1.THE GLASS CAPILLARY GAS CHROMATOGRAPHIC METHOD FOR THE ANALYSIS OF FATTY ACIDS

A gas chromatographic method was established for the analysis of fatty acid composition of oils. After methlation of the samples, the retention time of the fatty acid esters in the glass capillary coated with crosslin-ked methyl silicone was closely related to the number of carbon atoms.The reproducibity of this method was satisfactory. The relative standard deviation was 0.4-1.5%. Six kinds of vegetable oils were analyzed by this method, and the results were consistent with those reported by Canadian Cereal Association. For analysis of the free fatty acid composition of becte-ria, the results were comparable with those obtained from thin layer chroma-tography and chemical analysis.
2.Effect of remifentanil and fentanyl on anesthetic in elderly patient's with transurethral electro-vaporization of the prostate

Objective To compare the anesthetic effects of remifentanil and fentanyl in elderly patient undergoing transurethral electro-vaporization of the prostate.Methods Eighty cases of the elderly patients (age ≥60 years) with transurethral resection of the prostate operation in the hospital were randomly divided into remifentanil group(R group,n =40) and fentanyl group (F group,n =40).Patients in R group and F group were administrated remifentanil and fentanyl respectively for induction and maintenance of anesthesia.The following indices were recorded including the changes of emodynamics (lood pressure,heart rate) in before-and after-anesthesia,postoperative recovery (spontaneous breathing recovery time,open the recovery time,extubation time) and the adverse reaction within 24 hours after operation (awareness during operation,nausea,vomiting).Results Blood pressures,heart rate of after-anesthesia in R group were lower than that in before-anesthesia and F group,the difference were significant(Blood pressures:F within group =58.306,P within group =0.000,F interactive group =142.450,p interactive group =0.000,F between group =7.417,P between group =0.010;heart rate:F within group =41.512,P within group =0.000,F interactive group =269.103,P interactive group =0.000,F between grouP =39.703,P between group =0.000).Spontaneous breathing recovery time,opening eye time and extubation time in group R were significantly shorter than that of F group (Spontaneous breathing recovery time:(7.5 ± 1.3) min vs.(10.7 ± 3.1) min,t =3.011 ; Opening eye time:(13.5 ± 3.2) min vs.(18.3 ± 5.2) min,t =2.611 ; Extubation time:(24.4 ± 5.5) min vs.(29.2 ± 7.1) min,t =2.696 ; all P < 0.05).No significant differences were found in R and F group in terms of intraoperative awareness postoperative nausea,vomiting (P > 0.05).Conclusion Remifentanil used in elderly patients with transurethral electro-vaporization of the prostate was safer and more effective than fentanyl.Meanwhile postoperative recovery in Remifentanil patient was more rapidly.Therefore Remifentanil was ideal drug during the process clinical anesthesia.
3.The application of hip joint traction device in arthroplasty for serious developmental dysplasia of the hip

Objective To evaluate the application value of hip joint traction device in treating serious developmental dysplasia of the hip. Methods From April 2006 to October 2008, 6 cases Crowe Ⅳ hip dysplasia and 1 case dysplasia of acetabulum in coxa vara underwent arthroplasty. There were 2 males and 5 females with an average age of 30 years (range, 12-45). All patients were admitted to the hospital for the distraction, which was 1.0-2.0 cm on the first day and 1 mm/d thereafter, until slowly pulling femur head to the real acetabulum. Changing to the skin distraction with 3 kg, arthroplasty was performed after pin sites healing. Results Seven femur heads of 5 cases and two greater trochanters of 1 case were pulled into the real acetabulum, average distraction length was 4.45 cm, and average time was 43 d, no nerve injury and infection of pin sites. Soft tissue release and femur shortening were not required in arthroplasty. One case accelerated pulling speed to 1.25 mm/d when left femur head was distracted to 6.0 cm, so that the partial left deep peroneal nerve injury happened after 12 h. Ending distraction, bilateral THA was undergone by stages and femur shortening 3 cm in operation, nerve injury restored after half year. All patients were followed up for average 2.5 years (range, 1.5-3.0). In all patients, two limb lengths were equal, middle gluteal muscle myodynamia improved about 1-2 grades. The mean Harris hip score improved from 56.2 to 92.2 one year after operation. Conclusion The hip joint traction device could pulled femoral head to the real acetabulum rotating center, as a result, femur shortening and soft tissue release were not required while performing arthroplasty, and through which wound and operative time were decreased, middle gluteal muscle function was improved. However, we should strictly pulled the femur head at 1 mm/d to avoid nerve injury.
4.Relationship between the serum high-sensitivity C-reactive protein level,C-reactive protein 1059G/C gene polymorphism and cerebral infarction

