1.The Evaluation of Treatment Effect of Respiratory Disease for 68 Cases of VLBWI

Objective To discuss the main complication and death cause of VLBWI,and enhance survival rate. Methods We summarized our experience and analyzed distribution of respiratory disease diseases of 68 cases of VLBWI. Results 51 cases were cured and 9 cases were died. Conclusion The respiratory disease was the main complication and death cause of VLBWI. The survival rate of VLBWI could be improved by being in NICU,intervening early with medicine and strengthening management of respiratory tract.
2.Repair of soft tissue defects on the foot and ankle with flaps of cutaneous branches of the low medial leg

Objective To investigate clinical effects of cutaneous branch flaps of the low medial leg for the repair of soft tissue defects on the foot and ankle.Methods A flap with pedical of cutaneous branches of the low medial leg was used for the repair of 7 cases of skin and soft tissue defects on the feet and ankle from March 2003 to October 2005.The cutaneous branches of the posterior tibial artery were identified along the medial border of the tibia and between the soleus muscle and the flexor digitorum longus muscle.The flap was mobilized according to the site and length of the cutaneous branches,and was transferred to soft tissue defects for skin grafting. Results The operating time was 3~5 hours(mean,4.2 hours).The flaps survived completely in all the 7 cases.Follow-up checkups were carried out for 5~18 months(mean,10 months).The appearance and functions of the foot were satisfactory and met the requirements for daily activities.Conclusions The procedure can effectively repair soft tissue defects on the foot and ankle and does not sacrifice the major arteries.This flap is easy to be prepared.
3.Primary study on parkin gene deletion mutations in Chinese patients with familiel Parkinson's disease

Objective To explore parkin gene deletion mutations at exons 3 to 7 in Chinese familial patients with Parkinson's disease as well as the association with the clinical features.Methods DNA was extracted from 6 unrelated families of PD patients; the deletion mutations of parkin gene at exons 3 to 7 were identified by PCR amplification, agarose gel electrophoresis. and the clinical data were analyzed together with the above information.Results In the 6 unrelated families of PD patients, 1 case had exon 5 deletion, its hereditary manner was autosomal recessive inheritance,the patient's age at the onset was 60 years old,clinical feature was tremor,rigidity and bradykinesia,but no athetosis.In addition the deletion mutations of parkin gene at exons 3,4,6,7 were not found.Conclusion There are deletion mutations of parkin gene at exons 5 in families PD of Chinese patients.
4.Value of magnetic resonance imaging on local-regional staging of endometrial carcinoma

Objective To assess the value of magnetic resonance (MR) imaging (MRI) in depicting the depth of myometrial infiltration, cervical invasion and lymph nodes metastasis in patients with endometrial carcinoma compared with surgicopathologic findings. Methods Thirty-eight patients with endometrial carcinoma diagnosed by pathology were inspected by MRI in this prospective study. MR images were analyzed by two radiologists to report the depth of myometrial infiltration, infiltration of the uterine cervix and lymph nodes metastasis. MRI and surgicopathologic staging of endometrial carcinoma were based on FICO classification. MRI findings were compared with surgicopathologic findings. Results The overall accuracy rate of MRI in staging endometrial carcinoma was 77.1 %. The results of Kappa statistics showed that there was strong consistency between MRI and surgicopathology for staging of endometrial carcinoma (k= 0.677). Respective diagnostic accuracy, sensitivity, specificity, negative and positive predictive values in assessing myometrial infiltration(>l/2 depth) were 91.4 %, 83.3 %, 95.6 %, 91.7 % and 90.9 %; those in cervical infiltration were 94.2 %, 80.0 %, 96.7 %, 96.7 %, and 80.0 %; and those in lymph node assessment were 96.2 %, 66.7 %, 100 %, 95.8 % and 100 %. Conclusion MRI is highly accurate in depicting the depth of myometrial infiltration, cervical invasion, lymph nodes metastasis and local-regional staging of endometrial carcinoma.
5.Relationship of Claudin-1 and VEGF-C protein with colorectal cancer

