Objective To investigate the relationship between leukocyte counts and coronary artery stenosis as well as try to find the possible cause of coronary heart disease. Methods All the 220 patients underwent coronary angiography and leukocyte counts. The Gensini scoring system was used to determine the results of the coronary angiography. All patients were allocated into three groups according to the Gensini score. 74 cases with 0-3.00 score were classified into group CHD1 while 73 cases with 3.01-32.67 score were classified into group CHD2 and 73 cases with 32.68-180.00 score into group CHD3. The distribution of leukocyte counts in the three groups underwent analysis of variance and the relationship between leukocyte counts and the Gensini score was further investigated by means of multiple linear regression analysis. [WT5”HZ]Results The analysis of one way variance showed that neutrophil, monocyte and leukocyte counts were associated with the Gensini score. Multiple linear regression analysis adjusted for many risk factors for coronary heart disease showed that neutrophil counts had independent association with the Gensini score. Conclusion Neutrophil counts were independently associated with coronary artery stenosis.

Objective To study the value of B type ultrasonography guiding percutaneous radiofrequency ablation (PRFA) for hepatic cancer (HC). Methods 70 patients with primary or metastatic hepatic malignancies were treated by PRFA under ultrasound guidance through LeVeen multipolar array needle electrode and RF 2000. All patients were followed up by turnor marker, B type sounography and MRI monthly. Results PRFA were performed in 53 patients with hepatocellular carcinoma (HCC) and 17 cases with metastatic liver cancer. Liver function classfication was 53 cases in Child class A, 15 in class B and 2 in class C. In 21 patients with non-operated small HCC (≤5 cm), 12 of 17 patients with AFP positive were decreased to normal (70.6%) and decreased markedly in 4 (23.5%), and MRI showed the treated tumors were completely necrosis in 16 patients (76.2%). The 3, 6 month survival rates were 90.5%, 100% respectively in small HC groups, and 66.7% to 72.7% and 27.6% to 72.7% in large HC groups. Conclusions PRFA as a local thermal therapy is becoming a kind of new palliative treatment for hepatic malignancies, which not only is safe, minimally invasive, and effective for small ones; but also can be used with combination of TACE for larger ones.

Objective To investigate the diagnostic and prognostic values of 18 F?FDG PET/CT in pa?tients with cervical nodal metastases from carcinoma of unknown primary site (CCUP). Methods A total of 137 consecutive patients(95 males, 42 females, age range 24-84 (55.77±11.71) years) with histological proven CCUP who underwent 18 F?FDG PET/CT imaging to find primary tumors from March 2010 to June 2015 were retrospectively analyzed. The gold standard was the final pathological diagnosis or clinical long?term fol?low?up (≥6 months) results. The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PET/CT imaging in detecting primary tumors were calculated. Kaplan?Meier anal?ysis was conducted for survival analysis and to identify the presence of any prognostic factors, such as age, gender, presence or absence of distant metastasis, region of involved cervical lymph nodes, left (right)?side/both sides of lymph node involvement, histopathologic tumor type and primary site found or not by PET/CT imaging. Results Primary tumors were confirmed in 96 patients, of which 87 patients were diagnosed correct?ly by 18 F?FDG PET/CT imaging. The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 90.6% (87/96), 80.5% (33/41), 87.6% (120/137), 91.6% (87/95) and 78.6% (33/42) respectively. Primary tumors were found most commonly in the head, neck and lung. However, the false positive and negative lesions were also mainly seen in those sites. Distant metastasis and the inferior region of cervical lymph node metastasis were associated with the worst survival (χ2=20. 990 and 12?277, both P<0.01) . The ROC curve revealed that patients with SUVmax>6.5 were of higher mortality risk than those with SUVmax≤6.5 (χ2=7.120, P<0.01) . Conclusions Whole?body 18 F?FDG PET/CT scan is a significant and valuable tool in the identification of the primary tumor site in patients with CCUP . Further? more, according to the distant metastasis and region of cervical lymph nodes metastasis, 18 F?FDG PET/CT scans are of value in prognostic evaluation even if without the need to identify the primary tumors.

Objective To explore the resting state cortical activity and frontal asymmetry in alpha oscillations in bipolar depressive patients and its relationship with clinical symptoms. Methods Twelve bipolar depressive patients (pa?tient group) and twenty-four well-matched healthy volunteer (control group) were underwent whole head MEG recording. Individual spectral power and frontal asymmetry index were calculated by using permutation test to discover the differenc?es in δ, θ, α1, α2, α3, β bands between the two groups among the regions of interested (bilateral central, frontal lobe, temporal lobe, parietal lobe, occipital lobe). The correlation analysis were used to analyze the association between power of brain regions with significant difference and the Hamilton depression rating scale17 scores as well as factor items in patients. Results Compared with the control group, the activity of various regions was increased in the patient group as follows:theδband in the left central and left occipital lobes, theθband in the left occipital lobe, theβband in left cen?tral, right frontal, left parietal lobe and right parietal lobe. The power ofα2 andα3 frequency bands was decreased in the bilateral temporal lobes (P<0.05, uncorrected). A negative correlation was observed between the right temporalα3 power and recognition item scores for bipolar depression (P<0.05). Conclusion The present study suggests that bipolar depres?sive patients have impaired neural activity at many bands and the symptom of cognitive impairment may be associated with dysfunction ofα3 band.

