1.Serum Level and Clinical Significance of Visfatin in Patients with Active Inflammatory Bowel Disease

Background:The incidence of inflammatory bowel disease( IBD ) is increasing in recent years,however,its pathogenic mechanism has not been fully clarified. Previous studies revealed that adipokines played crucial roles in regulating intestinal inflammation. Aims:To investigate the role of visfatin,an adipocytokine,and its clinical significance in active IBD. Methods:Ninety-one patients with active IBD including 61 Crohn’s disease( CD)and 30 ulcerative colitis ( UC)at the First Affiliated Hospital of Soochow University and Suzhou Municipal Hospital from May 2015 to Dec. 2015 were enrolled in this study,and 48 healthy subjects were served as controls. Serum level of visfatin was determined by ELISA. Correlation of serum visfatin level with clinical features of IBD was analyzed,its diagnostic performance for IBD was assessed by ROC curve. Results:Serum level of visfatin was significantly higher in patients with active CD and UC than in healthy controls[(385. 24 ± 112. 64)pg/mL and(378. 91 ± 118. 57)pg/mL vs. (321. 11 ± 96. 27)pg/mL, P all ﹤0. 05]. Significant positive correlation was found between serum visfatin level and disease activity index(Mayo score)of UC( r =0. 398,P ﹤0. 05 ),however,no correlations were found between serum visfatin level and disease activity index of CD,CRP and ESR,two common inflammatory indicators for IBD and location of IBD(P all ﹥0. 05). The area under curve( AUC)of serum visfatin for diagnosis of CD and UC were 0. 654 and 0. 622,respectively;the diagnostic accuracy was relatively low. Conclusions:Serum visfatin might be associated with the active intestinal inflammation in IBD and has the potential to be served as a clinical index for active UC.

3.Debugging and Evaluation after Cochlear Implantation

Objective To investigate the adjustment methods and evaluation for children after cochlear implantation. Methods 6 children after artificial cochlear implantation were debugged by subjective psychological physical test, objective methods such as electricity induced composite action potential (ECAP), electrically evoked stapedius reflex threshold (ESRT) and electrically evoked auditory brainstem response (EABR). Results All the children got satisfied data. Conclusion For the children after cochlear implantation, objective methods such as ECAP, ESRT, EABR combined with subjective psychological physical test all together are helpful to obtain accurate threshold.
4.Effect of precondition with GBE50 and Salviae miltionrrhizae on cycloxygenase-2 and its downstream effectors contents in ischemia/reperfusion myocardium.

OBJECTIVETo investigate the changes in contents of cycloxygenase-2 (COX-2) and its downstream effectors in rat's myocardial ischemia/reperfusion (I/R) model and observe the effects of precondition with GBE50 (Ginkgo biloba extract 50) and Salviae miltiorrhizae (SM) on them.
METHODSRat's I/R model was established by 30-min left anterior descending coronary artery occlusion followed with 60-min reperfusion. Animals were divided into the model control group, the sham-operated group and the tested groups (received 1-week precondition with GBE50 and SM respectively via intragastric infusion before modeling). COX-2 mRNA expression in myocardium was detected by real-time PCR; contents of thromboxane B2 (TXB2) and 6-keto-prostaglandin F1alpha (6-keto-PGF1alpha) were measured by radioimmunoassay.
RESULTSThe mRNA expression of COX-2 in the model group was obviously higher than that in the sham-operated group (P < 0.001), while that in the tested groups was down-regulated significantly (P < 0.01), and the content of TXB2 as well as the ratio of TXB2/PGF1alpha was reduced significantly (P < 0.05). Besides, SM also showed the up-regulation effect on 6-keto-PGF1alpha content in myocardium (P < 0.05).
CONCLUSIONCOX-2 affects the myocardium through thromboxane A2 and prostacyclin after I/R; both GBE50 and SM can inhibit the production of COX-2, but they may act in different paths.

5.Effect of coking oven emissions on level of serum oxidation-reduction and peripheral white blood cellular ras P21 and P53 in coke oven workers.

OBJECTIVETo explore the adverse effects of coking oven emissions (COE) on the serum oxidation-reduction and relevant genes in the exposed workers.
METHODSFifty-six coke oven workers and forty controls were investigated. Serum Malondialdehyde (MDA) levels and the activities of total superoxide dismutases (T-SOD) were measured by spectrophotometrical method. Immunohistochemical method was used to assess the P21 and P53 levels in peripheral white blood cells.
RESULTSCompared with controls, the individuals exposed to COE had significantly increased levels of serum MDA [(5.30 +/- 2.29) nmol/mL, P < 0.01] and markedly decreased levels of T-SOD [(100.04 +/- 10.75) NU/mL]. Additionally, the median levels of P53 and P21 were markedly increased in the exposed individuals compared with the controls (21.4% and 23.2%, respectively, all P < 0.05).
CONCLUSIONThe findings indicate that occupational exposure to COE causes the rise of serum oxidation-reduction MDA and the fall of T-SOD, and increasing expression levels of P21 and P53 proteins before the occurrence of apparent clinical symptoms.