Objective To investigate the expression of tight junction protein 1(Claudin‐1) and vascular endothelial growth factor C(VEGF‐C) in colorectal carcinoma tissues and the relationship between them .Methods Totally 50 cases of colorectal carci‐noma ,paracancerous tissues and 50 cases of normal colorectal tissues from Department of Pathology were detected by immunohisto‐chemistry SP method ,according to the patient′s age ,gender ,lymph node metastasis ,depth of invasion ,clinical stage for comprehen‐sive comparison ,and analyze the Claudin 1 and VEGF‐C protein expression in colorectal cancer tissues .Results The positive rates of Claudin‐1 ,VEGF‐C in colorectal cancer tissues was significantly higher than that in paracancerous tissue and normal colorectal tissues (χ2 =32 .270 ,P=0 .000;χ2 =41 .209 ,P=0 .000) ,the positive rate of Claudin‐1 ,VEGF‐C in paracancerous cancer tissues was significantly higher than that in normal colorectal tissues (χ2 =7 .294 ,P=0 .007 ;χ2 = 5 .741 ,P=0 .017) .The expression of colorectal carcinoma Claudin‐1 had close relationship with differentiation ,invasion degree ,TNM stage ,lymph node metastasis(P<0 .05) ,the expression of VEGF‐C had colse relatons with infiltration degress ,TNM stage ,lymph node metastasis ,tumor size(P<0 .05) .The expression of colorectal cancer Claudin‐1 ,VEGF‐C had significantly positive correlation (χ2 =10 .953 ,P= 0 .001 ,r=0 .468) .Conclusion The expression of colorectal cancer Claudin‐1 ,VEGF‐C was significantly higher than that in paracancerous tis‐sue and normal colorectal tissue ,the expression of Claudin‐1 ,VEGF‐C had a certain relationship with clinical staging .
6.Analysis of clinical effect of involvement of clinical pharmacists in perioperative total parenteral nutrition prescription for cancer patients

Objective To evaluate the effect of the intervention of involving clinical pharmacists in perioperative total parenteral nutrition (TPN) prescription,including rationality of prescriptions,the length of hospital stay and economic evaluation.Methods We respectively selected TPN prescriptions in September to November 2015 (pre-intervention) and December 2015 to February 2016 (post-intervention) from a surgery department of Harbin Medical University Cancer Hospital.The data regarding rationality of prescription,length of hospital stay,and economic benefit were analyzed in order to evaluate the effect of involvement of clinical pharmacists in TPN prescriptions.Results A total of 81 patients with 115 TPN prescriptions before the intervention and 92 patients with 124 TPN prescriptions after the intervention were recruited.Through comparative analysis,the rationality of TPN prescriptions increased significantly after the participation of clinical pharmacists in terms of the rational glucose:fat ratio,energy:nitrogen ratio,electrolyte concentration,and the choice of vitamin preparations;and the utilization of controversial treatments such as invert sugar and other drugs into readyto-use TPN solution was decreased significantly;average daily cost of TPN was decreased from (1 021 ± 218) yuan to (860 ± 176) yuan (P <0.001);average duration of hospital stays was decreased but the difference was not statistically significant (P > 0.05).Compared with the data before the intervention,serum albumin levels after the intervention showed no statistically significant difference but pre-albumin levels were significantly higher after the participation of clinical pharmacists [(175.0 ± 77.9) g/L vs.(153.2 ± 64.8) g/L,P < 0.05].The liver and kidney function indicators showed no statistically significant differences between pre-and post-intervention (P > 0.05).Conclusion Engaging clinical pharmacists in TPN prescription can improve prescription rationality,ensure clinical drug safety,reduce the daily cost of nutrition treatment and thus reduce the economic burden of patients.
7.Risk factors and etiology of multiple drug-resistant bacterial infection of stroke-associated pneumonia pa-tients in intensive care unit

Ojective To determine the risk factors and the clinical distribution of multiple drug resistant bacteria in stroke- associated pneumonia (SAP) patients with multiple drug-resistant bacterial infections from in-tensive care unit, providing guidance for clinical treatment of SAP. Methods A retrospective study was de-signed to analyze the clinical data of the SAP patients from January 2012 to December 2015. Univariate analysis and multivariable regression analysis were taken for risk factors of MDR infections , and investigated the distribu-tion and drug resistance of MDR. Results There were 183 SAP patients, of which 131patients (71.6%) had MDR infection. There are 193 MDR strains in the 131 patients , the first 5 MDR strains were Acinetobacter bau-mannii, Pseudomonas aeruginosa, Klebsiella pneumoniae, Staphylococcus aureus and Escherichia coli. MDR in-fection is highly associated with sever unconsciousness , time of stay in ICU longer than 7 days , ventilator time longer than 7 days and late-onset pneumonia and the difference was significant (P < 0.05). Conclusion SAP patients with MDR bacterial infections are in association with the following risk factors: sever unconsciousness , time of stay in ICU longer than 7 days, ventilator time longer than 7 days and late-onset pneumonia. The rate of MDR bacterial infections was high , and these MDR strains were widely different degrees of resistance to many antibiotics.
8.Effects of Ulinastatin on pulmonary vascular endothelium permeability and respiratory function in ;patients with extrapulmonary acute respiratory distress syndrome

