Objective:To explore the influence of health coaching technology on self-management ability and negative emotions in patients with cirrhosis. Methods:In this single-center, randomized, single-blind controlled trial, 90 patients with cirrhosis hospitalized from May to October 2019 were selected as study subjects by means of convenience sampling method. Subjects were included in the control group (routine chronic disease management in the department of gastroenterology) and the experimental group (health coach technical intervention) with random number table method, with 45 cases each. The self-management Behavior Scale for patients with cirrhosis and the Depression-Anxiety-stress scale (DASS-21) were used to evaluate the intervention effect. Results:Two cases in the experimental group were lost to follow-up, and 3 cases in the control group were lost to follow-up. Finally, 43 cases in the experimental group and 42 cases in the control group completed the study. After the intervention, the experimental group's self-management scores and total scores were (23.02±1.68), (25.07±1.45), (17.72±1.64), (18.95±0.90), (84.77±3.32) points, the control group were (17.14±1.49), (23.43±1.77), (15.24±1.95), (15.88±2.26), (71.69±3.85) points, the difference between the two groups was statistically significant ( t values were 4.678-16.955, P<0.05). The scores of DASS-21 were (8.05±1.73), (7.02±1.85), (12.40±2.20) points in the experimental group and (10.10±1.83), (9.05±2.39), (14.02±1.89) points in the control group. The differences between the two groups were statistically significant ( t values were -5.300, -4.379, -3.659, all P < 0.05). Conclusions:The health coaching technique can effectively improve the self-management ability of patients with cirrhosis and reduce their negative emotions.

Objective To study the effects of APBV (airway pressure release ventilation) / BIPAP(bipha-sic positive airway pressure) on lung recmitment/open maneuvers in piglets with acute lung injury. Method The model of acute lung injury (ALI) was induced by E. coll. intraperitoneal injection in piglets. Based APRV/BI-PAP model, the different pressure combinations (Phigh/Plow) of RMs increased gradually, such as RMI(30/15),RM2(35/20), RM3(40/25), RM4(45/30),RM5(50/35), RM6(55/40), RM7(60/45) cmH2O. The effects of stepwise RMs were studied by computed tomography (CT) at iaspiratory phase. Meantime the oxygen index (PaO2/FiO2), hemodynamic parameter and mean pressure of airway (Pmean) were continuously observed. The piglets were killed when RiMs finished and pulmonary pathological examination were done routinely by optical microscope. Data was analyzed by self-contrast method, using SPSS 11.5 software package. Results were expressed as mean ± standard deviation (x±s). Multiple comparisons were made with One-way ANOVA. Pearson correlative analysis was used to describe the relativity of PaO2/FiO2 and the collapsed alveolar area. Changes were considered as statistically significant if P value was less than 0.05. Results Eight piglets with ALl model were successfully made and all of them showed different degree of alveolar collapse under chest CT scan. During RMs their PaO2/FiO2 increased obviously (P<0.05) were decreased obviously (P<0.05) too, specially after RM2 finished (P<0. 05). But the alveolar over-inflatian could be found in some non-diseased area. The heart rate (HR) increased and mean artery blood pressure (MAP) decreased significantly while the pressure combinations (Phigh/Plow) of RMs were added gradual]y ( P<0.05). Meantime the Pmean and Ppeak inspiratory pressure (PIP) of airway and central venous pressure (CVP) were increased significantly ( P<0.05). But when RMs were finished, all of these indexes were hack to the levels of pre-RMs. Even there were no barowaumas happened, such as pneumothorax and pneumomedistinum, the alveolar overdistention and interruption of the alveolar separation still could be seen by pathologic examination. Conclusions RMs could be done well by APRV/BIPAP. Phigh/Plow (35/20cmH2O) would be the best pressure combination with more efficacy of RMs and less influence on hemodynamics, airway pressure indexes and others. When the effect of RMs was satisfied enough, setting Phigh/Plow to 30/15cmH2O for 20 mitt may maintain the good efficacy of RMs.

