2.Influence of sitting position on pulmonary hemodynamics and intrapulmonary shunt in neurosurgery

Objective To study the effects of sitting position on pulmonary hemodynamics and intrapulmonary shunt (Qs/Qt) during neurosurgery. Methods Twenty eight patients scheduled for posterior fossa and posterior cervical spinal surgery, were anesthetized with sevoflurane fentanyl pancuronium or enflurane fentanyl pancuronium after intravenous induction. Pulmonary hemodynamics were monitored with Swan Ganz catheter technique and Qs/Qt was calculated from the data of arterial and mixture venous blood gas analysis, including the following parameters: CI, RAP, MPAP, PCWP, PVRI, pHa, PaCO 2, PaO 2, PaO 2/FiO 2 and Qs/Qt. The detection time points were as the follow: before anesthesia induction, before sitting position, immediately ,60 min and 120min after sitting position, and at the supine position after termination of surgery.Results After induction of anesthesia and during operation at sitting position, CI decreased significantly (P0.05).Conclusions During neurosurgical procedure under general anesthesia and sitting position, pulmonary vascular auto regulation can be preserved and pulmonary oxygenation can be kept in normal.
3.Effect of Chinese herbal immersion combined with Beecher's exercise in prevention and treatment of grade-0 diabetic foot

Objective To explore clinical effect of Chinese herbal immersion combined with Beecher's exercise for prevention and treatment of grade-0 diabetic foot.Methods A total of 107 patients who were diagnosed with grade-0 diabetic foot were selected in Department of Endocrinology of our hospital from July 2009 to October 2013.In addition to regular treatments,their feet were immersed in Chinese herbal liquid,and then were taken care by doing Beecher's exercise.The symptoms of diabetic foot were self-rated by each individual patient and mastering of knowledge about diabetic foot was evaluated both before the intervention and after three courses of treatment.Results After three courses of treatment,the self-rating symptom scored (4.37±1.45) totally,significandy lower than that before the intervention,(5.62±1.55),t=7.91,P < 0.05.The mastering of six testing indexes about diabetic foot obviously improved compared between those before and after three courses of treatment [62.62% (67/107)],23.36% (25/107),30.84% (33/107),46.73% (50/107),57.94% (62/107),50.47% (54/107) vs.96.26% (103/107)],86.92% (93/107),89.72% (96/107),85.98% (92/107),94.39% (101/107),87.85% (94/107),x2=37.78,87.35,77.46,36.92,39.16,35.05,all P < 0.05.Conclusions For grade-0 diabetic foot,Chinese herbal immersion treatment combined with Beecher's exercise can be applied straight forwardly on the disease focus;it's clinically safe and effective,and easy to be operated.It improves clinical symptoms of grade-0 diabetic foot by taking advantages of traditional Chinese medicine,prevent and control further development of diabetic foot and is valuable to be applied in clinical work.
4.Effect of Panax Notoginseng Saponins on Cerebral Water Content and Thrombin of Intracerebral Hemorrhage Rats

Objective To investigate the effects of Panax notoginseng saponins (PNS) on the cerebral water content and thrombin in the rats with intracerebral hemorrhage (ICH), and explore its mechnism for treating ICH. Methods SD male rats were randomly divided into blank group, sham operation group, model group and treatment group, and time points of 6, 24, 48, 72 h and 7 d were observed. ICH rat model was induced by collagenase and heparin in saline solution injected into caudate nucleus. The treatment group was injected PNS intraperitoneally once a day, and the cerebral water content was assessed by dry-wet weight method. Both content of fibrinogen (FIB) and thrombin clotting time (TT) in 48 h were measured. Results The cerebral water content of model group increased at every time point compared with blank group and sham operation group (P<0.05). The cerebral water content of treatment group was significantly decreased at every time point except for 6 h compared with model group (P<0.05). FIB of treatment group decreased compared with model group (P<0.05), while TT of treatment group was not significantly decreased (P>0.05). Conclusion PNS may play a role in treating ICH by decreasing cerebral water content, FIB and TT of ICH rats.
5.Effect of whole-process nursing intervention on comfort of patients undergoing radial artery puncture

