1.Effect of metformin on proliferation and apoptosis in human pancreatic cancer cell line CFPAC-1

Objective To investigate the effect of metformin on the proliferation and apoptosis in human pancreatic cancer cell line CFPAC-1,and to explore the potential mechanism.Methods Human pancreatic cancer CFPAC-1 cells were cultured in vitro,and were treated with metformin at different concentrations (1,2.5,5,10,20,40,60 mmol/L) for different durations (24 h,48 h and 72 h),and cells without treatment were considered as control group.Cell proliferation was evaluated by CCK-8,cell cycle was determined by flow cytometry,apoptosis was determined by Annexin V/PI double staining method,and Western blot was used to detect the protein expression of p-AMPK,FAS,cyclin D1,Bcl-xl,Bax,caspase-3 and survivin.Results Metformin could inhibit the proliferation of CFPAC-1 cells in a time and dose dependent manner.Forty-eight hours after 40 mmol/L metformin treatment,the proportion of CFPAC-1 cells in phase G0/G1 was significantly increased [(65.93 ± 0.27)% vs (42.89± 1.02)%],and the proportion of CFPAC-1 cells in phase G2/M,S was significantly decreased [(22.01 ± 2.95) % vs (38.28 ± 4.93) %,(13.58±0.43)% vs (20.12 ± 3.38)%],and the difference between the two groups was statistically significant (P <0.05).The apoptosis rate was increased from (3.01 ± 0.49) % to (32.97 ± 3.19) %,(P < 0.05) ; and the expression of p-AMPK,Bax,and caspase-3 was increased,while the expression of FAS,cyclin D1,Bcl-xl,survivin were decreased.Conclusions Metformin can inhibit proliferation and promote apoptosis of CFPAC-1 cells mainly by activation of AMPK pathway,and down-regulation of FAS,cyclin D1,survivin and Bcl xl/Bax ratio,as well as up-regulation of caspase-3.
2.The relation between the positive rate of platelet antibody and the frequency of platelet transfusion, the gender and the blood group of patients

Objective To study the relation between the positive rate of platelet antibody and the frequency of platelet transfusion,the total quantity of platelet transfusion,the gender of patients,the blood group of patients through statistic analysis of the data gathered from patients.Methods A total of 5 745 results of platelet antibodies detection assay throughout the year of 2016 were gathered,and statistical relation between the positive rate of platelet antibody and the gender of patients,the blood group of patients and the frequency of platelet transfusion were analyzed.Results The results of platelet antibodies detection assay show that 108 patients were positive among 5 745 patients,and the positive rate of platelet antibody was 1.87%.There were no relations between the positive rate of platelet antibody and the gender of patients or the blood group of patients,while the positive rate of platelet antibody and the frequency of platelet transfusion were positively correlated.Conclusion Platelet antibody production could impact the therapeutic effect of platelet transfusion.Patients with platelet antibodies should receive matching platelet to ensure the therapeutic effect.
3.Analgesic efficacy of ultrasound?guided adductor canal blockade after minor arthroscopic knee surgery

Objective To investigate the analgesic efficacy of ultrasound?guided adductor canal blockade (ACB)after minor arthroscopic knee surgery. Methods Sixty patients undergone minor arthroscopic knee surgery were randomly divided into group ACB(n=20)and group Control(n=20). All patients received spinal anesthesia. The patients in group ACB received ultrasound?gGuided ACB with 20 ml 0.5% ropivacaine,and patients in group Control received 20 ml saline after the surgery. In addition ,all patients have a basic analgesic regimen with etoricoxib. Visual analogue scales(VAS) during rest and passive movement ,additional analgesic dose and side effects were recorded at 4,8,12,24 h Post?operation. At 24 h post?operation,the numbers of patients who can walk for 5 meters were recorded. Results VAS during rest and movement at 4 h,8 h and 12 h post?operation in group ACB were significantly lower than those in group Control. And all patients could walk 5m at 24 h post?operation. No headache,nausea and vomiting,urinary retention and other adverse reactions were observed in group ACB. There were four patients who received additional analgesic and one patient vomitted. Conclusions Significant analgesic effect of the ACB could be detected after minor arthroscopic knee surgery ,with less reduction in requirements for supplemental opioids.
4.Effects of therapeutic dose on temperature rise induced by high intensity focused ultrasound in tissue.

