1.The effect of improved closed repair operation combined with YiQiZengYe traditional Chinese medicine for treating patients with severe rectocele

Objective To observe the clinical efficacy of improved closed repair operation combined with YiQiZengYe traditional Chinese medicine(TCM) for treating the patients with severe rectocele.Methods 60 patients with severe rectocele were randomly divided into treatment group and control group.The patients in treatment group received the treatment of improved closed repair operation combined with YiQiZengYe TCM,and the patients in control group received the treatment of conventional transvaginal incision repair.The clinical efficacy was evaluated 3 months after surgery.The clinical symptom score,rectocele depth of resting bit and force row,postoperative complications of two groups were comparatively analyzed.Following-up of 6 months after surgery,the postoperative recurrence was observed.Results The effective rate of the treatment group was 100.0%,and was significantly higher than that of the control group(70.0%) (x2 =10.588,P < 0.05) ; The complication rates of postoperative bleeding,pain,anal bulge and infection in the treatment group were 0 cases,3 cases(10.0%),7 cases(23.3%),1 cases (3.3%),respectively,wich were significantly lower than those of the control group [4 cases (13.3%),23 cases (76.7%),28cases (93.3%),9 cases (3.3 %)] (x2 =4.286,38.400,30.240,7.680,all P < 0.05).There was no postoperative recurrence in the treatment group.Conclusion The improved closed repair operation combined with YiQiZengYe TCM for treating the patients with severe rectocele can significantly improve the clinical efficacy,effectively improve prognosis and prevent recurrence.This treatment method is safe and reliable.
2.A HIGH POWER CO_2 LASER SCAPEL FOR HEMISPLENECTOMY AND TRAUMATIC RUPTURE OF THE SFLEFN——AN ANIMAL EXPERIMENTAL STUDY

Twety dogs were used to study a high Power Co_2 laser scapel for hemisplenectomy and traumatic rupture of the spleen by temporary occlusion of the spleen pedicle. The laser scapel with its point power of 160-200 w was used for a focusing cut and for stopping bleeding by the laser beam beyond the focus. The wound surface could be dried and coking good, without injecting adrenaline into the splenic artery. All the dogs survived after the experimental investgation and no infection or any more Post-oPerative bleeding was found. The spleen membrane was formed on the section of the spleen 1 year after operation. Compared with other operations for keeping the organ of spleen, this method safer and more reliable. It could be used in clinical practice with success.
3.The role of telomerase reverse transcriptase in monocrotaline-induced pulmonary hypertension in rats

AIM and METHODS: To explore the role of telomerase reverse transcriptase (TERT) in monocrotaline-induced pulmonary hypertension (PH), TERT was investigated by using immunohistochemistry and image analysis on the chronic PH induced by MCT in Sprague-Dawley rats. RESULTS: TERT was positive in pulmonary vascular smooth muscle cells, bronchial smooth muscle cells and chondrocytes in normal group. TERT in MCT group was more positive in pulmonary vascular smooth muscle cells and bronchial smooth muscle cells compared with normal group. CONCLUSION: Over expression of TERT in pulmonary vascular smooth muscle cells and bronchial smooth muscle cells is related to MCT-induced PH.

It was an dideal model of respiratory distress syndyomeinduccd in 59 rabbits by Ⅳ oleic acid injection. 6.Value of reconstruct anterior cruciate ligament for remaining anteromedial or posterolateral bundle and ruptured fiber

[Objective] To evaluate the curative effect of anterior cruciate ligament(ACL)reconstruction for remaining anteromedial or posterolateral bundle and ruptured fiber.[Method]Among 89 cases of ACL rupture,there were 32 cases of sports injury,24 cases training injury,23 cases traffic accidents and 10 cases sprained in daily life.ACL ruptured at the notch of the femur in 24 cases,and at the intercondyle of the tibial in 28 cases.Thirty-two cases were reconstructed with quadri-bundle Hamstring tendon knot press-fit technique and 20 cases were reconstructed with quadriceps tendon press-fit technique with autogenetic patellar bone.ACL anteromedial bundle(AMB)ruptured in 14 cases and posterolateral bundle(PLB)ruptured in 23 cases.The intact bundle and the remnants of ACL were preserved.AMB and PLB were reconstructed with double-bundle Hamstring tendon knot press-fit technique in 37 cases.Absorbable interference screws were used at the tibial tunnel and sutured tendon was knotted across the tibial bone bridge.[Result]The average duration of follow-up were 25 months(14-37 months).Lysholm knee function score increased by 27.7 scores,from average 67.5 scores of pre-operation to average 95.2 scores at the end of the follow-up.Rulermetr examination tibial translation was 4 mm?2 mm in 72 cases and 6 mm?3 mm in 17 cases.Lachman test was negative in 79 case,weakly positive in 8 cases and positive in 2 cases.The function of the knees were improved and the stabilization was strengthened after operation.According to knee therapeutic effect evaluation criterion,76 cases were excellent and 13 cases were good.[Conclusion]To keep the remaining of anteromedial or posterolateral bundle and ruptured fiber for ACL reconstruction is beneficial for the graft revascularization or healing,and is helpful for the position sense or rehabilitation.
7.Effect of extracorporeal shock wave on tendon-bone healing after reconstruction of the anterior cruciate ligament:an experimental study in rabbits

