1.Two-dimensional and three-dimensional dobutamine stress echocardiography in assessment of cardiac ischemia in the elderly

Objective This study was designed to test the feasibility and efficacy using live three-dimensional echocardiography (3D) to detect ischemia in elderly people during dobutamine-induced stress(DSE) and two-dimensional echocardiography (2D). The results were compared with that of the conventional methods. Methods Live three-dimensional echocardiography, a new 3D imaging technique, offers rapid acquisition with comprehensive simultaneous views of the left ventricle (LV), and it is applicable during stress. A total of 83 consecutive patients(aged 51-86 years)were screened for image quality by 2D and 3D within 30 s at baseline and at peak DSE. Results Live three-dimensional echocardiography and 2D showed good concordance in detection of abnormal LV wall motion at baseline (r=0.82, Kappa=0.56) and at peak DSE (r=0.81, Kappa=0.62). Left ventricular wall motion scores were similar at baseline and peak DSE using both techniques. Interobserver and intraobservr agreement for detection of ischemia at peak DSE were superior for3D DSE and at baseline for 2D DSE. 2.Clinical value of ultrasonic examination of related blood vessels before coronary artery bypass graft

Objective To investigate the value of screening bridge vessels before coronary artery bypass graft(CABG) by ultrasonic examination. Methods To analyze the ultrasonic result of internal mammary arteries,carotid arteries,and radial arteries before CABG. Results Among the 140 cases,11 internal mammary arteries were not qualified,14 of 74 radial arteries were not qualified. And the stenosis of carotid arteries were present in 28 cases. Conclusions It is valuable to examine the bridge vessels before CABG.
3.Comparison of live three dimensional transthoracic and transesophageal echocardiography in detection of thrombus in the left atrial appendage

Objective To evaluate live three dimensional transthoracic echocardiography (Live 3D TTE) for the detection of left atrial appendage thrombus(LAAT). Methods This is a prospective protocol recruiting 26 cases with mean age(61?7) years. Each patient received Live 3D TTE examination before transesophageal echocardiography (TEE). The images were analyzed with double blind rules. The results of Live 3D TTE were evaluated by the golden standard of TEE results. Results The sensitivity, specificity, accuracy, positive predictive value and negative value of Live 3D TTE were 73%, 60%,65%,57%,75% respectively. 4.Assessment of the left thoracic artery graft late postoperative patency by ultrasound contrast with angiography

Objective To access the left internal thoracic artery (LITA) graft late postoperative patency after coronary artery bypass grafting (CABG) by peripheral blood vessel ultrasound combined with color Doppler coronary flow imaging (CDCFI).In contrast with angiography,try to find available flow parameter to access graft patency.MethodsForty-six patients with CABG more than 1 year postoperatively followed-by angiography were detected by ultrasound.The LITA graft and left anterior descending artery were examined.Systolic and diastolic peak velocity(Smax,Dmax),velocity time integral(VTIs,VTId)of each segment were measured separately.The ratio of diastolic and systolic peak velocity (D/S),and diastolic velocity time integral fraction(DVTIF) were calculated.All patients were divided into groups according to angiography results.ResultsThirty -one LITA grafts were patent,11 were dysfunctional,4 were occlusive.According to the angiography results,the flow parameters of the proximal segment of LITA graft were significant.The D/S and DVTIF of patent group was higher than that of dysfunctional group.The diastolic peak velocity of distal segment of LAD of patent group was higher than that of dysfunctional group.ConclusionsPeripheral blood vessel ultrasound combined with CDCFI could provide the evidence to access the patency of the graft.It was an effective method for the clinical follow-up.
5.The role of spatio-temporal image correlation with tomographic ultrasound imaging in the prenatal diagnosis of conotruncal defects

