This paper reviewed the clinical application of video-assisted mediastinoscopy(VAMS),Including:(1)The methods of VAMS;(2)The diagnosis of thoracic disease by VAMS,such as stages of lung cancer,the diagnosis of occupying lesion in anterior or superior mediastinum;and the therapy which includes of resection of occupying lesion in mediastinum and esophageal carcinoma;primary palmar hyperhidrosis;malig-pleural effusion,drainage of mediastinat abscess;(3)The incidence of complication is low.
2.Analysis on efficacy of zoledronic acid combined with taxotere in treatment of breast cancer with bone metastases

Objective To investigate the efficacy and safety of zoledronic acid combined with taxotere in treatment of breast cancer patients with multiple bone metastases and negative hormone receptors.Methods Thirty-six breast cancer patients with multiple bone metastases were randomized devided into two groups: Group A(zoledronic acid plus taxotere) and Group B(zoledronic acid alone).The effect in the two groups was compared.Results Relief of bone pain,improvement in quality of life,and toxic side effects between the two groups showed no significant difference.The effect on bone metastases was 66.7%(12/18)in group A,which was significantly higher than that in group B(11.1%,2/18)(P

4.Multilocus sequence typing and genetic diversity ofCandida albicans in patients with vulvovaginal candidiasis

Objective To investigate the molecular epidemiology of 114C. albicans strains isolated from the vaginal discharge of female patients treated in three obstetrics and gynecology hospitals in Shanghai by analyzing the relationship between the main genotypes and resistance proifle, and the relationship between genetic diversity and cluster ofC. albicans.Methods A total of 114 strains ofC. albicans were collected from the Obstetrics & Gynecology Hospital of Fudan University, Shanghai First Maternity and Infant Hospital Corporation and the International Peace Maternity & Child Health Hospital of China welfare institute. Phylogenetic analysis of strains were carried out by eBURST.C. albicans strains were also analyzed by multilocus sequence typing (MLST). The susceptibility of theC. albicans strains was tested by ATB FUNGUS 3.Results A total of 47 diploid strain types (DSTs) were identiifed from the 114 strains, 30 of which were known types. DST 79 and DST 435 were the main types. Of the 114C. albicans strains, 96.5% were susceptible to lfucytosine, 100% to amphotericin B, 85.1% to lfuconazole, 55.2% to itraconazole and 84.3% to voriconazole.Conclusions The pathogenicC. albicans strains isolated from different obstetrics and gynecology hospitals in Shanghai were originated from multiple clones, the main type of which was DST 79 and DST 435 with certain degree of antifungal resistance. MLST typing suggests that genetic diversity is present in theC. albicans strains isolated in Shanghai area. The clustering analysis ofC. albicans strains is consistent with its genotypes.
5.Relationship between gastric schistosomiasis and gastric cancer

ObjectiveTo investigate the relationship between gastrc schistosomiasis and gastrc cancer.MethodsThe data of 11 cases of gastrc schistosomiasis were retrospective analyzed.Results 54.5% (6/11) of gastrc schistosomiasis cases complicated with gastrc cancer,and 83.3% (5/6) was well-differentiated and moderately differentiated.Seen around the parasite eggs were obvious intestinal metaplasia,glandhyperplasia,abnormalepitheliallesionsandother seriousatypicalhyperplasia.ConclusionsGastric schistosomiasis significantly increases the risk of gastrc cancer.Gnstrc schistosomiasis does not have specific symptoms,and it is clinically sumilar to chronic gastrtis.Therefore,patients with chronic gastrtis-likesymptoms in the schistosomiasis endemic areas should be subjected to an intensive test for gastrc schistosomiasis in the biopsy specimens collected from pyloric antrum and gastric lesser curvature.Patients infected by gastric schistosomiasis are strongly suggested to go regularly for gastric cancer check with gastroscopy.
6.Health risk warning system and its follow up results

Objective To explore the reporting process of our health risk warning system and its long-term effectiveness.Methods A total of 1 168 individuals who were identified as higher risk populations from July 2011 to June 2012 in our Health Mangement Center were served as the control group,and another 973 adults who were identified as higher risk individuals from July 2012 to May 2013 were assigned to the study group.Diagnosis and treatment were based on our health risk reporting process and follow-up system.Paired t test and paired contingency table x2 test were used for data analysis.Results The rate of follow-up (97% vs 100%,x2=30.503,P<0.05),consultation (83.48% vs 93.63%,x2=52.142,P< 0.05),mean treatment time ((3.0±0.5) vs (1.5±0.5) d,t=69.12,P<0.05) and patient satisfaction (87.84% vs 96.20%,x2=48.361,P<0.05) were significantly different between the two groups,although no statistically significant difference of incidence of adverse events was found (0.26% vs 0.10%,x2 =0.102,P>0.05).Conclusion Our health risk warning system may reduce the time for physical examination and improve disease diagnosis level and patients' satisfaction.Moreover,our health manatement system and health service quality should further be improved in practice.
7.Detection of plasma interleukin-6, C-reactive protein, D-dimer and fibrinogen among 96 patients with pneumoconiosis

