Objective To study the prevalence of intestinal metaplasia (IM) ,dysplasia and carcinoma at the distal esophagus and esophagogastric junctions as well as the relationship between SCJ and RE. Methods In 391 patients the main symptoms , RE and its degree by gastro endoscopy, and biopsy specimens from the mucosa just below SCJ were investigated. Results The prevalence of IM in LSBE,SSBE and GEJ is 26. 53% , 33.85% and 34.00% respectively; It increases with age after 40 years old and no difference occurs between' the male and female. ;12 cases of dysplasia were diagnosed ( low -grade 7 cases , medium and high-grade 5 cases) . Sixteen cases of cardia carcinomas and 1 case of esophageal adenocarcinoma were diagnosed; The longer the SCJ moved upward above GEJ, the higher the prevalence and severeness of RE . Conclusions 1. There is no difference about the prevalence of IM among LSBE,SSBE and GEJ; 2. It is important to pay attention to the diagnosis of dysplasia in the distal esophagus and esophagogastric junctions; 3. The prevalence of cardia carcinomas is much higher than that of esophageal adenocarcinoma.

Objective To evaluate the accuracy of high-aensitivity C-reactive protein (hs-CRP) in the prediction of perioperative cardiovascular accidents (CVAS) in patients with diabetes mellitus. Methods Ninetyone type 2 diabetic patients undergoing elective non-cardiac surgery were divided into 3 groups according to their blood concentrations of hs-CRP: low-concentration group < 1.0 mg/L ( group Ⅰ , n = 9); median-concentration group 1.0-2.9 mg/L (group Ⅱ ,n= 16) and high-concentration group 3.0-10.0 mg/L (group Ⅲ ,n =66). Risk factors for perioperative CVA were analyzed using Spearman rank correlation analysis. Various risk factors including sex, age, ASA physical status, BMI, NYHA claasification, smoking history ,preoperative ECG changes, duration, treatment and complications of diabetes mellitus, hypertension and blood lipids were correlated with perioperative CVA. The risk factors of which P values were less than 0.05 would enter the binary logistic regression analysis to stratify CVA-related risk factors. Area under the ROC curve was used to analyze the accuracy of the risk factors in prediction of perioperative CVA. Results The incidences of perioperative CVAs were 0, 31% and 35 % in Ⅰ ,Ⅱ and Ⅲ groups respectively. It was determined by Spearman rank correlation analysis that preoperative ECG,NYHA classification, age, preoperative cardiovascular color doppler ultrasound, blood hs-CRP concentration and ASA classification of physical status were important risk factors ( P < 0.05 or 0.01 ). Binary logistic regression .malysis indicated that preoperative ECG, NYHA classification and age were highly related to perioperative CVA (P <0.05 or 0.01).The area under ROC curve of preoperative ECG, NYHA classification, age and blood hsCRP concentration was 0.855, 0.755, 0.702 and 0.605 respectively. Conclusion The accuracy of blood concentration of hs-CRP in prediction of perioperative CVA in patients with diabetes mellitus is lower than that of the preoperative ECG, NYHA classification or age.

Objective To explore the protective effects of geranylgeranylacetone (GGA) on acute renal failure tats induced by isehemia reperfusion (IR) and the possible mechanism. Methods GGA (400 mg/kg) was administered to induce overexpression of heat shock protein 72 (HSP72) in the kidney of Sprague-Dawley (SD) rats. IR model was generated by temporary clamping the left renal artery for 45 minutes followed by right nephrectomy and 24 h reperfusion. A sham-operated group was used as normal control. 24 h after reperfnsion, rats were sacrificed. Blood was collected for measurement of serum creatinine (Scr) and blood urea nitrogen ( BUN ). Paraffin-embedded sections of the kidney were stained with PAS. Histological changes due to tubular damage were quantitated as tubular damage score. TUNEL assay was used to detect the apoptosis, and Western-blot was used to detect the expression of XIAP. Results After renal IR, the increased level of BUN and Scr, the tubular injury and the apoptosis of renal tubular epithelial cells were observed (P<0.01). At the same time, the decreased level of XIAP was observed (P< 0.01). Compared with the control groups, the level of HSP72 expression was up-regulated in oral administration of GGA group (P<0.05). The expression levels of BUN and serum creatinine were significantly decreased after IR injury in pre-conditioned rats with over-expression of HSP72 (P< 0.01 ). Kidney morphology was better preserved in GGA group. Rats with over-expression of HSP72 also revealed reduction of apoptotic cells by TUNEL stain and XIAP degradation by Western blot (P<0.05). Conclusion GGA attenuates renal IR injury at least in part through inhibiting tubular cell apoptosis by decreasing XAIP degradation and restoring XIAP protein level.

Objective: To investigate the effect of intravenous immunoglobulin in highly sensitized patients awaiting organ transplantation. Methods: IVIG was used to reduce donor specific anti HLA alloantibodies in vitro and in vivo . Fifteen patients received IVIG′s suppressive experiment in vitro by random panel lymphocytotoxicity test. The serum of patients were divided into 2 groups: one was diluted with equal volume of IVIG and the other was diluted with PBS solution, and then reacted with lymphocytes from healthy donors randomly. Of them 5 patients received the treatment by IVIG. Four patients were administrated with 0.5 g/kg. Period of treatment was 4 weeks. One patient received 8 weeks infusion in same dose, 2 patients resulted in PRA drop to 10% had received kidney and pancreas kidney transplantation. Results: The percentage of RPLT in experimental group was lower than that in control group. After large dose of infusion of IVIG, the patients showed a reduction in absolute PRA of 2% 51% (mean decrease: 23%). Two patients had undergone subsequent transplantation and no serious rejection occurred. Conclusion: Treatment with IVIG is a valuable tool for the transplantation of immunized patients. The effect of IVIG is dose dependent and can be achieved in 3 weeks.
