2.Serum Level and Clinical Significance of Visfatin in Patients with Active Inflammatory Bowel Disease

Background:The incidence of inflammatory bowel disease( IBD ) is increasing in recent years,however,its pathogenic mechanism has not been fully clarified. Previous studies revealed that adipokines played crucial roles in regulating intestinal inflammation. Aims:To investigate the role of visfatin,an adipocytokine,and its clinical significance in active IBD. Methods:Ninety-one patients with active IBD including 61 Crohn’s disease( CD)and 30 ulcerative colitis ( UC)at the First Affiliated Hospital of Soochow University and Suzhou Municipal Hospital from May 2015 to Dec. 2015 were enrolled in this study,and 48 healthy subjects were served as controls. Serum level of visfatin was determined by ELISA. Correlation of serum visfatin level with clinical features of IBD was analyzed,its diagnostic performance for IBD was assessed by ROC curve. Results:Serum level of visfatin was significantly higher in patients with active CD and UC than in healthy controls[(385. 24 ± 112. 64)pg/mL and(378. 91 ± 118. 57)pg/mL vs. (321. 11 ± 96. 27)pg/mL, P all ﹤0. 05]. Significant positive correlation was found between serum visfatin level and disease activity index(Mayo score)of UC( r =0. 398,P ﹤0. 05 ),however,no correlations were found between serum visfatin level and disease activity index of CD,CRP and ESR,two common inflammatory indicators for IBD and location of IBD(P all ﹥0. 05). The area under curve( AUC)of serum visfatin for diagnosis of CD and UC were 0. 654 and 0. 622,respectively;the diagnostic accuracy was relatively low. Conclusions:Serum visfatin might be associated with the active intestinal inflammation in IBD and has the potential to be served as a clinical index for active UC.
3.Debugging and Evaluation after Cochlear Implantation

Objective To investigate the adjustment methods and evaluation for children after cochlear implantation. Methods 6 children after artificial cochlear implantation were debugged by subjective psychological physical test, objective methods such as electricity induced composite action potential (ECAP), electrically evoked stapedius reflex threshold (ESRT) and electrically evoked auditory brainstem response (EABR). Results All the children got satisfied data. Conclusion For the children after cochlear implantation, objective methods such as ECAP, ESRT, EABR combined with subjective psychological physical test all together are helpful to obtain accurate threshold.
4.Antagonism of GBE50 on lesion of myocardial ischemia-reperfusion observed with myocardial mechanics

Objective To observe the preventive effect of extract of Ginkgo biloba (GBE50) on the le-sion of myocardial ischemia-reperfusion (I/R) in vitro in rats, and discuss the evaluation functions of myocardial mechanics and hemodynamics information.Methods SD rats (n=55) were randomly divided into 6 groups including the model group, high-dose, mid-dose and low-dose GBE50 groups, Danshen (Radix Salviae Miltiorrhizae) group and diltiazem group. All groups were given Langendorff heart perfusion in vitro for 15 minutes. The perfusion was stopped for 30 minutes to induce global ischemia and then reperfusion was given in all groups for 40 minute. The left ventricular pressure was recorded and the influences of different drugs on the changes of myocardial mechanics and hemodynamics were observed.Results After I/R the levels of heart rate (HR), mLVSP, developed pressure (DP) and ±dp/dt_(max) were all lower than those before I/R, and the levels of mLVDP and t-dp/dt_(max) increaed significantly (P<0.05 to P<0.001). Along with the prolongation of reperfusion time all indexes tended to normal level. The levels of DP and ±dp/dt_(max) were higher (P<0.05) and t-dp/dt_(max) and mLVDP were lower (P<0.05) significantly in the mid-dose GBE50 group, Danshen group and diltiazem group than those in the model group.Conclusion GBE50 has a significant preventive effect on heart function decline induced by the lesion of myocardial ischemia-reperfusion. The evaluation with myocardial mechanics and hemodynamics is stable and reliable.
5.Effect of precondition with GBE50 and Salviae miltionrrhizae on cycloxygenase-2 and its downstream effectors contents in ischemia/reperfusion myocardium.

OBJECTIVETo investigate the changes in contents of cycloxygenase-2 (COX-2) and its downstream effectors in rat's myocardial ischemia/reperfusion (I/R) model and observe the effects of precondition with GBE50 (Ginkgo biloba extract 50) and Salviae miltiorrhizae (SM) on them.
METHODSRat's I/R model was established by 30-min left anterior descending coronary artery occlusion followed with 60-min reperfusion. Animals were divided into the model control group, the sham-operated group and the tested groups (received 1-week precondition with GBE50 and SM respectively via intragastric infusion before modeling). COX-2 mRNA expression in myocardium was detected by real-time PCR; contents of thromboxane B2 (TXB2) and 6-keto-prostaglandin F1alpha (6-keto-PGF1alpha) were measured by radioimmunoassay.
RESULTSThe mRNA expression of COX-2 in the model group was obviously higher than that in the sham-operated group (P < 0.001), while that in the tested groups was down-regulated significantly (P < 0.01), and the content of TXB2 as well as the ratio of TXB2/PGF1alpha was reduced significantly (P < 0.05). Besides, SM also showed the up-regulation effect on 6-keto-PGF1alpha content in myocardium (P < 0.05).
CONCLUSIONCOX-2 affects the myocardium through thromboxane A2 and prostacyclin after I/R; both GBE50 and SM can inhibit the production of COX-2, but they may act in different paths.

6.Effect of coking oven emissions on level of serum oxidation-reduction and peripheral white blood cellular ras P21 and P53 in coke oven workers.

