Objective To investigate the correlation between specific features on contrast-enhanced CT and its clinical findings of adrenal tuberculosis so as to improve the diagnostic accuracy of the disease. Methods Contrast-enhanced CT features in 30 patients with documented adrenal tuberculosis were retrospectively evaluated blindly for the features of location, size, morphology, attenuation and enhancement patterns on CT images, and compared with clinical and pathological materials. Results The common CT manifestations were as follows:enlargement of the adrenal glands in all 30 cases(bilateral 90%,mfilateral 10%)including mass-like enlargement in 13 cases and enlargement but the contours preserved in 17 cases, heterogeneity(28 cases,93.3%),calcification(17 cases,56.7%),and low attenuation in the center with peripheral enhancement(16 cases,53.3%)of the lesions. After antituberculosis chemotherapy, 5 cases of enlarged adrenal glands decreased in size or returned to normal size and configuration, with disappearance of the central low attenuation and new appearance of dot-like calcification in 2 cases.Cochran Armitage trend test showed there was an increasing tendency of calcification rate with clinical duration(X~2= 7.47,P

OBJECTIVETo investigate the effects of the treatment of supplementing qi, nourishing yin, and promoting blood flow (SQNYPBF) on the serum levels of CRP, TNF-alpha, IL-1 and IL-6, as well as the expression of HLA-DR in the peripheral monocytes in septic patients suffering from stress-induced hyperglycemia.
METHODSIn the prospective randomized controlled study, eighty-five stress-induced hyperglycemia patients with sepsis were randomly assigned to the experimental group (45 cases) and the control group (40 cases). On the basis of routine therapies, including anti-infection, nutrition support, and the glucose control with insulin pump, patients in the experimental group additionally received the treatment of SQNYPBF (They were intravenously dripped with Shenmai Injection and Sulfotanshinone Sodium Injection, once daily, for 7 successive days). The serum levels of CRP, TNF-alpha, IL-1, and IL-6 and the HLA-DR expression of the peripheral monocytes were detected using ELISA before treatment and on the 8th day of the treatment. The total dose and the duration of insulin used, the morbidity of hypoglycemia, the APACHE II scores, and the mortality within 28-day hospitalization were compared between the two groups.
RESULTSThe total dose of insulin used, the duration of insulin used, the morbidity of hypoglycemia, the APACHE II score on the 8th day of treatment, and the mortality within 28-day hospitalization significantly decreased in the experimental group, when compared with the control group (P < 0.05, P < 0.01). There was no difference in the expression of HLA-DR, the serum levels of CRP, TNF-alpha, IL-1, or IL-6 before treatment between the two groups (P > 0.05). After treatment the serum levels of CRP, TNF-alpha, IL-1, and IL-6 significantly decreased (P < 0.05) and the expression of HLA-DR significantly increased in the two groups (P < 0.05). Better effects were shown in the experimental group (P < 0.05, P < 0.01).
CONCLUSIONSQNYPBF combined intensive insulin therapy could better improve the sepsis patients' immunity, decrease the plasma glucose level and duration, increase their survival rate, and improve their prognosis.

Objective To evaluate the effectiveness of newborn screening of hearing combined with deafness predisposing genes. Methods Through screening,514 newborns who may had the problem of hearing were classified as experimental group and the other 1 028 newborns were classified as control group by MALDI-TOF. Detecting the predisposing genes of GJB2,GJB3,12SrRNA,SLC26A4 including 20 hot spot mutations for these newborns. Results Among 514 subjects, 40 cases were found with deafness gene mutations, and the positive rate was 7. 47%. 7 cases were pathogenic mutation(1 was GJB2 235delC homozygous mutation,6 were GJB3 538C→T heterozygous mutation ),with the rate of 1. 36%,and 33 cases were heterozygous carrier, with the rate of 6. 62%. Among the control group,45 cases were found with deafness gene mutations, and the positive rate was 4. 38%. 3 cases were pathogenic mutation(1 was 12srRNA 1555A→G homozygous mutation,1 was GJB3 538C→T heterozygous mutation,1 was GJB3 547G→A heterozygous mutation),with the rate of 0. 29%,and 42 cases were carriers of heterozygous gene, with the rate of 4. 09%. The positive rate, the pathogenic mutation rate and the heterozygous carry rate of experimental group were higher than that of control group ,and the differences were significant(all p<0. 05). Conclusion The newborns who did not pass the hearing screening should be the target population for test of the deafness predisposing genes. Since the positive rate were still high,if condition permitted, the screening of hearing combined with deafness predisposing genes should be carried out in some areas.
