Objective One of the characteristics of rheumatoid arthritis was multiple joints involved symmetrically. The purpose was to study the method of early diagnosis and treatment for the atypical rheumatoid arthritis involving solitary joint. Methods From April 1997 to December 2002, 5 cases which were 1 male and 4 female patients with an average age of 36.8 years (range, 23 to 47 years), suffered from the swelling and pain of knee joint, and were diagnosed as early rheumatoid arthritis. The interval of the onset to surgical treatment ranged from 2 months to 3 years with a mean of 16.2 months. The patients were evaluated with serum test, joint fluids examination and radiological imaging. They were treated by the synovectomy under the arthroscopy, while the appearances of the joint under arthroscopy were recorded, and the synovial tissue was sent to the pathological examination. The improvements of the joints symptoms and the range of motion were assessed at final follow up. Results During the operation, the light green and little muddy joint fluids were found in all patients. ESR of the patients ranged from 29 to 51 mm/1h with an average of 32.2 mm/1h, all of which were higher than normal value, whereas, 3 cases had positive rheumatoid factor and 4 cases showed significant higher ?2-MG. The radiographic findings were osteoporosis in 3 cases, soft tissue swelling in 4 cases and narrowed joint space in 1 case. The synovium were proliferated and enlarged in dark violet, there were creeping pannus and necrosis fibrin deposition in the cartilage. All of the patients were diagnosed as rheumatoid arthritis confirmed by pathological changes of the synovium. The patients were followed up from 4 months to 5 years (mean, 18.7 months). Pain released in 5 cases, swelling reduced in 3 cases, the range of motion improved by 19 degrees. Unfortunately, all of the patients developed later into typical rheumatoid arthritis after the mean interval of 10.9 months (range, 3 to 25 months), the symptoms were consistent with the criteria of American Rheumatoid Association . Conclusion Solitary joint with rheumatoid arthritis is rare and may be the early status of rheumatoid arthritis. The specific appearance under the arthroscopy prompt the early diagnosis of the knee rheumatoid arthritis, and synovectomy by arthroscopy may be benificial to release the symptoms of rheumatoid arthritis.

To explore the electromagnetic compatibility in the shelter hospital system to adapt it to the future complex electromagnetic environment. The factors for electromagnetic interference were analyzed, and some measures were put forward including separating the interference source from the sensitive device, setting up shielding space, applying grounding measures, eliminating interference signals by filtering and etc. The electromagnetic compatibility in the shelter hospital system was detected through electromagnetic self-compatibility test and electromagnetic environ-mental suitability test for external radio frequency. Simulations and tests proved that the failures and mal-re-sponse due to electromagnetic interference were avoided in the shelter hospital system. The electromagnetic compatibility scheme can meet the requirements of emergency medical facilities for electromagnetic protection in field conditions.

Objective To observe the treatment effect of acupoint massage and auricular point sticking of cowherb seeds on blood pressure of hypertensive patients.Methods 100 cases of hypertensive patients were randomly divided into the observation group and the control group,50 cases in each.The observation group was given routine application of antihyportensive drags combined with acupoint massage and auricular point sticking of cowherb seeds.The control group received routine drug therapy.The blood pressure changes were compared between the two groups before and after treatment.Results The systolic blood pressure and diastolic blood pressure of both groups were lower after treatment compared with those before treatment.The total effective rate of blood pressure control was 98％ in the observation group,88％ in the control group,which showed no significant difference.Symptom control results showed that the total effective rate of the observation group was higher than that of the control group.Conclusions Acupoint massage and auricular point sticking of cowherb seeds in hypertensive patients can be an auxiliary pressure relief means,it can not only effectively lower blood pressure,but show good effect in alleviation of headache,vertigo,irritability.

Nonaqueous capillary electrophoresis is used for the determination of the contents of diosgenin and ruscogenin in Radix Ophiopogonis. The operating buffer was composed of 20 mmol x L(-1) Na2B4O7-HCl (pH 7.61) in 70% methanol. The applied voltage was 25 kV and detection potential was at +0.70 V. With these conditions, the components were successfully separated. The content of diosgenin in Radix Ophiopogonis was 0.018 mg x g(-1) and ruscogenin was 0.008 mg x g(-1). The average recoveries of diosgenin and ruscogenin were 102% and 99.2%, respectively. A new method of the quality control of diosgenin and ruscogenin in Radix Ophiopogonis is provided.

