1.The effect of alternative decompression body pad on the pressed skin of patients with spinal surgery

Objective To investigate the effect of alternative decompression body pad in the intraopera-tive pressure sore prevention.Methods Using random sampling methods,100 cases of patients were collect-ed and divided into the experimental group and the control group with 50 cases in each group,the experimental group adopted self-designed alternative decompression body pad and the control group used common body pad.The skin red,and hurt situation in the main parts of compression were observed,the average pressure of the main parts of compression,such as forehead,chest, anterior superior iliac spine was measured by means of stress-based software DZX29 figure measuring instrument, the above data underwent comparison.Results Statistical differences existed in skin red,skin damage and the average pressure between the two groups.Conclusions The alternative decompression body pad can effectively reduce the partial pressure of the vertical principal stress,and having an active role in the prevention of intraoperative pressure sores.But the prevention of pressure sores will still need to take comprehensive nursing measures.
2.Investigation on Water Source Pollution by Cryptosporidium parvum and Giardia lamblia in the Small Towns and Villages in Shenzhen,China

Objective To investigate the water source pollution by parasitic protozoa in small towns and villages in Shenzhen,China,in order to present the data to establish the national hygienic standard for controlling parasitic protozoa pollution.Methods The samples of water source were collected from 8 village level water plants in Shenzhen.Giardia lamblia and Cryptosporidium were detected,according to the EPA methods,by filtering,rinsing,magnetic isolation and staining.Results Giardia lamblia was found in water sources of 6 water plants and Cryptosporidium was found in one water plant.Conclusion At present,some water sources of village level water plants where the reservoirs were used as the water source in Shenzhen have been polluted by Giardia lamblia and Cryptosporidium,that indicates the potentiality of the water-born-diseases in that areas.
3.Change tendency of correlative test indexes in patients with type 2 diabetic mellitus and diabetic nephropathy

Objective To analyze the variation tendency of correlative test indexes in the patients with type 2 diabetic mellitus (T2DM) and diabetic nephropathy (DN) .Methods The urinary microalbumin (M‐Alb) ,blood glucose and renal function were de‐termined in 167 cases of T2DM .Then the cases were divided into the simple DM group ,early DN group and clinical DN group ac‐cording to the excretion rates of urinary M‐Alb .The determined results were compared among 3 group and analyzed .Results The age in the simple DM group was smaller than that in the early DN group and clinical DN group (P<0 .05) .The positive rates of u‐rinary M‐Alb and urinary protein were highest in the clinical DN group ,the differences among 3 groups had statistical significance (P<0 .05) .The difference of blood glucose indexes had no statistical difference between the simple DM group and early DN group , the clinical DN group was apparently higher than that in the simple DM group and early DN group (P<0 .05) .The levels of urea (Urea) and creatinine(Cr) in the simple DM group were significantly lower than those in the early DN group and clinical DN group ,the differences among 3 groups had statistical significance (P<0 .01) .Conclusion The severe the DN ,the higher the urina‐ry M‐Alb ,blood glucose and renal function indicators ,the higher the positive rates of urinary protein and urinary glucose ,the more significant the tendency of renal function damage .
4.Clinical observation of vacuum sealing drainage in osteomyelitis of sternum after cardio-thoracic surgery operation

Objective To explore detection value of vacuum sealing drainage in osteomyelitis of sternum after cardio-thoracic surgery operation.Methods 132 cases of osteomyelitis of sternum after cardio-thoracic surgery operation were randomly divided into two groups(research group and control group).The research group had 72 cases)and the control group had 60 cases.The control group was treated with conventional treatment.The research group was treated with vacuum sealing drainage.The average healing time and clinical therapeutic effect of the two groups were observed.Results The patients'age and the sternum of osteomyelitis was significantly related,with OR=1.153 and P<0.05.After treatment,the average healing time of the research group was(3.1±0.8)months.The average healing time of the control group was(7.2±1.5)months.The average healing time of the two groups had significant difference(P<0.05).Conclusion Vacuum sealing drainage in osteomyelitis of sternum after cardio-thoracic surgery operation has sure curative effect.It can change chronic wounds for acute tissue effectively,shorten the healing time.It has high application value in the first phase of treatment and the second phase of repair.
5.Change in kidney morphology after ischemia/reperfusion in a sheep model of acute heart failure supported by pulsatile catheter pump

