Objective To investigate the application of homemade stapler in anus-retained operation on low and super-low colon or rectal cancer.Methods From August 1991 to August 2006,this technique was used in 333 patients with low colon or rectal cancer,among which 96 patients were right hemicolon excision,ileum colon anastomosis,colon sigmoideum excision,colon descendens rectum anastomosis and 183 patients were rectum pro-excision,low pro-excision,54 patients were low pro-excision,super low anastomosis.The other traditional operations(59 patients manipulation anastomosis) were used in 166 patients.Results Fourteen patients had anastomotic fistula(one rectovaginal fistula) in group1 and two in group2.Two patients had pro-sacral bone bleeding,and one dead.Conclusion The stapling device was a safe and feasible choice in anus-saving operation of low rectal cancer.

ObjectiveTo explore the effects of personality characteristics and mental health of students on the career orientation.MethodsThe method of stratified random sampling was used in three universities of Jiangxi province.501 subjects of Arts,Science and Medical Science were tested with Holland career orientation test,symptom check list(SCL-90) and 16 personality factors(16PF).Structure validity analysis,analysis of variance and t -value test were employed with LISREL8.70 and SPSS13.0.Results ( 1 ) The proportion of students choosed the type of occupation were:social type 36％,management type 18％,conventional type 17％,reality type 14％,investigation type 9％,artistic type 6％ ; (2) Career orientation,personality characteristics and mental health had moderate effect on the career orientation.The correlation coefficients of personality characteristics and mental health to the vocational orientation were 0.63 and 0.44.The interaction correlation coefficient was 0.56.ConclusionPersonality characteristics and mental health of college students are important factors to the career orientation and decision-making of job seeking.The personality has stronger explanatory power than mental health.Great emphasis should be paid on both short-term psychological health of college students and long-term cultivation of healthy personality in the employment psychological guidance.

Objective To study the effects of sitting position on pulmonary hemodynamics and intrapulmonary shunt (Qs/Qt) during neurosurgery. Methods Twenty eight patients scheduled for posterior fossa and posterior cervical spinal surgery, were anesthetized with sevoflurane fentanyl pancuronium or enflurane fentanyl pancuronium after intravenous induction. Pulmonary hemodynamics were monitored with Swan Ganz catheter technique and Qs/Qt was calculated from the data of arterial and mixture venous blood gas analysis, including the following parameters: CI, RAP, MPAP, PCWP, PVRI, pHa, PaCO 2, PaO 2, PaO 2/FiO 2 and Qs/Qt. The detection time points were as the follow: before anesthesia induction, before sitting position, immediately ,60 min and 120min after sitting position, and at the supine position after termination of surgery.Results After induction of anesthesia and during operation at sitting position, CI decreased significantly (P0.05).Conclusions During neurosurgical procedure under general anesthesia and sitting position, pulmonary vascular auto regulation can be preserved and pulmonary oxygenation can be kept in normal.