Objective To investigate the application value of spatio-temporal image correlation (STIC) combined with tomographic ultrasound imaging(TUI) in the prenatal diagnosis of conotruncal defects(CTD).Methods Two-dimensional(2D) fetal echocardiography to screen and TUI-STIC volumes from 1508 cases of fetuses of high risk with congenital heart disease.Postnatal work-up and pathological results were available for all fetuses with CTD.Results Thirty nine cases with CTD were found by TUI-STIC while thirty five cases were found by 2D echocardiography,but TUI-STIC had new findings and corrected the diagnosis in 9 cases as compared with 2D echocardiography.The sensitivity,specificity,positivity predictive value,negative predictive value and accuracy of TUI-STIC in evaluating CTD were 97.5 %,100%,100 %,99.9 % and 99 %.The Kappa value of consistency test between 2DE and TUI-SIC was 0.244(P < 0.01),McNemar test showed that the difference was statistically significant (P < 0.01).Conclusions TUI-STIC allows a complete sequential analysis of fetal conotruncal defects and supplying additional information over 2D fetal echocardiography,it could improve the prenatal diagnosis rate.TUI-STIC is helpful in diagnosis of prenatal conotruncal defects.
6.Meta analysis of euthyrox on the blood lipid of patients with hypothyroidism

Objective To study the effect of euthyrox on blood lipids in patients with hypothyroidism throngh the meta analysis.Methods Patients with clinical hypothyroidism theated by euthyrox on blood lipid effect of randomized controlled trials were retried by Pubmed(2000/2014-07),EMbase (2000/2014-07),the Cochrane library(2014,7),the Chinese biomedical literature database(2000/2014-07),retrieved the time limit from January 2000 to July 2014.The paper were objective evaluated of the quality,Meta analysis with Rev Man 5.3 software system applicated by Cochrane collaboration.Results Euthyrox could significantly reduce cholesterol levels of hypothyroidism patients ,it was statistically significant difference(OR=0.63,95%CI:0.71 ~0.55,P<0.001);Euthyrox could significantly reduce triglyceride levels of hypothyroidism patients,difference was statistically significant(OR =0.29,95%CI:0.34 ~0.25,P <0.001 );Euthyrox could significantly reduce low-density lipoprotein cholesterol levels of hypothyroidism patients,it was statistically significant difference(OR=0.22,95%CI:0.30 ~0.14,P<0.001);euthyrox could obviously increase high-density lipoprotein cholesterol levels, it was statistically significant difference (OR =0.21, 95% CI:0.17 ~0.26, P <0.001 ). Conclusion Euthyrox could significantly reduce total cholesterol,triglyceride,low-density lipoprotein cholesterol(hdl-c)of hypothyroidism patients,and promote the synthesis of high-density lipoprotein cholesterol(hdl-c),could effectively promote the lipid metabolism,reduce blood fat level,prevent the happening of the disease of heart head blood-vessel hypothyroidism patients,have the guide meaning to clinical treatment.
7.A Tumor-Specific Tissue-Penetrating Peptide Enhances the Efficacy of Chemotherapy Drugs in Gastric Cancer.

PURPOSE: C-end rule (CendR) peptides are found to enhance the penetration of chemotherapeutic agents into tumor cells, while GX1 is a peptide that homes to gastric cancer (GC) vasculature. This study aimed to synthesize a novel peptide GX1-RPAKPAR (GXC) and to explore the effect of GXC on sensitizing GC cells to chemotherapeutic agents. MATERIALS AND METHODS: Intracellular Adriamycin concentration analysis was applied to conform whether GXC peptide increases the penetration of chemotherapeutic agents into GC cells in vitro. The effect of GXC peptide on sensitizing GC cells to chemotherapeutics was validated by apoptosis assay and in vitro/vivo drug sensitivity assay. The specificity of GXC to GC tissue was validated by ex vivo fluorescence imaging. RESULTS: In vitro, administration of GXC significantly increased Adriamycin concentrations inside SGC-7901 cells, and enhanced the efficacy of chemotherapeutic agents by decreasing the IC50 value. In vivo, FITC-GXC specifically accumulated in GC tissue. Moreover, systemic co-injection with GXC peptide and Adriamycin statistically improved the therapeutic efficacy in SGC-7901 xenograft models, surprisingly, without obviously increasing side effects. CONCLUSION: These results demonstrated that co-administration of the novel peptide GXC with chemotherapeutic agents may be a potential way to enhance the efficacy of anticancer drugs in GC treatment.