BACKGROUND: Acute renal failure following heart failure assisted circulation have been extensively reported. However, little data have been available concerning morphological analysis of kidney tissues under that condition.OBJECTIVE: To observe morphological change of ischemia/reperfusion kidney in a sheep pulsatile catheter (PUCA) pump short-term support for heart failure model and explore causes of acute renal failure in assisted circulation patients.DESIGN, TIME AND SETTING: Self-contrast animal experiment was performed at the laboratory of Department of Cardiothoracic Surgery, Renji Hospital of Shanghai Second Medical University between July 2003 and April 2004.MATERIALS: PUCA pump was provided by Gerhard Rakhorst, Professor of Biomedical Engineering, University of Groningen.METHODS: After ischemic heart failure in 10 sheep was induced successfully and subsequently ischemia/reperfusion kidney was developed, PUCA pump was activated to support the hemodynamics for 3 hours.MAIN OUTCOME MEASURES: Hemodynamic parameters were monitored and recorded before thoracotomy, heart failure, and every 45 minutes after the support. Kidney biopsy specimens for light and electron microscopy were obtained 3 hours after support.RESULTS: PUCA pump support was successful in 7 of 10 sheep for 3 hours. During support with the PUCA pump,Hemodynamic parameters gradually restored to normal and stable condition, and blood pressure was close to baseline at the end of experiment. On both light and electron microscopy examination, mild acute kidney change was observed after ischemia/reperfusion. Cytosis in renal glomerulus associated with vasodilatation hyperemia, endepidermis in renal tubules hydropic degeneration, vasodilatation hyperemia and Interstitial edema in renal medulla were the main findings.CONCLUSION: PUCA pump could successfully maintain the hemodynamics for 3 hours in a sheep acute heart failure model,but pathological change in ischemia/reperfusion kidney was remained. It is impossible to predict prognosis of renal function on hemodynamic data alone during support.
6.The application of pulsatile catheter pump support on cardiac resuscitation in sheep

Objective To assess the effect of the pulsatile catheter (PUCA) pump support on cardiac resuscitation in sheep,and to provide a new approach for saving cardiac arrest patients. Methods Cardiac arrest was induced by ventricular fibrillation in 11 sheep. These sheep were divided into three groups including no support (n=3), delayed support (n=2) and immediate support (n=6). Time for cardiac resuscitation and the ratio of success to failure in each group were recorded. Hemodynamic parameters including heart rate (HR), cardiac output (CO), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), central venous pressure(CVP),right ventricular end-diastolic pressure (RVEDP),left atrial pressure (LAP), and left ventricular end-diastolic pressure (LVEDP) were monitored and recorded at 5, 60 and 180 min after cardiac resuscitation with PUCA pump. Results Time for cardiac resuscitation in no support group, delayed support group and immediate support group was (38.3±5.8),(43.5±9.2) and (48.7±23.8)minutes, respectively(P>0.05),and the ratio of success to failure was 0/3,0/2 and 5/1, respectively ( P<0.05). After cardiac resuscitation with the PUCA pump support, MAP, SBP and DBP increased gradually(P<0.05).Conclusions PUCA pump can maintain the hemodynamic stability in a sheep model of cardiac arrest,and can thus increase the success rate of cardiac resuscitation. It may be suitable for resuscitating cardiac arrest patients.
7.Comparison of prognosis of skull base chordoma treated by surgical resection and gamma knife surgery.

OBJECTIVE:
To analyze the complication, survival and life quality, and to explore the efficacy and prognosis of skull base chordoma treated by open surgery and gamma knife surgery (GKS).
METHODS:
Thirty-eight patients with skull base chordomas who underwent an open surgery and another 19 patients who underwent GKS between January 2002 and January 2010 were followed up. The Karnofsky performance scale (KPS) at admission,discharge and follow-up, and recurrent rates were calculated through SPSS life-table, and survival rate curve was used to evaluate the efficacy and prognosis.
RESULTS:
Forty patients were followed up, and the survival rate of 1, 3 and 5 years was 92.6%, 74.3% and 60% in 27 patients with open surgery, and 100%, 83.3% and 62.5% in 13 patients with GKS respectively. There was no significant difference between the 2 groups (χ(2)=0.867, P=0.353). There was no significant difference in the KPS score between the 2 groups (P=0.138).
CONCLUSION
Despite the poor prognosis of skull base chordoma, GKS can effectively improve the survival rate and present life quality of patients with skull base chordoma.

