2.The Evaluation of Treatment Effect of Respiratory Disease for 68 Cases of VLBWI

Objective To discuss the main complication and death cause of VLBWI,and enhance survival rate. Methods We summarized our experience and analyzed distribution of respiratory disease diseases of 68 cases of VLBWI. Results 51 cases were cured and 9 cases were died. Conclusion The respiratory disease was the main complication and death cause of VLBWI. The survival rate of VLBWI could be improved by being in NICU,intervening early with medicine and strengthening management of respiratory tract.

4.Role of apparent diffusion coefficient for predicting the consistency of intracranial meningiomas

Objective To investigate the correlation between minimum apparent diffusion coefficient ( ADC) values and consistency of intracranial meningiomas of different parts and nature, providing radiographic evidence of selection for surgical approach. Methods The routine MRI, diffusion-weighted imaging imaging (DWI), ADC maps and ADC value of 23 patients (25 cases) who were confirmed as men-ingiomas through surgery and pathology were retrospectively analyzed and compared with the surgical findings and pathology results. Results ADC values in meningiomas with a hard consistency were significantly lower than those with a soft consistency. Meningiomas with low mini-mum ADC were considered to have a hard consistency. Conclusion The consistency of meningiomas is an important factor in developing the strategy of surgical resection and predicting the degree of removal. The minimum ADC value might have clinical use as a predictor of menin-gioma consistency.

Objective To evaluate the effect of inhalation of furosemide on lung function and airway hyperresponsiveness in asthmatic patients.Methods By double blind randomized controlled trial,the effect of inhalation of furosemide (20mg twice daily) on the lung function and airway hyperresponsiveness in the asthmatics was studied.Results Most of lung function tests of the observation group improved 10 days after inhalation therapy.The improvement of VC,FVC and FEV 1 was significant statistically.There was also slight improvement of lung function in the control group,but not statistically significant.but it was decreaded slass.Conclusion Asthma can be relieved by inhalation of furosemide.
7.Long-term effects of meniscal allograft transplantation to repair rabbit knee cartilage by different methods

BACKGROUND: There are no ideal substitutes for meniscus. Meniscel allograft transplantation has become a feasible method to retain maniscal function following injury. Currently, the meniscus conservancy methods include fresh meniscus, low temperature preserved meniscus; freeze-dried meniscus.OBJECTIVE: To observe the long-term effects of meniscus allograft transplantation by different methods.METHODS: A total of 70 adult New Zealand rabbits were used, and 30 were selected as meniscus donors. The remaining 40 rabbits were divided into 4 groups (n=10): after meniscectomy, control group was not treated; fresh group underwent fresh meniscus allograft transplantation; low temperature group underwent low temperature preserved meniscus allograft transplantation; freeze-dried group underwent freeze-dried meniscus allograft transplantation. Of 30 donors, 10 received fresh meniscus transplantation, and the other received temperature preserved meniscus and freeze-dried meniscus transplantation 1 week after low temperature and freeze dried preservation. The rabbits were sacrificed 12 months after surgery for gross and histological observation.RESULTS AND CONCLUSION: The meniscus allografts of rabbits healed well, and no rabbits died. Gross observation showed that the appearance and elasticity of fresh meniscus group was similar to normal meniscus; tibial plateau was covered well, with no additional wearing in the cartilage of tibial plateau or condyles of femur. In low temperature and freeze-dried preservation groups,the meniscus grafts healed well, but their volume was small and the elasticity was poor; tibial plateau was only partially covered.Pathological observation suggested that fresh meniscus group had orderly arranged collage fiber and similar chondrocyte appearance, number and distribution to normal meniscus, while sparse collagen fiber end small number of meniscus grafts were observed in the other groups. Results show that meniscal allogrefting can survive, retain certain function and structure and prevent the degenerative changes of the knee joint. Moreover, fresh meniscal grafts exhibit best effects, as the grafts following low temperature or freeze-dried preservation degenerate and their volume diminish 1 year following meniscectomy.
8.Accuracy of procalcitonin for diagnosis of sepsis in adults:a Meta-analysis

