1.Protective effect and antioxidative mechanism of α-lipoic acid in rats with acute pancreatitis

Objective To investigate the protective effects of α-lipoic acid in rats with acute panereatitis(AP)and its potential mechanism.Methods Wistar rats were randomly divided into four groups according to random number table:sham operation(SO)group,AP group,normal saline(NS)group and α- lipoic acid group with 30 rats in each group.AP model was induced by retrograde iniection of 3.5%sodium taurocholate into the pancreatobiliary duct.Rats in α-lipoic acid group immediately received α-lipoic acid intra- peritoneal injection at the dose of 1 mg/kg.Rats in NS group received sanle amount of normal saline.The rats were sacrificed at 1,3,6,9 and 12 h after AP induction.The serunl levels of amylase.TNF-α and ICAM-1 were measured.Pancreatic histological changes were observed.The activities of pancreatic SOD and MDA were measured. Results In rats of AP group,optical microscopy showed pancreatic edema,adhesion and necrosis. The semm amylase,TNF-α,ICAM-1 and MDA levels in pancreatic tissue 6h after operalion were(2211±547)U/L,(174.8±7.9)ng/ml,(49.3±8.0)ng/ml and(32.2±5.9)U/mg prot,respectively,in AP group;which were significantly increased when compared with those of SO group(P<0.05).Pancreatic SOD activity was(38.5±9.5)U/mg prot,which was signifieandy lower than(56.7±6.7)U/mg pint of SO group (p<0.05).The serum amylase,TNF-α,ICAM-1 and MDA levels in pancreatic tissue 6 h after operation in α-lipoic acid group were(1478±642)U/L,(164.8±6.2)ng/ml,(37.5±3.9)ng/ml and(20.2 ±8.4)U/mg prot,respectively;which were significantly decreased when compared with those of AP group(P<0.05).Pancreatic SOD activity was(66.0±8.6)U/mg prot,which were significantly hisher than(38.5±9.5)U/mg prot of AP group(P<0.05).Condusiors The pathogenesis of AP wag associated with oxiddative stress,and α-lipoic acid as an antioxidant played a role in the treatment of AP.the possiblemechanismsincludedinhibitedproduction of TNF-α and ICAM-1.
2.Analysis of the clinical effect of whole boby γ-knife in treatment of non-small cell lung cancer:a report of 42cases

ObjectiveTo evaluate the effect and complication of whole γ-knife stereotactic radiotherapy on the non-small cell lung cancer and explore the value of γ-knife stereotactic radiotherapy in treatment of non - small cell lung cancer. Methods42 patients with non -small cell lung cancer:γ-knife radiotherapy patients ,4 ~ 8Gy once a day,five times a week,4 ~ 10times and 40 ~50Gy in total. ResultsThe overall response rate (CR + PR) in γ-knife was 71.4%. The1-,2-and 3-year survival rates were 95.5%, 59.5%, 33.3%. ConclusionCompared with conventional radiation,γ-knife stereotactic radiotherapy could achieve tumor dose escalation,and give the normal tissue better protection at the same time. It could improve the tumor control rate of non-small cell lung cancer,improve long survival rate,and so could reduce the radiation reaction.
3.The effect of carbon fiber couch on dose distribution of conformal intensity modulated plan

Objective To evaluate the effect of carbon fiber couch on dose distribution of radiotherapy planning and verification pass rate.Methods Establishing the carbon fiber treatment couch model in Pinnacle8.0m Treatment Planning system (TPS),and then this model was used to correct dose calculations of oblique fields in the treatment plans of 10 cases of nasopharyngeal carcinoma,10 cases of breast cancer and 10 cases of lung cancer and evaluate the effect of carbon fiber couch on the whole dose distribution of the plans.Then these plans were measured by three-dimensional dose verification equipment Delta4 to confirm the improvement extent of Gamma pass rate after considering the carbon fiber treatment couch.Results For the majority of plans,when the carbon fiber couch was taken into consideration,the target doses was significantly reduced (4772 cGy-7266 cGy vs.4859 cGy-7347 cGy,P=0.000-0.002) and the relative deviation of D95 was 1% to 3%.Measurement results of Delta4 showed that Gamma pass rate (3 mm/3% criteria) increased in all plans (96.4%-98.8% vs.93.4%-97.3%,P =0.000),some of that were up to 5 percentage when the couch model was applied.Conclusions Target doses will be overestimated if the treatment couch is ignored in TPS measurement.,However it should arouse enough attention when the disease with smaller doses corresponding gradient.
4.Study on the Quality Standard for Tiandijing Pills

