Objective To explore the expression of CD14 and TLR4 ( Toll like receptor - 4) in peripheral blood monocytes during the different stage of cancer patients. Methods The expression of TLR4 and CD14 on peripheral blood monocytes was determined by flow cytometry in 50 healthy control group and 80 patients with malignant tumor who were divided into stable group and advanced group according to the change of the size of tumor measured by image and/or the outcome of tumor makers. Results No significant difference was found in the expression level of CD14 between the different stage of malignant tumor ( stable group and advanced group) and healthy controls [ (6. 1600% ? 2. 32217% ,5. 8629% ? 1. 91896% ) vs (5. 5580% ? 1. 42272% ) ]. 2.Gemcitabine single-agent in the treatment of elderly advanced cancer

Purpose:To evaluate the clinical efficacy ,clinical-benefit-response (CBR) and toxicity of gemcitabine single-agent in the treatment of elderly advanced cancer. Methods:54 elderly patients with grade Ⅲ/Ⅳ were treaded with gemcitabine 1 000 mg/m~(2) on day 1,8 and 15 in each 28-day cycle. The clinical efficacy and toxicity were estimated according to the standard of WHO and the CBR was evaluated simultaneously. Results:Of 32 advanced NSCLC patients , obtained the partial response (PR) of 21.88% .The CBR rate of 90.7% was reached in 54 patients. The toxicity was mild and well tolerated for elderly patients. Conclusions:Gemcitabine single-agent in the treatment of elderly advanced cancer is highly effective, low toxic and safe. Therefore, it could be considered as first line therapy for elderly advanced cancer.
3.Cilia Toxicity and Mucosal Irritation of Gastrodin Nasal Temperature-sensitive in situ Gel

Objective:To evaluate the nasal safety of gastrodin nasal temperature-sensitive in situ gel through the studies on cilia toxicity in toads and mucosal irritation in rats. Methods:The toads were randomly divided into four groups, saline group, gastrodin in situ gel group, blank gel matrix group and sodium deoxycholate group, and the cilia toxicity was observed in vivo by a toad palate meth-od. The rats were randomly divided into three groups, saline group, gastrodin in situ gel group and blank gel matrix group, and the mucosal irritation was studied in rats through the observation of nasal mucosal pathological changes and behavioral indices. Results:Compared with the saline group, gastrodin in situ and blank gel matrix showed no notable effect on the cilia movement function in toads, and the effect on cilia movement of sodium deoxycholate showed statistically significant difference when compared with that of sa-line, gastrodin in situ gel and blank gel matrix (P<0. 01). During and after the treatment, no sneezing appeared in the rats. Com-pared with that in the saline group, the number of scratching nose in the gastrodin in situ gel group and blank gel matrix group in-creased (P<0. 05) without difference between the groups (P>0. 05), and after the 2-day withdrawal, that in the gastrodin in situ gel group and blank gel matrix group decreased significantly when compared with that at the last administration (P<0. 05) and showed no notable difference when compared with that in the saline group (P>0. 05). The number of inflammatory cells in the nasal mucosa in the gastrodin in situ gel group and blank gel matrix group increased complicated with congestion and cilia falling off, and after the with-drawal, the mucosal morphology in the three groups showed no significant difference. Conclusion:The local application of gastrodin in situ gel has high security, which is valuable to be studied further.
4.Chromosome translocation t (11;18) and expression of BCL10 protein in gastric mucosa-associated lymphoid tissue lymphoma

Objective To detect chromosome translocation t (11;18)(q21;q21) and expression of BCL10 protein in gastric mucosa associated lymphoid tissue (MALT) lymphoma. Methods We, combining clinical and pathological data, detected API2 MLT fusion by reverse transcriptase polymerase chain reaction (RT PCR)as well as the expressions of BCL10 protein and Ki 67 by immunohistochemistry in cases of gastric MALT lymphoma and follicular gastritis (FG) and analyzed the relationships among them. Results API2 MLT fusion was detected in three cases (2 low grade and 1 low to high grade) out of 14 cases of gastric MALT lymphoma, and no in 8 cases of FG. BCL10 protein was weakly expressed in cytoplasm of B cells of germinal center in lymph follicles of FG; but abundantly in cytoplasm of tumor cells in gastric MALT lymphoma; 42.5% of the latter showed weak expression in nucleus. The expression of Ki 67 was significantly higher in low to high and diffuse large B cell lymphoma cases than in low grade cases( P 0.05), the frequency of nuclear expression of BCL10 increased with the increased expression of Ki 67. Conclusions Both API2 MLT fusion and BCL10 nuclear expression are associated with transformation of gastric MALT lymphoma from LG to HG. RT PCR technique used to detect API2 MLT fusion may be an important tool in identification of t(11;18)(q21;q21).
5.Determination of T Lymphocytes and Trace Elements in Spleen from Rats Infected with Toxoplasma gondii

