1.Operative treatment of comminuted fractures of distal tibia

[Objective]To investigate the curative effect and methods of distal tibial fracture by operation.[Method]From May 2004 to Feb.2005,30 cases of distal tibial fracture were treated in the department.All cases were treated with open reduction,internal fixation with medial anatomic plates,and with bone-grafting prophylacticly in all cases.The operation would be done on 10~14 days after fracture.They were followed up and analyzed retrospectively.[Result]All cases were followed up for 8~17 months(average 13.9 months).All wounds were primary healing and all fractures obtained bone-union.Twenty-two cases were the excellent,6 cases were the good according to Mazur's criteria.The excellent and good rate was 93.3%.All cases had no severe complication,such as wound infection,cutaneous necrosis and bone non-union.[Conclusion]Anatomic plate of distal tibia is a perfect choice for treatment of distal tibial fracture.Operative juncture and prophylactic bone grafting were very important to obtain satisfactory effect.
2.Primary investigation on therapeutic effect of stem cell transplantation in patients with leukemia based on changes in haptoglobin

Objective To explore the effects of different approaches of stem cell therapy in patients with leukemia on haptoglobin(Hp).Methods The trial includes four patients treated by mesenchymal stem cell(MSC)with hematopoietic stem cell cotransplantation(HSCT)and two patients treated by hematopoietic stem cell transplantation(HSCT).Haptoglobin in the plasma,collected from different therapeutic stages,of six patients was separated by two-dimensional gel electrophoresis(2-DE),and then followed by the identification with MALDI-TOF/TOF mass spectrometry.Results The abundance of haptoglobin alpha and beta chains with different modifications decreased along with an extending therapeutic time window.This tendency was more significant in HSCT group.Conclusion The haptoglobin may be a potential biomarker for the prognosis in patients with leukemia treated by stem cell transplantation.

Objective: To investigate the effects of the cooperative actions of isoflavone, vitamin C and E on the expression of acute C reactive protein (CRP) and interleukin 6 (IL-6) in the patients with typeⅡdiabetic mellitus (DM). Method: Ninety six patients with DM were divided randomly into 4 groups, DM control group(B), and groups receiving low(C), medium (D) , and high (E) dose of the compound antioxidant of isoflavone, vitamin C and E respectively. Objective To determine the association between weight gain induced by atypical antipsychotic and the polymorphisms of MC4R gene rs12970134.Methods 62 patients who had weight gain more than 7% of their pre -drug body weight were selected as study group,and 62 patients who had weight gain less than 7% of their pre -drug body weight were selected as control group.The polymorphism of MC4R gene rs12970134 was analyzed by using polymerase chain reaction and directly sequencing technology.Results There were no significant differences in the frequency of MC4R gene rs12970134 genotypes and alleles between the two groups(χ2 =0.648,P =0.723;χ2 =0.679,P =0.410).While after the treatment with atypical antipsychotic,the weight gain degree in patients with GG genotypes was less than patients with GA /AA genotypes[(22.18 ±0.33)kg/m2 vs.(23.53 ±0.58)kg/m2 ](t =-2.167,P =0.032).Conclusion The polymorphisms of MC4R gene rs12970134 maybe affect the weight gain degree in patients after treatment with antipsychotic.
6.A cumulative Meta analysis of association between H .Pylori infection and pancreatic cancer

Objective To obtain a reliable estimate of the risk of H. pylori infection in causing pancreatic cancer ,by perform‐ing a M eta‐analysis of the existing observational studies evaluating the association .Methods Observational studies comparing the prevalence of H. pylori infection in patients with pancreatic cancer and healthy controls were identified through systematic search in the Medline ,EMBASE ,the Cochrane ,PubMed ,VIP database .H. pylori infection was confirmed by serological testing using an anti‐gen‐specific enzyme‐linked immunosorbent assay .Pooled adjusted odds ratios (AOR) and associated 95% confidence intervals (CI) were obtained by using a Dersimonian and Laird random‐effects model .Results Six studies involving a total of 2 335 patients met our eligibility criteria .A significant association between H. pylori seropositivity and development of pancreatic cancer (AOR=1 .38 , 95% CI:1 .08-1 .75 ;P=0 .009) was seen .No significant association had been seen on pooled analysis of the three studies assessing the relationship between CagA positivity and pancreatic cancer (AOR=1 .14 ,95% CI:0 .66-1 .97 ,P=0 .639) .Conclusion The da‐ta suggests an association between infection with H. pylori and the development of pancreatic cancer .Further research is needed to confirm our findings .

Objective To develop information system for radiotherapy. Methods The radiotherapy information the system adopts B/S structure mode,ACCESS 2010 as the database Server at the front desk, running on the hospital local network,background database is called Oncentra TPS and SQL Server 2008 in Mosaiq system, using ASP programming language network, the system is in Macromedia Dreamweaver 8 platform development. Based on the Internet information services ( IIS) 6. 0 6. 0 build system server service components,IE browser is used to implement the client access server capabilities. Results The information system including system Settings module,physics teacher module,the doctor module,technician module,data statistics and analysis, data download, seven modules such as video teaching. Conclusions The Radiotherapy information system is real?time performance, data security, stable operation, is the key construction for efficient utilization of resources in radiotherapy.
8.Correlation between cognitive disorder after acute cerebral infarction with serum uric acid by multivariate analysis

