1.Clinical Study of Cyclosporin A for Psoriasis in China.

BACKGROUND: The efficacy of Cyclosporin A(CyA) in the treatment of psoriasis is well recongized, the dosage less than 5mg/kg per day(mg/kg/d) is recommended. OBJECTIVE: The purpose of this study is to evaluate the indication, dose, course and side-effects of CyA therapy for psoriasis, especially in China. METHODS: Forty patients with psoriasis were treated with doses of CyA 5mg/kg/d, compared with 20 patients treated with Tigason lmg/kg/d by Global PAST. RESULTS: 1. Of 40 patients 33 patients (82.5%)were cured clinically in 4-5weeks and all the patients(100%) had responses to the therapy. 2. The efficacy and the Global PASI reduction of CyA therapy were significantly better than that of Tigason therapy. 3. During the CyA therapy, no severe side-effects and obvious laboratory changes were found. CONCLUSION: CyA(5mg/kg/d) was satisfactory for psoriasis, especially for psoriasis pustulosa, arthropathica and erythroderma. CyA was found to be superior to Tigason. The therapy course and side-effects seemed to be related to the dosage.

The resident match system established in 1952.And since then almost all of the senior medical students and international students have applied a position of resident training in this national system.They seek a respective resident program and hospital for training while the hospital hires students they preferred.The match procedure includes submitting application,interview of selected candidates,submitting rank-order list of both parties,centralizing matching service by computer and announcement of the results in Match Day.This is a relatively stan-dardized,rational and fair system which we can learn something from.
3.Effects of ultrasonic cavitation on cultured cell membrane in vitro

Objective To investigate the effects of ult rasonic cavitation on cell membrane cultured in vitro. Methods The rat pulmonary artery vascular smooth muscle cells were divided into three groups, control group, simple ultrasound radiation group and contrast plus ultrasound radiation group.The latter two groups were exposed to ultrasound(665 kHz, 0.5- W/cm 2) for 20 s but the last group with contrast agent. Immediately after the experiment or 24 hours after treatments, electron microscopic scanning was used to observe the membrane change. Results Compared with control group, the cell structure and cell surface had no apparent changes in the second group treated with ultrasound alone. In the third group, nearly 20% cells exhibited small holes with diameter mostly about 1-2 ?m in the cell surface and returned to normal appearance after 24 hours later. Conclusions Ultrasonic cavitation can cause small transient holes in cell surface and hence increase permeability of the cell membrane. It might be the main mechanism for gene delivery with ultrasound contrast agents.
4.Observation on the effect of limited open reduction and Kirschner wire fixation in the treatment of calcaneal fractures

Objective To observe the effect of limited open reduction and Kirschner wire fixation in the treatment of calcaneal fractures.Methods Clinical data of 23 patients with calcaneal fracture treated with limited open reduction and Kirschner wire fixation were collected,and the efficacy was evaluated according to Maryland foot score.Results The B(o)hler corner and Gissane angle of 23 patients recovered well without any complications.Conclusion For the patients with Sanders Ⅱ and Ⅲ calcaneus fracture,the limited open reduction and Kirschner wire fixation has good effect.
5.Significance of u-PA and PAI-1 Expression on Diseased Glomeruli

Purpose To investigate the significance of u-PA and PAI-1 expression in various types ofglomerulonephritis. Methods The expression level of u-PA, PAI-1, type-Ⅳ collagen and PCNA positivenuclei in 120 cases of renal biopsies from patients with various types of glomerulonephritis were detected byimmunohistochemical method. Both the staining intensity of u-PA and PAI-1 in glomeruli were quantitativelyanalyzed by image analysis. Results The expression intensity of u-PA and PAI-1 was different in varoustypes of glomerulonephritis, which were significantly higher than that of the minor lesion group. Meanwhile,the intensity of PAI-1 stain was significantly higher than that of u-PA in various types of glomerulonephritis( P < 0.05). The increase of u-PA expression was closely related to increase of type- Ⅳ collagen synthesis andhypercellularity in glomeruli(r = 0. 761 and 0. 811, P< 0.05), while the expression of PAI-1 was closelyrelated to the increase of Col-Ⅳ synthesis other than the cell proliferation in the glomeruli. ConclusionsThe over expression of u-PA and PAI-1 may play an important role in contributing to the pathogenesis anddevelopment of glomerulonephritis.
6.Expression of apoiipoprotein H in childhood primary nephrotic syndrome