Objective:To establish the quality standard for Tiandijing pills. Methods:A microscopic method was used to identify the crush herbs. TLC was performed to identify Rhizoma Polygonati and Fructus Ligustri Lucidi. HPLC was used to determine the con-tent of gastrodine. Results:The microscopy could identify earthworm and Acorus tatarinowii. The identification characteristics by TLC were distinct and highly specific. Gastrodine could be determined in HPLC. The linearity of gastrodine was good within the range of 19. 80-158. 40 μg·ml-1(r=1. 000 0). The average recovery of gastrodine was 98. 33% (RSD=0. 45%, n=6). Conclusion:The methods of identification and quantification are simple and reproducible, which can be used for the quality control of Tiandijing pills.
5.Research on classification of prescriptions in Treatment Record of Hundreds of Selected Ancient Prescriptions based on hierarchical clustering

Objective:Based on systematic idea of clustering methods, a novel approach for classifying prescriptions was explored in the light of the method of classifying analogous prescriptions.Methods:A total of 581 ancient prescriptions were selected from Treatment Record of Hundreds of Selected Ancient Prescriptions, and the standardized names of Traditional Chinese Medicines in these prescriptions were entered in Microsoft Excel 2007, then imported to SPSS 24.0 to generate dendrograms using the hierarchical clustering function. The classification of 581 selected ancient prescriptions was analyzed. Results:All 581 prescriptions could be classified into 86 categories through repeated clustering, the largest group has 29 prescriptions, the smallest group has 2 prescriptions, and the average group has about 6.75 prescriptions. In general, the later the intercepted group, the lower the similarity of its internal prescriptions.Conclusion:This method could realize the classification of prescriptions in the light of the method of classifying analogous prescriptions, which may help to break the original thinking bondage and further deepen the understanding of compatibility rules of prescriptions. However, its disadvantage lied in that the theoretical clues would be reduced when analyzing the compatibility rules of prescriptions and the issues of drug dosage, nature, taste and meridian categories were not specially considered.
6.A case report of Guillain-Barré syndrome associated with subarachnoid hemorrhagic aneurysm embolization

A case of a 47-year-old female patient who had a severe headache for 2 days was reported. Spontaneous subarachnoid hemorrhage (SAH, hunt-Hess classification Ⅱ level)-anterior communicating aneurysm rupture was diagnosed by brain CT and digital subtraction angiography (DSA) examination.Intravascular interventional aneurysm embolization was performed, and the patient appeared progressive limb weakness 5 days after operation. Physical examination revealed grade Ⅱ upper limb muscle strength and grade Ⅰ lower limb muscle strength. The muscle tone of the extremities was decreased,and the tendon reflex disappeared. The pinprick pain was decreased below the double wrist joints and the ankle joints. After neuroelectrophysiology test and cerebrospinal fluid assay, the patient was diagnosed as Guillain-Barré syndrome associated with subarachnoid hemorrhagic aneurysm embolization.
7.The therapeutic effects of Chinese formulated products combined with glucocorticoid for treatment of patients with refractory heart failure

