1.Influence of olanzapine combined with risperidone on the serum level of homocysteine in patients with schizophrenia

Objective To explore the influence of olanzapine combined with risperidone on the serum level of homocysteine (Hcy) in patients with schizophrenia.Methods Participants included sixty patients diagnosed with schizophrenia,patients were divided into two groups by means of random digital method.The group A was given olanzapine treatment,and the B group was given olanzapine combined with risperidone therapy.After two months follow-up,the curative effect and the level of Hcy were observed.Results Before treatment,Hcy level [(24.4 ± 6.1)μmol/L,(25.8 ± 7.7)μmol/L] and HH number (22 cases,23 cases) in the two groups had no obvious differences (t =1.11,x2 =1.34,all P > 0.05) ;2 months after treatment,Hcy level (11.6 ± 8.6) μmol/L and HH number(16 cases) in group B were significantly lower than those in the group A [(16.3 ± 3.8) μmol/L,8 cases],the differences were statistically significant (t =4.65,x2 =4.32,all P < 0.05).Conclusion Olanzapine combined with risperidone in treatment of schizophrenia have good curative effect,it can improve the symptoms of patients,decrease serum Hcy level and HH number.

Objective To explore the characteristics of T cell immunity in peripheral blood of patients with carboxypeptidase - H antibody (CPH-Ab). Methods Forty-two latent autoimmune diabetes in adults (LADA) patients with CPH-Ab~+ alone, 20 Type 2 diabetes mellitus patients (T2DM), and 22 healthy controls were selected and their peripheral blood mononuclear cells were isolated. Human recombinant carboxypeptidase (CPH) protein was expressed and further used as a stimulant in Enzyme-linked immunospot (ELISPOT) assay to detect IFN-γ-Th1 and IL-4-Th2 cells in the 3 groups. Th1/Th2 ratios were also calculated. CPH-Ab and glutamic acid decarboxylase antibody (GAD-Ab) were determined by radioligand assay. Results Compared with healthy controls and T2DM, IFN-γ-Th1 and IL-4-Th2 numbers did not increase significantly in CPH-Ab~+ group, nor did the Th1/Th2 ratios (P >0. 05). We further divided the CPH-Ab~+ patients into a short duration group (n = 22) and a long duration subgroup (n = 20) according to the duration of 3 years. CPH-IL-4-T in the short duration subgroup was significantly higher than that in T2DM and healthy controls (1. 8 vs. 0.2 and 0.3, both P < 0. 05) and we did not find any factor that was significantly correlated with the IL-4 spots number. There were not any significant differences in T cell responses to phaseolus vulgaris agglutinin (PHA) among all groups (P>0.05). Conclusion CPH does not directly involve in the cellular pathological mechanism of LADA. Anti-CPH immunity may be associated with more slowly aggressive beta cell autoimmunity.
4.Relations between pulse pressure and LDL-C for elderly patients with hypertension

Objective To explore the relations between pulse pressure and LDL-C for elderly patients with hyperten-sion. Methods A total of 451 elderly patients with hypertension were selected and divided into four groups. Group A was 65 to 69 years of age, group B was 70 to74 years of age, group C was 75 to 79 years of age, and group D was ≥80 years of age. They were also divided into four groups according to PP levels: PP1 group ≤40 mm Hg, PP2 group 41 to 60 mm Hg, PP3 group 61 to 80 mm Hg, and PP4 group>80 mm Hg. They were divided into LDL-C group 1 (≥2.6 mmol/L) and LDL-C group 2 (<2.6 mmol/L) according to their LDL-C levels. Distribution of different age groups in four PP groups and the effects of different levels of LDL-C on pulse pressure were analyzed. Results (1) The propor-tions of PP2 group among the four age groups were 59.41%, 48.84%, 55.45% and 51.67%. The proportions of PP4 a-mong the four age groups were 4.95%, 3.88%, 3.96% and 7.50%. (2) Compared with PP1 group, the proportion of LDL-C ≥2.6 mmol/L in PP2 group, PP3 group and PP4 group was significantly increased. Conclusion (1) PP levels in different ages groups are concentrated on 41-60 mmHg. Among elderly patients with hypertension>80 mmHg, the pro-portion of patients with advanced age (≥years of age) is relatively higher. (2) LDL-C is the risk factor of increased pulse pressure for elderly patients with hypertension.

Blast injury of the chest injury is the most common wound in modern war trauma and terrorist attacks, and is also the most fatal type of whole body explosion injury. Most patients with severe blast injury of the chest die in the early stage before hospitalization or during transportation, so first aid is critically important. At present, there exist widespread problems such as non-standard treatment and large difference in curative effect, while there lacks clinical treatment standards for blast injury of the chest. According to the principles of scientificity, practicality and advancement, the Trauma Society of Chinese Medical Association has formulated the guidance of classification, pre-hospital first aid, in-hospital treatment and major injury management strategies for blast injury of the chest, aiming to provide reference for clinical diagnosis and treatment.
6. Clinical observation of 43 cases of acute poisoning caused by herbicide marked diquat

Objective:
To analyze the relationship between clinical characteristics and prognosis of patients with acute herbicide poisoning marked diquat.
Methods:
A multi-center, retrospective clinical study of patients with acute diquat poisoning admitted into Emergency Department was conducted from June 2015 to August 2018 in 8 hospitals in Jiangsu Province.
Results:
A total of 43 patients (22 males and 21 females) were collected and the peak age of poisoning ranged 20-39 years old. The only route of poisoning was ingestion. Among these cases, suicide was the most common cause of poisoningaccounting for 90.70%. In emergency treatment, the constituent ratios of gastric lavage, hemoperfusion and glucocorticoid were 87.50%, 72.50% and 42.50%, respectively. The total mortality increased to 60.00% after follow-up, while the in-hospital mortality was 18.60%. The mortality of patients with toxic dose < 50 mL was 11.11%.
Conclusions
The incidence of acute herbicide poisoning with "diquat" as commercial component is gradually increasing. At present, the mortality is very high. Ingestion poisoning dose is the key factor affecting prognosis, and the prognosis of patients with oral dose > 50 mL is poor.
