[Objective]To determine whether discectomy resulted in improvement in low back pain(LBP)associated with lumbar disc herniation. [Method]Two hundred and one consecutive single-level lumbar discectomy were followed up,one hundred patients underwent microendoscopic discectomy(MED group) and 101 patients were treated with conventional open discectomy(COP group).Outcomes were assessed using Visual Analog Scale(VAS) and patient satisfaction.The changes in LBP were compared between the MED group and COP group.The effects of severity of low back pain before surgery on the improvement of back and leg pain were also determined.The mean duration of follow-up was 45 months.[Result]The mean VAS leg pain significantly decreased from 70.02?15.80 before operation to 7.73?12.13 at final follow-up.A significant improvement was also noted in LBP,the mean VAS LBP score was 43.61?26.18 before surgery and 16.89?14.73 at the time of final follow-up.The important improvement in LBP and leg pain was seen in 70.1%～78.1% and 97.0% patients respectively.Improvements in back pain and leg pain were correlated(r= 0.1656,P=0.0304).The rate of important improvement of low back pain was greater in MED group than in COP group.The rate of important improvement in LBP was similar in the patients of a preponderance of LBP,a preponderance leg pain and an equivalence of LBP and leg pain,the patients with a preponderance of LBP showed a lower rate of important improvement in leg pain than the others.[Conclusion]Discectomy for relief of sciatica of lumbar disc herniation provides effective relief of LBP.The MED is more likely effective as a surgery way to relief LBP.Furthermore,it certified that the severe LBP maybe a negatively predictive factor for the follow-up outcome of leg pain.

[Objective]This study was designed to construct a recombinant adenovirus vector contains LMP-1 gene,and investigate the osteoinductive activity of MSC which were transfected recombinated adenoviral vector carrying LMP-1 gene.[Methods]Total RNA was extracted from mt osteoblast and the LMP-1 gene was acquired by RT-PCR,the LMP-1 gene and entry vector pENTR/D-TOPO were used to create the entry clone with the directional TOPO clone technology,then the entry clone and the expression vector were used to create the expression clone throush the LR recombination reaction.The adenovirus expression clone was linearized by PacI and transfected to the 293A cell line to harvest a high titer.Ad-LMP-1 was infected into the 3rd passage MSC,the expression of LMP-1 was detected by Western blot.The osteogenic activity of MSC was evaluated by the expression of collagen Ⅰ,ALP,osteocalcin and the formation of bone nodule.[Result]The LMP-1 gene was successfully acquired and confirmed,the entry clone and the expression clone were both verified by enzymes digestion,and the expression clone was further confirmed by sequenced.The expression of LMP-1 was detected successfully in MSC.The increasing expression of collagen Ⅰ,osteocalcin.ALP and bone nodule were observed by comparing to the control group.[Conclusion]Gateway technology not only make construction of the pAd-LMP-1 recombination adenovirus vector simple and fast,but also get a high transfection efficacy in MSC.LMP-1 gene can induce the osteoblast differention of MSCs,and improve its osteogenic activity.The adenovirus vector is reliable to be used in further gene therapy research.

Objective To investigate the awareness of osteoporosis and its influential factors in middle-aged and older people. Methods By using self-designed questionnaire, 354 middle-aged and old people were investigated as to their awarness of osteoporosis in Chajiao street community in Guangzhou. Among them, 58 people were measured the bone density in the calcaneal bone by Ultrasound. Results The average score of the questionnaire was 9.46 ± 5.13, and the accuracy was 39.4%. In the diagnosis and treatment, the accuracy was just 17.8%. The diet (the accuracy in 54%) and sport (the accuracy in 50.3%) were good. The statistical analyses showed the linear relationship between the T value of bone density and the age (P = 0.021), between the T value and the score (P = 0.047). Wilcoxon rank sum test found that the T value between female and male was significantly different (Z = -3.749, P < 0.000). With multiple linear regression analysis, the T value had a linear correlation with the score (F = 4.224, P = 0.045) positively. Conclusions Currently, the awareness of osteoporosis in the middle-aged and old people was inadequate. To better prevent osteoporosis, the propogation of knowledge and guidance provided by clinicians in osteoporosis should be improved necessarily and emergently.

This article devotes itself to evaluating the biocompatibility of synthetic macromolecular resin class of Comfort denture adhesive, which was developed by the present authors. Acute toxicity, haemolysis, cytotoxicity, sensitization and oral mucous stimulation were tested with standard method (ISO7406-1997 and YY0268-1995). The results showed that no toxic effect was observed with in vivo tests and no cytotoxic effect was observed with in vitro MTT assay. Haemolysis rate of the material (2.95%) indicated good hemaocompatibility. No local mucous membrane irritation reaction was noted after the relevant tests. The developed Comfort denture adhesive exhibited good biocompatiblility.

OBJECTIVETo prepare a new synthetic macromolecular resin class of denture adhesive (named Comfort-DA).

METHODSTwelve different formulae for denture adhesive were designed by orthogonal design. Each factor affecting tensile bond strength of adhesive was analyzed by ANNOVA to select the best formula. Then the chemical and physical properties, such as tensile bond strength, pH value, and viscosity, were evaluated and compared with Fittydent in vitro also.

RESULTSThe effect of content of polyvinyl acetate and cellulose ether on tensile bond strength exhibited no statistical significance (P > 0.05), while ethyl alcohol and CMC-Na showed statistical significance and mutual action (P < 0.05). Results of testing viscosity and shearing stress showed that there was no significant difference between Fittydent and Comfort-DA (P > 0.05), while Fittydent exhibited a significantly lower tensile bond strength than the prepared one (P < 0.05). The prepared adhesive demonstrated a neutral or slightly basic pH.

CONCLUSIONSThe generally favorable in vitro performance of the prepared adhesive (Comfort-DA) formulation indicated that it might be efficacious denture adhesive.

OBJECTIVETo observe whether olfactory ensheathing cells could be used to promote axonal regeneration in a spontaneously nonregenerating system.

METHODSAfter laminectomy at the lower thoracic level, the spinal cords of adult rats were exposed and completely transected at T10. A suspension of ensheathing cells was injected into the lesion site in 12 adult rats, and control D/F-12 (1:1 mixture of DMEM and Ham's F-12) was injected in 12 adult rats. Six weeks and ten weeks after cell transplantation, the rats were evaluated by climbing test and motor evoked potentials (MEPs) monitoring. The samples were procured and studied with histologicl and immunohistochemical methods.

RESULTSAt the 6th week after cell transplantation, all the rats in both the transplanted and control groups were paraplegic and the MEPs could not be recorded. At the 10th week after cell transplantation, of 7 rats in the control group, 2 rats had muscles' contraction of the lower extremities, 2 rats had hips and/or knees' active movement; and 5 rats' MEPs could be recorded in the hind limbs in the transplanted group (n=7). None of the rats in the control group had functional improvement and no MEPs recorded (n=7). Numerous regenerating axons were observed through the transplantation and continued to regenerate into the denervated host tract. Cell labelling using anti-Myelin Basic Protein (MBP) and anti-Nerve Growth Factor Receptor (anti-NGFR) indicated that the regenerated axons were derived from the appropriate neuronal source and that donor cells migrated into the denervated host tract. But axonal degeneration existed and regenerating axons were not observed within the spinal cords of the adult rats with only D/F-12 injection.

CONCLUSIONSThe axonal regeneration in the transected adult rat spinal cord is possible after ensheathing cells transplantation.
