3.Primary intensive insulin therapy improves islet ?-cell function in type 2 diabetes——A dream or reality?

Chinese Journal of Endocrinology and Metabolism 1986;0(04):-
Hyperglycaemia attenuates?-cell insulin secretion in response to plasma glucose elevation and reduces insulin-mediated glucose transport. The long-standing severe hyperglycaemia may irreversibly damage?-cells. However, in this regard, there are increasing evidences showing that short-term intensive insulin therapy may significantly improve the response to endogenous insulin by normalizing blood glucose, so as to induce a "honeymoon period" for several years in a considerable number of subjects with newly diagnosed type 2 diabetes. Thus, a question has been raised: whether or not insulin therapy should be taken as a part of the strategy of the primary treatment for type 2 diabetes? A number of experts believe that it is an important therapeutic strategy, however, others hold that insulin therapy may yield more cost than benefit and suggest that to halt this cycle of progressive metabolic deterioration by any means may transiently restore metabolic re-compensation in type 2 diabetes. This issue was discussed in the fourth meeting of China Group on Insulin Secretion and in 66th ADA scientific session this year. Here we offer our consideration on this question for your reference.

Chinese Journal of Cancer Biotherapy 1995;0(02):-
The cell suspensions prepared from surgically resected hepatoma specimens were used to immunize BALB/c mice and, with the hybridoma technique, a battery of high affinity monoclonal anti-hepatoma antibodies were obtained which were designated HAbl8, Fll, E5 and A10 separately. Immunohistochimical staining showed that the above 4 antibodies possessed good selective reactivity with hepatoma tissue. After radioiodination of Fll, E5, A10, HAbl8 IgG and It's F(ab')_(2) fragments, the labelled reagents were employed for radioimmunoimaging in hepatoma-bearing nude mice and the in vivo detection appeared promising, with tumor/non-tumor ratios being 6.88, 5.14, 5.67, 5.15 and 14.47 respectively. The in vivo localization ablities of the antibodies seemed better compared to other similar findings published elswhere (Dunk AA, 1987). Also, ~(131) I -HAbl8 I gG and its radiolabelled fragments were utilized for radioimmunotherapy in hepatoma-bearing nude mice, with complete response rate and partial response rate being 42%(5/12) and 50% (6/12)respectively. When the HAbl8 conjugate with radioiodine was introduced for the in vivo imaging in hepatoma patients, a positivity rate of 86.5% (45/52)was witnessed, with the smallest tumor foci detected being only 0.5cm in diameter. In the in vivo targeting therapy with the immunoconjugate, a partial response rate of 69.6% (16/23) was obtained. In summary, the antibodies reported here have lended a novel regime for the present comprehensive therapy protocol of hepatoma.

Chinese Journal of Diabetes 2008;0(12):-
Objective To investigate the significance of serum IP-10 levels in different types of diabetic patients. Methods Serum IP-10 level in 78 cases with type 1 diabetes,49 cases with type 2 diabetes and 33 cases of healthy controls was measured with ELISA assays. Type 1 diabetic patients were divided into classic type 1 diabetes (n=39) and latent autoimmune diabetes in adults (LADA,n=39) according to their disease process, and they were also divided into autoimmune type 1 diabetes (n=58) and idiopathic type 1 diabetes (n=20).Type 2 diabetic patients, according to carotid intima-media thickness (IMT),were divided into patients with (n=24) or without atherosclerosis (n=25). Results 1) Serum IP-10 levels were not statistically different among type 1 diabetes, type 2 diabetes and healthy controls. 2) Serum IP-10 concentrations in autoimmune type 1 diabetes with positive islet autoantibody were higher than those in healthy controls(184.96?104.48pg/ml vs 146.10?74.61pg/ml,P=0.03);while there were no difference in type 1 diabetes with variable disease durations. 3) No difference in IP-10 levels was found between type 2 patients with(173.5?69.6pg/ml) and without atherosclerosis (188.5?79.7 pg/ml). Conclusions Serum IP-10 level is augmented in autoimmune type 1 diabetes.

OBJECTIVE To find out the prevalent distribution and multidrug resistance trend of meticillin-resistant Staphylococcus(MRS) and to prevent fulminant prevalence of MRS and opt for effective therapeutic means.METHODS The bacterium was(identified) by the way of API Staph and the TH-16S′s coding tube. The(antimicrobial) susceptibility testing was adoped by ATB-STAPH5 and MRS was examined by dilution and K-B.All statistical analyses were performed using SPLM 3.0 software.RESULTS The isolation rate of meticillin-resistant S.aureus(MRSA) and meticillin-resistant coagulase negative Staphylococcus(MRCNS) was 38.09%,20.00%and 87.65%,89.00%,(respectively) in 2 years.Along with the age of patients, the infection from MRS was(increasing). The isolation rate of MRSA was 28-26%,but that of MRCNS was more than 80% from S.epidermidis,S.haemolyticus,S.hominis,and S.saprophyticus subsp saprophyticus. All parts of our body can be(infected) by MRS.The more than 30% MRSA were multidrug resistant and the approximately 11.87-13.75% MRCNS were also multidrug(resistant).(CONCLUSIONS) The isolation rate of MRSA from national surveillance(network) is not(different) with that of MRCNS.

0.05).In LADA patients, multipe linear regression analysis showed that the risk factors related to the intima media thickness of common carotid artery were age, smoking and LDL-C and the risk factors related to the intima medial thickness of femoral artery were age,sex and LDL-C.Conclusion The prevalence of subclinical atherosclerosis between LADA and type 2 diabetes is similar.LDL-C,age,sex and smoking may be the risk factors of subclinical AS in LADA patients.

Tianjin Medical Journal 2014;(3):251-253
Objective To investigate the risk factors of dual anti-platelet treatment in upper gastrointestinal hemor-rhage after percutaneous coronary intervention (PCI). Methods Clinical data of 2004 PCI patients with dual anti-platelet treatment were collected, including gender, age, body mass index (BMI),history of drug therapy, history of digestive disease, history of smoking and drinking, history of hypertension, history of diabetes, history of cerebrovascular disease and history of combined treatment with a proton pump inhibitor (PPI). The different characteristics of gastrointestinal bleeding were com-pared. The risk factors of upper gastrointestinal bleeding were analyzed. Results The risk factors for PCI patients with up-per gastrointestinal bleeding were age>65 years of old, history of smoking, history of hypertension, abnormal platelet count, creatinine and hemoglobin. The combined treatment with a proton pump inhibitor was a protective factor associated with gas-trointestinal bleeding. Conclusion We should pay attention to the upper gastrointestinal bleeding induced by dual anti-platelet treatment, and take appropriate measures to reduce the incidence of gastrointestinal bleeding in PCI patients.
