1.Minimally invasive operation in the treatment of severe hypertensive cerebral hemorrhage in basal ganglia

Objective To investigate the clinical effect of minimally invasive surgery via basisylvian fissure approach in the treatment of severe hypertensive cerebral hemorrhage in basal ganglia. Methods The Study Group included 28 patients who received a minimally invasive surgery via basisylvian fissure approach, while the Control Group had 26 patients who underwent a traditional hematoma removal. Comparisons were made between the two groups on short-term (1 month) and long-term (6 months) effects. Results The rates of excellent or good short-term outcomes and mortalities were 53.6% (15/28) and 17.9% (5/28) in the Study Group, and 46.2% (12/26) and 15.4% (4/26) in the Control Group, respectively, without significant differences between the two groups (?2=0.297, P=0.586; ?2=0.000, P=1.000). However, the activity of daily life (ADL) grade I~Ⅲ accounted for 63.2% (12/19) of long-term survivors in the Study Group, which was superior to the Control Group (23.6%, 4/17) (z=-2.189, P=0.029). Conclusions Minimally invasive surgery for severe hypertensive cerebral hemorrhage in basal ganglia provides better life quality of survivors after operation than traditional hematoma removal.

3.Curative effect analysis of neuroendoscopic surgery via superior frontal sulcus in the treatment of ;hypertensive intracerebral hemorrhage

Objective To investigate the surgical technique and the curative effects of neuroendoscopic surgery via superior frontal sulcus in the treatment of hypertensive intracerebral hemorrhage. Methods The clinical data of 63 patients with hypertensive intracerebral hemorrhage were analyzed retrospectively. Thirty-one of them were treated by neuroendoscopic surgery via superior frontal sulcus(neuroendoscopic surgery group), and 32 of them were treated by mini- invasive drainage (conventional therapy group). All of them were followed up for 6 months, and were assessed by the activity of daily living (ADL) scale. Results After treatment, all patients reviewed CT. The clear rate of hematoma in neuroendoscopic surgery group was 86.0%, in conventional therapy group was 23.3%, and there was significant difference (P<0.05). There were one death case in neuroendoscopic surgery group and 2 death cases in conventional therapy group. The survival patients were followed up for 6 months .The rate of better prognosis in neuroendoscopic surgery group was 83.3%(25/30), in conventional therapy group was 53.3%(16/30), and there was significant difference (P<0.05). Conclusions The surgical technique of neuroendoscopic surgery via superior frontal sulcus in the treatment of hypertensive intracerebral hemorrhage is safe and effective.
4.Characteristics of hysteromyoma MRI images

Objective To study the common imaging characters of the human tissues such as tumors in MRI based on hysteromyoma in hope of getting the results that the same tissue has the same MR characters and identifying the common,comparatively steady MR figs sectional area which can provide related references for the computer-based automatic recognition orgnization and basis for the question such as whether the same MR figs has the same hyperacoustic impedance characters.Methods We analysed all the images with hysteromyoma on MRI T2 and got the rules on MR figs change with the application of the MRI image analysing software which specially designed by Microsoft Visual C++ 6.0 for the research.Results(1)No significant difference was found in MR figs sectional area in the different periods of the same patient under the same MRI test condition.(2)No significant difference was found in MR figs sectional area in different patients under the same MRI test condition.(3)Significant difference was found in MR figs sectional area after the patient received the HIFU treatment under the same MRI test condition.Conclusion(1)Human tissues have some common MRI imaging characters in the same MRI test equipment under the same MRI test condition.(2)Imaging characters of the human body tissues in MRI can be altered after HIFU treatment.
5.Automatic drawing on target therapy area in HIFU treatment based on MRI and CT imagings

Objective To study automatic drawing on target therapy area based on MRI and CT imagings in order to get the precision less than 1 mm in HIFU treatment.Methods Based on MRI and CT imagings,automatic image was recognized and the frame was extracted by image processing and image segmentation,etc..Results The system of software we are studying can be effectively used to recognize medical automatic images and get target therapy areas' image drawings.Conclusion This system of software will be used in medical precise treatment systems,such as HIFU 3D Treatment Regional Planning System.
6.Early image alternations in 55 patients with primary liver cancer treated by high intensity focused ultrasound

ObjectiveTo evaluate the early image alterations of primary liver cancer (PLC) treated by high intensity focused ultrasound (HIFU). MethodsA total of 55 PLC patients underwent extracorporeal HIFU ablation. Color Doppler ultrasound, digital substraction angiography (DSA), computed tomography (CT) or magnetic resonance imaging (MRI) were performed to monitor the therapeutic response.ResultsImage examinations obtained 2 weeks after HIFU ablation showed significant response of PLC. MRI was the best for evaluating HIFU efficacy. Unenhanced MRI revealed signal alterations on both T 1 and T 2 weighted images, and enhanced MRI demonstrated the absence of contrast enhancement in the treated HCC and a thin peripheral rim of enhancement surrounding the coagulation necrosis. ConclusionMRI rapidly assesses HIFU induced complete ablation of HCC, residual viable cancer tissure in treated region, and coagulation extent of HIFU treatment.
7.Enhancing effects of microbubble contrast agent of different dosage on high-intensity focused ultrasound ablation in goat liver in vivo

