1.Investigation of Indoor Air Pollution and Citizen Health Knowledge in a City

Objective To understand the current situation of indoor air pollution,citizen health knowledge and the demand for detection of indoor air pollution.Methods The decorated rooms finished for more than 6 months were selected for indoor air quality test and the on-the-spot questionnaire investigation was conducted in Feb-Jul,2006.Results The indoor air was polluted in about 30% of the investigated rooms.Formaldehyde was the predominant pollutant,but the pollution level was different among the different function districts.The pollution in the children's bedroom and the parents bedroom was more serious.About 60% of the citizens participated in the investigation knew the cause of the indoor air pollution,but only 10% of them had the demand for indoor air quality test.Conclusion The indoor air is seriously polluted in this city at present.The related health knowledge and the indoor air quality test demand of the citizens is in the lower level,the economic situation and education are the main influence factors.
2.Clinical study of thymalfasin combined with radiotherapy for non-small cell lung cancer

Objective To discuss the clinical effect of thymalfasin combined with radiotherapy for non -small cell lung cancer (NSCLC).Methods According to digital table,61 patients with NSCLC were randomly assigned to combined therapy group and radiotherapy group.The control group(radiotherapy group)only used radia-tion therapy.The treatment group(combination group)used thymalfasin combined with radiotherapy.The tumor lesion changes,immune indicators and the quality of life were observed.Results The effective rate of the tumor lesion changes in the treatment group was 83.9%,which in the control group was 76.7%(χ2 =0.725,P >0.05)).The levels of CD +4 ,CD +4 /CD +8 in the treatment group were (49.29 ±4.48)%,(1 .91 ±0.44),which were significantly higher than (33.1 7 ±1 .07)%,(1 .38 ±0.48)in the control group (t =1 4.69,5.34,all P <0.05).The improve-ment rate of digestive tract reaction in the treatment group was 74.2%,the body -weight ratio was 38.7%,the increased rate of KPS(karnofsky performance score)was 64.5%,while those of the control group were 23.3%, 1 3.3%,36.7% (χ2 =5.87,6.1 2,3.87,all P <0.05).Conclusion Compared with the use of radiotherapy only, thymalfasin combined with radiotherapy for NSCLC was more effective for patients in tumor lesion response,cell immu-nity and the quality of life.
3.Reflection on reinforcing the construction of the public health system in the rural areas

The paper reviews the evolution of the medical and health cause in the rural areas of China and argues that at present the system of public health services in the rural areas is confronted with a number of problems, including poor infrastructure, lack of highly qualified medical personnel, resurgence of a variety of extremely harmful contagious diseases, and lack of basic medical security for the farmers. It is therefore necessary to enhance understanding of the importance of reinforcing the construction of the public health system in the rural areas, expand financial input, set up a system of pooling resources for serious illnesses, and establish sanitation and antiepidemic networks and socialized health care networks in the rural areas.

5.Evaluation of therapeutic effects with microspheres embolization for giant hepatocellular carcinoma

Objective To evaluate the clinical therapeutic effect with microspheres embolization for giant hepatocellular carcinoma(HCC). Methods A prospective study was performed for 156 HCC patients needed for TACE, then randomly signed them into group M (embolized by microspheres alone), group L (embolized by lipiodol alone) and group M + L (embolized by microspheres combined with lipiodol). TACE of group M was performed by 300-700 μm microspheres. Group L selected lipiodol alone to embolize. While group M + L were embolized by about 1/3-1/2 lipiodol of total embolized volume and then microspheres feeding individual tumor vessels. Stoped the procedure when caused terminal vessel blockade. Before and after therapy, patients'liver function, serum α-fetoprotein level (AFP), responses of tumor, complications related to embolization and survive rates of 180 days and 360 days were analyzed among three groups.Enumeration data such as survive rates and positive response were compared by χ2 test, while measurement data were analyzed by one-way classification. Results The positive response (CR + PR) of group M was 38.5% (20/52)and (CR + PR + SD)73. 1% (38/52). Survive rates of 180 days and 360 days were 88. 5% (46/52) and 82. 3% (43/52). While those of group L were 42. 3% (22/52) ,76.9% (40/52),86.5%(45/52), 75.0% (39/52) and group M + L were 55. 8 % (29/52), 88.5 % (46/52), 94. 2% (49/52),86. 5% (45/52). Significant difference of survival rates and total effective between group M + L and M,M + L and L was found(χ2 = 6. 27,6. 16, P < 0. 05). At the same time adverse responses of incidence and persis time such as febricity, nausea and vomit, hepatic pain were more lower in group M + L and group M.Three groups febricity rate were 63.4% (33/52), 86. 5% (45/52) and 69. 2% (36/52), nausea and vomit were 67.3 % (35/52) ,84. 6% (44/52) and 76. 9% (40/52), hepatic pain were 59. 6% (31/52), 86. 5 %(45/52), 73. 1% (38/52) (χ2 = 6.55, 6. 22, 6. 90, P < 0. 05). Conclusion Embolization with microspheres can get longer and more effective treatment. TACE with microspheres and lipiodol for embolizing HCC was more effective than with Microspheres or lipiodol alone.
6.DNA sequence homology analysis of Trichophyton rubrum isolates from multiple body sites of patients with onychomycosis