6.A system review on efficacy and safety of Shuxuetong Injection in treatment of acute cerebral infarction

Objective Using Ginkgo biloba extract injection as control drug,to systematically review the efficacy and safety of Shuxuetong Injection in the treatment of acute cerebral infarction.Methods Database including PubMed,Medline,CNKI,VIP and WanFang Data were searched to collect randomized controlled trials (RCTs) about Shuxuetong Injection versus G.biloba extract injection in the treatment of acute cerebral infarction from database setup time to July of 2016.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.The meta-analysis was conducted by RevMan 5.2 software.Results Total of 11 RCTs,1 338 patients were included.Meta-analysis showed that Shuxuetong Injection was significantly better than G.biloba extract injection in clinical total effective rate [RR =1.17,95%CI(1.11,1.23),P < 0.01],reducing neurological deficit score [MD =-4.46,95%CI (-6.07,-3.25),P < 0.01] and improving life ability score [MD =13.98,95%CI (11.30,16.65),P < 0.01],there was no serious adverse reaction in both groups.Conclusion Current evidence shows that Shuxuetong Injection is effective and safe in the treatment of acute cerebral infarction better than G.biloba extract injection.

Objective To study on epidemiologic characteristics of human plague cases in Sanjiangyuan Area,and provide theoretical basis to work out the preventive measures.Methods Based upon the epidemiology information from the human plague case data bank of Qinghai Province,human plague data were analyzed retrospectively in Sanjiangyun Area by sorting,verifying and summing up of these data,including some of case file and monitoring data.Results Except for 12 years in the period of 1960 to 2006,there were human plague cases happened every year.The morbidity occurred mainly in 12 counties of 4 states,including Yushu,Guolou,Huangnan and Hainan,and Tanggula Town of Geermu City,a total of 85 human plague episodes were occurred,resulting 238 onsets,134 deaths,and a matality rate of 56.30%.The sources of infection were respectively Himalayan mormot 27.31%(65/238),artiodactyls 14.71%(32/238),carnivora 2.10%(5/238),Lagomorpha 0.42%(1/238),the pneumonic plague patient 49.16%(117/238),and biting of flea 6.30%(15/238).The prevalent season was from May all the way to November,the peak-months were August and September.After October,the sheep as the source of infection initiating human being plague accounted for 23.53%.Among the clinical types,the most prevalent type was pneumonic type(61.34%),and the rest,glandular type(17.23%),septic type(16.81%)and other types(4.62%),but the first plague case in each epidemic was mainly the glandular plague.Conclusions In recent years,the tendency of human plague prevalence increases in Sanjiangyuan Area,it is urgent to improve and adjust the prevention and treatment measures in time.
9.An analysis on the distribution and drug resistance of pathogenic bacteria in infectious diarrhea among infants

Objective To learn the main pathogenic microorganisms,and to acquire the data of pathogens composition anddrug resistance of infant diarrhea disease in Wenzhou City.Methods The diarrheal stool specimens of <5 years oldchildren were collected from outpatients and inpatients in children′s hospital and were cultured and detected.Results Atotal of 201 strains pathogenic bacteria were separated from 51 7 clinical specimens,and the rate of total detection is38.88%,of which included 74 strains intestinal pathogenic bacteria and 1 27 strains diarrhea virus,with the rate of detection1 4.31 % and 24.56%,respectively.Among intestinal pathogenic bacteria,the E.coli was the most common bacteria(36strains,48.65%),followed by salmonella(31 strains,41 .89%).Diarrhea virus included norovirus type 2 (56 strains,44.09%),norovirus type 1 (24 strains,1 8.90%),A group rotavirus (56 strains,31 .50%)and goblet virus(7 strains,5.51 %).The resistance rate of Salmonella to ampicillin was 64.52%,and that to ampicillin,tetracycline,compound newMing were more than 70.00%.The resistance rate of Rifampicin.Novobiocin in all bacteria reached 1 00%.Conclusion E.coli and salmonella epidemic strains were the common pathogenic bacteria in infectious diarrhea among infants inWenzhou City.The predominant viruses were norovirus and rotavirus group A.The drug resistance of various pathogenicbacteria was different,and active surveillance should be strengthened.
10.Endothelial progenitor cells related gene expression changes before and early after revascularization in patients with acute myocardial infarction.

OBJECTIVEThe purpose of this study was to observe the endothelial progenitor cells (EPCs) related gene expression changes before and early after revascularization in patients with acute myocardial infarction.
METHODSPeripheral blood samples were taken from patients with acute anterior myocardial infarction 6 hours and 7 days after PCI and stenting. Mononuclear cells (MNCs) were isolated by Ficoll-density centrifugation and cultured in M-199 medium. After 14 days culture, attaching cells incorporated DiI-acetylated low-density lipoprotein (EPCs) were collected and RNA was isolated by Trizol for microarray analysis on 24 genes associated with permissibility/vessel tone (angiotensin system: ACE, AGTR-1, AGTR-2; NO system: eNOS; prostacyclin system: COX-2; endothelin system: ET-1, ETA, ETB; superoxide anions system: SOD-1), angiogenesis (adhesion molecule: CDH5; growth factors and receptors: VEGFR1, VEGFR2, VEGF) and endothelial cell activation (adhesion molecules expression: ICAM1, ICAM2, ICAM3, PECAM-1, E-Selectin, L-Selection, VCAM1; change phenotype from antithrombotic to prothrombotic: tPA, uPA, PAI, vWF). VEGFR2, PECAM-1 and VE-cadherin positive cells were identified by flow cytometry.
RESULTEight gene expressions (AGTR-1, AGTR-2, COX-2, eNOS, ET-1, ETA, VEGF) were significantly downregulated 7 days post PCI compared to pre-PCI (P < 0.05). Flow cytometry results showed that VEGFR2 positive cells were also significantly reduced post PCI than that of before PCI (P < 0.05).
CONCLUSIONPCI down-regulated endothelial progenitor cells related gene expressions in patients with acute myocardial infarction.