Based on the well-known Pennes' bioheat equation, we analyzed theoretically the temperature rise in tissue during high intensity focused ultrasound(HIFU) irradiation, and measured the focal temperature rise induced by HIFU in the treatment of freshly excised cow liver with different acoustic intensity and exposure time. The results showed that a threshold exposure time always existed under a certain acoustic intensity. Temperature rise was slow when exposure time exceeded the threshold exposure time. The greater the acoustic intensity was, the earlier the threshold time appeared. The focal temperature rise and the relative cumulative thermal dose (RCTD) increased with the increase of acoustic intensity and exposure time. For a certain therapeutic dose, the effects of acoustic intensity on focal temperature rise were more distinct than the effects of exposure time on focal temperature rise. Therefore, the optimal HIFU therapeutic dose should meet the need, i.e. moderate acoustic intensity, and the exposure time be the threshold exposure time under this acoustic intensity.

5.High intensity focused ultrasound alone for malignant solid tumors.

OBJECTIVETo investigate the efficacy and side-effects of high intensity focused ultrasound (HIFU) in the treatment of malignant solid tumors.
METHODSThirty such patients who refused surgery and or refractory to chemotherapy, were treated by HIFU alone, with the efficacy and side effects monitored in terms of vital organ signs observation, functional assay of important organs, imaging examinations: digital subtraction angiography (DSA), CT, MRI, single photoemission computed tomography (SPECT), large core needle biopsy, complications or metastasis.
RESULTSAfter HIFU therapy, the vital signs remained stable and the function of heart, lung, kidney and liver was also normal. DSA images showed that small or larger arteries were not damaged. After a follow-up of 10 - 38 months (23.1 months), 26 patients (87%) have been alive. The volume of the tumor underwent complete regression in 10 patients. Shrinkage of tumor volume >/= 50% was observed in 13. Eight of 13 patients were examined by large core needle biopsy, all showing necrosis and/or fibrosis though 3 patients (10%) had local recurrence. Two of these were retreated again by HIFU and the locally recurrent tumor became well controlled. New metastases developed in 5 patients after HIFU therapy. Two patients suffered from peripheral nerve injuries, most of which have recovered during the follow-up. However, one patient developed skin injury.
CONCLUSIONHigh intensity focused ultrasound alone is effective and safe in the treatment of malignant solid tumors.

6.Endovenous laser ablation combined with subfascial endoscopy therapy for venous ulcer of the lower =limbs

Objective To study the effect of endovenous laser ablation combined with subfascial endoscopy therapy for venous ulcer of the lower limbs.Methods Clinical data of 112 patients undergoing endovenous laser ablation combined with subfascial endoscopy therapy for venous ulcer of the lower limbs from Oct 2011 to Feb 2016 was retrospectively evaluated.Results Patients had average 2-6 perforating veins in their affected lower limbs.Procedures were successful in all the patients,all the superficial varicose veins disappeared after 1 month and 1 year of ultrasound follow-up,perforating veins closed,ulcer healed,and chromatosis alleviated.Conclusion Endovenous laser ablation combined with subfascial endoscopy therapy for venous ulcer of the lower limbs is effective,safe and quick recovery.

Objective To compare the efficacy, safety, and survivability of TCbHP versus AC-THP in the neoadjuvant therapy of HER2-positive breast cancer in real-world. Methods Clinical data of patients with HER2 positive breast cancer, who have received TCbHP or AC-THP as neoadjuvant therapy and completed surgery in 11 third-class hospitals in various cities of Hebei Province, were retrospectively collected.The total pathological complete remission (tpCR) rate, the incidence of grade 3 or higher adverse reactions and the completion rate of the given approaches were compared. Results A total of 110 cases were collected, including 78 cases in the TCbHP group and 32 cases in the AC-THP group.The tpCR rate of the TCbHP group was higher than that of the AC-THP group, but the difference was not statistically significant (64.10%

