1.X-ray measurement of metacarpal and phalange bones in the right hand of adult patients with Kaschin-Beck disease in Qinghai Province

Objective By X-ray measurement of metacarpal and phalange bones in the right hand of adult patients with Kaschin-Beck disease in Qinghai Province,to provide anatomic data and information for clinic diagnosis of the disease and application of hand surgery.Methods Fifty-four cases of adult patients with Kaschin-Beck disease,who came from Guide County and Xinghai County in Hainan Prefecture,Qinghai Province,were selected as investigation subjects,then their right hand X-ray film were taken,metacarpal and phalange bones were measured by electronic digital vernier caliper.All data were analyzed with SPSS 19.0.Results The axis length of the metacarpal bones was in the order of:the 2nd metacarpal > 3rd metacarpal > 4th metacarpal > 5th metacarpal > first metacarpal.The axis length of proximal phalange bones was in the order of:the middle finger proximal > ring finger proximal > index finger proximal > little finger proximal > proximal thumb,while the middle,distal phalanx axis length varied greatly.The length of metacarpal and phalange bones of all male was longer than that of female(all P < 0.01).The width of the metacarpal bones was in the order of:the 2nd metacarpal > 3rd metacarpal > 5th metacarpal > 4th metacarpal.The width of the 2nd-5th metacarpal of male and female was (8.57 ± 0.90),(8.25 ± 0.80),(6.84 ± 0.69),(7.70 ± 0.77)mm and (7.40 ± 0.74),(7.36 ± 0.70),(6.00 ± 0.57),(6.62 ± 0.65)mm,respectively,the differences were significant(t =5.24,4.32,4.85,5.55,all P < 0.01).The matacarpal bone index of female[(8.23 ± 0.90)mm] was significantly larger than that of male[(7.61 ± 0.76)mm,t =2.73,P < 0.01].Conclusion X-ray measurement of hand may provide reference information for diagnosis of Kaschin-Beck disease and determination of metacarpal and phalange length in hand surgery.
2.Investigation on Kashin-Beck disease in Xinghai County,Qinghai Province in 2006

Objective By investigating newly diagnosed Kashin-beck disease,to understand distribution range,epidemic intensity and prevalence trend of Kashin-Beck disease(KBD)in Xinghai County.Method According to monitoring method on national KBD,we carried on epidemical investigation,clinical examination and X-ray photograph on school children aged 7-13 years in 6 villages from 4 towns in Xinghai County,and hair,grain,water and soil were sampled to detect selenium content using naphthalene fluorescence.Resuit Three hundred and seventy.three school children aged 7-13 years underwent the clinical examination,the prevalence rate was 12.06%(45/373);347 school children were went on X-ray,the detection rate of X-ray was 21.61%(75/347).The selenium content was the lowest in Quanqu Village,Ziketan Town,which were(77.70±42.04),(7.44±6.93)μg/kg respectively;the selenium content Was highest in Cainaihai Village,Qushian Town,which were(103.88±58.57),(29.58±24.11)μg/kg respectively;level of in internal and external environment in 4 KBD region were all low.Conclusion These four region are new KBD region.The prevalence state of KBD is serious,pathogenetic factor of Kashin-Beck disease is very active.
3.Influence of 256-slice Spiral CT Perfusion Imaging on the Cerebral Blood Flow before and after Cranioplasty

Objective To evaluate changes in cerebral blood flow before and after cranioplasty by 256-slice Spiral CT perfusion imaging,and evaluate the effect of cranioplasty on the cerebral blood flow in patients with skull defect.Methods 256-slice spiral CT scan was performed in 20 cases with early cranioplasty surgery,CTP check time points were 1 to 2 days before and 10 to 14 days after cranioplasty surgery.We recorded the the CBF and CBV of the cortex,basal ganglia,and thalamus and other parts,MTT on rCBV,parameter values rCBF,MTT and 1TrP etc.and analyzed and compared.(RCBF,rCBV,MTT and TTP) Results The CBF of cortex after cranioplasty at injured side had statistically significant increase (P<0.05).The CBF of cortex,basic nuclei,thalamus on contrateral had no statistically significant difference.The cerebral blood flow on both sides of the basal ganglia and the thalamus was increased after surgery,but there was no significant difference between before and after surgery (P>0.05) Conclusion Cranioplasty can significantly improve the ipsilateral cortex cerebral blood flow,and CT brain perfusion can accurately assess changes in brain tissue blood flow before and after cranioplasty.