8.Efficacy and prognosis of trigeminal neuralgia treated with surgical excision or gamma knife surgery.

OBJECTIVE:
To evaluate the curative effects and complications when using microvascular decompression (MVD) or gamma knife surgery (GKS) to treat trigeminal neuralgia (TN). And to investigate the prognosis for TN after these treatments.
METHODS:
Sixty-one TN patients treated using MVD and eighty-six TN patients treated using GKS were enrolled by means of telephone, letter or out-patient recheck; the patients had originally presented from December 1996 to June 2010. A chi-square test was applied to follow-up data on pain relief after 1 month, 6 months and 12 months, and at the final follow-up.
RESULTS:
One hundred and two patients were followed for 3-151 months. Cumulative pain relief rates at 1, 6, 12 months and at end times were 90.48%, 95.24%, 92.86%, and 95.24%, respectively, for forty-two MVD-treated patients, and 23.33%, 83.33%, 86.67% and 90% for sixty GKS-treated patients, respectively. The effects of two methods for treatment of TN was not statistically different (χ(2) = 2.053, P=0.152). Pain relief rates in the short-term (first month) demonstrated statistically significant differences (P<0.01), but pain relief rates in the long-term showed no significant differences (P>0.05).
CONCLUSION
Immediate pain relief with MVD treatment is higher than with GKS, but in the long term both treatments were comparable.

9.Multidisciplinary team working for diagnosis and treatment of fetal intestinal volvulus: report of two cases

We report the diagnosis and treatment of two cases of fetal intestinal volvulus. Case 1 presented to Gansu Provincial Maternity and Child-care Hospital due to reduced fetal movements at 33 +4 weeks of gestation. Case 2 was referred to our hospital from a local hospital because of fetal bowel dilatation by ultrasound at 32 +5 weeks. Both cases were found to have fetal bowel dilatation with typical "whirlpool" or "coffee bean" signs on ultrasound after admission. After multidisciplinary consultation and discussion, an emergency cesarean section was performed, during which the two neonates underwent surgical operation and resection of necrotic bowel loops after confirming the diagnosis of volvulus and intestinal necrosis. Case 2 suffered from pulmonary artery thrombosis after the bowel surgery, and underwent pulmonary artery incision and embolectomy within 24 hours. Both newborns recovered well after the operation, whose growth parameters and nervous system development was normal for follow-up.
10.A study on keloid fibroblast induced THP-1 derived macrophage polarization and inflammatory factor TNF-αexpression and its feedback effect

Objective:To investigate the effect of keloid fibroblasts on the polarization and expression of inflammatory factors of M0 macrophages and possible mechanisms, and provide theoretical basis for new targets for keloid therapy.Methods:Keloids, normal skin tissues and paraffin specimens from patients undergoing plastic surgery in the First Affiliated Hospital of Sun Yat-sen University from November 2020 to September 2021 were collected, and fibroblasts of keloids and normal skins were isolated and co-cultured with M0 cells formed form THP-1 by phorbol ester (PMA)-stimulation to detect the expression of macrophage polarization markers and cytokines. Besides, keloid fibroblasts were treated with exogenous tumor necrosis factor-α(TNF-α) to detect its effect on the proliferation and extracellular matrix expression.Results:Macrophages were dominated by CD163 + (M2) in keloid tissues. Moreover, M0 cells expressed more TNF-α when co-cultured with keloid fibroblasts, compared with those with normal skin fibroblasts, in which, the positive staining rates of TNF-α were 19.32% and 29.52% respectively by flow cytometry. Furthermore, the proliferation was promoted and the expression of extracellular matrix proteins (COL3A1 and FN1)and Vimentin were upregulated in keloid fibroblasts under TNF-α stimulation. However, there was no significant difference in the expression of polarization surface markers CD86 and CD163 in macrophages, when co-cultured with keloid fibroblasts or normal skin fibroblasts. Conclusions:Keloid fibroblasts promote the expression of TNF-α in macrophages, which in turn promotes the proliferation and extracellular matrix secretion of keloid fibroblasts.
