1.Literature review of domestic and international research on patient satisfaction

This paper elaborates on the connotation and development path of patient satisfaction theory, organi-zes and concisely reviews the status quo and progress of domestic and international research on patient satisfaction, and clarifies the three aspects of research on patient satisfaction:research into instruments of measurement, research into evaluation models, and research into analytical methodology. The problems and deficiency of existing research are also analyzed, specifically: first, the long-term reliance on the customer paradigm in the field of marketing re-search;and secondly, the lack of a unified, standardized patient satisfaction assessment system. Based on introspec-tion, some suggestions are put forward:first, the next step should be to focus on the innovation of patient satisfaction analysis theories and methods using comprehensive multidisciplinary perspectives;and secondly, methods for researc-hing how patient satisfaction data acquisition channels affect the results should be actively explored and an optimal method should be sought.
2.An initial analysis on the status quo of applying diagnosis test in imaging scientific study in China

Objective To analyze the status quo of applying diagnosis test in imaging scientific study in China, and to see whether reliable diagnosis can be provided to clinicians for accurate guidance of clinical management. Method We manually looked up diagnosis tests published in 1979~1999 "Chinese Journal of Radiology" that is quite influential in our country. Then we evaluated each of these diagnosis tests according to the International Standards. Results We totally searched 2 888 articles in which only 9% were diagnosis tests. Of these diagnosis tests, only 8.9% applied blind comparison with Gold Standard, 24% calculated sensitivity, specificity and accuracy in the same time. And only 0.5%?6% and 5.7% respectively calculated likelihood ratios, negative predictive value and positive predictive value. Conclusion In China, the quality and quantity of the diagnosis tests applied in the specialty of imaging scientific study is far from meeting the clinical demand. Improving methods of scientific study and carrying on more diagnosis tests of high quality are of important significance to improving the diagnostic level of imaging.
3.Sodium pyruvate protect occipital cortex of rats with repetitive and severe neonatal hypoglycemia detected by high resolution 1H nuclear magnetic resonance spectroscopy

Objectives To investigate the occipital cortex metabolite alterations in repetitive and severe neonatal hypoglycemia rats treated with sodium pyruvate and to reveal the protective role of sodium pyruvate using high resolution 1H nuclear magnetic resonance spectroscopy.Methods Thirty-six 2-dayold Sprague-Dawley rats were randomly divided into hypoglycemia group and pyruvate group with 18 rats in each group.Rats in both groups received intraperitoneal injections of insulin (40 U/kg body weight) at 2,4 and 6 days of age to induce severe hypoglycemia (blood glucose value ≤ 1.4 mmol/L).In the hypoglycemia group,2.5 hours after insulin injection,intraperitoneal injection of 50% glucose (2 ml/kg) was administered to terminate hypoglycemia,while in the pyruvate group,50% glucose (2 ml/kg) and sodium pyruvate solution 2.5 ml/kg (500 mg/kg) were injected.Terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling assay was used to observe the status of injured neurons in six neonatal rats,and metabolite changes in occipital cortex of the other 12 rats were detected by 1H nuclear magnetic resonance spectroscopy.The difference between the two groups was compared by independent-samples t test.Results Neonatal rats of both groups reached severe hypoglycemia level 2.5 hours after insulin injection.Compared with hypoglycemia group,pyruvate group had fewer injured neurons (45±5 vs 113 ± 12,t=0.782,P=0.013) and lower injured index in the occipital cortex (0.15 ± 0.03 vs 0.36 ± 0.06,t=l.143,P=0.020).Pyruvate group showed significant decreases in the concentration of taurine [(13.31 ± 2.06) vs (18.44 ± 3.86) mol/kg,t=8.231],glutamine[(1.50 ± 0.24) vs (2.02 ± 0.40) mol/kg,t=3.137],glutamate[(7.04 ± 0.95) vs (9.40 ± 1.73) mol/kg,t=6.449],aspartate[(1.51 ± 0.28) vs (2.15 ± 0.58) mol/kg,t=2.561] and creatine [(6.37±0.99) vs (8.46± 1.77) mol/kg,t =4.226] in the occipital cortex (all P'<0.017).Conclusions Simultaneous use of glucose and sodium pyruvate to terminate hypoglycemia in repetitive and severe neonatal hypoglycemia rats can effectively alleviate severe hypoglycemia-induced occipital lobe damage via regulating excitatory amino acid neurotransmitters,energy metabolism and other metabolic pathways.
4.Exogenous hydrogen sulfide inhibits expression of tissue factor induced by ox-LDL through reducing generation of ROS and inhibiting NF-κB activation in endothelial cells