5.Effects of total flavones of Elsholtzia splendens in isolated ischemia/reperfusion rat hearts.

OBJECTIVETo investigate the influence of total flavonoids of Elsholtzia splendens (TFES) on isolated ischemia/reperfusion rat hearts and its underlying mechanisms.
METHODSHearts isolated from male SD rats were perfused on the Langendorff apparatus and subjected to global ischemia for 30 min followed by 120 min of reperfusion. The cardiac infarct size was measured by TTC staining. Hemodynamic parameters and the level of lactate dehydrogenase (LDH) in the coronary effluent were measured. Absorbance at 520 nm was determined in isolated cardiac mitochondria exposed to 200 micromol/L CaCl2 to detect the opening of the mitochondrial permeability transition pore.
RESULTSPretreatment with TFES (1, 10, 100 microg/ml) for 5 min decreased infarct size and LDH release and improved the recovery of the left ventricular developed pressure. In mitochondria, the decrease of absorbance at 520 nm evoked by CaCl2 was greatly inhibited by TFES.
CONCLUSIONTFES prevents myocardial ischemia/reperfusion injury, and this cardioprotective effect is probably via inhibiting mitochondrial permeability transition pore opening.

6.Design, synthesis and antiproliferative activities of artemisinin derivatives substituted by N-heterocycles.

Increasing attention has been focused on the antitumor activity of artemisinin derivatives in recent years, for artemisinin had been reported to have cytotoxic effects against HL-60, P388 and MCF-7 tumor cells. We report here the synthesis and evaluation for antitumor activity of a series of artemisinin-ether derivatives bearing tetrahydropyrrole, morpholine, piperidine, substituted piperidines and azoles with various linkers. Sixteen 10-O-substituted dihydroartemisinin derivatives were designed and synthesized, all of which have never been reported in literatures and whose antiproliferative effects on human breast cancer MCF-7, MCF-7/Adr and HL-60 cells were determined by MTT assay or direct cell counting. Each of these artemisinin derivatives possessed better effects than dihydroartemisinin evidently against HL-60 and MCF-7 cells growth, while less potent than doxorubicin. All target compounds exhibited significantly improved potency compared to DHA and doxorubicin on the doxorubicin-resistant MCF-7/Adr cells, so did they in their sensitive counterparts MCF-7 cells. Among them, compounds GF02, GH04 and ZH04 showed strong activity against these three cell lines growth. Further research is undergoing.

7.Effect of Kangnaoye on angiogenesis in rats after cerebral ischemic reperfusion

Objective To observe the effect of Kangnaoye on angiogene-sis in rats after focal cerebral ischemia reperfusion.Methods The male SD rats were randomly divided into 6 groups: sham operation group , model group, Kangnaoye high, middle and low dosages groups (24,12,6 g· kg -1· d-1), and nimodipine group(1 mg· kg -1· d -1).The mod-els of cerebral ischemia reperfusion were established in rats by methods of middle cerebral artery occlusion ( MCAO).Immunohistochemical method was used to observe the change of the microvessel density ( MVD ) after cerebral ischemic and the expression of vascular endothelial growth ( VEG), HGF and CD31 protein of rats which were killed at the 0.5, 3, and 7 d of reperfusion injury respectively 2 h after cerebral ischemia.The nervous function deficit scores were evaluated at the 2 , 24 , and 48 h af-ter reperfusion.Results Compared with model group , the neurological behavior performance in Nimodipine and Kangnaoye treatment groups were significantly improved , with an increased number of MVD , and en-hanced vascular endothelial growth factor ( VEGF) and hepatocyte growth factor( HGF) protein levels ( P<0.05 or P<0.01 ) , especially in the Kangnaoye middle dosage group.Conclusion Kangnaoye has neuropro-tective effect against focal cerebral ischemic injury by improving the ex-pression of VEGF and HGF , which is one of possible anti -ischemic mechanisms.
8.Colon metastasis of chromophobe renal cell carcinoma with sarcomatoid change