Objective To evaluate the correlation between serum uric acid with cognitive disorder after acute cere?bral infarction by prospective study. Methods Four hundred consecutively enrolled patients of acute cerebral infarction were divided into no cognitive impairment group and cognitive impairment group according to the assess of Montreal Cog?nitive Assessment (MoCA). Univariate analysises were conducted in the potential risk factors of cognitive impairment in?cluding age, sex, smoking, alcohol, hypertension, diabetes, dyslipidemia, level of education, infarction in key parts, atrial fibrillation, serum uric acid, blood homocysteine between two groups. The statistically significant indicators in univariate analysises were used as independent variables and the scores of MoCA were used as the dependent variable to conduct multiple linear regression analysis. The assessment on the risk of cognitive impairment after cerebral infarction were con?ducted according to serum uric acid, sex, age and TOAST classification further. Results Serum uric acid was indepen?dent risk factors of cognitive disorder after acute cerebral infarction. The risk of cognitive disorder after acute cerebral in?farction was significantly increased in patients with high level of serum uric acid than with normal level and the relative risk was 1.35,95%CI(1.098,1.660). Especially for the young, male or patients with cerebral infarction in classification of small artery occlusion, the risk increased further, and the relative risk was 1.513, 95%CI(1.092, 2.096)1.412, 95%CI (1.125, 1.771)and 1.464, 95%CI(1.128, 1.900)respectively. Conclusion Exaltation of Serum uric acid was indepen?dent risk factor of cognitive disorder after acute cerebral infarction. The risk of cognitive disorder after acute cerebral in?farction was significantly increased in patients with high level of serum uric acid than with normal level, and especially for the young, male and patients with cerebral infarction in classification of small artery occlusion, the risk increased fur?ther.
9.Alteration of cerebral regional homogeneity within sensorimotor network in patients with cervical spondylotic myelopathy after spinal cord decompression:a resting-state functional MRI study

Objective To assess the altered regional homogeneity (ReHo) of local intrinsic cerebral activity within sensorimotor network(SMN) in patients with cervical spondylotic myelopathy (CSM) before or after spinal cord decompression using functional MRI (fMRI). Methods Twenty-one CSM patients who would decompress spinal canal, and 21 healthy volunteers (age, gender and level of education matched) were enrolled from June 2013 to April 2014. All the patients underwent rs-fMRI examination before and 3 months after spinal cord decompression. ReHo measurement was performed statistically within a SMN mask. A second-level random-effect 2-tailed Student's t test was applied to compare the ReHo results between pre-and post-operation CSM patients and healthy volunteers. A second-level paired 2-tailed Student's t test was applied to compare the ReHo results between pre-and post-operation CSM patients. Pearson correlation analysis was performed to assess the correlations between the altered ReHo and clinical evaluation. Results Compared with healthy volunteers, pre-operation patients showed significantly lower ReHo in the left postcentral gyrus/precentral gyrus, together with enhanced ReHo in the right superior parietal lobule (GRF correction, P<0.05). Post-operation CSM patients showed significantly lower ReHo in the right superior parietal lobule comparing with healthy volunteers, as well as enhanced ReHo in the left postcentral gyrus/precentral gyrus comparing with pre-operation (GRF correction, P<0.05). Abnormal ReHo areas in CSM patients demonstrated no significant correlation with clinical measurements (P>0.05) between pre-operation and post-operation. Conclusions Myelopathy in cervical cord may affect intrinsic cerebral activity, as patients with CSM show disrupted regional homogeneity within sensorimotor network. The change of ReHo following decompression suggests that central plasticity may influence functional recovery.
10.Design and application of screw-assisted reduction of fractured vertebrae

BACKGROUND:Screw fixation of the fractured vertebral body can share the stress of implant, increase fixing strength, and help to maintain the stability of late fracture. However, whether it can assist reduction using fracture vertebral screw, but no relevant clinical reports were found at present. OBJECTIVE:To explore the effectiveness of self-designed screw-assisted reduction of fractured vertebrae. METHODS:161 cases of thoracolumbar fracture who had been hospitalized between June 2001 and June 2009 were enrol ed in this study, including 101 males and 60 females, at the age of 22-67 years, averagely 36 years. By Denis classification, 64 cases affected burst fractures and 97 flexion-compression fractures. Fracture levels involved T11 (9 cases), T12 (63 cases), L1 (74 cases) and L2 (15 cases). The self-designed method of reduction assisted with screw insertion was used. The efficacy of this reduction method was evaluated by operation time, intraoperative bleeding, fracture union time, height ratio of anterior borders of injured and normal vertebrae, sagittal Cobb’s angle, and reduction of fragments.
RESULTS AND CONCLUSION:The operation time ranged from 60 to 150 minutes, averaging 80 minutes. The intraoperative bleeding ranged from 67 to 750 mL, averaging 98 mL. The fol ow-up time ranged from 16 to 42 months in 161 patients. Fracture union time ranged from 10 to 18 weeks, averaging 12.6 weeks. The ratio of anterior heights and sagittal Cobb’s angle of fractured vertebrae were significantly improved from (54.39±9.60)%and (22.55±7.90)° respectively preoperati on to (82.80±6.63)%and (8.91°±5.85)° 12 months postoperation (P<0.05). The size of spinal canal was increased by (46.5±2.6)%postoperatively. CT scan revealed satisfactory fragment reduction and no deep infection appeared. These findings suggest that our self-designed screw-assisted reduction of the fractured vertebrae can improve the reduction quality in the treatment of thoracolumbar fractures.