Objective:To investigate the related factors of length of stay in type 2 diabetes mellitus (T2DM) with urinary tract infection, and provide reference for clinical nursing. Methods:A retrospective analysis was used to collect patients ′ general demographic data, disease history, HbA1c, and other laboratory indicators, and etc. A total of 228 hospitalized patients with confirmed T2DM with urinary tract infection in Shanxi People ′s Hospital from January 2019 to September 2020 were selected as the study subjects. The patients were divided into observation group ( n=79) and control group ( n = 149) according to their length of stay. The electronic medical history and related laboratory examination indexes of the two groups during hospitalization in the Department of Endocrinology were analyzed retrospectively. The general demographic data, disease history, glycosylated hemoglobin and other laboratory indexes of the two groups were statistically analyzed. Results:The results of single factor analysis on the influence of length of stay showed that there were statistically significant differences in age, gender, medicare payment method, Charlson Complication Index (CCI), treatment, glycosylated hemoglobin, procalcitonin (PCT) ( χ2 values were from 5.107 to 20.134, t value was 3.946, Z value was -2.914, P<0.05). Logistic regression analysis showed that female ( OR=0.405, 95% CI 0.172-0.951), age≥70 years old( OR=0.528, 95% CI 0.263-0.922), insulin treatment ( OR=0.479, 95% CI 0.269-0.854), CCI≥3 points ( OR=2.555, 95% CI 1.346-4.851), payment method of new rural cooperative medical insurance ( OR=0.273, 95% CI 0.120-0.620), resident medical insurance ( OR=0.184, 95% CI 0.083-0.405), others ( OR=0.367, 95% CI 0.157-0.862), PCT( OR= 0.252, 95% CI 0.290-0.877), and HbA1c ( OR= 0.81, 95% CI 0.705-0.930) were the influencing factors for the length of stay in patients of T2DM with urinary tract infection. Conclusion:Patients have a longer hospital stay in women, age ≥70 years old, and insulin treatment, CCI≥3 points, HbA1c and PCT. Compared with the employee medical insurance, patients have a shorter hospital stay with new rural cooperative medical insurance, resident medical insurance and other payment methods. Therefore, clinical testing ability should be further improved to effectively reduce the pain and economic burden of patients.

OBJECTIVE: To determine the efficacy of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in the detection of radiation-induced myocardial damage in beagles by comparing two pre-scan preparation protocols as well as to determine the correlation between abnormal myocardial FDG uptake and pathological findings. MATERIALS AND METHODS: The anterior myocardium of 12 beagles received radiotherapy locally with a single X-ray dose of 20 Gy. 18F-FDG cardiac PET/CT was performed at baseline and 3 months after radiation. Twelve beagles underwent two protocols before PET/CT: 12 hours of fasting (12H-F), 12H-F followed by a high-fat diet (F-HFD). Regions of interest were drawn on the irradiation and the non-irradiation fields to obtain their maximal standardized uptake values (SUVmax). Then the ratio of the SUV of the irradiation to the non-irradiation fields (INR) was computed. Histopathological changes were identified by light and electron microscopy. RESULTS: Using the 12H-F protocol, the average INRs were 1.18 +/- 0.10 and 1.41 +/- 0.18 before and after irradiation, respectively (p = 0.021). Using the F-HFD protocol, the average INRs were 0.99 +/- 0.15 and 2.54 +/- 0.43, respectively (p < 0.001). High FDG uptake in irradiation field was detected in 33.3% (4/12) of 12H-F protocol and 83.3% (10/12) of F-HFD protocol in visual analysis, respectively (p = 0.031). The pathology of the irradiated myocardium showed obvious perivascular fibrosis and changes in mitochondrial vacuoles. CONCLUSION: High FDG uptake in an irradiated field may be related with radiation-induced myocardial damage resulting from microvascular damage and mitochondrial injury. An F-HFD preparation protocol used before obtaining PET/CT can improve the sensitivity of the detection of cardiotoxicity associated with radiotherapy.

[Objective] To evaluate the efficacy of autologous whole blood injections in patients with chronic spontaneous urticaria and positive autologous serum skin test (ASST).[[Methods]] After assessment of clinical history, patients with chronic spontaneous urticaria underwent skin prick test (SPT) and ASST.Then,100 patients with positive ASST but negative SPT for common allergens were randomly classified into treatment group (n =60) and control group (n =40).Oral loratadine was given to all the patients with a gradual tapering to the least maintenance dose.Patients in the treatment group were also injected with autologous whole blood once a week for 12 times.Patients were evaluated by urticaria activity score (UAS) and dermatology life quality index (DLQI) at the baseline, the end of the 3rd and 6th month after the initial treatment. The total amount of antihistamines required for the control of urticaria every month was calculated. The UAS,DLQI,accumulative amount of administrated antihistamines, and the diameter of wheal/flush induced by autologous serum were compared by t test before and after the treatment, and the efficacy was compared by rank sum test between the two groups.[Results] No significant difference was observed between the control and treatment group in UAS at the baseline (5.73 ± 0.51 vs.5.32 ± 0.79,P> 0.05).The UAS reached 1.57 ± 1.42 and 0.69± 0.92 with a decrease rate of 69% and 81% in the treatment group, and 3.65 ± 1.53 and 2.65 ± 1.61 with a decrease rate of 35% and 53% in the control group, respectively at the end of the 3rd and 6th month, and statistical difference was observed for the decrease in both groups at the two time points (all P < 0.05).The total amount of antihistamines required for the control of urticaria per month averaged 8.63 pills and 3.83 pills respectively in the treatment group after 3 and 6 months of treatment, significantly less than that in the control group (16.85 and 15.27 pills, respectively).[Conclusion]s The combination of oral antihistamine and autologous whole blood injections can not only reduce disease activity and improve patients' quality of life, but also decrease the total amount of antihistamines required for the control of urticaria.
