1.Suggestions and Analysis on the Status Quo of Chinese Prescription-checking Pharmacists

OBJECTIVE:To cultivate qualified prescription-checking pharmacists and to ensure patients'rational drug use. METHODS:The status quo of prescription-checking pharmacists in China was analyzed and some concrete suggestions on how to cultivate qualified prescription-checking pharmacists were made.RESULTS:The phenomena of low educational level and low professional title,lack of professional knowledge and medical knowledge,lack of staff members were widespread in today's Chinese prescription-checking pharmacist professionals.CONCLUSION:The rational drug use can only be realized through being fully aware of the importance of prescription-checking pharmacists,enhancing whose admittance qualifications,working out professional standards for whom,clearly defining their responsibilities,strengthening their cultivation and increasing their numbers.
2.Autologous nucleus pulposus induces rat dorsal root ganglion damage without mechanical compression

BACKGROUND: Resent studies demonstrate that sciatica is caused by inflammatory reaction of nucleus pulposus, and dorsal root ganglion (DRG) is in involved in this process; however, its pathophysiological changes remain poorly understood. OBJECTIVE: To explore pathogenesis of sciatica related to nucleus pulposus protrusion without mechanical compression. METHODS: Totally 24 male SD rats were randomly divided into the experimental group and control group, with 12 rats in each group, The coccygeal intarvartebral discs of rats were opened to obtain 5 coccygeal nucleus pulposus, mixed with 50 μL physiological saline to prepare suspension. Rats in the experimental group were prepared non-compressive models by injecting suspension to the epidural cavum of rats. Meantime, the same volumes of physiological saline were injected into rats in the control group. The rat hind paw mechanical withdrawal thresholds were measured, pain-related behavior and dorsal root ganglion morphology were observed. RESULTS AND CONCLUSION: In the absence of mechanical pressure, disappearance of the nucleolus, degeneration of the nuclear membrane and significant hyperalgesia derived by auto-transplantation of nucleus pulposus to the epidural cavum of rats were observed. Dorsal root ganglion cells often had hyperemia, edema and intracytoplasmic vacuoles, with large and irregular nuclei. Prominent Nissl bodies were not seen. The Inflammatory reaction derived by autograft nucleus pulpcsus is an important factor in DRG injury and sciatica.
3.The level and clinical significance of serum anti-lysosomal associated membrane protein-2 antibody in patients with ANCA-associated glomerulonephritis

Objective To investigate the relationship of the serum anti-lysosome associated membrane protein 2 (LAMP-2) antibody levels and anti-neutrophil cytoplasmic antibodies (ANCA)-associated glomerulonephritis.Methods Thirty-three patients with new onset ANCA-associated glomerulonephritis and thirty healthy controls were enrolled.ANCA detection was performed using indirect immunofluorescence (IIF).Enzyme-linked immunosorbent assay (ELISA) was used to detect myeloperoxidase (MPO),proteinase-3 (PR3) and other ANCA-associated antibodies including LAMP-2.The cut-off value of the serum anti-LAMP-2 antibody was determined by a receiver operating characteristic (ROC) curve.Results The serum levels of anti-LAMP-2 antibody in new onset ANCA-associated glomerulonephritis patients were significantly higher than remission stage ANCA-associated glomerulonephritis patients and healthy controls (P < 0.05).The serum levels of anti-LAMP-2 antibody showed no visible difference between the remission ANCA-associated glomerulonephritis patients and healthy controls (P > 0.05).The levels of anti-LAMP-2 antibody showed a strong positive correlation with ESR,Scr,BUN,proteinuria,crescent proportion and Birmingham vasculitis activity score (BVAS) and a negative correlation with Ccr,Hb and Alb.Conclusions Anti-LAMP-2 antibody is correlated with the activity of ANCA-associated glomerulonephritis and the severity of renal damage.It may be a useful indicator on the activity of ANCA-associated glomerulonephritis.

