1.Discussion of How to Guarantee the Interests of Both Doctor and Patient after Implementation of Drug“Zero Profit”

OBJECTIVE:To promote the implementation of drug“zero profit”,and ensure the interests of both doctor and pa-tient. METHODS:Through introducing the related information of drug management and compensation mechanism of drug“zero profit”,the advices were put forward for the heavy financial burden,shortage of hospital income and medical accessibility,etc. RESULTS&CONCLUSIONS:Drug“zero profit”could promote the reform of public hospital. If both doctor and patient could ob-tain benefits,it is very important to strictly enforce compensation mechanism and increase the government investment. Meanwhile, medical service price should be appropriately adjusted to reflect technical value,knowledge value and risk value of medical staff;preparyment of the total amount should be propeled by project,disease,head and other payment methods to control unreasonable medical expenses growth;mechanical of the hospital operating service should be improved to consciously reduce the dependence on drug profits.
2.Research methods of ″18 incompatible medicaments″incompatibilitytoxicitycharacterizationoftraditional Chinese medicine based on early evaluation of overall toxicity

Currently,the toxicity study of traditional Chinese medicine is faced with the following problems. Firstly,the evaluation in vitro cannot fully reflect the true state of the body. Secondly,the traditional method is not sensitive enough to the early toxicity. Lastly,the toxicity evaluation indexes cannot determine whether the compatibility of traditional Chinese medicine produces toxicity or increases toxicity systematically. The paper proposed a synthesized early evaluation research method for target organ toxicity induced by traditional Chinese medicine:screening,validation,optimization and application. This method mainly inoolves early target organ toxicity biomarkers in screening,optimi?zation,validation,biological significance explanation,and application to the traditional Chinese medicine incompatibility based on the metabolic dynamic fingerprint spectrum in order to obtain biomarkers of target organ toxicity that are sensitive and precede conventional biochemical indices for early evaluation . We attempted to analyze the pattern of chang of the biomarkers for animals acted by″18 incompatible medicaments″compatibility combination. We found that Radix Aconiti Lateralis Preparata with cardiotoxicity were compatible with Rhizoma Pinelliae,and that Trichosanthes kirilowii Maxim,Fritillaria,Ampelopsis Radix and Bletilla striata without non-cardiotoxicity produced and increased cardiotoxicity systematically.
3.Multi-slice computed tomography for diagnosis of mucinous cystic neoplasms and serous cystadenomas of the pancreas.

OBJECTIVETo assess the value of multi-slice computed tomography (MSCT) in the diagnosis and differential diagnosis of pancreatic mucinous cystic neoplasms and serous cystadenoma.
METHODThe MSCT images were reviewed for 19 pathologically confirmed cases of pancreatic mucinous cystadenomas and 13 cases of pancreatic serous cystadenomas (n=13) treated in our center between July, 2003 and December, 2009. The CT features were analyzed including the tumor location, contour, dimension of the largest cyst, cystic wall, septation, presence of calcification, solid component, pancreatic atrophy, main pancreatic duct dilatation, and lesion margins.
RESULTSSignificant differences were found between the two groups in lesion diameter (P=0.009), cyst distribution of the largest cyst (>2 cm) (P=0.01), and the presence of solid component (P=0.02).
CONCLUSIONMSCT can be of important value in the diagnosis and differential diagnosis of pancreatic mucinous cystic neoplasms and serous cystadenomas.

4.Study on vertebral degenerations of coal miners with low back pain by means of computerized tomography.

OBJECTIVETo study lumbar vertebral degenerations of coal miners with low back pain.
METHODS40 underground miners with low back pain, aged from 30 to 50 years and employed for a length from 10 to 30 years, were taken as observation group. 30 ground workers without low back pain but with almost the same age, employment length and height etc. were selected as control group. Intervertebral discs at L3/L4, L4/L5, L5/S1 were scanned with computerized tomography, comparing herniated discs, sagittal diameter of vertebral canal, height of lateral recess, vertebral hypertostosis, ligament hypertrophy and calcification between two groups.
RESULTSL3/L4, L4/L5, L5/S1 herniated discs and narrow lateral recess in observation group(0.33 +/- 0.15, 0.53 +/- 0.25, 0.45 +/- 0.18 and 0.40 +/- 0.08, 0.31 +/- 0.05, 0.37 +/- 0.07) were more serious than that in control group(0.28 +/- 0.11, 0.32 +/- 0.21, 0.37 +/- 0.19 and 0.42 +/- 0.10, 0.43 +/- 0.07, 0.40 +/- 0.06), but only with significant difference at L4/L5 (P < 0.01). Sagittal diameter of vertebral canal in observation group is narrower than that in control group but of little significance(P > 0.05). Cases of vertebral hyperostosis, ligament hypertrophy and calcification were found more frequent in observation group(45.00%, 42.50%, 22.50%) than in control group(23.33%, 16.67%, 16.67%), but only ligament hypertrophy was remarkable(P < 0.05).
CONCLUSIONSAs compared with control group lumbar vertebral degenerations are more serious in underground miners with manifestations like herniated disc and narrow lateral recess mainly at L4/L5.
