0%, with Dermatophagoides farinae (Der. f) and Dermatophagoides pteronyssinus (Der.p) as the most common allergens; the positive rate of inhalant allergen was obviously higher than that of ingestive ones, with significantly statistical difference(χ2=2006.557,P<0.01). Most of patients were allergic to double allergens; the intensity of Der.f and Der.p mainly presented as (++++), with no significant difference(Z=-0.391, P=0.696). 2. There was significant difference of variation with seasons(χ2=34.254, P<0.01). 3. No significant difference of positive rate were observed in different AR-courses(χ2=16.102, P<0.01). 4. Significant difference of positive rate was found among different age-groups; The positive rate was increased along with growing age, got peak at group of 10-12 years old, and then got down after that. CONCLUSION Dust mite was the main allergen coursed AR in Wenzhou area. Seasons and age were two important factors effecting on positive of SPT and onset of AR. The positive rate of allergens was related to age.

Objective:To evaluate the diagnostic value of neck imaging reporting and data systems (NI-RADS) based on MRI in extracavity local recurrent nasopharyngeal carcinoma (ELRNPC) and posttreatment changes (PTC).Methods:From April 2015 to September 2020, 33 cases of ELRNPC and 37 cases of PTC confirmed by pathology or follow-up were retrospectively enrolled at Zhongshan City People′s Hospital. Two radiologists independently evaluated the conventional MRI findings using NI-RADS criteria in the first step, then evaluated with conventional MRI and diffusion weighted imaging (DWI) sequences in the second step. All images were re-evaluated by one senior radiologist in the same steps after three months. Inter- and intra-reader agreements were assessed with Cohen′s Kappa test. Receiver operating characteristic curves were generated to assess the diagnostic values of NI-RADS categories between ELRNPC and PTC. The area under the curve (AUC) was compared by Delong test.Results:Inter- and intra-reader agreements of Kappa value were 0.742 and 0.909 for conventional MRI and 0.807 and 0.934 for conventional MRI with DWI. In the differential diagnosis of ELRNPC and PTC, the AUC, sensitivity, and specificity of NI-RADS categories based on conventional MRI were 0.932 (95%CI 0.846-0.978), 87.9% (95%CI 71.8%-96.6%), 94.6% (95%CI 81.8%-99.3%), and of NI-RADS based on conventional MRI with DWI were 0.991 (95%CI 0.933-1.000), 93.9% (95%CI 79.8%-99.3%), 97.3% (95%CI 85.8%-99.9%), respectively. There was a statistical difference between the AUCs of the two categories ( Z=2.20, P=0.028). Conclusions:For both the NI-RADS based on MRI with or without DWI, the differential diagnostic value of ELRNPC and PTC is excellent, while the consistency and diagnostic performance are more substantial when combined with DWI.