BACKGROUND: Smoking is an important cause of cardiovascular disease, its definite mechanism in inducing cardiovascular disease is still unclear, and whether there is linear correlation between the amount of cigarette smoking and cardiovascular risk still needs to be investigated.OBJECTIVE: To analyze the association between cigarette smoking and the severity of coronary atherosclerosis.DESIGN: A retrospective investigation and comparative study.SETTING: The First Affiliated Hospital of Nanjing Medical University.PARTICIPANTS: Totally 500 consecutive patients, who underwent coronary angiography for suspected or known coronary atherosclerosis, were selected from the First Affiliated Hospital of Nanjing Medical University from April 2004 to April 2005. The inclusive criteria included patients with history of chest pain and/or ischemic changes of electrocardiography (ECG), and those with suspected or known coronary artery disease by coronary angiography. Patients with spastic angina pectoris (acetylcholine-positive) were excluded. Patients with infectious processes within 2 weeks before catheterization, heart failure (Killip Class≥ 2 after acute myocardial infarction), hepatic dysfunction, vascular disease (aortitis should be treated with prednisolone), familial hypercholesterolemia, thyroid dysfunction, or adrenal dysfunction were also excluded. There were 370 males aged 42-76years with an average of (61±11) years and 130 females aged 45-75 years with an average of (61±12) years. There were no significant differences in the sex, age and general information among all the patients. This study was approved by the ethics committee of the First Affiliated Hospital of Nanjing Medical University and informed consent was obtained from each patient.METHODS: ① According to the smoking habit, 500 patients with suspected or known coronary artery disease were divided into smoking group (n=254) and non-smoking group (n=246). ② All the patients received anthropometric measurements including body mass index (BMI), systolic blood pressure (SBP) and diastolic blood pressure (DBP). Blood samples were drawn from every patient at admission to the detect the leukocyte counts in peripheral blood, including total leukocyte count, neutrophil count, eosinophil count, lymphocyte count, monocyte count and basophil count. ③ The severity of coronary atherosclerosis was defined by the Gensini score system, based on the hypothesis that the severity of coronary artery disease should be considered as a consequence of the functional significance of the vascular narrowing and the extent of the area perfused by the involved vessel or vessels. In this scoring system, a greater reduction of the lumen diameter was assigned a higher score than a distal lesion. ④The association of smoking amount with Gensini score, anthropometric measurements and leukocyte count were studies with the Spearman correlation analysis.MAIN OUTCOME MEASURES: ① Results of BMI, SBP, DBP, leukocyte count and Gensini score; ② Results of the Spearman correlation analysis on the association of smoking amount with Gensini score, anthropometric measurements and leukocyte count.RESULTS: ① The total leukocyte count, neutrophil count, monocyte count in peripheral blood and Gensini score were significantly higher in the smoking group than in the non-smoking group.② The Spearman correlation analysis indicated that the amount of cigarette smoking was significantly associated with the total cigarettes smoked, total leukocyte count, neutrophil count, monocyte count and Gensini score (r=0.109, 0.100,0.135, 0.139, P < 0.05-0.01).CONCLUSION: Smoking is significantly associated with the severity of coronary atherosclerosis, and the inflammatory response may be part of the mechanisms underlying the association between coronary artery disease and cigarette smoking.

OBJECTIVE:
To investigate the changes of electrode impedance, THR/MCL values, and dynamic range (DR) in Combi 40+ cochlear implant after implantation.
METHOD:
A respective study was carried out collecting 20 consecutively implanted children's electrode impedances, THR/MCL values, and DR at seven time point during the first three years after implantation. Their variation and correlations were analyzed.
RESULT:
Overall, electrode impedances were lowest during the operation, and significantly rise to the highest at the first stimulation, then followed by a gradual decrease. After three months, electrode impedance of apical and medial cochlear segment were basically stable, while that of the basal segment was gradually increased. Dynamic range (DR) of apical and medial group electrode increased early after the operation and showed a stabilization from the second year, whereas that of basal group have a downward trend since the first year. However, the electric charge of each group increased significantly after three months, and then become stable after first year. Otherwise, a stronger negative rectilinear correlation was found between impedance changes with DR than with THR/MCL level.
CONCLUSION
The electrode impedances vary clue to different electrode position. Measuring the electrode impedance can effectively evaluate the working status of Combi C40+ cochlear implant. The dynamic range of the electrode was negatively correlated with the impedances, which made it possible to predict the width of the dynamic range by measuring the impedance 3 or 6 months after operation.