Objective To explore the relationship between the level of high-sensitivity C-reactive protein (hs-CRP),CRP 1059G/C gene polymorphism and cerebral infarction(CI).Methods The CRP 1059G/C genotype and allele frequencies were assayed by polymerase chain reaction-restricted fragments length polymorphism (PCR-RFLP) in 105 patients with CI and 121 controls.The level of serum hs-CRP was detected by immune-turbidimetry.The relationship between the condition of CI patients,the level of serum hs-CRP and CRP 1059G/C genotype and allele frequencies were anaysed.Results Compared to the control group,the CRP 1059G/G genotype and G allele frequencies in CI group were statistically higher,G/C+C/C genotypes and C allele frequencies were statistically lower(all P

6.Screening and identification of proteins that interact with receptor for advanced glycation end products (RAGE) via T7 select phage display system

Objective To screen and identify proteins that interact with receptor for advanced glycation end products (RAGE) with T7 select phage display system. Method GST-tagged fusion protein of RAGE was coated on glutathione suporflow resin and binding screens were performed with human lung T7 phage library. Results After 6 rounds of biopanning, 64 independent plaques were scraped and processed with EDTA. The inserted fragments in these plaques were amplified by PCR and the PCR products were purified by a gel recovery method. The sequences of the insertions were identified and analyzed with BLAST program in GeneBank. Twenty-five clones of encoding proteins were found. Conclusions T7 select phage display system is a convenient, rapid and effective method for screening binding proteins. This study provides an approach to explore complicated functions and novel mechanisms of RAGE-related signal transduction pathways.
7.The correlation of HIF-1α,MACC1 and FOXO4 in gastric carcinoma pathology and prognosis

Objective To explore significance and impact of hypoxia inducible factor 1α,metastasis associated with colon cancer 1 and transcription factor FOXO4 in gastric carcinoma and clinical pathological characteristics and prognosis .Methods 110 samples diagnosed as gastric carcinoma and 46 samples diagnosed as pericarcinomatous tissue which was 5 cm far away from cancer were collected in first affiliated hospital of Anhui medical university from December 2006 to May 2008 .Immunohistochemical stai-ning was performed in these samples to detect the expressions of HIF-1α,MACC1 and FOXO4 .And the correlation between devel-opment of gastric carcinoma and expressions of HIF-1α,MACC1 and FOXO4 was analyzed with the help of patients′messages .Ka-plan-Meier method and Cox regression were used to analyze patients′5 years survival rate and the factors .Results The expression levels of HIF-1α(P<0 .001) and MACC1(P<0 .001) in gastric carcinoma tissues were significantly higher than pericarcinomatous tissues ,however the expression levels of FOXO4(P<0 .001) was significantly decreased in gastric carcinoma tissues .Statistical a-nalysis revealed that the expressions of HIF-1αand MACC1 were apparently increased with the lower degree of cell differentiation , the greater degree of tumor infiltration ,accompanied with lymph node metastasis and higher TNM scores ,but lowered the expres-sion of FOXO4 factor .There were significant correlations between expressions of the three factors in gastric carcinoma .The expres-sions of HIF-1α,MACC1 and FOXO4 had an apparent influence on the patients′5 years survival rate .And they were independent factors on the prognosis of gastric carcinoma .Conclusion HIF-1α,M ACC1and FOXO4 are abnormal expression in gastric cancer tissue ,and the clinical pathological features of gastric cancer and the prognosis of patients with gastric cancer are closely related .
8.The distribution of Schmorl's nodes in patients with low back pain or radiculopathy and their correlation with degeneration of lumbar intervertebral discs

Objective To evaluate the distribution of Schmorl's nodes (SN) in patients with low back pain or radiculopathy,and to analyze the correlation between SN and degeneration of lumbar intervertebral discs.Methods In 2012,1024 patients with low back pain or sciatica were examined by magnetic resonance (MR) scan and plain film.There were 448 males and 576 females,with an average age of (54.3 ± 12.7) years (range,22-90).The features distribution of SN in lumbar endplate on age,sex,segment and the type and grade of intervertebral disc degeneration were analyzed retrospectively.Results Among 5120 lumbar intervertebral segments of the 1024 patients,295 (28.8%) cases and 532 (5.2%) endplates were involved with SN,302 located in the cranial and 230 in the caudal endplate.According to percent prevalence per lumbar segment,L1,2 was the most common level (29.1%),followed by L3,4 (23.7%) and L2,3 (21.1%).The incidence of SN was positively correlated with elder age,but not with sex,body weight,height,or body mass index.SN occurred more often in bulging,extrusion,Modic changes,spondylolisthesis and osteophyte comparing with normal disc or protrusion or high intensity zone.The distributions of the grade of intervertebral disc degeneration were significantly different between groups with and without adjacent SN.Intervertebral discs with adjacent SN were more degenerated than those without adjacent SN.The number,size and volume of SN were associated with the degrade of intervertebral discs degeneration.SN was divided into two types,i.e.,acute edematous SN and non-acuteSN,according to the signal type on T1-and T2-weighted MR images,and the degeneration of the corresponding intervertebral discs was more severe in the latter than the former group.Conclusion SN occurred more often in the upper lumbar spine and cranial endplate in patients with low back pain or radiculopathy.SN were correlated with elder age and the degeneration of lumbar intervertebral discs.The number,size,volume and signal type on MR images of SN impacted on its correlation with lumbar intervertebral disc degeneration.
9.An important factor for cage retropulsion after lumbar interbody fusion——the injury of the posterior part of endplate cranial-adjacent to fusion

