1.Discussion on the Improvement of Ethical Review Level of Ethics Committee Based on the Construction of Ethical Review Platform of Traditional Chinese Medicine

Though there is a great progress in research of medical ethics in China, a gap still remains between China and the developed countries now. Medical ethics committees are established one after another in many hospitals, but the level of standardization construction and ethical review level differ in thousands ways. The construction of ethical review platform on TCM can standardize the construction and investigation procedure, and can further enhance the ethical review level of medical ethics committee. Among them, institutional improvement of regulations and procedures can be acquired in short time, but the enhancement of ethical review level needs a long way to go. This article aimed to probe the thinking and methods on the enhancement of ethical review level through the practice and experience of the construction of TCM ethical review platform.
2.Central Mechanism of Acupuncture at Baihui for Memory Impairment after Stroke: A Resting- state Functional Magnetic Resonance Imaging Study

Objective To explore the central mechanism of acupuncture at Baihui (GV20) for memory impairment after stroke. Methods 32 stroke patients were randomized to receive acupuncture at Baihui (GV20) (as observation group) and acupuncture at Yanglingquan (GB34) (as control group) for 8 weeks. At the meantime, all participants received routine treatment including physical and occupational therapies. They were scanned with resting-state functional magnetic resonance imaging (fMRI) to find functional connectivity and assessed with Wechsler Memory Scale (WMS) before and after treatment. The results of WMS and functional connectivity were analyzed with Pearson's correlation. Results The memory scores and memory quotient improved significantly after treatment in the observation group (P<0.05). The functional connectivity significantly increased occurred in the left hippocampus to right inferior frontal gyrus, right middle frontal gyrus and left inferior frontal gyrus; and right hippocampus to left middle frontal gyrus, left superior frontal gyrus and left parietal lobe. Significant correlations were found between memory quotient and functional connectivity of hippocampus to frontal lobe and left parietal lobe in the observation group. There was no statistical significance in memory scores and memory quotient in the control group. The functional connectivity significantly decreased in left hippocampus to right middle occipital gyrus, and right hippocampus to right superior temporal gyrus and right posterior lobe of cerebellum. There was no statistical correlation between functional connectivity and results of WMS. Conclusion The acupuncture at Baihui can improves memory ability of stroke patients, which may associate with the increase of functional connectivity of hippocampus with frontal lobes and parietal lobes.
3.Genotyping of Candida Strains from Patients with Vulvovaginal Candidiasis by Use of Random Amplified Polymorphic DNA

Objective To genotype Candida strains by random amplified polymorphic DNA(RAPD)technique,and to analyse the analogy of different Candida strains based on similarity coefficient.Methods Candida strains were isolated from vaginal secretion taken from patients with vulvovaginal can-didiasis by Sabouraud' s dextrose agar,C.albicans was differentiated from non-C.albicans strains by germ tube test,chlamydospore test and CHROMagar-Candida,and further identified by API20c Aux test Kits.The genomic DNA of45Candida isolates,including Candida albicans,Candida glabrata,Candida krusei,Candida parapsilosis and Candida tropicalis were amplified by RAPD,and DNA polymorphisms of inter-species and intraspecies were evaluated by using the DNA pattern-clustering of5primers.Results Thirty strains of C.albicans could be classified into7groups,and further divided into19subgroups,while15strains of non-C.albicans could be identified to species level.C.albicans isolates were related to each other with the similarity coefficients of more than90%,while different Candida species were connected to each other with the similarity coefficients of80%~90%.Conclusion C.albicans,which is the predominant pathogen of vulvovaginal candidiasis,could be classified into different genotypes by use of RAPD method.
4.In Vitro Anti-candidal Activity of the Essential Oil of Illicium verum

Objective To investigate the in vitro anti-candidal activity of the essential oil of Illicium verum (EOIV) alone and in combination with fluconazole. Methods The minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) of EOIV were determined in 130 clinically isolated Candida strains by NCCLS-M27-A microdilution method, and fluconazole was used as positive control. Meanwhile the checkerboard microdilution method was applied to assay the combined effect of EOIV and fluconazole in 18 candidal strains. Results For the 18 candidal strains the MICs and the MFCs of EOIV were 1 517.16 ?g/mL and 2 248.55 ?g/mL for C. albicans, 1 169.24 ?g/mL and 2 338.49 ?g/mL for C. glabrata, 1 320.03 ?g/mL and 1 741.79 ?g/mL for C. parapsilosis, 1 203.50 ?g/mL and 2 407.01 ?g/mL for C. tropicalis, 1 516.32 ?g/mL and 2 144.40 ?g/mL for C. krusei, and 1 072.64 ?g/mL and 2 144.40 ?g/mL for C. guilliermondii, respectively. Significant synergistic and additive effects were observed after the combination of EOIV with fluconazole, and no antagonism was found. There was no significant difference in the mean fractional inhibitory concentration index (FICI) between the fluconazole-susceptible and the fluconazole-resistant candidal strains (P = 0.671). Conclusion EOIV has antifungal effects on medically important Candida spp.. The combination of EOIV with fluconazole presents a synergistic and additive effects.
5.Observations on theEfficacy of Wrist-ankle Acupuncture plus Continuous Exercise Therapy for Post-stroke ShoulderPain

