Objective To compare the bony fusion in different degrees of vertebral plate hinge fracture after single open-door cervical expansive laminoplasty with centerpiece mini-plate fixation. Methods The clinical data of 79 patients received the unilateral simple open-door cervical expansive laminoplasty with centerpiece mini-plate fixation from January 2010 to December 2012 were retrospectively analyzed. The vertebral plate hinge fracture was divided into incomplete fracture and complete fracture according to thepostoperative CT scan. The complete fracture was divided into four types:type Ⅰ in which there was no displacement;type Ⅱ in which there was mild to moderate displacement;type Ⅲ in which there was complete displacement or separated; type Ⅳ in which there was hinge' s collapse into cervical canal. The hinge fracture healing was observed and the difference of different types of fracture healing rate was compared. Results Follow-up of 12-45 (21.2 ± 5.6) months.CT scan found 395 segments 1 week after the operation, vertebral plate hinge complete fracture accounted for 58.2% (230/395).And type Ⅰ accounted for 66.1% (152/230),type Ⅱ accounted for 25.7%(59/230),type Ⅲ accounted for 6.5%(15/230) and type Ⅳ accounted for 1.7% (4/230).The fusion rate of incomplete fracture at 6 months after the operation was higher than that complete fracture [97.6%(161/165) vs.80.9%(186/230)],there was significant difference (P< 0.05).The fracture rate in different types of complete fracture at 3,6 months after the operation had significant difference (P < 0.05).Type Ⅲ fracture had the lowest healing rate. Conclusion Type Ⅰ fracture is the major type for vertebral plate hinge complete fracture, complete fracture get a relatively bad bony fusion and type Ⅲ fracture has the lowest healing rate.

Objective To analyze the results of treatment of complicated tibial plateau fractures with open reduction and internal fixation with bilateral plates .Methods 29 elder patients with C type tibial platform fractures underwent internal fixation with bilat-eral plates and bone grafting .Fixation with bilateral plates was performed during operation with dynamic compression plate or 1/3 Tubular steel plate placed medially and the dissection plate or LISS system ones inserted laterally after the underlying separation . Meanwhile ,sufficient bone grafting during operation could effectively support reduction of articular surface .Results All patients were followed up of an average of 13 .8 ± 2 .43 months .According to Rasmussen score standard ,the excellent and good rate was 82 .7% .There was significant difference of TPA and PA between postoperation and postoperation one year later .Conclusion It is a simple and effective method to treat complicated elder tibial platform fractures with bilateral plates .It will improve the effect and re-duce complications significantly to perform sufficient bone grafting intraoperatively ,and make the drainage unobstructed and do the early functional exercises postoperatively .

ObjectiveTo discuss the role of transient receptor potential vanilloid 1 (TRPV 1) in the regulation effect of electroacupuncture on gastric motility.MethodTRPV1 gene knock-out mice (KO mice) and wild-type C57BL/6 mice (WT mice) were selected to receive acupuncture at Zusanli (ST36),Quchi (LI11), Zhongwan (CV12), and Weishu (BL21), and the intragastric pressure was observed before and after acupuncture.ResultElectroacupuncture at Zusanli caused both excitation and inhibition in WT mice, predominated by mild excitation, while electroacupuncture at Weishu, Quchi and Zhongwan all caused inhibition effect; in the KO mice, electroacupuncture at Zusanli, Quchi, Zhongwan, and Weishu all inhibited gastric motility. Conclusion TRPV1 bears certain regulating effect on gastric motility, andacupuncture can inhibit the gastric motility in TRPV gene KO mice.

ObjectiveTo explore the effect of earlier rehabilitation on activities of daily living(ADL) of patients with spinal cord injury(SCI).Methods50 SCI patients received earlier rehabilitation and improvement of ADL of patients was evaluated.ResultsAfter two months treatment, the scores of Barthel index, functional independence measure(FIM) grade of patients increased significantly compared with that of before treatment(P<0.05) and ADL improved.ConclusionEarlier rehabilitation can improve ADL of SCI patients.

The zinc chloride and cystine resistant strain of S.cerevisiae YZM-14(ZnCl2r,Cysr) was screened with the mutant processing of the protoplast of S.cerevisiae by combinative mutagens of ultraviolet and nitrite. The glutathione(GSH) production(84.72 mg/L),dry cell weight(7.63 g/L) and the intracellular GSH content(11.10 mg/g) of YZM-14 were 2.79,1.63 and 1.71 times compared with that of the initial strain. The biosynthetic process of GSH was divided into three phases according to the time course of the specific cell growth rate and GSH yielding coefficient. In the second phase, the metabolic flux of the pentose phosphate pathway and the GSH precursors biosynthetic pathway of the mutant strain increased by 8.1 mmol/(g?h),compared with that of the initial strain.Furthermore, the metabolic flux of the organic acids secretion of the mutant strain decreased.Through these mechanisms, the utilization efficiency of the carbon sources was enhanced and high production of GSH was obtained.

This paper introduces the work on the design and implementation of a medical record management subsystem, which is a part of a telemedicine system based on Microsoft .NET. First, an analysis is given on components and structure of medical record, together with an explanation for what is needed in a teleconsultation, how they should be organized and realized in a database, and how to deal with consultation data granularity using XML in combination with database; second, the work done during the realization stage is expatiated, including system analysis and design, database design, and system implementation.