Aim To investigate the mechanism for the inhibitory effect of hydrogen sulfide on the expression of tissue factor(TF)induced by oxidative low-density lipoprotein(ox-LDL)in endothelial cells.Methods Human umbilical vein endothelial cells (HUVECs ) were treated with 50 mg·L-1 ox-LDL in the absence or presence of different concentrations of NaHS (25 , 50,100 and 200 μmol·L-1 )for 24 h.The mRNA expression and protein content of TF in HUVECs were determined by reverse transcription PCR and ELISA, respectively.The content of intracellular reactive oxy-gen species (ROS)was determined by DCFH,an oxi-dative sensitive fluorescent indicator.The activation of nuclear factor-kappaB (NF-κB)was estimated by its expression in nuclear extracts analyzed by Western blot.Results Ox-LDL induced TF mRNA expression and increased TF protein content in HUVECs.The in-crease in intracellular ROS production and the activa-tion of NF-κB were observed in HUVECs treated with ox-LDL.However,NaHS could markedly inhibit the increases in TF mRNA and protein levels induced by ox-LDL.Also the elevation of intracellular ROS pro-duction and the activation of NF-κB elicited by ox-LDL were significantly suppressed by pretreatment with NaHS.In addition,pretreatment with BAY 1 1-7082 (10 μmol·L-1 ),the inhibitor of NF-κB or N-acetyl-L-cysteine(1 mmol·L-1 ),an antioxidant,could also decrease the TF mRNA and protein level as well as ROS production and NF-κB activation induced by ox-LDL in HUVECs,similar to the effects of 200 μmol· L-1 NaHS.Conclusion The mechanism for the in-hibitory effect of H2 S on the ox-LDL- induced TF ex-pression in endothelial cells may be related to inhibi-ting intracellular ROS production and subsequently NF-κB activation.
5.MRI Features in Children with Cerebral Palsy Induced by Chronic Kernicterus

Objective To study the characteristic findings on magnetic resonance imaging (MRI) in children with cerebral palsy (CP) induced by chronic kernicterus.Methods The MR imagings in 13 CP cases induced by chronic kernicterus were retrospectively studied, which were compared with 6 healthy and age-matched controls. 13 cases were finally diagnosed as dyskinetic cerebral palsy (athetoid subtype).Results In 13 cases, all showed symmetric high signals in the the globus pallidus on T2-weighted imaging. There were not apparent abnormality on T1-weighted imaging.Conclusion The bilateral high intensity signals in the globus pallidus on T2-weighted imaging are the characteristic findings on MRI in CP children induced by chronic kernicterus. Paying attention to the globus pallidus on MRI may be useful in determining the type of cerebral palsy.
6.The aesthetic analysis of midface ratio after folding aponeurosis of levator palpebrae superioris muscle in the ptosis correction

Objective To measure the ratio of the length between upper edge of eyebrow and edge of lower eyelid(L1)/the length between edge of lower eyelid and nasal base(L2)in the fordless-eyelid and upper lid ptosis patients who received levator aponeurosis folding surgery. To analyze the change in the proportion of midfa-cial aesthetics. Methods From December 2015 to October 2016,a total of 21 patients were recruited as study group. Those patientswere diagnosed with foldless eyelid and upper lid ptosis. They received levator aponeurosis folding procedures were carried out in our departmentand the pre- and post-operative photographs were collected. Full-face pictures of 20 Chinese female movie stars were prepared as control group. The ratiosof L1/L2(J)were measured via Adobe Photoshop CS6. The statistical significance was analyzed and the change of midfacial propor-tion was evaluated. Results In study group,the mean value of pre- and post-operative L1/L2(J1)is 0.746 and (J2)0.657 postoperatively. In control group,the mean value of L1/L2(J3)is 0.667. Statistical differences showed between the pre- and post-operative samples and between pre-operative samples and control group. There was no statistical difference between post-operative and control group samples. The ratios between L1 and L2 in post-opera-tive samples are close to those in control group ,which are closer to the golden radio. Conclusions Levator aponeurosis folding can effectively lower eyebrow in patients with upper lid ptosis and shorten the length between eyebrow and lower eyelid. Therefore harvesting a more appropriate and harmonious ratio of L1/L2 according the golden section theory. The changes in the proportion of midfacial aesthetics after operation could be used as a key point in the pre-operative communication and post-operative effect evaluation ,besides the size of palpebral fissure and corneal exposure rate.