ObjectiveTo investigate the clinical efficacy of wrist-ankle acupuncture plus continuous exercise therapy for post-stroke shoulder pain.MethodsEighty patients with post-stroke shoulder pain were randomly allocated to treatment and control groups, 40 cases each. The control group received continuous exercise training with an upper limb multi-joint rehabilitation training instrument and the treatment group, wrist-ankle acupuncture in addition. The affected limb SEP N9 and N13 latency values, shoulder pain severity (the VAS score) and the upper limb motor function score were observed in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.ResultsThe total efficacy rate was 90.0% in the treatment group and 75.0% in the control group; there was a statistically significant difference between the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in SEP N9-N13 wave interval on the affected side in the two groups (P<0.01). There was a statistically significant post-treatment difference in SEP N9-N13 wave interval on the affected side between the treatment and control groups (P<0.05). There were statistically significant pre-/post-treatment differences in the VAS score and the Fugl-Meyer score in the two groups (P<0.01). There were statistically significant post-treatment differences in the VAS score and the Fugl-Meyer score between the treatment and control groups (P<0.01).ConclusionsWrist-ankle acupuncture plus continuous exercise therapy is an effective way to treat post-stroke shoulder pain.
6.Research progress in animal models of oral squamous cell carcinoma and common oral mucosal diseases

The pathogeneses of oral squamous cell carcinoma and most oral mucosal diseases are unclear. Therefore, establishing animal models with similar pathogeneses is significant for clinical prevention, diagnosis, and treatment of related diseases. At present, scholars have established animal models for different focuses. This paper aims to introduce the methods for establishing animal models of oral squamous cell carcinoma and common oral mucosal diseases, compare their advantages and disadvantages, and provide evidence for related basic research.
7.Meta-analysis on the clinical effects of allogenic acellular dermal matrix treatment for diabetic foot ulcer

Objective To analyze the clinical effects of allogenic acellular dermal matrix (ADM) treatment for diabetic foot ulcer (DFU) with meta-analysis.Methods Databases including PubMed,Cochrance Library,Embase,and OVID EBM Reviews were searched using key words " diabetic foot,diabetic ulcer,diabetic wound,acellular dermal matrix,acellular dermal regenerative tissue matrix,acellular regenerative tissue matrix,AlloDerm,SureDerm,KaroDerm,Graftjacket,Hyalomatrix PA,and Hyalograft 3D",and Chinese Biological Abstracts,Chinese Journals Full-text Database,VIP Database,and Wanfang Database were searched using key words in Chinese version”糖尿病足,糖尿病足溃疡,异体脱细胞真皮基质,真皮基质”to obtain the published trials of allogenic ADM treatment for DFU from January 1990 to July 2015,and then meta-analysis was used to analyze the trials.The measurement indexes were wound contraction rate,wound healing rate,complete epithelization time of wound,and complication rate.Meta-analysis was conducted by RevMan 5.2 statistical software.Results A total of 5 trials involving 426 DFU patients were included,with 224 patients in group ADM who received allogenic ADM treatment and 202 patients in conventional treatment group (CT) who received conventional moist treatment.There was no statistically significant difference between group ADM and group CT in wound contraction rate,with mean difference 20.34 (with 95% confidence interval-25.27-65.94,P =0.38).The wound healing rate of patients in group ADM was higher than that in group CT,with relative risk (RR) 1.90 (with 95% confidence interval 1.29-2.80,P <0.01).There was no statistically significant difference between group ADM and group CT in complete epithelization time of wound,with mean difference 1.20 (with 95% confidence interval-2.93-0.52,P =0.17).The complication rate of patients in group ADM was lower than that in group CT,RR =0.54 (with 95% confidence interval 0.38-0.76,P < 0.001).Publication bias might exist in wound contraction rate,wound healing rate,complete epithelization time of wound,and complication rate.Conclusions Compared with conventional moist treatment,allogenic ADM treatment can accelerate wound healing and reduce complication rate in patients with DFU.