Objective To observe the therapeutic effect of Qili Qiangxin capsule combined with glucocorticoid for treatment of patients with dilated cardiomyopathy accompanied by refractory heart failure. Methods A prospective study was conducted. Forty-eight patients with dilated cardiomyopathy and refractory heart failure in Hebei Cangzhou Central Hospital were enrolled,and they were randomly divided into three groups:control group,treatmentⅠand treatmentⅡgroups(each,16 cases). All groups were treated with conventional anti-heart failure western treatment, meanwhile additionally prednisone was given to treatment groupⅠand groupⅡ,firstly 40 mg/d,then the dosage of 5 mg was decreased in every 5 days until reaching 5 mg per day;in treatment groupⅡ,besides the same treatment of group I,the traditional Chinese medicine therapy Qili Qiangxin capsule 4 granules(one capsule 0.3 g)each time and 3 times a day was added,2 months being the therapeutic course in all the patients. The clinical efficacy and cardiac functional indexes,such as the left ventricular end-diastolic volume(LVEDV),the left ventricular end-systolic volume(LVESV),the left ventricular ejection fraction(LVEF)and the plasma B type brain natriuretic peptide (BNP),etc. were observed in 1 week and 2 months after treatment. Meanwhile the electrocardiogram(ECG),aspartate transaminase(AST),alanine aminotransferase(ALT),urea nitrogen(BUN),serum creatinine(SCr),blood routine, urine routine examination and the adverse effects were investigated. Results The total effective rates in treatment groupⅠand treatment groupⅡwere significantly higher than those in the control group〔after treatment for 1 week:81.2%(13/16),81.2%(13/16)vs. 43.8%(7/16);after 2 months:87.5%(14/16),93.7%(15/16)vs. 50.0%(8/16), all P<0.05〕. After treatment,the LVEDV,LVESV and BNP were lowered and the LVEF was increased in the three groups,and the above indexes in treatment groupⅡwere improved more significantly than those in groupⅠ〔LVEDV (mL):142.4±33.0 vs. 174.8±52.5,LVESV(mL):111.6±23.7 vs. 132.4±29.0,LVEF:0.421±0.037 vs. 0.390±0.045,BNP(μg/L):1.944±0.751 vs. 3.038±1.905,all P<0.05〕. Conclusion Qili Qiangxin capsule combined with glucocorticoid may effectively improve the cardiac function and clinical symptoms in near and forward future in patients with dilated cardiomyopathy accompanied by refractory heart failure,thus it may elevate the patients' life quality.
8.Forkhead or winged helix transcription 3 and interleukin-17 expression in colon cancer and clinical significance

Objective To observe the expression of forkhead or winged helix transcription 3 (Foxp3) in colon cancer tissue and paracancerous tissue,and detect the level of serum interleukin (IL)-17 in colon cancer patients and healthy human,to explore the changes of regulatory T cell (Treg cell) and Th17 cell in the process of occurrence and development of colon cancer.Methods The expression of Foxp3 in 56 patients with colon cancer and paracancerous tissue and 15 cases with normal colon tissue was measured by immunohistochemical SP method and the level of serum IL-17 was determined by enzyme-linked immunosorbent method.Results The level of serum IL-17 in colon cancer tissue was higher than that in normal colon tissue [(9.1 ± 2.3) ng/L vs.(6.2 ± 1.5) ng/L],and there was significant difference (P =0.007).Foxp3 positive cell number in colon cancer tissue was more than that in normal colon tissue and paracancerous tissue (24.1 ± 6.4 vs.2.7 ± 1.1 and 8.7 ± 2.3),paracancerous tissue was more than normal colon tissue,and there was significant difference (P < 0.01).The level of serum IL-17 in colon cancer tissue with TNM Ⅲ was higher than that in TNM Ⅰ-Ⅱ [(8.5 ± 2.1) ng/L vs.(5.4 ± 0.9) ng/L],Foxp3 was more than that in TNM Ⅰ-Ⅱ (25.8 ± 6.2 vs.18.2 ± 4.4),and there was significant difference (P< 0.01).There was no significant difference in the level of serum IL-17 between middle-high differentiated adenocarcinoma and low differentiated adenocarcinoma [(9.4 ± 1.1) ng/L vs.(8.9 ± 1.8) ng/L] (P > 0.05).Foxp3 in middlehigh differentiated adenocarcinoma was more than that in low differentiated adenocarcinoma (26.8 ± 5.5 vs.17.2 ± 3.2),and there was significant difference (P < 0.01).Conclusions Detection of IL-17 and Foxp3 can provide a new way of targeting therapy for colon cancer.IL-17 levels and Foxp3 expression are closely related to the immune status of local tumor tissues,the joint detection is benefitial to the further understanding of the patients with tumor immune state,provide basic information for tumor immunotherapy.
9.Investigation on development status of radiotherapy in Guangxi Zhuang autonomous region