Objective To evaluate the effect and safety of Xiaoyin Ointment for blood-heat syndrome of psoriasis and to study further its effect on single symptoms of psoriasis such as erythema,scale,infiltration,pruritus and lesion area. Methods The 99 selected patients were randomized into 3 groups and were treated by Xiaoyin Ointment,Shidu (Damp and toxin removal) Ointment,and Vaseline Ointment respectively for 8 weeks. All patients of 3 groups took the Keyin Yifang (First prescription for psoriasis) as the basic medication. After treatment,the general effect and the effect on skin lesion were evaluated objectively,and the time-effect indices and the safety were assessed as well. Objective To investigate the difference in expression profiles of micro-RNA (miR-NA) between human epidermal stem cells and epidermal keratinocytes.Methods (1) Human primary epidermal stem cells and keratinocytes were obtained with enzyme digestion method and type Ⅳ collagen coated chosen method.Growth of cells cultured in vitro was observed by inverted microscope.Monoclonal antibody of integrinβ1,keratin 1 (CK1),CK10,and CK19 were detected by immunocytochemical staining.(2)Total RNA was respectively isolated from epidermal stem cells and epidermal keratinocytes by Trizol-based single-step procedure,detected by formaldehyde denaturing gel electrophoresis,purified by mirVanaTM miRNA isolation kit,and then labeled and hybridized by miRNA labeling and hybridization kit.Images of hybridization were analyzed using the Feature Extraction (Version 10.7).Data normalization and difference analysis were performed with GeneSpring (GX 10.0).Moreover,miRNA microarray results were confirmed by RT-PCR.(3) Target genes of differently expressed miRNA were predicted.Results Epidermal stem cells exhibited rapid adherence to type Ⅳ collagen and formed distinct clones after 3 days of culture; expressions of integrinβ1 and CK19 were positive.Keratinocytes could not adhere rapidly to type Ⅳ collagen and formed few clones after 3 days of culture ; expressions of CK1 and CK10 were positive.(2) Of the epidermal stem cells,31 miRNAs were up-regulated and 153 down-regulated.Besides,significantly up-regulated miRNAs were hsa-miRNA-125b-3p,hsa-miRNA-197-5p,and hsa-miRNA-376a-3p,while significantly down-regulated miRNAs were hsa-miRNA-203,hsa-miRNA-29b-3p,and hsa-miRNA-34a-3p.Findings of RT-PCR on hsa-miRNA-197-5p and hsa-miR-29b-3p revealed high concordance with the microarray results.(3) Some miRNAs target genes indicated that miRNA related to cell proliferation,differentiation,apoptosis,aging and other biological characteristics.Conclusion Distinct differences in miRNA expression profiles are detected between human epidermal stem cells and keratinocytes and this may correlate closely with their different biological characteristics such as proliferation and differentiation ability.

Objective To study the pharmacokinetics of aristolochic acid A in Radix Aristolochiae and the compound preparation of Guanxinsuhe Capsule in mice in vivo after single-dose oral administration and observe the difference of aristolochic acid A absorption and distribution. Methods Aristolochic acid A assay was performed by RP-HPLC on a Waters apparatus with a DiamonsilTM C18 column (250 mm × 4.6mm, 5 μm), a mobil phase: a mixture of methanol-water-acetic acid (72: 27 : 1), flow rate: 1.0 mL/min, detection wavelength: 315 nm, and column temperature: 20 ℃. Results Mice were given Radix Aristolochiae and Guanxinsuhe Capsule by ig at the same level of 2. 5 mg/kg of aristolochic acid A, respectively, which were suspended in 0. 3% CMC-Na solution. Plasma concentrations were determined by RPHPLC. After single-dose ig administration of Radix Aristolochiae or Guanxinsuhe Capsule to mice, the mean plasma concentration-time courses of aristolochic acid A obtained fitted the one-compartment model.The main pharmacokinetic parameters of aristolochic acid A in Radix Aristolochiae, t1/2ka, t1/2 ke, tmax,AUC, Cmax are 5. 103 min, 43. 63 min, 17.89 min, 80. 45 (μg · min)/mL, and 0. 916 8 μg/mL; the rela tive pharmacokinetic parameters in Guanxinsuhe Capsule are 5. 294 min, 43.50 min, 18. 32 min, 33.08(μg · min)/mL, and 0. 381 8 μg/mL. Conclusion The Cmax of aristolochic acid A in Guanxinsuhe Capsule is significantly less than that in Radix Aristolochiae, which indicates that the compound compability could decrease the absorption of aristolochiae acid A.

