1.Volumetric modulated arc therapy for nasopharyngeal carcinoma patients with poor compliance: setup error and expansion margin of target volume

Objective To investigate the setup error for nasopharyngeal carcinoma (NPC) patients with poor compliance using kV cone-beam computed tomography,and to calculate the expansion margin from the clinical target volume (CTV) to planning target volume (PTV).Methods In 45 NPC patients from 2013 to 2015,the setup error,95% confidence interval (CI)-1 for random error,and PTV-1 value were calculated.Moreover,in 16 NPC patients with poor compliance based on five verifications (random error not within 95% CI-1),the setup error,95% CI-2 for random error,and PTV-2 value were calculated.For the 16 special patients,PTV-1 and PTV-1 combined with PTV-2 were used to develop the plan-1 and plan-2,respectively.The dosimetric difference between plan-1 and plan-2 was evaluated.Results Both PTV-1 and PTV-2 had the largest expansion margin in the y direction.The CTV of plan-1 could not meet the requirement of the prescription dose after the setup error was introduced.Compared with plan-1,the V95% and D95 values for the CTV of plan-2 were increased by 6.26% and 4.43%,respectively.The D01 value was significantly larger in plan-2 than in plan-1 (P=0.005),which,however,met the clinical requirement.Conclusions In patients with poor compliance,the dose to target volume can be effectively elevated and the normal tissue can be spared from damage when PTV-1 combined with PTV-2 is selected as expansion margin.

3.Clinical research progress on traditional Chinese medicine in treating chronic obstructive pulmonary disease

This paper reviews the clinical research literature of traditional Chinese medicine (TCM) on treating chronic obstructive pulmonary disease (COPD) in recent years. It is believed that TCM has curative effect in the treatment of acute exacerbation or remission of COPD. The treatment principle for acute stage of COPD includes phlegm, heat and dampness, and the chronic stage lung, spleen and kidney. TCM can improve the clinical symptoms and quality of life, with various treatments and less adverse reactions. However, there are some limitations such as small sample size, lack of in-depth discussion on the mechanism of trwatment, lack of standardized syndrome differentiation and outcomes evaluation standards.
4. The role of human papillomavirus 16 early genes E2 and E6 and heterogeneous nuclear ribonucleoprotein E2 in cervical carcinogenesis and their interaction effect

Objective:
To investigate the role of human papillomavirus (HPV) 16 early genes E2 and E6 and heterogeneous nuclear ribonucleoprotein (hnRNP) E2 and their interaction effects in the progression of the cervical cancer.
Methods:
Women with normal cervix (NC), low cervical intraepithelial neoplasia (CIN Ⅰ) and high cervical intraepithelial neoplasia (CIN Ⅱ/Ⅲ) from the cervical lesions cohort in Jiexiu County of Shanxi Province from June 2014 to September 2014, and patients with cervical squamous cell carcinoma (SCC) treated at the Second Hospital of Shanxi Medical University in the same period were enrolled in this study. There were 257 participants, about 67 NC cases (26.07%), 69 CIN Ⅰ cases (26.85%), 68 CIN Ⅱ/Ⅲ cases (26.46%), and 53 SCC cases (20.62%), respectively. The information of demographic characteristics, life health habits and cervical lesions were collected by using the structured questionnaire. Cervical exfoliated cells and cervical biopsy tissues were collected to detect the infection of HPV16 and the protein expression levels of hnRNP E2, HPV16 E2 and E6. According to the median-value of the protein expression levels of hnRNP E2, HPV16 E2 and E6 and E2/E6 ratio in the NC group, the study participants were divided into the high and low expression groups/ratio groups. The multivariate logistic regression model was used to analyze the correlation between HPV16 early gene E2 and E6, hnRNP E2 and cervical cancer. The interaction effect was analyzed by using the generalized multifactor dimensionality reduction (GMDR) model.
Results:
The ages of NC, CIN Ⅰ, CIN Ⅱ/Ⅲ and SCC groups were (47.00±9.07), (47.64±7.35), (46.37±8.67) and (51.26±8.03) years old, respectively. Objective To understand the distribution of virulence gene and the epidemiological characteristics of diarrheagenic Escherichia(E.) coli (DEC) from diarrheal patients in Beijing. Methods Stool specimens from diarrheal patients were cultured which were collected from the hospitals under sentinel surveillance program,during 2012-2013. DNA was examined by real-time PCR. Results 253 out of 6 370 specimens were positive for DEC detection with the rate as 4.0%. A total number of 262 DEC strains were isolated. Two different pathotypes of DEC strains with mixed infection,were isolated from 9 specimens. Different pathotypes would show the following profiles:42.8% for enteropathogenic E. coli (EPEC) including 42.0% atypical and 0.8% typical;38.9% for enterotoxigenic E. coli(ETEC)including 24.8%st positive,9.9%lt positive and 4.2%st and lt both positive;15.3% for enteroaggregative E. coli(EAEC);2.7% for enteroinvasive E. coli(EIEC);one strain STEC with serotype O26 ∶ K60. ETEC had obvious characteristics on age. All kinds of DEC were isolated throughout the year with seasonal fluctuation. Conclusion DEC isolates from diarrheal patients in Beijing were dominated by EPEC and ETEC,with atypical ones accounted for the majority of EPEC. One specimen was found under mixed infection. Pathotypes DEC were found to have different age and seasonal distributions.
6. Research progress of traditional Chinese medicine in treating chronic obstructive pulmonary disease with anxiety and depression

To review the literatures on the treatment of COPD combined with anxiety and depression treated by TCM in recent years, which conclude that to treat this kind of disease is usually by taking drugs, as well as non-drug therapy such as traditional Chinese exercises, acupuncture and massage, and music, etc, which could improve patients unhealthy emotions and quality of life. The recent clinical researches on TCM treatment has been increased, but the overall number of the research of this kind is limited, with problems such as small sample size, singular observing indicator, and requiring multi-center research. There still lacks related basic research and needs more attention.
