Objective To compare the efficacy and safety of Foley catheter (FC) and prostaglandins (PG) for cervical ripening used in labor induction of late pregnant women with unfavorable cervix. Methods Several databases including Cochrane Library,Embase,PubMed,Elsevier,Wanfang Medical Database, CNKI, VIP were searched for collecting the related literatures according to criteria for inclusion and exclusion.Meta analysis was performed by applying software Review Manager 5.2. Results Thirty randomized controlled trials were included. The FC group presented significantly lower risk of excessive uterine contraction [OR= 0.38,95%CI(0.19,0.75),P<0.05] than the PG group. There was no significant difference between the two groups in terms of Cesarean section rate, vaginal delivery rate, Bishop score improvement, duration of induction to delivery, neonatal birth weight, or in neonatal outcome. Conclusion This study shows that FC has good promoting effect of cervical maturity and maternal ̄ neonatal outcomes, and rarely produces excessive uterine contraction. It is safe and equally effective as PG does. The approach is easy to operate and worth to apply in clinical use.

To establish a method of directional differentiation and efficient production of neurons from embryonic stem cells (ES cells) in vitro, based on the 4-/4+ protocol described by Bain, a new method was established to induce ES cells differentiating into neurons by means of three-step differentiation using all-trans retinoic acid (ATRA) combined with astrocyte-conditioned medium (ACM) in Vitro. The totipotency of ES cells was identified by observation of cells' morphology and formations of teratoma in immunocompromised mice. The cells' differentiation was evaluated continuously by the detection of the specific cellular markers of neural stem cells, neurons and astrocytes, including nestin, NSE and GFAP using immunohistochemistry assay. The NSE positive cells' ratio of the differentiated cells was determined by flow cytometry. It was found that the transparent circular clusters surrounding embryoid bodies induced with combining induction protocol formed just after 24 h and gradually enlarged later. This phenomenon could not be observed in EBs induced only by ATRA. The NSE positive cells' ratio in the cells induced with ATRA and ACM was higher than that of the cells induced by ATRA at different time points of differentiation, and finally reached up to 73.5% among the total differentiated population. It was concluded that ES cells could be induced into neurons with high purity and yield by means of inducing method combining with ATRA and ACM.

Objective To diagnose a rare variant Hb Shaare Zedek in a Chinese family, and describe its clinical feature. Methods Blood samples of seven members of three generation were collected.Standard blood routine analysis, hemoglobin electrophoresis and blood gas analysis were used for phenotype analysis.Gap-PCR and reverse dot blot analysis were used to detect common thalassemia mutations.DNA sequence analysis of the human α and β globin genes were used to identify the mutation site of these samples. Results The result of blood routine analysis was normal in the proband, but a abnormal hemoglobin band (22.4%,capillary assay) was found by hemoglobin electrophoresis. In the blood gas analysis, the partial pressure of oxygen and blood oxygen saturation were 72.0 mm Hg ( 1 mm Hg =0.133 kPa) and 93.0% in the proband. The heterozygous mutations in αl globin gene at codon 56 ( AAG > GAG)which leaded Glu substitution to Lys were identified in the proband.Other family members who carried the same mutation showed similar phenotype, with abnormal hemoglobin band ( 22.4% - 23.9%,capillary assay),low partial pressure of oxygen (59.0 - 72.0 mm Hg) and blood oxygen saturation (91.0% -93.0%).Conclusions The heterozygote of Hb Shaare Zedek leads to slight symptoms with abnormal hemoglobin band, decreased partial pressure of oxygen and blood oxygen saturation.Discovery of this mutation enriches the abnormal hemoglobin spetrum of Chinese people, and it is useful for the clinical diagnosis and genetic counseling of hemoglobinopathies.

OBJECTIVE:
To explore the genetic basis for a fetus with hydrocephalus.
METHODS:
The fetus was found to have hydrocephalus upon ultrasonography duringthe second trimester. Following induced abortion, fetal tissue was collected for the extraction of DNA and whole exome sequencing.Sanger sequencing was used to verify the suspected variants in the family.
RESULTS:
The fetus was found to harbor a hemizygous c.620A>G (p.Tyr207Cys) variant of the L1CAM gene (OMIM 308840),for which his mother and sister were heterozygous carriers. The same variant was not found in his father, uncle and grandparents.Based on the standards and guidelines of the American College of Medical Genetics and Genomics, the variant was predicted to be likely pathogenic (PM1+PM2+PP3+PP4).
CONCLUSION
The hemizygous c.620A>G (p.Tyr207Cys) variant of the L1CAM gene probably underlay the hydrocephalus in this fetus.

Objective:
The aim of this study was to compare the efficacy and safety of metformin/thiazolidinediones (TZDs) / glucagon-like peptide 1 (GLP-1) analogs (triple therapy) with conventional glucose-lowering therapy(conventional therapy) for patients with type 2 diabetes and metabolic syndrome.
Methods:
A prospective randomized-controlled 26-week study was carried out. A total of 82 patients with type 2 diabetes and metabolic syndrome were randomized to receive either triple therapy protocal or just conventional therapy, altogether with 41 cases in each group.
Results:
HbA1C value was significantly reduced in triple therapy group versus the conventional therapy group [(2.23±1.75)%