Bisecting N-acetylglucosamine(GlcNAc),a GlcNAc linked to the core β-mannose resi-due via a β1,4 linkage,is a special type of N-glycosylation that has been reported to be involved in various biological processes,such as cell adhesion and fetal development.This N-glycan structure is abundant in human trophoblasts,which is postulated to be resistant to natural killer cell-mediated cytotoxicity,enabling a mother to nourish a fetus without rejection.In this study,we hypothesized that the human amniotic membrane,which serves as the last barrier for the fetus,may also express bisected-type glycans.To test this hypothesis,glycomic analysis of the human amniotic membrane was performed,and bisected N-glycans were detected.Furthermore,our pro-teomic data,which have been previously employed to explore human missing proteins,were ana-lyzed and the presence of bisecting GlcNAc-modified peptides was confirmed.A total of 41 glycoproteins with 43 glycopeptides were found to possess a bisecting GlcNAc,and 25 of these gly-coproteins were reported to exhibit this type of modification for the first time.These results provide insights into the potential roles of bisecting GlcNAc modification in the human amniotic membrane,and can be beneficial to functional studies on glycoproteins with bisecting GlcNAc modifications and functional studies on immune suppression in human placenta.
9.Development and effects of bundled and continuous nursing measures for patients during daytime chemotherapy after gastric cancer surgery

Objective:To develop bundled and continuous nursing measures for patients during daytime chemotherapy after gastric cancer surgery and explore its effects.Methods:Literature search, expert consultation and field visits were used to understand the needs of patients during daytime chemotherapy after gastric cancer surgery, specify the corresponding measures, and determine the final evidence-based bundled and continuous nursing measures. Totally 62 patients undergoing the first chemotherapy after gastric cancer surgery from May to December 2018 were included into the control group, and 53 patients undergoing the first chemotherapy after gastric cancer surgery from January to July 2019 were included into the intervention group. Patients in the control group received conventional continuous nursing, while patients in the intervention group received evidence-based bundled and continuous nursing. The laboratory tests and chemotherapy compliance during the eight chemotherapy treatments were compared between the two groups of patients, and the quality of life and satisfaction with the medical and nursing staff after the eight chemotherapy treatments of the two groups of patients were observed.Results:After the implementation of evidence-based bundled and continuous nursing measures, the compliance of the intervention group was higher than that of the control group ( P<0.05) . There were statistically significant differences in the quality of life between the two groups, except for cognitive function, sleep and economic status ( P<0.05) . The satisfaction with the medical and nursing staff in the control group was 79.03% (49/62) lower than 92.45% (49/53) in the intervention group, and the difference between the two groups was statistically significant ( P<0.05) . Conclusions:The evidence-based bundled and continuing nursing measures can improve the compliance, quality of life and satisfaction with the medical nursing staff in patients undergoing the first chemotherapy after gastric cancer surgery.
10.Effect of oral nutrition supplements cluster care in home care of postoperative patients with gastric cancer

Objective:To construct a cluster care of oral nutrition supplements (ONS) , and to explore its effect in home care of postoperative patients with gastric cancer.Methods:Literature search was conducted to review the status quo of ONS compliance at home in patients with gastric cancer after surgery, and analyze the existing problems. We summarized and formulated the cluster care plan according to the evidence, and established the home ONS management team and process. From November 2020 to October 2021, 107 patients with gastric cancer who needed to implement ONS at home in Gastrointestinal Surgery of Ningbo First Hospital were selected as research objects by convenience sampling. Postoperative patients with gastric cancer from November 2020 to April 2021 were set as the control group ( n=40) , and those from May to October 2021 were set as the observation group ( n=67) . The control group was treated with conventional ONS, and the observation group was treated with home ONS cluster care plan on the basis of the control group. The compliance of nutritional support, weight gain, body mass index increase and follow-up experience between the two groups were evaluated. Results:After six months of intervention, the scores of ONS compliance, weight gain, body mass index increase and follow-up experience in the observation group were significantly higher than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The cluster care plan improves the ONS compliance at home, nutritional status and satisfaction with the follow-up by medical and nursing staff in patients with gastric cancer after surgery, which is worthy of clinical promotion.