Objective To investigate causes and treatment strategy of cage retropulsion after lumbar interbody fusion,in particular the correlations between the injury of the posterior part of endplate cranially adjacent to fusion and cage retropulsion.Methods Data of 7 patients with cage migration after lumbar interbody fusion from August 2011 to July 2014 in our hospital were retrospectively analyzed.There were 4 males and 3 females,aged from 45-74 years (average,53.6 years).All patients took X-ray and CT scans examinations 3 d postoperatively,and they were followed up at 3,6,12 months after operation.The characteristics of the patients and the injury of the endplate adjacent to fusion were analyzed to identify possible risk factors of cage retropulsion.Moreover,corresponding managements were performed and outcomes were recorded.Results All 7 cases of cage retropulsion occurred within 3 months post-operatively (rang,15-67 d).Posterior part of the endplate superiorly neighboring to fusion was found injured on the decompression side in all cases with cage retropulsion,and that led to an increase in the posterior intervertebral space height at the fusion segment.Other factors,including pear-shape type endplate,and improper operative manipulations may add the risk of cage retropulsion.These 7 patients complained mild back pain or no back pain,and received corresponding treatments including bed rest,activity limitation,and oral administration of anti-inflammatory analgesics.The clinical symptoms and degrees of cage retropulsion were not aggravated during follow-up.Conclusion The injury of the posterior margin of end-plate superiorly neighboring to fusion on the decompression side which caused increased posterior intervertebral space height may be key to postoperative cage retropulsion.Prevention of possible iatrogenic injury to the endplate may be effective to reduce the risk of cage retropulsion.
10.Online health education by family physician based on the wearable devices

Objective To explore the effect of online health education by family physician using the wearable devices, and provide the basis in theory and practice to improve the level of primary health care in community. Methods A total of 132 community residents (78 for male and 36 for female, all of them are WeChat user) with hypertension in one residential area who had regularly participated in the activity of volunteer blood pressure measurement organized by community health service center in wujiaochang street from November 2015 to February were chosen as the participants. The residents who wore the wearable device given by the community hospital uniformly were set as the study group (60 people), while the remaining 72 residents were set as the control group. Both groups received the daily online health education provided by the family physicians through their smartphone. We compared the diversity of health cognition, controlling behavior and controlling results to blood pressure of both groups before and after the studyrelatively, then we comprehensively analyzed the effect of use of wearable devices in family physicians ' online health education. SPSS 18.0 was used for data analysis. The categorical data of two groups were compared with chi-square test and the comparison of continuous data was performed with t test. Results Totally 114 community residents were qualified in the study, and the initial backgrounds of study group and control group had no significant difference. After receiving the online health education information for three months, both groups achieved improvement on the cognition and related self-control behavior of hypertension except for several items, and the study group did better than control group in most items. The physical examination showed that the systolic blood pressure of the study group after the intervention [(131.46 ± 12.89) mmHg] (1 mmHg=0.133 kPa) was lower than before [(137.24 ± 12.27) mmHg] and the difference was significant (t=9.2, P<0.01); the diastolic blood pressure of the study group after the intervention [(78.29±8.91) mmHg] was lower than that of before intervention [(80.75±10.25) mmHg] and the difference was significant (t=2.3, P<0.05). Meanwhile, after intervention the systolic blood pressure of study group [(131.46 ± 12.89) mmHg] was lower than that of control group [(133.27 ± 12.7) mmHg] and the intervention was significant (t=2.1, P<0.05). Conclusion Long-term use of wearable devices help family physicians improve the effect of health education, consequently the community hospitals are responsible to enhance the input and management of the informatization of family physicians' service, and assist them to broaden the form and content of health education.