Objective To determine the level of five trace elements(Fe 2+ ?Cu 2+ ?Zn 2+ ?Ca 2+ ?Mg 2+ )in the spleen and changes of T lymphocyte and its subtype variations in peripheral blood from the rats infected with Toxoplasma gondii . Methods Twenty rats were randomly and equally divided into two groups: control group and experiment group. Each rat in the experiment group received an ip injection of 2 ml normal saline containing 1.5?10 6 tachyzoites of T. gondii . On the 64th day after injection of T.gondii , the changes in T lymphocytes (TL) and their subgroups, the helper T lymphocytes (Th) and the suppressor T lymphocytes(Ts) in the peripheral blood of the rats with T.gondii were determined by the assay of the lymphocytes labeled with intercellular acid ? naphthyl acetate esterase. All the rats were killed and the atomic absorption method were used for detecting the level of trace elements in the spleen tissue. 6.One case report of systemic onset juvenile idiopathic arthritis with atlantoaxial subluxation as the initial manifesta-tion

Objective To explore the clinical features of systemic onset of juvenile idiopathic arthritis (SoJIA) with atlantoaxial subluxation as the initial manifestation. Methods The clinical data from one SoJIA patient with atlantoaxial subluxation as the initial manifestation were retrospectively analyzed. Results A 9-year-old boy presented the head and neck movement disorder as the ifrst symptom, developed fever on the third day, then rapidly progressed on 23rd day, with a sharp decline in red blood cell, platelet, and hemoglobin, blood coagulation dysfunction, and a large number of bilateral pulmonary exudation. Ultrasonography showed excess lfuid in abdomen, chest, and pericardium. According to the 2001 version of the revised International College of Rheumatology standard, the diagnosis of SoJIA combined macrophage activation syndrome was conifrmed. Then treated with hemoperfusion, ifltration, and methylprednisolone combined with cyclosporine A, the disease was in remission. Conclusion For children with spontaneous atlantoaxial subluxation accompanied by the systemic symptoms, with no obvious skeletal deformity or acute inlfammation etc., should be alerted to the consideration of systemic juvenile idiopathic arthritis.
7.Pharmacodynamics interaction between propofol and remifentanil under different harmful stimulations analyzed by response surface methodology

Objective To observe the patients hemodynamic reactions to the different stimula-tions under different blood concentrations of propofol with remifentanil.Exerting the response surface methodology,the half of the maximum effective concentration of propofol and the interaction index of propofol with remifentanil were calculated.Methods A total of 120 patients for scheduled abdominal surgery,randomly divided into 12 groups(n = 10).TCI 12 different concentration combinations of propofol with remifentanil,also the the hemodynamic reactions to harmful stimulations of intubation and incising were observed.The data were analyzed by responding surface analysis.Results The in-teraction index of propofol and remifentanil under intubation was 0.523 0±0.223 7,that under inci-sing 0.361 3±0.148 4,half of the maximum effective concentration of propofol for intutation and in-cising was (6.878 7 ± 1.832 1 ) μg/ml and (4.41 1 6 ± 0.902 5 ) μg/ml respectively. Conclusion Propofol and remifentanil have a synergistic inhibitory effect on harmful cardiovascular reaction among Chinese.During the endotracheal intubation,the requirements of half of the maximum effective concentration of propofol and intensity of the synergy with remifentanil are greater than those during the incising.
8.STUDIES ON CHARACTERISTICS OF SNAIL DISTRIBUTION IN CANAL AREA INSIDE EMBANKMENT AND FACTORS FORMING HIGH RISK AREA OF SCHISTOSOMIASIS