Objective:
To investigate the plasma levels of interleukin-6 (IL-6), C-reactive protein (CRP), D-dimer (D-D) and fibrinogen (Fib) among patients with pneumoconiosis, so as to provide insights into the prevention of thrombosis among patients with pneumoconiosis.
Methods:
Ninety-six male coal workers with stable-stage pneumoconiosis admitted to China Pingmei Shenma Group Occupational Disease Prevention and Control Hospital from February 2019 to February 2021 were included in the pneumoconiosis group, and 43 male healthy volunteers in the hospital during the same period were selected as the control group. The plasma D-D, Fib, IL-6 and CRP levels were detected from subjects in the two groups. The associations of plasma D-D and Fib levels with IL-6 and CRP levels were examined using Pearson correlation analysis among pneumoconiosis patients.
Results:
Participants in the pneumoconiosis group and the control group had a mean age of (52.91±3.89) and (52.64±4.12) years, D-D of (1.28±0.91) and (0.44±0.11) mg/L, Fib of (4.41±0.98) and (2.88±0.61) g/L, IL-6 of (0.63±0.19) and (0.42±0.06) ng/L and CRP of (3.30±1.65) and (1.35±0.12) mg/L, respectively. Higher plasma D-D, Fib, IL-6 and CRP levels were detected in the pneumoconiosis group than in the control group (all P<0.05). The plasma D-D level correlated positively with IL-6 level among pneumoconiosis patients (r=0.347, P<0.001).
Conclusion
High plasma IL-6, CRP, D-D and Fib levels are detected among patients with pneumoconiosis, and the plasma D-D level correlates positively with IL-6 level among patients with pneumoconiosis.
8.The screen of serum microRNA in Ⅳand Ⅳ+Ⅴ lupus nephritis patients

Objective To preliminary screen serum microRNA which was related to class Ⅳ and class Ⅳ+Ⅴ lupus nephritis (LN)by using microRNA array technology,and explore the role of serum microRNA in renal pathological classification of lupus nephritis.Methods Eight serum samples of lupus nephritis patients from biological specimen library in our hospital were enrolled. Experimental group was composed of 4 class Ⅳ cases and 4 class Ⅳ+Ⅴ cases,control group was composed of 4 healthy persons. MicroRNA array was used to screen the differentially expressing microRNA.Results There were part of same miRNA in class Ⅳand classⅣ+Ⅴ LN,among which expression of 22 miRNA were significantly different,29 miRNA of classⅣ was unique,3 miRNA of class Ⅳ+Ⅴ was unique.Conclusion Serum miRNA has a potential to serve as a biomarker to identify class Ⅳ and classⅣ+Ⅴlupus nephritis.
9.Chronic kidney disease in 5 708 people receiving physical examination

Objective: To investigate chronic kidney disease (CKD) and its risk factors in people receiving physical examination. Methods: hTis retrospective study included people over 20 years old who had physical examination in the Health Management Center of Third Xiangya Hospital from Janurary 2008 to June 2011. CKD and its risk factors as well as questionnaire were recorded. hTe risk factors were analyzed by multivariate logistic analysis. CKD was deifned by kidney damage (microalbuminuria≥30 mg/L) and/or hematuria and/or reduced kidney function [evaluate glomerular ifltration rate (eGFR)<60mL/(min.1.73 m2)]. We counted eGFR according to the modiifcation of diet in renal disease (MDRD). Results: A total of 5 708 physical examination reports were included. The detection rate of albuminuria, reduced renal function and hematuria was 25.0%, 1.7% and 1.1%. hTe detection rate of CKD was 25.6%, and detection rate of CKD stage 1-5 was 17.8%, 6.7%, 1.1%, 0 and 0, respectively. Multivariate logistic analysis indicated that diabetes mellitus, hypertension, hypercholesterolemia, male, age, and smoking were the risk factors for CKD. Increasing physical activity was the protective factor against CKD. Conclusion: High prevalence of CKD in people receiving physical examination is found in Changsha, especially stage 1 and 2 CKD. Physical examination is important to screen CKD. Stopping smoking, control of blood glucose, blood pressure, blood lipids and increasing physical activity may help reduce the prevalence of CKD.
10.Efficacy of different doses of sufentanil in minimum alveolar concentration of sevoflurane in patients undergoing bronchoscopy

Objective To investigate the effects of different doses of sufentanil on the minimum alveolar concentration (MAC) of sevoflurane for sedation in patients undergoing bronchoscopy. Methods ASA physical status I orⅡpatients of both genders, aged 20 ~ 65, undergoing bronchoscopy under general anesthesia,were randomly divided into 4 groups (n=20 each):control group (group C) and different doses of sufentanil groups (Sl, S2 and S3 groups). Sufentanil 0.1, 0.2 and 0.3 μg/kg in 5 mL of normal saline was intravenously infused before induction of anesthesia in groups of SI S2 and S3 respectively. While 5 mL of the normal saline was given instead in the group C The patients were mechanically ventilated after insert laryngeal mask. Anesthesia was maintained with inhalation of sevoflurane. Each time the concentration of sevoflurane at end expiration increased/decreased in the next patient depending on the concentration of sevoflurane at end expiration with which the former had no cough. The ratio between the two consecutive concentrations was 1.1. The middle point between the positive response and negative response served as a crossover pair. After at least 7 independent crossover pairs were observed in each group. The MAC and 95%confidence interval of sevoflurane were calculated. The time of anesthesia induction and analepsis was recorded. Results The MAC (95%CI) of sevoflurane was 3.0%(2.8%~3.3%), 2.3%(2.1%~2.5%), 1.9%(1.6% ~ 2.2%) and 1.6% (1.3% ~ 1.9%) in groups of C, S1, S2 and S3 respectively. The MAC of sevoflurane was significantly lower in groups of S1, S2, S3 than in the group C, and in groups S3 than in the group S1 (P<0.05). The time of anesthesia induction was significantly shorter in groups of S2, S3 than in the group C and significantly longer in groups S3 than in the group C. Conclusion Sufentanil of 0.1, 0.2, 0.3 μg/kg can significantly decrease the MAC of sevoflurane in patients undergoing bronchoscopy in a dose-dependent manner.