5.Percutaneous central venous catheter drainage for interventional treatment of complications in abdominal surgical diseases

Objective To discuss the effica of central venous catheter drainage in interventional treatment of abdominal surgical complications.Methods The clinical data of 72 cases of abdominal surgical diseases were retrospectively analyzed,the complications included 23 cases of ascites,19 cases of bile leakage,8 cases of pancreatic fistula,7 cases of pancreatic necrosis and 15 cases of abdominal abscess,they all underwent percutaneous central venous catheter drainage under the guidance of ultrasound or CT.Results Central venous catheter was placed in 72 cases successfully in one time,the cathetem were kept for 2 to 37 days,5 discharged cases with catheter underwent regular reexamination in the clinic.3 cases of ascites were replaced for catheter shedding; 17 cases for occlusion recieved catheter irrigation and guidewire dredging;2 cases of pancreatic necrosis and 1 case of peripancreatic abscess were cured by repeated puncture and replacement of large-bore catheter.2 cases of pancreatic necrosis,1 case of pancreatic fistula and 1 case of appendicitis underwent laparotomy finally; 1 case of postcholecystectomy bile leakage underwent endoscopic stent therapy; 1 cases of severe pancreatitis complicated with ARDS was treated in ICU.Conclusion Central venous catheter drainage guided by ultrasound or CT for treatment of abdominal surgical complications was not only safe but also effective,and is worthy of popularization and application.
6.Complications after carotid artery stenting in patients with carotid artery stenosis

Objective To analyze complications of carotid artery stenting (CAS) and preventions.Methods Clinical data and treatment outcomes of 72 consecutive patients ( 80 stents ) from July 2006 to January 2012 with carotid stenosis were analyzed.Asprin 100 mg and clopedigrel 75 mg were given orally 5 days before CAS.Distal embolic protection device were implanted in all patients,pre-dilatation was done for those with carotid stenosis > 90% and post-dilatation was done for those of residual stenesis > 30%.Severe complications of CAS mean death,myocardial infarction (MI) and stroke.Other minom included transient ischemic attack ( TIA ),hyperperfusion and intracranial hemorrhage ( ICH ),bradycardia and/or hypotension,hypertension,access hematoma or bleeding.Results In 72 patients a total of 80 self-expandable bare stents were successfully implanted.Distal embolic protection devices were used in all cases.Combined procedure was taken in 5 cases including OPCABG in 2 cases,left subclavain artery stenting in 2 cases and renal artery stenting in 1 case.The overall in-hospital complications was 37.5% (27 of 72).Of these events,1 case had minor strokes defined as a modified Rankin Scale score less than 3 at 1-year follow-up,2 patients (2.78% ) experienced a hemispheric TIA (neurological symptoms that resolved within 24 hours),1 patient experienced hyperperfusion syndrome.The overall mortality rate was 0,21 cases (29.2% ) experienced hemodynamic instability (hypotension in 15 cases,bradycardia in 5 cases and hypertension in 1 case) and 2 others had access hematoms.The 30-day death/stroke/myocardial infarction risk was 1.39% (1 minor stroke).Conclusions Hemedynamic instability (hypotension and bredycardia) is main complications of carotid artery stenting in patients with carotid artery stenosis,severe complications are rare.
7.Studies on characteristics and function of alpha2-adrenoceptors intact lymphocytes isolated from rat spleen

~3H-Clonidine, a potent and selective alpha2-adrenergic agonist was used to label alpha2-a-drenoceptors in intact lymphocytes isolated from rat spleen. Binding of ~3H-Clo-nidine was rapid(t1/2: 2min)and readily reversed by 10umol?L~(-1) clonidine(t1/2: 3-4min). ~3H-Clonidine saturationexperiments indicated a single c1ass of site with a K_D of 6.57?1.63nM and Bmax of 72.4?13.4 fmol/10~7 lymphocytes. Adrenergic agonists competed for ~3H-clonidine binding site with anorder of potency:epinephrine)norepinephrine)isoproterenol. These results show the presence ofalpha2-adrenoceptors in splenic lymphocytes. Computer analysis of competition experiments withadrenergic agonists revealed three classes of sites: high affinity site, medium affinity site and lowaffinity site. The affinity of high affinity site is 2-3 orders of magnitude higher than the one ofmedium affinity site, whereas the latter is the same orders higher than low affinity site Using im-proved Mishell-Dutton method, 10umol?L~(-1) clonidine suppressed the Ig M synthesis and thesuppression was blocked by 10 umol?L~(-1) phentolamine. These results indicate the suppression ofIg M antibody response to SRBC in vitro by clonidine is mediated by the alpha-adrenoceptors.