Zishi township of Jiangling city is located in the canal area inside embankment,a heavy endemic area of schistosomiasis japonica in Hubei province. The population and cattle preva-lence was 29.5% and 66.7% respectively in 1992. There are 66 canals and it covered a total area of 300000m2. 97.0% of the canals and 99.3% of the total area had snail inhabitants, and 7.6% of the snails distributed in canal was infected which covered 18.3% of the total area. The snails were mainly distributed in the main canal and their branches. The rate of snail distribution was 45.2% and 39.8% respectively. Cattle was pastured all year round in this area. Examination of scattered cattle feces and water revealed that the major factor of water contamination was scattered cattle feces. It explained why the infection rate of snail was high and the distribution of infected snail was extensive and how the major high rise area inside embankment was formed.
9.Effect of electroporation mediated transfecting recombinant plasmid pIRES-hVEGF165-EGFP on angiogene-sis of distraction area during early mandibular distraction osteogenesis

Objective To explore the effect of electroporation mediated gene therapy on angiogene-sis of the distraction area during early mandibular distraction osteogenesis (DO). Methods Thirty-two New-Zeland rabbits were randomly divided into 4 groups: group A: recombinant plasmid pIRES-VEGF165-EGFP and electroporation; group B: recombinant plasmid pIRES-VEGF165-EGFP; group C: normal saline (NS) and electroporation and group D: control group. The rabbits were sacrificed at 1d, 3d, 7d and 14d after injection, respectively. The distraction area tissue was removed for histological examination and electron microscopy, and immunohistochemical stain for CD34 was performed to detect the microvessel density. Results Generation of vascular endothelial cells (VEC) in the group A and group B were active, and majority of VEC in groups C and D took on early change of cataplasia and apoptosis. The immunohistochemical stain for CD34 showed that it expressed weakly at the first day after transfection, and at 3,7,14 days after transfection, CD34 of VECs in the distraction area expressed positively. CD34 expression was the strongest in group A (P<0. 05), and there was significant difference among three groups and different time, respectively.Compared to each other, CD34 of VECs expressed positively with a tendency to rise in the groups A and B. But it fluctuated at the level of the expression at the first day in the groups C and D. Conclusion Electroporation-mediated transfecting recombinant plasmid could promote angiogenesis during early stage of mandibular DO. It could promote local vascular proliferation and penetration, increase the blood flow of broken ends in fractured bone. It indicates that electroporation-mediated transfecting recombinant plasmid play an important role in regulating and promoting growth and reparative process of the bone.
10.Value of dual-phase 99Tcm-MIBI scintigraphy in the localization and diagnosis of secondary hyperparathyroidism

Objective To explore the clinical value of dual-phase 99Tcm-MIBI scintigraphy in the localization and diagnosis of secondary hyperparathyroidism (SHPT).Methods A total of 20 patients (8 males,12 females; average age 49.6 years) with uremic SHPT who underwent parathyroidectomy from 2010 to 2013 were retrospectively analyzed.All patients underwent 99Tcm-MIBI SPECT/CT and 19 underwent color Doppler ultrasonography (CDUS).Post-excisional histopathology was considered as the gold standard.The diagnostic efficacies of 99Tcm-MIBI and CDUS for SHPT were calculated.The correlation between T/NT ratio in delayed imaging and the volume of excised parathyroid and the intact PTH (iPTH) were analyzed.x2 test,Pearson or Spearson correlation analysis were used to analyze the data.Results The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of 99Tcm-MIBI SPECT/CT and CDUS in the diagnosis of SHPT were 66.67% (44/66),100%(14/14),100% (44/44),38.89%(14/ 36),72.50% (58/80) and 78.19%(43/55),52.38%(11/21),81.13%(43/53),47.83(11/23),71.05% (54/76),respectively.There were significant differences in specificity and positive predictive value (x2 =9.33,9.26,both P<0.05),but no significant differences in the sensitivity,negative predictive value and accuracy (x2 =1.97,0.04,0.46,all P>0.05).T/NT ratio correlated with serum iPTH and parathyroid volume (r=0.638,rs =0.571,both P<0.05).Conclusions The specificity of 99Tcm-MIBI SPECT/CT is superior to CDUS in the diagnosis of SHPT.Dual-phase 99Tcm-MIBI SPECT/CT could locate the hyperfunctional parathyroid gland and provide the basis for surgical treatment.