Objective To study the effect of whole-process nursing intervention on comfort of patients undergoing radial artery puncture. Methods 100 patients with radial artery puncture from July, 2007 to June, 2008 were divided into the control group and the experimental group with 50 cases in each group accord-ing to time sequence. Routine whole nursing mode was used in the control group, the whole-process nursing intervention mode based upon routine nursing mode under the instruction of evidence-based method was used in the experimental group. Pain, psychological tensity, body numbness, success rate of radial artery puncture, patients' satisfaction degree were compared between the two groups. Results Every indexes of the experi-mental group were better than those of the control group. Conclusions The whole-process nursing interven-tion mode can promote the comfort degree of patients undergoing radial artery puncture.
6.Effects of lotusine on the action potentials in myocardium and slow inward current in cardiac purkinje fibers

AIM To study the effects of Lotusine(Lot), a pure alkaloid extracted from the green seed embryo of Nelumbo nucifera Gaertn, on the action potentials(APs) in myocardium and slow inward current(Isi) in cardiac Purkinje fibers(PF). METHODS standard microelectrode and two microelectrodes voltage clamp techniques were used. RESULTS Lot 1~100 ?mol?L -1 could concentration dependently prolonged APD 20 and APD 90 of fast AP and increase the contractile force(Fc) in guinea pig papillary muscles. In papillary muscles pretreated with reserpine, similar results were also observed but the effect of Lot was weaker than those pretreated without reserpine. In guinea pig left atria, Lot 30 ?mol?L -1 could partly antigoniste the shortening APD effect of acetylcholine. Lot 3~100 ?mol?L -1 could enhance the action potential amplitude(APA)and maximal velocity of phase 0 depolarization ( V max ) of slow AP of papillary muscles induced by high K + (24 mmol?L -1 )and isolated sinoatral node(SAN) pacemaker cells of rabbits with dose dependent manner,But not obvousely short the sinus cycle length of SAN. Moreover, Lot increased the Isi of in canin cardiac PF with with time dependent and dose dependent manner. CONCLUSION The results suggest that Lot has effect of prolongation of APD and increase of Ca 2+ influx and these are very important in contribution to its positive inotropic effect, which may be related to inhibition of phosphodiesterase Ⅲ.
7.Variation of dissolution rate of main active ingredient of compatibility of coptis and scutellaria

Objective: To study the scutellaria and coptis in different proportions, the main active ingredient of baicalin, berberine dissolution rate changes and to explore the effect of traditional drug pair of scutellaria and coptis on the dissolution rate of active ingredient. Methods: Based on the clinic common used prescriptions of scutellaria with coptis, ratio of 1:0,1:1,1:2,1:3,2:1,2:3,3:1,3:2,0:1 was selected, after the water boiling reflux extraction, refining separation, various products were obtained for the test, under the optimized conditions of RP-HPLC analysis to compare chromatographic fingerprints and to examine the main component baicalin, berberine in aspects of relative peak area ratio of changes and compatibility relations. Results: Compatibility of Scutellaria baicalensis and Coptis at different proportions can influence the dissolution rate of baicalin, Berberine Hydrochloride. There was a non-linear relationship of dissolution rate of baicalin and Scutellaria baicalensis. The best ratio of scutellaria ratio was 3:1, the best compatibility ratio of coptis was 1:3. Conclusion: Different compatibility proportions of scutellaria and coptis will result in different dissolution rate of baicalin and berberine.
8.Inhibition of invasion and metastasis by imrecoxib and its mechanisms in lung adenocarcinoma A549 cell xenograft in nude mice

Background and purpose:Cyclooxygenase-2 (COX-2) participates in angiogenesis and lymph node metastasis of lung cancer. COX-2 inhibitors could inhibit invasion and metastasis of lung cancer. This study aimed to investigate the inhibition of invasion and metastasis by COX-2 inhibitor imrecoxib in xenograft tumor of lung adenocar-cinoma A549 cell in nude mice and to explore its possible mechanisms, in addition, to observe the efficacy of imrecoxib combined with lobaplatin.Methods:Thirty male BALB/c nude mice were injected subcutaneously with A549 cells into the right axillary region to establish xenograft models. Twenty-nine successfully modeled mice were randomly divided into four groups: control group (n=7), imrecoxib group (n=8), lobaplatin group (n=7), imrecoxib combined with lobaplatin group (n=7). The control group was treated with the same amount of sterile distilled water and injected with the same amount of 0.9% sodium chloride solution via caudal vein. The treatment group was treated with imrecoxib tablets 40 mg/kg per day through gavage and injected with lobaplatin 7.5 mg/kg per week via caudal vein respectively. The diet, physical activity and other normal conditions of nude mice were observed everyday. After 6 weeks, 29 mice were sacrificed and transplanted tumor tissues were cut off. The expression of PTEN, cortactin protein and mRNA were detected by immunohistochemistry and real-time PCR. The data were analyzed with one-way anova and non-parametric test.Results:In the last week, the diet and physical activity of all nude mice were less than before, and they became thinner, which were more obvious among the mice in lobaplatin group and imrecoxib combined with lobaplatin group. Compared with the control group, the expression of PTEN protein and mRNA were significantly increased in imrecoxib group and imrecoxib combined with lobaplatin group (P<0.001, respectively). Compared with the control group, the expression of cortactin protein and mRNA were significantly decreased in imrecoxib group and imrecoxib combined with lobaplatin group (P<0.001, respectively). PTEN and cortactin protein, PTEN and cortactin mRNA had significantly negative correlation (r=-0.660, -0.983,P<0.001, respectively). Conclusion:Imrecoxib can inhibit non-small cell lung cancer invasion and metastasis which may be involved in upregu-lating PTEN protein and reducing cortactin protein. Imrecoxib could enhance the effect of lobaplatin chemotherapy.
9.Regulatory effect of hydrogen sulfide on endoplasmic reticulum stress and lipid metabolism in livers in apoE knockout mice