OBJECTIVE:To prepare Weikang capsule and establish its quality standard METHODS:Weikang capsule was prepared with gentamycin sulfate,vitamin B1,vitamin B2,vitamin B6 and vitamin B12 A polarimetry was established for determining the content of gentamycin sulfate in Weikang capsule RESULTS:There was a good linearity between optical rotations and concentrations of gentamycin sulfate from 2 000IU/ml to 12 000IU/ml with a regression equation of ?=0 00 767+1 77 003C,r=0 9 999 The average recovery rate of gentamycin sulfate in Weikang capsule was 100 1%,RSD was 0 79%(n=6) The results of polarimetry and microbioassay were nearly equivalent CONCLUSION:The preparation process of Weikang capsule is simple,and polarimetry for determination of the content of gentamycin sulfate in Weikang capsule wes rapid and reliable

Retinol binding protein 4 (RBP4)was a class of secreting protein,mainly synthesized by the liver,widely distribu-ted in the human body blood,urine and other body fluids.It plays an important role in assisting the physiological function of vitamin A[1].Recent research shows that RBP4 was a new kind of adipocytokine,participated in insulin resistance and occur-rence of type 2 diabetes,and had a closed relationship with diabetic nephropathy,nutritional disease.This article describes the function of RBP4,review the relationship between RBP4 and nutritional or other type of diseases,and new clinical detec-tion method with RBP4.
10.The analysis of pulmonary infect flora distribution and risk factors of stroke patients with tracheotomy in intensive care unit

Objective To investigate pulmonary infect flora distribution characteristics of stroke patients with tracheotomy in Intensive care unit (ICU) and analyze the risk factors.Methods A total of 792 cases of ICU stroke patients was selected,including 426 cases of tracheotomy patients,and 366 cases of non-tracheotomy patients.The incidence of lung infections was compared.Bacteriological examination was used for tracheotomy bacteriological studies for lung infected stroke patients with tracheotomy.Bacteria infection's characteristics was observed.The risk factors were analyzed.Results (1) The tracheotomy patient 's lung infection rate was 23.00%,higher than 9.56% of the non-tracheotomy patients,the difference was statistically significant (X2 =19.125,P <0.05);(2) For lung infection-occurred patients with tracheotomy of ICU,gram-negative bacteria infection rate was 63.97%,significantly higher than 19.85% of gram-positive bacteria and 16.18% of fungi,the difference was statistically significant (x2 =18.255,17.042,P < 0.01);(3) Lung infection rate of ICU stroke tracheotomy patients with unconscious,dysphagia,hospitalization time > 14 d,blood glucose levels ≥≥7.8 mmol/L,and based diseases was significantly higher than that of patients with conscious,non-dysphagia,hospitalization time ≤<14 d,blood sugar level <7.8 mmol/L,and no based diseases (P < 0.05);(4) Consciousness,dysphagia,hospitalization,blood sugar levels,and the underlying disease were the independent risk factors of lung infection in ICU stroke tracheotomy patients (OR1 =11.528,OR2 =8.046,OR3 =15.174,OR4 =7.795,ORs =10.784,P <0.05).Conclusions Gram-negative bacteria is the main reason for pulmonary infections in ICU stroke tracheotomypatients patients with stroke.State of consciousness,invasive treatment,hospitalization,blood sugar levels,and the underlying disease are the independent risk factors.
