2.Expression and clinical significance of SFRP4 in pancreatic ductal adenocarcinoma

Purpose To investigate the expression of secreted frizzled-related protein 4 (SFRP4) and to evaluate its clinical significance in pancreatic ductal adenocarcinoma (PDAC).Methods RT-PCR was performed to analyze SFRP4 mRNA expression level in 30 paired PDAC lesion and matched adjacent non-tuimor tissue.Immunohistochemistry staining detection of 205 matched cases tissue microarray was conducted to explore SFRP4 protein expression pattern.The correlation between SFRP4 and clinical characteristics was also analyzed,including overall survival.Results SFRP4 expression pattern both at mRNA and protein level in PDAC lesion was higher than that in matched adjacent non-tumor tissue.At mRNA level,to found that expression of SFRP4 was elevated in 90% (27/30) of PDAC tissues (P =0.007 2).To found that high expression of SFRP4 was detected in 56.5% (116/205) of PDAC tissue,while only 28.8% (59/205) in the adjacent non-tumor tissue.Moreover,no significant association was observed between SFRP4 expression and clinical characteristics.Kaplan-Meier survival analysis showed high level of SFRP4 expression was correlated with poor overall survival (x2 =3.467,P =0.024).Conclusion SFRP4 can be a novel prognostic biomarker in PDAC.
3.Acetylized Racemization Process and Kinetics of D-penicillamine

OBJECTIVE:To study the process of acetylized racemization of D-penicillamine in the acidic condition. METHODS:The acetylized racemic mixture of D-penicillamine was prepared by racemizing acetyl chloride in acetic acid solu_tion with D-penicillamine as feedstock.The preparation process was optimized with the quantities of solvent and acetyl chloride,the reaction temperature,the reaction time etc.as parameters.The influence of reaction temperature,reaction time on the specific optical rotation in the acetylized process was determined and the kinetic equation of acylation process was computed. RESULTS:The optimal condition for racemization was the following,the quantity ratios of D-penicillamine-acetic acid -acetyl chloride were 1∶7∶2,the reaction temperature was 80℃ and the reaction time was 5h.The kinetic equation of acylation process fitted first order linear relation. CONCLUSION:This preparation process is mild and simple,and it offers direct feedstock for the preparation of D,L-penicillamine.
4.Kinetic Migration Behaviors of Formaldehyde and Acetaldehyde from Paper Packaging Materials into Food Stimulant Tenax

The migration of two potential contaminants, formaldehyde and acetaldehyde, from two paper materials into food simulant Tenax was studied. A rapid and simple one-step extraction-derivatization method combined with ultra high performance liquid chromatography was established for the determination of formaldehyde and acetaldehyde in paper packaging materials and food stimulants. The linear correlation coefficients (R2) were greater than 0. 9999 in the range of 0. 14-35. 7 mg/m2 for both formaldehyde and 0. 20-49. 1 mg/m2 for acetaldehyde, with detection limits of 0. 03 mg/m2 for formaldehyde and 0. 04 mg/m2 for acetaldehyde. The recoveries were 90. 1%-108. 6% for paper sample and Tenax. The migration behaviors of formaldehyde and acetaldehyde at different temperatures and migration times were investigated. Both for formaldehyde and acetaldehyde, the amount of migration gradually increased with time first, then decreased, finally reached a steady value. The migration rates of formaldehyde and acetaldehyde were differently influenced by temperature. After reaching a steady value, the maximum migration rate for formaldehyde was at 30℃, and for acetaldehyde was at 70℃ and 50℃. The migration rates of formaldehyde and acetaldehyde greatly varied, and the migration rate of acetaldehyde was far more than the migration rate of formaldehyde when reaching a steady value.
5.Diagnosis of Neonatal Congenital Syphilis with Treponema pallidum IgM Western Blot