Objective To investigate the regulatory effect of hydrogen sulfide (H2S) on endoplasmic reticulum stress (ERS) and lipid metabolism in livers in apoE knockout (apo E-/-) mice.Methods Male C57BL/6 J mice and homozygous apoE mice were fed with a western type diet and randomly divided into four groups:C57BL/6 J control group (injected intraperitoneally with normal saline),apoE group (injected intraperitoneally with normal saline),apoE-/-+NaHS group (injected intraperitoneally with an H2S donor NaHS 56μmol · kg-1 · d-1) and apoE-/-+ DL-propargylglycine (PPG) group (injected intraperitoneally with an acystathionine-γ-lyase inhibitor PPG 30 mg ·kg-1 · d-1).After 10 weeks,all mice were sacrificed and plasma lipids were detected.Lipid deposition was determined by oil red O staining.Glucose-regulated protein78 (GRP78),Thr-981 phosphorylated double-stranded RNA-activated protein kinase like endoplasmic reticulum kinase (PERK),subunit of eukaryotic translation initiation factor-2α (elF-2α),low density lipoprotein (LDL) receptor (LDLR),sterol regulatory element binding protein-2 (SREBP-2) in the livers were detected by Western bloting.The expressions of GRP78 and SREBP-2 mRNA were analyzed by realtime polymerase chain reaction (RT-PCR).Results Compared with C57BL/6 J control group,plasma levels of total cholesterol (TC),triglyceride (TG) and low density lipoprotein (LDL),liver lipid content and expressions of GRP-78,PERK and eIF-2α were significantly increased in apoE-/-mice,but body weight did not change.Compared with apoE-/-mice,plasma LDL level was decreased,liver lipid deposition was improved,expressions of GRP-78 and PERK in livers were increased,and the ratio of p-eIF-2α/ eIF-2α was increased in apoE-/-+NaHS group,but expression levels of SREBP-2 and LDLR in liver did not change.Conclusions H2S decreases serum LDL level and liver lipid content,and up regulates GRP78 protein and mRNA expressions,promotes PERK and eIF2α phosphorylations,improves endoplasmic reticulum function,but has no effect on the expressions of SREBP-2 and LDLR in apoE-/-mice fed with a high-fat diet.
10.Evaluation of short axis systolic function of single left ventricles using velocity vector imaging

Objective To investigate the short axis systolic function of children with single-left ventricle(LV),and to appraise the clinical value of velocity vector imaging(VVI) on assessing it.Methods The study group consisted of 14 patients with single-LV.The control group consisited of 14 age-matched normal children.VVI was used to analyse the circumferential and radial strain and strain rate of regional single-LV at the level of papillary muscle.Results Compared with values in control group,single-LV circumferential strain values were significantly lower in anterior septum,posterior septum,anterior wall,lateral wall and inferior wall(all P <0.05).Single-LV circumferential strain rate values were significantly lower in anterior septum,posterior septum,anterior wall,lateral wall and posterior wall compared with controls(all P <0.05). In control group,circumferential strain and strain rate were higher in inferior septum and anterior septum compared with other segments( P <0.05).Single-LV radial strain values were significantly lower in all segments compared with values in control (P < 0.05).In control group,radial strain and strain rate of papillary muscle level showed no significantly difference( P >0.05).Conclusions Circumferential and radial systolic ventricular function are impaired in children with single-LV.VVI can beused as a quantitative tool in evaluating the short axis systolic function of single-LV.