Objective To investigate the development status of tumor radiotherapy in Guangxi Zhuang autonomous region in order to provide reference for the decision-making of the competent administrative departments for public health.Methods A questionnaire survey was conducted by the aid of on-site inspection and telephone or Email investigation among the medical institutions in Guangxi to know the current status of radiotherapy units and settings,human resources,allocation of radiotherapy facilities,and standardization of medical practices.Results Up to the end of August 2014,radiotherapy was carried out in 39 hospitals,with other 5 hospitals under preparation for construction.Among these 40 hospitals were three-dimensional conformal radiotherapy in 38,intensity modulated radiation therapy (IMRT) in 23,and image guided radiation therapy (IGRT) in 5.There were 48 linear accelerators,15 afterloading therapy apparatus,and 4 γ-knife or X-knife in Guangxi.There were 647 professional personnel (except nurses),including 322 radiation oncologists,100 radiotherapy physicists,213 radiotherapy technologists and 12 professional engineers.There were 2 000 to 2 500 hospital beds and 20 000 cancer patients had received radiotherapy in 2013.1 600 to 2 000 people received treatment on average every day.Conclusions The present radiotherapy resources in Guangxi are insufficient and the input to radiotherapy equipment is deficient,with low proportion of cancer patients treated with radiotherapy.Because of the uneven radiotherapy quality among different hospitals,the reasonable planned development and unified diagnostic and treatment practices are required to improve radiotherapy level in Guangxi.For shortage of medical staff and high-quality physical and technical personnel,the improvements are needed by fostering and introduction of personnel.Compared with developed regions in China,the introduction of new technology is largely needed.
10.Changes in drug resistance and distribution of Streptococcus pneumonia infection from 2001 to 2011 in Shenyang area

Objective To investigate the dynamic changes of infectious distribution,current resistant situation of Streptococcus pneumoniae(SP) and resistant to penicillin and other 10 kinds of antibiotics for the guidance of clinical therapy of SP.Methods Bacteria were identified by French BioMerieux API identification system,VITEK 2-COMPACT identification system and Optochin test,bile solubility test.In vitro,drug susceptibility test to penicillin,cefotaxime,and other 10 kinds of antibiotics were conducted with K-B disk diffusion method combined with the concentration gradient method.Drug susceptibility results were analyzed from 2001 to 2011.Results In recent 11 years,SP antimicrobial resistance rate had increased year after year,especially rates of penicillin-resistant and penicillin not-sensitive had increased significantly,rising from 8.3%( in 2001 ) to 72.7% ( in 2011 ) ( meningitis 86.4%,non-meningitis 59.0% ).Resistance rate of cefotaxime,belonging to third-generation cephalosporin had already accounted for 54.0% ( meningitis 64.0%,non-meningitis 44.0% ) ;and the resistance rate of macrolide antimicrobial was 100% ; vancomycin-resistant strains and linezolid-resistant strains had not been detected.Conclusion Penicillin non-susceptible SP has accounted for a high percentage,antimicrobial resistance of Streptococcus pneumoniae penicillin,cephalosporins,macrolides are in serious condition.Vancomycin and linezolid should be preferred or the right antibiotics be chosen according to vitro susceptibility results of isolates in clinical treatment of severe infections caused by SP.
