1.Determination of geniposide in Diaojinyao powder by HPLC

Objective To establish a high performance liquid chromatography (HPLC) method for the determination of geniposide in Diaojinyao powder. Methods The HPLC system consisted of the Fortis-C18 (4.6 mm × 250 mm, 5μm) column, the mobile phase consisted of HCN∶H2O (12∶88) , the flow rate was 1.0 ml/min, the column temperature was 30℃, The UV detector was set at 238 nm. Results The linear response range was 0.049~0.980mg/ml ( r=0.999 9). The average recovery of geniposide was 99.1%, and RSD was 1.66%. Conclusion The method is simple, rapid, accurate and repeatable. It can be applied in determination of Geniposide in Diaojinyao Powder.
2.Experience of vagina reconstruction with axial pattern skin flap

Objective To summarize the experience of vagina reconstruction with six kinds of axial pattern skin flaps Methods Six kinds of axial pattern skin flaps have been used for vagina reconstruction in 126 cases,including of hypogastric skin flap,thoraco umblical skin flap,labia minora skin flap,scrotal skin flap,penis skin flap and pudendal femoral skin flap Results All flaps were survived,but one case of necrosis occurred in hypogastric skin flap Conclusion The pudendal femoral skin flap is the first choice of flap for vagina reconstruction The advantages of this method are (1) the reconstructed vagina has good sensation and flexibility;(2)the linear scars of donor site are hidden and inconspicuous;(3)the flap is close to recipient site the procedure is simple
3.Determination of hesperidine inErchen mixture by HPLC

Objective To establish a method for the determination of hesperidine inErchen mixture. Methods HPLC method was used with Fortis C18(250 mm×4.6 mm,5μm)column and the mobile phase consisted of acetonitrile-acetic acid-water(22∶4∶74). The flow rate was 1.0 ml/min,the column temperature was 30℃, and the UV detector was set at 283 nm.Results The linear ranges of hesperidine was 2.86-57.20μg(r=0.999 8). The average recovery of Aloin was 99.4%.Conclusion The method is rapid, reliable, simple, repeatable and with high specificity. It can be an important supplement for Erchen mixture elevated standard.

As the research work went further and more detailed, a variety of new treatments compete to come out.However, it remains unclear that how the antigen works to distinguish cancer cells and normal cells.Neoantigen, which is located in the tumor cell surface of a specific antigen, its presence makes human immunotherapy into new areas which may make personalized treatment possible in the near future.Emerging data suggest that the identification of such newantigens is a major factor in clinical immunotherapy.They can form a biomarker in cancer immunotherapy to provide targets for a variety of therapeutic approaches to attack, which allows T cells to selectively enhance the immune response against this class of antigens.
5.Clinic significance of neutrophil-iymphocyte ratio in the early-stage diabetic nephropathy

Objective To investigate neutrophil lymphocyte ratio (NLR) in early-stage diabetic nephropathy and its clinic significance. Methods The 145 subjects were divided into two groups:the healthy control group (n=54) and early stage diabetic nephropathy group (n=91). The numbers of neutrophils (N) and lymphocytes (L) as well as the NLR values of peripheral blood and other biochemistry index were examined. Factors of early stage diabetic nephropathy were calculated us?ing variance and logistic regression analysis. Results Creatinine(Cr), total cholesterol(TC), triglyceride(TG), LDL-C, neu?trophils number and CRP in DN group were significantly higher than those of the control group and lymphocytes numbers of DN group were significantly lower than that of the control group (P<0.05 respectively);NLR values were significantly higher in diabetic nephropathy group compared with those of healthy control group(2.52±0.57 vs 1.82±0.60,t=6.997, P<0.01). Lo?gistic regression analysis showed that the risk factors of DN include NLR, TG and total cholesterol. NLR ( P <0.001, OR=8.951, OR 95%CI:3.595-22.287) was significantly associated with DN. Conclusion High NLR values may be a predic?tive and reliable marker ofearly-stage DN.
6.The influence of EGR-1 on NF-κB in HTLV-1 Tax protein positive cells

Objective To research the relation of early growth response gene-1(EGR-1) and NF-κB in human T-cell leukemia virus type 1(HTLV-1) Tax protein positive cells. Methods RT-PCR was used to amplify the aimed segments EGR-1 cDNA which was then inserted into an eukaryotic expression plasmid pcDNA3.0 to construct pcDNA3.0-EGR-1. The constructed plasmid was transfected into TaxN and TaxP cells by Tfx-50-mediated transfer method, the expression levels of EGR-1, p65 and Tax mRNA in transfected cells were assay by RT-PCR after 48 h post-transfection, the proteins of EGR-1 and p65 were detected by Western blot after 48 h post-transfection too. The constructed plasmid and pNF-κB-luc reporter gene plasmid was co-transfected into TaxN and TaxP cells by Tfx-50-mediated transfer method, and the activity of luciferase was assay after 48 h post-transfection. Results The results showed that the eukaryotic expression plasmid pcDNA3.0-EGR-1 was successfully constructed. The mRNA and protein expression of EGR-1 could be promoted significantly by Tax. EGR-1 can promote the mRNA and protein expressions of p65 in TaxP cells, the activity of NF-κB was up-regulated by EGR-1 too. Conclusion EGR-1 maybe involve in adult T-cell leukemia(ATL) by increasing the activation of NF-κB.
7.Incision choice of sentinel lymph node biopsy in breast cancer