[Objective]To explore the effects of extracorporeal shock waves(ESW) therapy on tendon-bone healing after reconstruction of the anterior cruciate ligament(ACL).[Method]Animal model of reconstruction of the anterior cruciate ligament in 18 healthy F/M New Zealand white rabbits(2~3 months old) were established using digital long extensor tendon and randomly classified into 2 groups.The ESW group was given extracorporeal shock waves therapy on 24 hours after reconstruction of the anterior cruciate ligament while the control group with only reconstruction of the anterior cruciate ligament but no more therapy.All animals were sacrificed at 2、4、8 weeks after operation for histological examinations.The specimens were stained with hematoxylin-eosin and Masson trichrome to investigate pathology change of the tendon-bone healing tissue.The ink intravascular injection was used to observe the neo-vascularization of tendon-bone healing tissue.[Result]The histological examination revealed that at 4 weeks after operation,the ESW group,tendon-bone interface was filled with proliferated fibroblasts and chondroblasts,the collagen fibers were obviously increased and regularly arranged.At 8 weeks after operation,the healing tissue at the bone-tendon interface had developed into dense connective tissue,the collagen fibers were formed in abundance and the perpendicular collagen fibers were regularly and longitudinally arranged.The transition zones of collagen fibers,fibrocartilage and bone(the structure alike a direct ligamentous insertion) occurred in the local region of tendon-bone interface.The histological observations showed that the tissue healing at tendon-bone interface of the ESW group were quicker than those of the control group.The proportion of neo-vessels at the tendon-bone interface of the ESW group were significantly higher than those of the control group at 4 and 8 weeks after operation(respectively P=0.028,P=0.008).[Conclusion]Extracorporeal shock wave treatment can significantly improve the tendon-bone early healing after reconstruction of the anterior cruciate ligament in rabbits.

Colostomy,which is an important means of surgical treatment of colorectal cancer,leads to the colostomy fecal leakage,odor,skin infections and other complications.Artificial anus plugging device as an alternate device to colostomy bag,can achieve bowel control,streamline operations,good blocking effect and mprove the quality of life.Therefore,in this review,the author will discuss the research progress of artificial anus plugging device,including its froms,materials and functions in the treatment of fecal incontinence.
9.Effects of alanyl-glutamine dipeptide administration on postoperative intestinal permeability and immmunologic function

Objective: The study was designed to observe the effects of alanyl-glutamine(Aln-Gln) dipeptide on postoperative intestinal permeability and immmunologic function in abdominal patients. Methods: A prospective,randomized and controlled trial was taken.20 patients who underwent abdominal operation were divided into two groups: study group(intravenous administration of Aln-Gln dipeptide,0.5 g/(kg?d),for 4 days,n =10) and control group(equal normal saline as placebo,for 4 days,n =10).Serum concentrations of glutamine(Gln) and diamine oxidase(DAO),blood total lymphocyte count(TLC) and human leukocyte antigen(HLA)-DR and urine lactulose/mannito(L/M) ratio were measured before and after operation. Results: Serum Gln concentration was significantly decreased in control group and increased in study group on postoperative day 5.Urine L/M ratio and serum DAO level were significantly increased in control group and decreased in study group after operation.Blood TLC and HLA-DR expression in the study group were significantly higher than those in control group on postoperative day 5. Conclusion: Intravenous administration of Aln-Gln dipeptide can increase the serum Gln level,decrease the intestinal permeability,maintain the intestinal barrier and improve the immmunologic function in the early period of postoperative patients.Aln-Gln dipeptide can be used in the fast track surgery to help patients recover rapidly.
10.A neuroelectrophysiological study of the effect of mild hypothermia on acute severe cerebrovascular disease

Objective To evaluate the effect of mild hypothermia on acute cerebrovascular diseases. Methods Sixty-two cases of severe cerebrovascular diseases were randomized into hypothermia group and control group. In hypothermia group the patients were cooled to 34~35℃ for 48h ,while the patients in the control group treated by routine methods. Median nerve short latency somatosensory evoked potentials (SLSEP) and brain-stem auditory evoked potentials (BAEP) were recorded before cooling and 30 minutes, 24, 72, and 120 hours after cooling.