Objective To profile genotypes of Trichophyton rubrum isolates from different body sites in patients with onychomycosis. Methods DNA was extracted from 30 T. nibium isolates from 10 patients with onychomycosis, and subjected to PCR with tandemly repetitive subelement 1 (TRS1 )-specific primer to analyze the number of repetitive elements in the non-transcribed spacer (NTS) region of ribosomal DNA gene, and to random primer amplification with the random primer OPAA11. The genotype variety was evaluated for T. rubrum isolates from different body sites of patients with onychomycosis. Results All the strains were classified into 5 genotypes based on the copy number of TRS1, and into 11 genotypes by RAPD analysis. The genotypes of T. rubrum seemed unrelated to sites of infection. Genotype diversity was observed among T. rubrum strains from different body sites of the same host in 7 out of the 10 cases as shown by amplification of TRS1 region, in 8 out of the 10 cases as demonstrated by RAPD analysis. Conclusion A single host with onychomycosis could harbor multiple genotypes of T. rubrum at different body sites, suggesting external sources of infection rather than infection from a different site in the same individual.
7.Clinical Observation of the effect of Shenmai injection on congestive heart failure

Objective To observe the effect of Shenmai injection on congestive heart failure(CHF) and the effect on thyroid hormone as well as medicine security. Methods 52 patieits with CHF were enrolled,Shenmai injection was used with regular western medicine in the treatment. And observed its effect and test serum thyroid hormone: TSH,T3 ,T4 ,rT3. Another 50 CHF patients were enrolled as a control. Results The results of observation were that 25 cases were very effective,23 were effective,4 had no effect and the total effective rate was 92%. The results of control group were that 21 were very effective, 16 were effective, 13 had no effect and the total effective rate was 74%. The differences between the two groups were significant (P > 0.05). By comparing two thyroid hormone T3, T4, rT3, the observation T3 and T4 was obviously higher than the control. rT3 was lower than the control. Conclusion Shenmai injection could efficiently improve the heart function of CHF,cure arrhythmin and correct the illness caused by thyroid hormone.

10.Evaluation and clinical application of the subaxial cervical spine injury classification system

Objective To evaluate the interobserver reliability and intraobserver reproducibility of the subaxial cervical spine injury classification system (SLIC) and identify its guidance on clinical treatment of spinal cord injuries. Methods Thirty consecutive patients with subaxial cervical injury ad-mitted into our hospital between December 2007 and July 2008 were enrolled in this study and underwent neurological examinations followed by X-ray, CT, and MRI examinations. SLIC was evaluated in aspects of fracture morphology, disco-ligamentous complex (DLC) status and ncurologic status. According to the morphology in image, the fractures were classified into normal, compression, burst, distraction and trans-lation/rotation. According to injury severity, DLC was sorted into intact, indeterminate and disrupted types. Neurologic status was classified into intact, nerve root injury, complete/incomplete spinal cord in-jury and continuous spinal cord compression. The clinical and radiographic data of 30 patients with subax-ial cervical injury were evaluated prospectively by four groups of surgeons to count SLIC scores and decide treatment based on SLIC scores. The evaluation was done again three months later. Cohen' s unweighted kappa coefficients were calculated for the key parameters of each classification system ( morphology, neurologic status, DLCstatus, total score, and proposed management) to analyze the reliability and repreducibility of SLIC. Based on SLIC, the treatment method was selected and the neurological function recovery and the complications observed. Results The interrater Kappa statistics of all subgroups ( morphology, DLCstatus, total score, proposed management) were within the range of moderate to substantial reliability (0.47-0.69). Kappa coefficient was 0.83 for neurologic status, with high reliability. There was no statistical difference upon Kappa values in two evaluations. The interrater Kappa statistics of all sub-groups ( morphology, DLCstatus, total score, proposed management) were within the range of moderate to substantial reproducibility (0.53-0.78). Kappa coefficient was 0.89 for neurologic stares, with high repreducibility. The recovery rate of neuroiogie function was 79.2%. There was no aggravation of neurological status, with low incidence of complications. Conclusions SLIC has advantages of high reliability and reproducibility, simple use, exact evaluation and can facilitate decision-making in treatment of subaxial cervical spine injures.