Objective To study the expression of apolipoprotein H (ApoH) in childhood primary nephrotic syndrome (PNS) and to discuss its role in PNS. Methods Immunohistochemistry staining and real-time quantitative polymerase chain reaction(RT-PCR) were performed to evaluate the expression of ApoH in renal tissues of 78 patients with PNS and 14 cases of normal controls. Serum albumin, serum lipid, proteinuria and urinary retinol binding protein (RBP) were tested before renal biopsy in all patients. Tubulointerstitial lesions were investigated. Results (l)There was positive expression of ApoH in renal tissues of PNS patients and normal controls,mainly in the proximal tubules by immunohistochemistry staining. ApoH mRNA was also shown in all renal tissues by RT-PCR. ApoH protein expression was positively correlaed with its mRNA expression(r=0.264, P 0.05) whereas these data displayed no significant difference between two groups. 7.Treatment of 58 Cases of Tonsillitis by Pricking Technique

Ashi point (tonsil) in combination of the pricking and bleeding technique on Shaoshang (LU 11 )and Shangyang (LI 1) were used to treat 58 outpatients of acute tonsillitis. The results showed cure in 38 cases,remarkable effect in 17 cases and failure in 3 cases by one treatment, and the total effective rate in 95%.

9.Effects of inhalation-intravenous general anesthesia on stress response of patients with liver hydatidosis for pericystectomy

Objective To investigate the changes of stress response on inhalation-intravenous general anesthesia during pericystectomy for Liver hydatid cyst. Methods Thirty ASA I-II patients with liver hydatidosis for pericystectomy were studied. The blood from right jugular vein were collected for measurement of serum cortisol (Cor) concentrations and glucose (BG) levels, and MAP, HR and BIS were observed and recorded at different point-times: pre-intubation for 1 min, post-intubation for 3 min, pre-incision for 1 min, post-incision for 3 min, post-incision for 10 min, post-surgical exploration 10 min. The data of pre-intubation and pre-incision served as controls. Results The levels of Cor concentration of post-incision for 10 min were decreased than those of pre-intubation and pre-incision (P<0.05); MAP of post-incision for 10 min and post-surgical exploration for 10 min were increased than those of pre-intubation and pre-incision (P < 0.01); For HR, the data of post-surgical exploration 10 min was much quicker compared with pre-incision (P < 0.01), which is slower than that of pre-intubation(P<0.01). Conclusion Inhalation-intravenous general anesthesia may inhibit the stress response during intubation,incision and surgical exploration for liver hydatidosis pericystectomy. Hemodynamic changes during incision may reflect the trend of stress response in advance.
10.Role of K~+ channels in attenuation of pulmonary vasoreactivity following chronic hypobaric hypoxia in rats

AIM and METHODS: To determine the role of different K +-channels in attenuation of vasoreactivity of intrapulminary artery rings induced by chronic hypoxia. RESULTS: ①Acute hypoxia-induced pulmonary vasoconstriction(HPV) could be significantly attenuated by chronic hypobaric hypoxia for 15 days and for 30 days. ②HPV could be significantly potentiated by ATP-sensitive K +-channel (K ATP ) blocker or Ca 2+ -activated K +-channel (K Ca )blocker, and the potentiated scope in chronic hypoxic group was much higher than that observed in control group. ③ Delayed rectifier K +-channel (K DR )blocker showed no effect on HPV in both control group and chronic hypoxic group.CONCLUSIONS: Both K ATP and K Ca play an important modulating role in HPV and it's potentiation may be a critical mechanism for the attenuated vasoreactivity to acute hypoxia; following chronic hypobaric hypoxia.
