Objective To investigate the effect of the intraoperative wake-up test on the postoperative delirium in patients undergoing scoliosis operation under analgesia with sufentanil. Methods Sixty ASA Ⅰ or Ⅱ patients , aged 12-60 yr, scheduled for scoliosis surgery, were randomly divided into 2 groups ( n = 30 each) : control group (group C) and intraoperative wake-up test group (group W) . The intraoperative wake-up test was not performed during operation in group C. Anesthesia was induced with target-controlled infusion of sufentanil with the target effect-site concentration set at 0.5 ng/ml and iv injection of propofol 1-2 nig/kg. As soon as the patients lost consciousness, tracheal intubation was facilitated with 0.15 mg/kg cisatracurium besylate. The patients were mechanically ventilated. Anesthesia was maintained with target-controlled inhalation of sevoflurane (target end-tidal concentration 0. 8%-1. 5%), target-controlled infusion of sufentanil (target effect-site concentration 0.2-0.3 ng/ml) and iv infusion of cisatracurium besylate 0.1 mg- kg-1·h-1 . In group W, the patients underwent the intraoperative wake-up test. Postoperative analgesia was provided with sufentanil. The occurrence of intraoperative awareness and postoperative delirium was recorded. Results No significant difference was found in the incidences of intraoperative awareness and postoperative delirium between the two groups. Conclusion The intraoperative wake-up test is not the risk factor for postoperative delirium in patients undergoing scoliosis surgery under analgesia with sufentanil, and inhibition of perioperative pain and intraoperative awareness by analgesia with sufentanil may be involved in the mechanism.

ObjectivesTo retrospectively analyze the clinicopathological features of neuroblastoma (NB) and investigate the signiifcance of abnormality ofN-myc and anaplastic lymphoma kinase (ALK) gene copy number change as well asALKmu-tations in NB.Methods Eighty-three NB patients were collected and classiifed into different subgroups according to the clinical stage and histology. Fluorescence in situ hybridization (FISH) was performed to detect the abnormalities ofN-mycandALK genes. The extracted DNA was ampliifed by PCR and sequenced to investigate the point mutations of theALK gene. Follow-up data were collected and survival analysis was performed.ResultsFISH detection showed that the aberration ofN-mycgene copy number presented as gain and ampliifcation. The aberration ofALK gene presented as point mutation and gain. It was shown that 17 cases had the abnormality of bothN-myc andALK gene. Survival analysis showed that the prognostic factors included the clinical stage, age and abnormality ofN-myc genes.ConclusionDetection ofN-myc andALK abnormality in NB would be helpful for evaluating the prognosis and providing theoretical basis forALK target therapy.

Objective Though the research on the satisfaction of environment and factors influ encing the growth of the talented in medical science in Xinjiang, the author focuses on the shortcomings and reasons of current development of the talented, and provides specific suggestions and improvement for the cultivation of the talented. Methods Using Xinjiang Medical University First Affiliated Hospital as the research object, author investigates the influential factors of growth, and the satisfaction of environment though the questionnaires. Results According to the principal components analysis, the factors which influence the growth the talented in medical science are put in flowing order:political factor>environmental factor>education factor>family factor.According to the data envelop ment analysis,in the satisfaction of environment of hospital technological innovation, the talented in hospital technological innovation consider that the cultivation plan, research expenses, support for scientific research, recruitment, and social welfare are at a better satisfaction rate;but the duration of academic research, income policies, selection of the talented, and the outcome of the research are at a low satisfaction rate. Conclusions Based on the current development and cultivation of the talented in the innovation of medical science, there are still many shortcomings.Hospital must continue interna tional cooperation, insist to do the cultivation of the talented and the innovation team, and complete the selection mechanisms,in order to give enough time for the scientific research, and create a good atmosphere of medical science innovation at the same time.Only when the services for the talented are well-prepared,Xinjiang medical science innovation can achieve enough support and motivation for the development of the talented.

Objective To analysis the cause of primary graft failure of unrelated cord blood transplantation with high-dose of CD34 + cells in treatment of acute myelocytic leukemia (AML) /myelodysplastic syndrome (MDS).Methods A 4-year-old girl was diagnosed AML/MDS at the Department of Pediatric Hematology and Oncology of West China Second University Hospital of Sichuan University. She presented completely remission after induction and consolidation chemotherapy. She received unrelated partially human leukocyte antigen (HLA)-mismatched cord blood transplantation. We investigated the treatment outcomes of UCBT and associated complications. Results The patient suffered primary graft failure and then received secondary haploidentical hematopoietic stem cell transplantation (HSCT)from her mother. However, she suffered fatal multiresistant Acinetobacter spp septicemia. She died due to respiratory failure on 7 d after the second transplantation. Conclusions In this case, hematopoietic stem cells with high dose of CD34 + cells could not overcome the risk of primary graft failure and HLA disparity. The patient's primary graft failure was associated with platelet transfusion refractoriness and potent immunologic dysfunction, especially the anti-HLA donor specific antibodies before unrelated cord blood transplantation.

Organ Transplantation 2014;(6):383-388
Retinoid X receptor (RXR) acts as ligand-dependent transcription factors playing an important role in regulating a serial of physiological processes, such as embryo development and organ homeostasis. At the molecular level,RXRs exert their functions by inter-activating with multiple signal pathways to regulate target gene expression which control cell growth, differentiation, survival and death. The interference in the network of RXR and other signal pathways has turned RXR into an attractive drug target.