Objective To study the clinical function of different incision in sentinel lymph node(SLN)biopsy in breast cancer.Methods 40 patients were early-stage breast cancer who were performed breast-conserving surgery for SLN biopsy with different incisions were retrospectively analyzed during November 2003 to November 2007.Results (1)Transversely incision(cleavage lines)were 35 cases;(2)arc incision were 3 cases;(3)one incision were 2 cases.Among the 40 cases,there are 30 cases which were performed breast-conserving surgery because the SLN and tumor-margin were negative with the quick frozen section during the operation,the rest of the SLN who had the contains cancer had to be performed mastectomy surgery.Conclusion When breast cancer conserving surgery are performed,transversely incision is the best incision for SLN biopsy.
8.Early gastroscopy after closure of gastroduodenal perforation by conservative treatment

Objective To evaluate early diagnosis by gastroscopy for the causes of gastroduodenal perforation after successful non-surgical treatment.Method Gastroduodenal perforation patients suspected of benign ulcer in origin on hospital admission undergoing successful non-surgical treatment were examined by gastroscopy within days after the closure.Results Among 284 patients undergoing gastroscopy,277 cases (97.5%) were diagnosed as gastroduodenal ulcer,3 cases (1.1%) as gastric carcinoma,1 case (0.4%) of duodenal diverticulum.There were not major complications developing after gastroscopy.Conclusion Early gastroscopy performed after closure by non-surgical treatment in gastroduodenal perforation patients is safe,helping make definite diagnosis for the cause of perforation.
9.Reasons for anastomotic leakage following the learning curve by laparoscopic anterior resection of rectal cancer

Objective:To investigate the reasons of anastomotic leakage following learning curve by laparoscopic anterior resection of rectal cancer.Methods:From December,2011 to March,2015,the clinical information of 179 patients in our hospital who underwent dixon of rectal cancer were collected.The patients were divided into a laparoscopic learning group,a laparotomy group and a laparoscopic group,The reasons of anastomotic leakage for each group were comparatively analyzed.Repeated cutting of anastomotic stoma was compared between the laparoscopic learning group and the laparoscopic group.The male,age,obesity,nutrition complications and the position of anastomotic stoma were compared among the 3 groups.Results:The rate of anastomotic leakage in the laparoscopic learning group was significantly higher than that in the laparotomy group and the laparoscopic group (P<0.05).Repeated cutting was a significant risk factor in the laparoscopic learning group (P<0.05),but not in the laparoscopic group.Except obesity,the four factors were significant risk factors in the laparoscopic learning group (P<0.05).All of the five factors were not the significant risk factors in the laparotomy group and the laparoscopic group (P>0.05).Conclusion:The operation technical shortcoming is the major factor in the learning of the laparoscopic anterior resection of rectal cancer.In order to reduce the rate of anastomotic leakage in the learning curve period,the selection of patients following the laparoscopic anterior resection of rectal cancer should avoid the following factors:male,older age,the low position of the tumor and the nutrition complications.
10.The relationships of hyperuricemia with clinical and Oxford classiifcations in children with IgA nephropathy

Objective To investigate the relationships of hyperuricemia with clinical and Oxford classifications in children with IgA nephropathy.Methods Clinical data of 213 children with IgA nephropathy were retrospective analyzed. According to the levels of blood uric acid, these children were divided into two groups, hyperuricemia group (n?=?51) and normal uric acid group (n?=?162). Differences of clinical index and Oxford classiifcation between the two groups were observed. Results The 24 h urinary protein, level of serum creatinine, glomerular ifltration rate (eGFR) in hyperuricemia group were signiifcantly higher than those in normal group (P all?0.05). Oxford classiifcation showed that the degree of mesangial cell proliferation (M1) and renal tubular atrophy/interstitial ifbrosis (T1-2) in hyperuricemia group were more serious than in normal uric acid group (P all?0.01).Conclusions Children with IgA nephropathy combined with hyperuricemia were worse in clinical index and pathological manifestations. Increased serum uric acid may be one of the important factors that contribute to poor prognosis of children with IgA nephropathy.