Objective To investigate the enhancing effect of microbubble contrast agent SonoVue of different dosage on high-intensity focused ultrasound (HIFU) ablation in goat livers in vivo. Methods Twenty goats were divided into 4 groups randomly. Animals in group 1,2 and 3 were bolus-injected of 0.01 ml/kg,0. 03 ml/kg and 0.05 ml/kg of SonoVue intravenously before HIFU exposure, respectively,and those in group 4 were not given injections as control. After injection 20s, the livers were ablated using HIFU performed in the manner of a single dot set by a computer system using a clinical device. The frequency of HIFU was 0.8 MHz,the intensity of HIFU was 19 100 W/cm2 ,the distance from skin to the target liver tissue was 30 mm,the exposure time was set at 15 s for all animals in the four groups. All animals were euthanized 7 days after HIFU, volumes of coagulated necrosis were measured. Pathological examinations were performed to analysis the exposure regions. Results Under the same parameters of exposure, coagulated volumes in group 1,2 and 3 were larger than those in group 4, the difference was significant (P < 0.05),and the coagulated volumes increased gradually with the dosage of SonoVue increasing from group 1 to group 3, the difference was significant (P<0.05). Pathological examinations confirmed that there were no residual unaffected tissues within the exposed volume. Two remarkable changes were observed in one goat in group 37 days after HIFU:the surrounding adjacent tissue outside the reactive zone necrotized and the skin were destroyed. Conclusions The enhancing effects of microbubble contrast agent in HIFU ablation is related with the dosage of the microbubble contrast agent SonoVue. The higher the SonoVue dosage,the larger the volume destroy in the target tissue.
8.The effects of low intensity pulsed ultrasound on repair of articular cartilage injury in rabbits

Objective To investigate the effects of low intensity pulsed ultrasound ( LIPU ) on articular car-tilage injury in rabbits. Methods Ten adult New Zealand white rabbits with bilateral, full-thickness osteochondral defects in the cartilage surfaces of intercondylar fossas were used as model. The left knees in these rabbits treated with LIPU were used as experimental group. The right knees treated with sham intervention were used as controls. All the rabbits were sacrificed at the 8th week postoperation for gross appearance grading, histological grading and semi-quan-titation of proteoglycan in the repairing tissue. Results Compared with controls, defects treated with LIPU im-proved significantly as shown by gross appearance grades, histological grades and optical density of toluidine blue at the 8th week postoperation (both P <0.05). Conclusion LIPU could accelerate repairing of articular cartilage in-jury.
9.Effects of safflower injection on serum superoxide dismntase and malonyldialdehyde with sudden deafness

Objective To explore the treatment mechanism of safflower injection on sudden deafness through the changes of the hearing threshold, serum superoxide dismntase (SOD) and malonyldialdehyde (MDA) before and after treatment. Methods 40 cases of sudden deafness were divided into two groups: groupⅠ(group with safflower) and groupⅡ (group without safflower). The concentrations of serum MDA and SOD and hearing threshold were detected before and after treatment. 25 cases of normal hearing as the control group. Results (1) The total efficiency in the group Ⅰwas higher than the group Ⅱ (P<0.05). (2)The concentration of SOD was lower and the concentration of MDA was higher than that of the control group before treatment in both two groups (P<0.05). The concentration of SOD was higher and the concentration of MDA was lower after treatment in group I (P<0.05). The concentrations of SOD and the MDA had no significant change in groupⅡ (P>0.05). Conclusion Safflower injection could scavenge oxygen free radical and improve the quality of hearing recovery.
10.Effect of ultrasound in repair of rabbit articular cartilage injury

Objective To observe the effect of low intensity pulsed ultrasound (LIPU) in repair of the articular cartilage injury in rabbits. Methods Osteochondral defects in the cartilage surfaces of femoral intercondylar fossae were made in 33 healthy adult New Zealand White rabbits, of which the knee lesions of the left hind legs were treated with LIPU (LIPU group) while that of the right ones used as control group. All animals in two groups were sacrificed respectively at the 2nd, 4th and 8th weeks postoperatively for gross appearance observation and histological examination. Results The ultrasound-treated tissues became more mature with in advance repair or regeneration. The repaired tissues in the LIPU group were near to normal hyaline cartilage tissues at the 8th week postoperatively, while the repaired tissues in the control group were weak, near to fibrocartilaginous tissues. Conclusion LIPU can promote repair of articular cartilage injury in rabbits.