Objective To evaluate the clinical usefulness of Treponema pallidum IgM Western blot (TP-IgM-WB) test in the diagnosis of neonatal congenital syphilis. Methods Eight cases of neonatal congenital syphilis, whose mothers were diagnosed as syphilis at different pregnant stages, were tested by TRUST, TPPA, FTA-ABS-19S-IgM and TP-IgM-WB. The results and clinical manifestations were analysed. Results Three of 8 cases presented clinical manifestations of congenital syphilis, including typical cutaneous lesions, syphilitic pneumonia, and multi-system failure. The others were asymptomatic. In all 8 cases TP-IgM-WB was positive. Of them 7 cases FTA-ABS-19S-IgM was positive. Six cases (including 3 symptomatic) were positive to all four tests. One case was positive to FTA-ABS-19S-IgM and TP-IgM-WB, but negative to TRUST and TPPA. One case was negative to FTA-ABS-19S-IgM, but positive to TPPA, TP-IgM-WB and TRUST, and TRUST titer was 4 times as high as her mother. Conclusions In view of its high specificity and sensitivity, TP-IgM-WB could be used as a confirmatory test for the diagnosis of both symptomatic and asymptomatic neonatal congenital syphilis.
6.Short-term efficacy of sevelamer hydrochloride on hyperphosphatemia in patients undergoing maintenance hemodialysis

Objective To evaluate the short-term efficacy and safety of sevelamer hydrochloride in treating maintenance hemodialysis (MHD) patients with hyperphosphemia.Methods A multicenter,open-labeled,self-control study was performed.Phosphate binders were discontinued during a two-week washout period.Patients with more than 1.78 mmol/L serum phosphorus after two-week washout period were eligible for the trial.The dose was adjusted every two weeks as necessary to achieve serum phosphorus control. Sevelamer hydrochloride was administered to 138 MHD patients for 10 weeks and a second two-week washout period followed.Results A total of 111 from 138 patients fulfilled the whole 14-week study. Mean serum phosphorus and calcium-phosphate products starte to decline after two-week sevelamer hydrochloride treatment. By the end of 10-week sevelamer hydrochloride treatment, mean serum level of phosphorus [(1.85±0.50) vs (2.57±0.54) mmol/L,P<0.01],calcium-phosphate product [(4.16± 1.72) vs (5.79 ± 1.50) mmol2/L2,P<0.01 ] and low density lipoprotein [(1.64±0.76) vs (2.31 ±0.87) mmol/L,P<0.01] were significantly decreased,while the adjusted serum level of calcium and serum intact parathyroid hormone kept steady.Both serum phosphorus and calcium-phosphrus product increased after the second washout period, but the levels were still lower as compared to pre-treatment [(2.26±0.71) vs (2.57±0.54) mmol/L; (5.12±1.63) vs (5.79±1.50) mmol2/L2,P<0.01].Of the 138 patients involved,214 episodes in 106 patients and 121 episodes in 89 patients were reported as adverse events and adverse drug reaction respectively. Gastrointestinal symptoms,of which most were mild or moderate,happened to 68.1% (94/138) patients. Conclusions Sevelamer hydrochloride can control serum phosphorus and reduce the levels of calcium-phosphorus product and cholesterol.Slight gastrointestinal symptoms like constipation are common during the treatment.
7.Monitoring results of coal-burning-borne endemic fluorosis in Yunnan Province in 2020

Objective:To learn about the current situation of prevention and control of coal-burning-borne endemic fluorosis (coal burning fluorosis for short) in Yunnan Province, and comprehensively evaluate the implementation effect of prevention and control measures.Methods:From August to November 2020, according to the requirements of the Monitoring Program of Yunnan Province for Coal-burning-borne Endemic Fluorosis (2019 edition), monitoring was carried out in all natural villages in the disease areas of 13 counties (cities and districts, hereinafter referred to as counties) of the province, and 30 households were selected from each natural village to monitor the use of stoves and the formation of related healthy living behaviors. At the same time, children aged 8 - 12 years in the natural villages in the disease areas were examined for the prevalence of dental fluorosis. The evaluation for control and elimination of disease areas was carried out in accordance with the Evaluation Approach for Control and Elimination of Priority Endemic Diseases (2019 edition).Results:A total of 392 244 households were investigated, and the rates of qualified improved stoves, the correct utilization of qualified improved stoves and correct drying of corn and pepper were 99.42% (389 982/392 244), 99.70% (388 796/389 982) and 99.07% (388 579/392 244), respectively. A total of 227 057 children aged 8 - 12 years were examined, 9 413 children of dental fluorosis were detected, the detection rate of dental fluorosis was 4.15%, the dental fluorosis index was 0.10, and the prevalence intensity was negative. Taking the county as a unit, except Fuyuan and Zhenxiong, which reached the control standard of the disease area, the remaining 11 counties reached the elimination standard.Conclusion:The prevention and control measures on coal burning fluorosis are implemented well in Yunnan Province and healthy lifestyle has gradually taken shape, with the overall condition of the disease reduced.
8.Quantitative characterization of cell physiological state based on dynamical cell mechanics for drug efficacy indication