We present a rare case of colonic metastasis of renal cell carcinoma (RCC) and review the literature.A 54-year-old male was referred to our hospital with a history of bloody stools and fever.A dght kidney tumor measuring about 10 cm in diameter was found by abdominal computed tomography.Right radical nephrectomy and a right hemicolectomy with ileotransversostomy were performed.Pathological diagnosis was chromophobe RCC with sarcomotoid change involving the colon.Chromophobe RCC with sarcomotoid change is very rare.
9.Scientific Evaluation of TCM Clinical Outcomes Rating Scale for Heart Failure Based on Patients Report.

OBJECTIVETo evaluate the reliability, validity, and responsiveness of traditional Chinese medicine (TCM) clinical outcomes rating scale for heart failure (HF) based on patients' report.
METHODSTCM clinical outcomes rating scale for HF (TCM-HF-PRO) were evaluated based on 340 HF patients' report from multiple centers. The completion of the investigation was recorded. Cronbach's α coefficient and split-half reliability were used for reliability analysis, and factor analysis was used to assess the construct validity of the rating scale. Pearson correlation analysis was then used for criterion validity analysis. Discriminant analysis was used to assess the responsiveness of the scale. All 340 HF patients having complete TCM-HF-PRO data were assigned to the treatment group and the control group by central randomization. The total TCM-HF-PRO scores of the two groups were compared using paired t-test to reflect the longitude responsiveness of the scale before treatment and at week 2 after treatment.
RESULTS(1) The recycling rate of the scale was 100.0%. One of them was not filled completely, which was rejected thereby. So the completion rate was 99.7%. The completion time for TCM-HF-PRO scale ranged 15 to 25 min. (2) The Cronbach's α coefficient of rating scale was 0.903, split-half reliability was 0.844 and 0.849. (3) Confirmatory factor analysis showed that 7 factors and items formed according to maximum load factor basically coincided with the construct of the rating scale, 7 factors accumulated contribution rate was 43.8%. TCM clinical outcomes rating scale for HF based on patients' report was relatively better correlated with the Minnesota living with HF questionnaire (r = 0.726, P < 0.01). (4) Discriminant analysis showed that the rating scale correctly classified more than 78.8% of case studies having confirmed initial differential diagnosis by experts. The total scale of the rating scale decreased more in the two group after treatment, with significant difference as compared with before treatment (P < 0.01.
CONCLUSIONTCM clinical outcomes rating scale for HF based on patients' report had good reliability, validity and responsiveness, hence it could be used to assess clinical efficacy for HF patients.

10.Identification of Model Parameters Basing on Matched Processing between Simulated and Recorded sEMG Signals

Objective To identify the model parameters of surface Electromyography (sEMG) by comparison between simulated and recorded signals. Methods A physiological model of sEMG signal was established basing on several logical hypothetical conditions, such as motor unit action potentials (MUAP), motor unit recruitment and firing behavior caused by excitation, architecture of volume conductor and other simulated factors. According to the matched shapes between the simulated and recorded sEMG signals, a group of model parameters was obtained; according to the similar power spectrum variations of real sEMG signals, decreased muscle fiber conduction velocity (MFCV) was applied to simulate the sEMG signals of the fatigued muscle. Results The experimental results showed that the simulated superimposed MUAP shapes could be matched with the recorded MUAPs satisfactorily by adjusting some proper physiological parameters of the model. When the MFCV of each fiber was assumed to decrease, the mean and median frequency (MNF, MDF) of the simulated sEMG signals declined, and this phenomenon was very similar to that of the recorded sEMG signals and could be used to interpret the muscle fatigue process. Conclusion This model provides an effective approach to simulate real sEMG signals, and the simulated signals can also be used to help the analysis of recorded sEMG signals.