Objective To investigate the phenotype,number and distribution of inflammatory cells in early and late stages of spontaneous regression of halo nevi,and to elucidate the immunological mechanisms for spontaneous regression of these nevi.Methods Halo nevi,their surrounding non-lesional skin,and normal control skin were examined by immunohistochemical staining with monoclonal antibodies to CD3,CD4,CD8,CD20,CD1a,CD56 and CD68.Staining results were observed and analyzed by the computer image analysis system,image-pro plus 6.0.Results The number of CD4+,CD8+,CD20+,CD1a+cells,along with the diameter of CD1a+and CD68+ cells was significantly increased in the lesions of early and late stage of spontaneous regression of halo nevi than in non-lesional skin and normal control skin(both P＜0.01).The ratio of CD8+/CD4+ cells in the lesions of late stage of spontaneous regression was also higher than that in the lesions of early stage (2.05∶1 VS 1.82∶1).A massive infiltrate of CD8+ cells was observed in the nests of nevus cells.ConclusionsCD4,CD8,CD20,CD1a,CD56 and CD68 positive cells are all involved in the spontaneous regression of halo nevi,and CD8+ cells may play a predominant role in this process.

Objective To investigate the clinical and laboratory characteristics of anticentromere antibody (ACA)and anti-SSA antibody expressions in patients with Primary Sj(o)gren's Syndrome (PSS). Methods Twelve PSS patients with ACA positive but SSA negative(ACA PSS)and 19 PSS patients with SSA positive but ACA negative(SSA PSS)were enrolled into the study and classified into two groups. We compared the age,laboratory data,occurrence of Raynaud's phenomenon(RP),and histological changes in minor labial salivary glands biopsies of the patients from two group. Results The mean age of the ACA PSS group(68.4 ± 7.9)years was significantly higher than that of the SSA PSS(54. 6 ± 16. 2)years group(P ＜ 0. 05). Serum IgG level of ACA PSS group(17. 89 ±4. 08)g/L was close to the normal range,which was significantly lower than that of SSA PSS(27.90 ±6. 72)(P ＜0. 01). Leukocytopenia was less frequently observed in ACA PSS than in SSA PSS(P ＜ 0. 05),the difference between two groups was statistically significant. We also found more frequent RP in the ACA PSS group than SSA PSS group(P ＜ 0. 05). Conclusions Our data confirm that ACA positive PSS differs from SSA positive PSS at several clinical respects and laboratorial examinations.

Before performing patient testing with commercial microbial test systems,each laboratory must verify that it can obtain performance specifications comparable to those of the manufacturer.This includes trueness,precision (reproducibility),and reportable range of test results,and verifying that the manufacturer's reference ranges are appropriate for the laboratory's patient population.American Clinical and Laboratory Standards Institute has set up a committeeto develop a verification process and a quality assurance program for commercial microbial identification system and antimicrobial susceptibility testing system,in order to provide recommendations for US Food and Drug Administration (FDA).This guidance is applicable to instrument systems widely used in clinical laboratories and can also be used for manual testing of microbiological identification and antimicrobial susceptibility testing.The aim of this article is to provide advice for the microbial identification system and antimicrobial susceptibility testing system verification process,based on principles of microbiological identification and antimicrobial susceptibility and CLSI M52 guideline.

OBJECTIVE: To better implement Edition 2005 of China Pharmacopeia in order to ensure the safety and rationalization of drug use.METHODS:551 categories of medicinal materials and cut crude drugs in Edition 2005 of China Pharmacopeia(Volume One) was classified on the basis of descriptions of their toxin and precautions.RESULTS:10 categories were classified as extremely poisonous,37 toxic,25 slightly toxic,479 nontoxic.5 categories were classified as contraindication in pregnancy,28 prohibited in pregnancy,38 cautiously used in pregnancy.1 category was classified as prohibited for children.So_me varieties of “Eighteen incompatible medicaments” and “Nineteen medicaments of mutual antagonism” were not mentioned.CONCLUSION:Although there is only small part of categories with toxin and precautions,much attention should be paid to.The use of these materials or drugs should strictly abide by Edition 2005 of China Pharmacopeia.

Objective To explore the clinical value of the interventional and comprehensive treatment of iliac-femoral venous thrombus(I-FVT).Methods 32 patients with I-FVT were underwent interventional therapy.First the filter was implanted into the inferior vena caval via opposite side of femoral vein,then the catheteres were implanted into the pathologic regions within the vena to process the emboluses,and balloon-directed extend when necessary.Results The procedure of treatment was successful in all patients.The iliac-femoral veins were patent,swollen and pain symptom of lower limb disappeared gradually after operation.The followed-up study the longest period for 30 months showed no severe complications and recurrence.Conclusion The interventional and comprehensive therapy is of ideal effect on treating I-FVT,occuring interventional and comprehensive treatment has ideally effect to cure the patients with I-FVT.