The continuous advancement of molecular detection technology has greatly propelled the develop-ment of precision medicine for lung cancer.However,tumor heterogeneity is closely associated with tumor metastasis,recurrence,and drug resistance.Additionally,different lung cancer patients with the same genetic mutation may exhibit varying treatment responses to different therapeutic strategies.Therefore,the development of modern precision medicine urgently requires the precise formulation of personalized treatment strategies through personalized tumor models.Lung cancer organoid(LCO)can highly simulate the biological characteristics of tumor in vivo,facilitating the application of innovative drugs such as antibody-drug conjugate in precision medicine for lung cancer.With the development of co-culture model of LCO with tumor microenvironment and tissue engineering technology such as microfluidic chip,LCO can better preserve the biological characteristics and functions of tumor tissue,further improving high-throughput and automated drug sensitivity experiment.In this review,we combine the latest research progress to summarize the applica-tion progress and challenges of LCO in precision medicine for lung cancer.
9.Machine Learning-Based Prediction of COVID-19 Severity and Progression to Critical Illness Using CT Imaging and Clinical Data

Objective:
To develop a machine learning (ML) pipeline based on radiomics to predict Coronavirus Disease 2019 (COVID-19) severity and the future deterioration to critical illness using CT and clinical variables.
Materials and Methods:
Clinical data were collected from 981 patients from a multi-institutional international cohort with real-time polymerase chain reaction-confirmed COVID-19. Radiomics features were extracted from chest CT of the patients. The data of the cohort were randomly divided into training, validation, and test sets using a 7:1:2 ratio. A ML pipeline consisting of a model to predict severity and time-to-event model to predict progression to critical illness were trained on radiomics features and clinical variables. The receiver operating characteristic area under the curve (ROC-AUC), concordance index (C-index), and time-dependent ROC-AUC were calculated to determine model performance, which was compared with consensus CT severity scores obtained by visual interpretation by radiologists.
Results:
Among 981 patients with confirmed COVID-19, 274 patients developed critical illness. Radiomics features and clinical variables resulted in the best performance for the prediction of disease severity with a highest test ROC-AUC of 0.76 compared with 0.70 (0.76 vs. 0.70, p = 0.023) for visual CT severity score and clinical variables. The progression prediction model achieved a test C-index of 0.868 when it was based on the combination of CT radiomics and clinical variables compared with 0.767 when based on CT radiomics features alone (p < 0.001), 0.847 when based on clinical variables alone (p = 0.110), and 0.860 when based on the combination of visual CT severity scores and clinical variables (p = 0.549). Furthermore, the model based on the combination of CT radiomics and clinical variables achieved time-dependent ROC-AUCs of 0.897, 0.933, and 0.927 for the prediction of progression risks at 3, 5 and 7 days, respectively.
Conclusion
CT radiomics features combined with clinical variables were predictive of COVID-19 severity and progression to critical illness with fairly high accuracy.
10.A Meta-analysis of quantitative evaluation of lumbar intervertebral disc degeneration by functional MRI T1ρ

Objective To investigate the change rule of T1ρ value in the process of lumbar intervertebral disc degeneration(IVDD)based on Pfirrmann grading by Meta-analysis.Methods PubMed,EMBASE,Cochrane Library,CNKI,Wanfang Data,VIP and Sinomed were searched to collect studies on quantitative assessment of IVDD using T1ρ imaging technology.The retrieval time limit was from the establishment of the database to December 20,2022.Meta-analysis was performed using RevMan 5.4 and Stata 14.0 software.Results A total of 12 articles were included,and the numbers of Pfirrmann grade Ⅰ-Ⅴ lumbar discs were 316,1 460,769,430 and 98,respectively.T1ρ relaxation time decreased gradually with the increase of the grade of degeneration.The T1ρ values of grade Ⅰlumbar discs were significantly higher than those of grade Ⅱ lumbar discs[weighted mean difference(WMD)=14.55,95%confidence interval(CI)6.35-22.75,P<0.01],and the T1ρ values of grade Ⅱ lumbar discs were significantly higher than those of grade Ⅲlumbar discs(WMD=34.20,95%CI 27.05-41.34,P<0.01).The T1ρ values of grade Ⅲ lumbar discs were significantly higher than that of grade Ⅳ lumbar discs(WMD=22.94,95%CI 17.08-28.80,P<0.01).The T1ρ values of grade Ⅳ lumbar discs were significantly higher than that of grade Ⅴ lumbar discs(WMD=9.35,95%CI 6.81-11.89,P<0.01).Conclusion T1ρ imaging technology can objectively and quantitatively evaluate degeneration at different stages,especially sensitive to IVDD in the early and middle stages,which can provide imaging evidence for clinical diagnosis of early IVDD.