AIM:To prepare Wurenchun Dispersible Tablet(Fructus schisandrae Chinensis)and investigate the dissolution of it.METHODS:The optimal formulation of Wurenchun Dispersible Tablets was established by the single factor experiments.The dissolution of deoxyschisandrin from Wurenchun Dispersible Tablets was determined by HPLC.RESULTS:Wurenchun Dispersible Tablets prepared by the mixture of 50 g extraction and 125 g CaHPO_4 plus excipent(MCC∶CMS-Na 3∶1)could disintegrate and disperse well within 3 min in(20?1)℃ water.The dissolution of deoxyschisandrin from Wurenchun Dispersible Tablets was higher than 80% in 15 min.CONCLUSION:Wurenchun Dispersible Tablets were stable,dissolved fast and completely.The preparation could promote the dissolution of total lignans.

Objective To assess clinical results between proximal-first sequential anastomosis technique (POPCAB) and conventional technique off-pump coronary artery bypass grafting(COPCAB) in 72 patients over 65 years old.Methods Between Jan 2008 and Oct 2012,72 patients with multiple vessel coronary artery disease underwent off-pump coronary artery bypass grofting.They were divided into two groups,POPCAB and COPCAB groups.Preoperative and postoperative variables of two groups were compared.Results There were no significant difference in sex,age,history of acute myocardium infarction,hypertension,diabetes and number of bypass grafts between two groups.The ICU stay time in POPCAB group was less than in COPCAB group(P ＜0.05).There were lower creatine kinase MB and cardiac trop I after operation in POPCAB group compared with COPCAB group(P ＜ 0.05).Conclusion The results confirm that promixal-first sequential anastomosis technique remains a valuable option of surgical myocardial revascularization,and may optimize the outcome in the aged.

Objective To compare the clinical effect of laparoscopic and open appendectomy in the treat-ment of perforated appendicitis.Methods 107 patients with perforated appendicitis were selected as the research object and were divided into two groups.The observation group(54cases)received laparoscopic appendectomy while the control group (53cases)received traditional open appendectomy.The size of incision,the amount of bleeding during operation,time of operation and hospitalization and other perioperative indexes and the incidence rate of complications were compared between the two groups.Results The operation time between the two groups had no statistical significance(t =1.286,P >0.05 ).The incision size [(2.5 ±0.3)cm],the amount of intraoperative bleeding[(10.3 ±3.4)mL]and hospitalization time[(5.4 ±0.9)d]of the observation group were better than those of (4.2 ±0.4)cm,(19.4 ±4.2)mL and (6.5 ±1.1 )d in the control group,the differences were statistically significant(t =24.901,12.329,5.666,all P <0.05).The occurrence rate of complications of the observation group was 3.4%,which was lower than 15.1% of the control group,the difference was statistically significant(χ2 =3.940, P <0.05 ).Conclusion Laparoscopic appendectomy has the advantages of less trauma,faster recovery,less complications,shorter hospitalization time,etc.And its effect is better than open appendectomy and worthy of populari-zation and application.

Objective · To study the electrophysiological effect of (S)-OTS·HCl on the heart. Methods · The conventional intracellular recording, electrocardiograph (ECG) and Langendorff cardiac perfusion technique were employed to investigate the effect of (S)-OTS·HCl on in-vivo and in-vitro hearts of guinea pigs and rabbits. Results · (S)-OTS·HCl could bind to M2 muscarinic receptors and dose-dependently prolong the RR intervals significantly in vivo. It had no effect on resting potential (RP), action potential amplitude (APA), and maximum upstroke velocity of phase 0 (Vmax) of ventricular myocytes. Instead, 1×10-5 mol/L (S)-OTS·HCl could shorten the action potential duration at 50 percent repolarization (APD50) and APD90 to 91.6% and 90.9%, respectively. And the spontaneous depolarization rate of phase 4 (SDR) of sinus nodes was reduced to its 13.7% when rabbit sinus nodes were exposed to 1×10-7 mol/L (S)-OTS·HCl. (S)-OTS·HCl could inhibit Ca2+channel effectively. It decreased APA and Vmax of sinus nodes and attenuated the cardiac contractility in vitro. Conclusion · (S)-OTS·HCl is a potent cholinergic agonist and has negative chronotropic, dromotropic, and inotropic effects on hearts via binding to M2 muscarinic receptors.