ObjectiveTo assess the clinical value of procalcitonin (PCT) in the diagnosis of sepsis in adults.Methods An extensive search for related literature from the Wanfang data, CNKI, VIP, Medline/PubMed, Embase/OvidSP and the Cochrane Library up to December 2014 was performed. The articles, including prospective observational studies or randomized controlled trials, regarding PCT for the diagnosing of sepsis were enrolled. Only patients older than 18 years were included. Patients with sepsis, severe sepsis, or septic shock served as the experimental group, and those with a systemic inflammatory response syndrome (SIRS) of non-infectious origin as control group. The language of literature included was English or Chinese. The quality of the studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool. Heterogeneity, pooled diagnostic odds ratio (DOR), pooled sensitivity, pooled specificity, pooled positive likelihood ratio, pooled negative likelihood ratio, the area under the summary receiver operating characteristic curve (SROC) and subgroup analysis were analyzed with the software of Metadisc 1.4.Results A total of 6 385 published reports were collected, and among them 24 met the inclusion criteria, including a total of 3 107 patients. The studies showed substantial heterogeneity (I2 = 69.4%), and random effect model was used for Meta analysis, showing that the pooledDOR was 10.37 [95% confidence interval (95%CI) = 7.10-15.17]. No evidence of a threshold effect was found (Spearman correlation coefficient = 0.27, calculated by logarithm of sensitivity and logarithm of 1-specificity,P = 0.20). TheDOR values of pooled and each study were not distributed along the same line in forest plots, and Cochran-Q = 78.33,P = 0.000 0, showing that there was heterogeneity in result from non threshold effect. Except for partial heterogeneity caused by non threshold effect, the result of Meta regression analysis including PCT detection method, categories of disease, research location and so on showedP values were all higher than 0.05. Thus, the heterogeneity could not be explained by Meta regression analysis. The pooled sensitivity was 74% (95%CI = 72%-76%), the pooled specificity was 70% (95%CI = 67%-72%), the pooled positive likelihood ratio was 2.79 (95%CI = 2.31-3.38), the pooled negative likelihood ratio was 0.34 (95%CI = 0.28-0.41), and the pooled AUC was 0.83 (95%CI = 0.79-0.87). AUC in medical patients was 0.80 (95%CI = 0.75-0.85), which was higher than that in surgical patients [0.71 (95%CI = 0.65-0.81)].Conclusions Our results indicate a moderate degree of value of PCT for diagnosis of sepsis in adult patients. The diagnostic accuracy in medical patients is higher than that in surgical patients. PCT is a good auxiliary biomarker for diagnosis of sepsis.

10.Frequency of metabolic syndrome and correlation with metabolic syndrome in patients with psoriatic arthritis

Objective:To investigate the frequency of metabolic syndrome (MS) in patients with psoriatic arthritis (PsA) and further analyze the correlation of MS and its components with clinical features of PsA.Methods:Data including demographics, clinical manifestations, laboratory tests, MS-associated features (height, weight, waist circumference, blood pressure, serum lipid spectrum, and so on) and history of complications (hypertension, diabetes mellitus, atherosclerosis, coronary heart disease, and cerebral vascular disease) were collected from PsA patients in our hospital from Jan 2017 to Sep 2019. The frequency of MS in PsA patients was calculated and the association between PsA clinical manifestations and MS as well as its components was investigated.Results:One hundred and sixty-two PsA patients who fulfilled the Classification Criteria for Psoriatic Arthritis (CASPAR) were recruited. Hypertension was identified in 36 (22.2%) patients, diabetes mellitus in 28(17.2%) patients, coronary heart disease in 11(6.7%) patients, and cerebral vascular disease in 7 (4.3%) patients. Based on the criteria of the International Diabetes Federation (IDF), 58(35.8%) patients were diagnosed as MS. Compared with MS-free patients, patients with MS, hypertension or diabetes mellitus were older [(54±10 vs 44±13; 56±11 vs 45±12; 54±11 vs 44±13, respectively, t=5.058 , 4.450, 5.150, P<0.01 for all], with higher disease activity [DAPSA scores 16.75(11.25, 26.7) vs 8.8(4.8, 16.4), 16.3(9.6, 27.8) vs 10.0 (5.1, 18.0), 14.4 (9, 25.7) vs 9.5 (5, 17.7), Z=4.539 , 3.046, 3.063, P<0.01]. There was a positive correlation between the sum of components of MS and DAPSA score ( r=0.27 , P<0.01), but multiple linear regression showed no correlation between each component with DAPSA score ( P>0.05) except for hypertension ( P<0.01, standard coefficient=0.334) and elevated fasting blood glucose ( P=0.023, standard coefficient=0.247). PsA patients with hypertension had higher ESR [16.5 (9.5, 34.25) mm/1 h vs 10 (5, 24.5) mm/1 h, Z=2.127, P=0.012]. CRP level was higher in patients with dyslipidemia [5.6(2.1, 17.8) mg/L vs 3.7(1.5, 6.5) mg/L, Z=2.543, P<0.01]. Prevalence of inflammatory back pain was also higher in dyslipidemia patients (41.3% vs 22.4%, χ2=5.901, P=0.016). DAPSA score was higher in dyslipidemia patients (14.1 vs9.9, P=0.031). Conclusion:MS and its components are not rare comorbidities in PsA patients. PsA patients with MS tend to be older with higher disease activity, which calls for more attention.