Objective:
To compare the treatment effect of surgical clipping and endovascular coil embolization for aneurysmal subarachnoid hemorrhage (aSAH) in the elderly.
Methods:
Elderly patients with aSAH (aged >65 years) treated in Shanghai Pudong Hospital from January 2009 to December 2017 were enrolled retrospectively. They were divided into craniotomy clipping group and endovascular intervention group according to the treatment strategy. The Glasgow Outcome Scale was used for short-term outcome assessment at discharge, 4-5 were defined as good outcome, and 1-3 were defined as poor outcome. Long-term follow-up was performed to assess clinical outcomes using the modified Rankin Scale, 0-2 was defined as good outcome and 3-6 were defined as poor outcome. This paper is to establish a three-dimensional finite element model (3D-FEM) of pelvic floor levator ani muscles in an old healthy women. We acquired the image data of the pelvic bones and pelvic floor muscles from CT and MRI scanning in a non-pregnant old healthy female volunteers. The 3-D reconstruction and mesh optimization of the whole pelvic bones and muscles with application of image processing software Mimics12.0 and Geomagic9.0 were obtained. Then we built the 3D-FEM of the musculoskeletal system of the pelvic bones and levator ani muscles with Ansys11.0 software. We obtained an accurate 3D-FEM of pelvic bones and levator ani muscles in the older healthy woman. The results showed that it was reliable to build 3D-FEM with CT and MRI scanning data and this model could vividly reflect the huge space anatomy of the real pelvic floor levator ani muscles. It avoids the defects to gain the model from the body of anatomical specimens in the past. The image data of model are closer to vivisection, and the model is more conducive to the latter finite element analysis.

9.Evaluation of the high-quality development of tertiary public hospitals in C city based on entropy weight TOPSIS combined with RSR method

Objective:To comprehensively evaluate the high-quality development of some tertiary public hospitals in C city, and explore the high-quality development path of municipal public hospitals, so as to provide a basis for the formulation of relevant policies.Methods:Based on the relevant policy documents of the evaluation indicators for the high-quality development of public hospitals, and around the four dimensions of medical quality, economic efficiency, sustainable development and satisfaction, the entropy weight TOPSIS-RSR method was adopted to comprehensively evaluate the high-quality development of some tertiary public hospitals in C city in 2019 and 2020. It included all five municipal comprehensive tertiary public hospitals in C city, and included one provincial and two county-level tertiary public hospitals for comparative study.Results:In 2019 and 2020, the number of medical cooperation, the surplus of income and expenditure, and the entropy weight of scientific research project funding per 100 health technicians were large, which were the key indicators to evaluate the high-quality development of public hospitals, with the values of 0.096 and 0.109, 0.119 and 0.141, 0.123 and 0.124, respectively. According to the best classification principle, the provincial, municipal and county-level public hospitals in C city could be divided into three levels of " good" , " medium" and " poor" , and the development gap between these levels was large.Conclusions:The development of municipal public hospitals in C city has achieved remarkable results, but there are still such problems as uneven distribution of medical resources, insufficient motivation for talent training, relatively lagging discipline construction, and insufficient advantages and characteristics. In this regard, we should further optimize the allocation of medical resources, improve the operation and management system, clarify the social functions of public hospitals, and scientifically allocate core resources.
10.A case report of mucinous tubular and spindle cell carcinoma of the kidney

Mucinous tubular and spindle cell carcinoma of the kidney is a rare subtype of renal carcinoma. In July 2019, one case of mucinous tubular and spindle cell carcinoma of the kidney was admitted to our hospital, The laparoscopic partial nephrectomy was performed, and there was no recurrence or metastasis during the follow-up period of 13 months. The disease has no obvious clinical manifestations and its diagnosis depends on pathological and immunohistochemical features. Surgical treatment is the main treatment for this disease, and most patients have a good prognosis.Howerer, the possibility of progression remains in the late stage of the disease.