Objective To observe the effects of Ulinastatin on pulmonary vascular endothelium permeability and respiratory function in patients with extrapulmonary acute respiratory distress syndrome(ARDS exp). Methods The data of 39 patients with ARDS exp were retrospectively analyzed.According to whether treated with Ulinastatin or not, all patients were divided into Ulinastatin group(n = 21)and control group(n = 18); The level of extravascular lung water index (EVLWI), pulmonary vascular permeability index(PVPI) and respiratory function were measured before and after the treatment. Results The mortality rate of Ulinastatin group was lower than that of control group (28.6% vs 38.9%). The time of decreasing EVLWI, PVPI and improving PaO2/FiO and respiratory function in Ulinastatin group was shorter than that of control group, and the effect was superior. Conclusion Ulinastatin could reduce EVLWI and PVPI, improve pulmonary compliance and oxygenation, and reduce mortality rate in patients with extrapulmonary acute respiratory distress.
9.Effects of PP242 on the Expression of Apoptosis Protein Bcl-2 in Human Lens Epithelial Cells

Objective To explore the effects of PP242 on the expression of apoptosis protein Bcl?2 in human lens epithelial cells(HLECs). Meth?ods Immortal HLECs SRA01/04 were cultured and treated with different concentrations of PP242. The cell growth was examined by MTT assay at 24 h and 48 h after PP242 treatment. Real?time quantitative polymerase chain reaction(RT?PCR)and Western blot were adopted to detect the mRNA and protein expression of Bcl?2 respectively. Results The cultured HLECs SRA01/04 were collected and treated with different concentra?tions(100,250,500,750,1 000,1 500 nmol/L)of PP242. After treatment for 24 h and 48 h,the inhibitory rate in each well was determined by MTT assay. The inhibition rate was as below,24 h:7.55%,9.43%,16.98%,22.64%,26.42%,30.19%;48 h:11.11%,23.81%,36.51%,42.86%, 49.21%,63.49%. Compared with the control group,the difference was statistically significant(P<0.05). Compared with the control group(0 nmol/L),the expression of Bcl?2 protein was significantly decreased with the increasing concentrations of PP242(P<0.05),while the expression of Bcl?2 mRNA was inhibited by PP242 based on the results of RT?PCR. Compared with the control group(0 nmol/L),the RT?PCR results of Bcl?2 mRNA were 0.723±0.039,0.517±0.028,0.353±0.052,0.167±0.046,respectively(P<0.05). Conclusion PP242 could inhibit cell proliferation of HLECs in vitro with a concentration and time depended manner. Bcl?2 protein is expressed in HLECs,which could be down regulated by PP242 treatment.

BACKGROUND: Poly(ethylene glycol)-b-poly(L-lactide)-b-poly(L-glutamic acid) (PEG-PLA-PGL) tri-block copolymers have good applied foreground in constructing tissue engineering scaffold materials. Whether endothelial cells survive and grow on the materials has a direct influence on the application as a biodegradable material for the scaffold of endothelial cell vector.OBJECTIVE: To explore the cytocompatibility of PEG-PLA-PGL tri-block copolymers with human umbilical vein endothelial cells (HUVECs).DESIGN: Randomized control observation.SETTING: the Second Hospital of Jilin University.MATERIALS: The experiment was carried out in the Department of Pathobiology, School of Basic Medical Sciences, Jilin University from February to October in 2006. Human umbilical cord about 20 cm length came from one neonatal infant who was delivered normally after enough months in the Department of Gynecology and Obstetrics, the Second Hospital of Jilin University. Human umbilical cord was sampled in the informed consents of the infant's family member. The experimentation was authorized by the medical ethic committee of the hospital. PEG-PLA-PGL membranes were provided by Changchun Institute of Applied Chemistry, Chinese Academy of Sciences. Inverted microscope and phase-contrast microscope were bought from Olympus Company (Japan).METHODS: HUVECs cultivated and grew steadily, were inoculated onto PEG-PLA-PGL membranes, serving as the experiment group. While the culture medium without PEG-PLA-PGL membranes were taken as the control group.①Cytocompatibility of PEG-PLA-PGL membranes with HUVECs was evaluated by observing cellular growth through phase-contrast microscope.②The proliferation index of cells was detected by MTT method in 1, 3, 5 and 7 days after inoculation.MAIN OUTCOME MEASURES: ①Cytocompatibility of PEG-PLA-PGL membranes with HUVECs;②The proliferation index of cells in l, 3, 5 and 7 days after inoculationRESULTS: ①Cytocompatibility of PEG-PLA-PGL membranes with HUVECs: The observation result of phase contrast microscopy showed that, endothelial cells planted on the PEG-PLA-PGL membranes began to attach and stretch after being planted 4-6 hours. Three days later, cells grew in colonies rapidly, after 5 days, colonies began to fuse and seemed like cobble-stone. The cells were shuttle or polygon in shape after passages. There were no significant differences between the experiment and control group. Cells cultured on PEG-PLA-PGL membranes for 15 days grew in inserts with membranes, but they didn't grow into patches through scanning electron microscope.②The proliferation index of cells: No significant differences of the proliferation index of cells were detected by MTT method in 1, 3, 5 and 7 days after inoculation between experiment group and control group (P>0.05).CONCLUSION: Endothelial cells grow well in PEG-PLA-PGL membranes, and the two have good cytocompatibility.