Cell mechanics is essential to cell development and function,and its dynamics evolution reflects the physiological state of cells.Here,we investigate the dynamical mechanical properties of single cells under various drug conditions,and present two mathematical approaches to quantitatively character-izing the cell physiological state.It is demonstrated that the cellular mechanical properties upon the drug action increase over time and tend to saturate,and can be mathematically characterized by a linear time-invariant dynamical model.It is shown that the transition matrices of dynamical cell systems signifi-cantly improve the classification accuracies of the cells under different drug actions.Furthermore,it is revealed that there exists a positive linear correlation between the cytoskeleton density and the cellular mechanical properties,and the physiological state of a cell in terms of its cytoskeleton density can be predicted from its mechanical properties by a linear regression model.This study builds a relationship between the cellular mechanical properties and the cellular physiological state,adding information for evaluating drug efficacy.
9.Analysis of the Marketing Approval of Pediatric Drugs in the United States and Its Enlightenment to Pedia tric Drug Use Guarantee in China

OBJECTIVE:To provide sugge stions for improving the variety of pediatric drugs and ensuring the safety of pediatric drug use in China. METHODS :The historical evolution of laws and regulations on the marketing approval of pediatric drugs in the United States and the implementation results of relevant policies were summarized. Combined with the current situation of the development of pediatric drugs in China ,some suggestions were put forward to ensure the accessibility and safety of pediatric drug use in China. RESULTS & CONCLUSIONS :Since 1994,the United States had issued a series of laws and regulations to encourage the development of pediatric drugs. At present ,the marketing approval of pediatric drugs were mainly based on the two laws of Best Pediatric Drug Act(BPCA)and Pediatric Research Equality Act (PREA). From 1998 to 2019,the amount of supplements of pediatric drug information in drug instructions of the United States showed a fluctuating growth. As of April 2020,854 kinds of drugs had been modified in pediatric instructions ,792 of which had been carried out post marketing pediatric clinical research ,and the problem of incomplete pediatric instructions had also been greatly improved. At present ,China’s policies on pediatric drugs mainly included encouraging R&D innovation ,giving priority to review and approval ,and strengthening R&D technical guidance. Although certain achievements had been made ,there were still some problems ,such as imperfect policies and regulations ,and great difficulties carrying out pediatric drug clinical trials. It is suggested that our country should draw lessons from the American regulations on pediatric drugs ,pediatric research and the catalogue of pediatric drugs ,and establish a system and catalogue of ped iatric drug use suitable for China ’s national conditions ,so as to improve the effectiveness ,safety and accessibilityof pediatric drugs.
10.T cell receptor β-chain CDR3 spectratyping and cytomegalovirus activation in allogeneic hematopoietic stem cell transplant recipients.

To explore the association between T-cell receptor beta variable (TCR BV) complementarity determining region 3 (CDR3) spectratyping and CMV activation in the recipients of allogeneic hematopoietic stem cell transplantation (HSCT).Fluorescence quantitative PCR melting curve analysis was used to sequence 24 TCR BV families in 7 HSCT recipients and 3 healthy controls. CMV-pp65 antigenemia was measured by immunohistochemical staining. Plasma IgM specific for CMV was identified using ELISA. Relationship between TCR BV families and CMV activation was statistically analyzed.Twenty-four TCR BV families were expressed in 3 healthy controls, while TCR BV CDR3 sequencing results in 7 recipients turned out to be BV9, BV11, BV17, BV20 and so on. Amino acid sequence features were as follows:TCR BV9 contained "QVRGGTDTQ", TCR BV11 contained "VATDEQ" and "LGDEQ", TCR BV17 contained "IGQGNTEA", and TCR BV20 contained "VGLAANEQ". Five recipients suffered from pp65 antigenemia in 3 month after transplantation, and pp65-positive cells ranged from 2 to 15 per 5×10white blood cells. Three recipients were CMV-IgM positive. No significant differences were found in TCR BV families between pp65-positive recipients and pp65-negative recipients (all>0.05). But there was statistically significant difference in frequency of TCR BV11 between CMV-IgM negative recipients and CMV-IgM positive recipients (<0.05).T cell immune response was characterized by special TCR BV CDR3 spectratyping in HSCT recipients, and TCR BV11 expression may be associated with CMV activation.