OBJECTIVETo explore the adverse effects of coking oven emissions (COE) on the serum oxidation-reduction and relevant genes in the exposed workers.
METHODSFifty-six coke oven workers and forty controls were investigated. Serum Malondialdehyde (MDA) levels and the activities of total superoxide dismutases (T-SOD) were measured by spectrophotometrical method. Immunohistochemical method was used to assess the P21 and P53 levels in peripheral white blood cells.
RESULTSCompared with controls, the individuals exposed to COE had significantly increased levels of serum MDA [(5.30 +/- 2.29) nmol/mL, P < 0.01] and markedly decreased levels of T-SOD [(100.04 +/- 10.75) NU/mL]. Additionally, the median levels of P53 and P21 were markedly increased in the exposed individuals compared with the controls (21.4% and 23.2%, respectively, all P < 0.05).
CONCLUSIONThe findings indicate that occupational exposure to COE causes the rise of serum oxidation-reduction MDA and the fall of T-SOD, and increasing expression levels of P21 and P53 proteins before the occurrence of apparent clinical symptoms.

7.Virtual reality system for diagnosis and therapeutic planning of cerebral aneurysms.

BACKGROUNDThe virtual reality (VR) system can provide the neurosurgeon to intuitively interact with and manipulate the three dimensional (3-D) image similarly to manipulate a real object. It was seldom reported that the system was used in diagnosis and treatment of cerebral aneurysms. This study aimed to investigate the application of VR system in diagnosis and therapeutic planning of cerebral aneurysms.
METHODSA total of 24 cases of cerebral aneurysms were enrolled in this study from 2006 to 2008, which diagnosed by 3-D digital subtraction angiography (3D-DSA) or VR-based computed tomography angiographies (CTA). The VR system and 3D-DSA system were used to observe and measure aneurysms and the adjacent vessels. The data of observation and measurements were compared between VR image and 3D-DSA image. All the patients underwent surgical plan and simulated neurosurgical procedures in the VR system.
RESULTSThere were 28 aneurysms detected in VR system and 3D-DSA system. The VR system generated clear and vivid 3-D virtual images which clearly displayed the location and size of the aneurysms and their precise anatomical spatial relations to the parent arteries and skull. The location, size and shape of the aneurysms and their anatomical relationship with the adjacent vessels were similar between 3-D virtual image and 3D-DSA, but the spatial relationship between aneurysms and skull only been displayed by VR system. This VR system also could simulate simple surgical procedures and surgical environments.
CONCLUSIONSThe VR system can provide a highly effective way to provide precise imaging details as same as 3D-DSA system and assist the diagnosis of cerebral aneurysms with virtual 3-D data based on CTA. It significantly enhances the chosen therapeutic strategy of cerebral aneurysms.

8.Endoscopic papillary balloon dilatation vs. endoscopic sphincteropapillotomy for common bile duct stones: a meta analysis.

OBJECTIVETo evaluate the safety and efficacy between endoscopic papillary balloon dilatation (EPBD) and endoscopic sphincteropapillotomy ( EST) for common bile duct stones using meta-analysis method.
METHODSRandomizd controlled trials comparing EPBD with EST for common bile duct stones and published from January 1990 to July 2012 were recruited. This meta-analysis was conducted to estimate short-term and long-term complications. Fixed random effect model or random effect model was established to analyze the data.
RESULTSTwelve randomizd controlled trials were included in this analysis. These studies included 1865 patients, 925 of them were treated with EPBD and 940 were treated with EST. The analysis of basic characteristics of these included studies showed that: compared to EST, patients in the EPBD group were younger (OR = -1.16, 95% CI: -1.49 to -0.84, P = 0.00), while in two groups, there were no significant difference (P > 0.05) in gender proportion, average size of stones, number of gallstones, previous cholecystectomy, the number of merged duodenal diverticulum, common bile duct diameter, the total follow-up time. Also, compared to EST, the overall stone clearance in the EPBD group was lower (OR = 0.64, 95% CI: 0.42 to 0.96, P = 0.03), pancreatitis incidence was higher (OR = 2.67, 95% CI: 1.61 to 4.43, P = 0.00), incidence of bleeding (OR = 0.12, 95% CI: 0.04 to 0.34, P = 0.00), acute cholecystitis (OR= 0.39, 95% CI: 0.18 to 0.84, P = 0.02), total long-term complication rate (OR = 0.53, 95% CI: 0.36 to 0.77, P = 0.01), stone recurrence rate more than a year were lower (OR= 0.48, 95% CI: 0.26 to 0.90, P = 0.02). While in two groups, there were no significant difference (P > 0.05) in the stone removal on 1 '' attempt, the total near-term complications and acute cholangitis.
CONCLUSIONSOn the basis of lower rates of bleeding, EPBD seems to be preferred strategy over EST for endoscopic remove of common bile duct stones in patients who have coagulopathy. Although stone recurrence rate more than a year of EPBD is lower, but the overall stone clearance rate is lower and the risk of pancreatitis is higher than that of EST.

OBJECTIVETo probe into the current situation and problems of establishing control group in clinical studies of acupuncture and moxibustion for treatment of periarthritis of shoulder at home.
METHODSTo retrieve the literatures of establishing control groups in clinical studies of acupuncture and moxibustion for treatment of periarthritis of shoulder in magazines at home, and based on key problems in establishment of control group, make a Excel form to take out relative contents in the literatures and classify, summarize and statistically analyze.
RESULTSIn establishing control group in clinical studies of acupuncture and moxibustion for treatment of periarthritis of shoulder, a lot of problems exist, most studies do not have the basis of establishing control group, and also do not establish control group for the purpose of the study, making the whole test have error in the stage of program design, and leading to unreliable for the results of clinical control studies.
CONCLUSIONIn clinical researches of acupuncture and moxibustion, a standard for guiding establishment of control group is eagerly needed.
