1.Evaluation of reliability and validity of the serf-assessed questionnaire of the elderly health care needs

Objective To test the reliability and validity of the self-assessed questionnaire of the elderly health care needs.Methods Based on the Zuluaga-Raysmith model, the initial questionnaire was formulated by literature reviewing and interviews.Through Delphi experts consultation, a questionnaire containing 12 factors, 61 items was constructed.A total of 750 elderly residents were investigated to test the reliability and validity of the questionnaire.Results The effective rate of questionnaires was 88.67% (665/750).The total Cronbach α was 0.871.The test-retest reliability of all the items were all greater than 0.75.The item-level content validity index were all greater than 0.80.The scale-level content validity index was 0.95.14 factors were extracted by exploratory factor analysis, explaining 66.653% of the total variance.Conclusions The elderly health care needs self-assessed questionnaire is comprehensive with good stability, which can be used in a multi-center, large sample research.The research may help to get a deep understanding of health care needs of the elderly as well as influencing factors.
2.Modified Kock method of bladder reconstruction with ileum (report of 51 cases)

Objective To investigate the clinical effects of the modified Kock method of bladder re- construction with ileum,and to provide the objective basis for wide application of this technique.Methods A total of 51 patients with bladder cancer(T_2N_0M_0 stage tumor in 37 cases,T_3N_0M_0 stage tumor in 14; and pathologic gradeⅡ-Ⅲin all)were included.After radical cystectomy,modified Kock reconstruction of bladder with ileum was performed in them.The procedure consisted of preparing the segment of the ileum for reconstruction of the reservoir,anastomosing the low part of the neobladder with the urethra and regaining the original urinary conduit.Results The mean operative time was 6.5h(range,5.5-8.5h);mean in- traoperative blood loss was 650ml(range,300-1200ml),with blood transfusion in 31 cases.Perioperative complications included stress ulcer in 6 cases and urinary leakage in 1.The other 44 cases had no severe complications.Four cases died of cancer metastasis at 6-18 months after operation.During a follow-up of 8 -32 months,the other 47 cases recovered well and have been alive till now.Two cases had ureteral urine reflux with no impairment of renal function.The daytime urinary continent rate was 100%;and nocturnal uri- nary incontinence occurred in 6 cases.Conclusions The modified Koek reconstruction of bladder with il- eum can improve the patients'quality of life with fewer complications,therefore is a better treatment choice for infiltrative bladder cancer after radical eystectomy.

4.Effects of Se-riched soybean peptide on antioxidant function in rats of fatty liver caused by high-fat diet.

OBJECTIVETo explore the effect of Se-riched soybean peptide (SSP) on antioxidant function in rats of fatty liver caused by high-fat diet.
METHODSForty Wistar rats were divided into 4 groups randomly and fed with standard diet and water (NC), high-fat diet and water (HC), high-fat diet and SSP (0.1 g/d) (SeH), standard diet and SSP (0.1 g/d) (SeN) respectively. After 10 weeks, the rats were killed to investigate the pimelosis level in liver tissues by Sudan III staining and the expression of hepatic GRP78 by immunohistochemical analysis. We also analyzed the changes of liver function, blood lipid, the glutathione peroxidase (GSH-Px), superoxide dismutase (SOD) activity and malondialdehyde (MDA) content in livers and serum.
RESULTSThe pimelosis level, total cholesterol (TC), triglyceride (TG), MDA contents and the expression of GRP78 in HC group were significantly higher than those in NC, SeN, SeH groups. The activities of GSH-Px and SOD in liver and serum were markedly up-regulated in SeH (P < 0.01). There was no significant difference between NC and SeN groups.
CONCLUSIONSSP can improve liver cell injury and the antioxidant functions in rats with fatty liver effectively and decrease the expression of GRP78 in liver.

5.Protective effect of lycium barbarum polysaccharides against heat stress-induced germ cell apoptosis in rats and its mechanism.

OBJECTIVETo investigate the protective effect of lycium barbarum polysaccharides (LBP) against heat stress-induced apoptosis of germ cells in rats and its action mechanism.
METHODSNinety male Sprague-Dawley rats were randomly divided into five groups of 18 each: control, heat stress (HS), high-dose LBP, median-dose LBP and low-dose LBP. The rats of the three LBP groups were given LBP by intragastric administration at 100 mg/(kg x d), 50 mg/(kg x d) and 10 mg/(kg x d) respectively for 14 days, and on the 15th day they, together with those of the HS group, were exposed to a heat of 43 degrees C for 30 minutes. At 24 h, 48 h and 7 d after heat stress, the animals were killed by cervical dislocation, followed by observation of the apoptotic germ cells by TUNEL, determination of the expression of Caspase-3 by immunohistochemistry and detection of cytochrome C in the cytosol by ELISA.
RESULTSCompared with the HS group, the three LBP groups showed statistically significant decreases in the apoptosis index (P<0.05), the expression level of Caspase-3 in germ cells (P<0.05) and the concentration of cytochrome C in the cytosol (P<0.05).
CONCLUSIONLBP can inhibit cytochrome C release from mitochondria, decrease the expression of Caspase-3 and hence reduce the apoptosis of germ cells. It thus can be deduced that LBP can protect germ cells against apoptosis via the mitochondrial pathway.

6.Relationship between the long type PML-RAR alpha and the prognosis of patients with acute promyelocytic leukemia.

OBJECTIVETo explore the relationship between long (L) type PML-RAR alpha fusion gene and the prognosis of patients with acute promyelocytic leukemia (APL).
METHODSPML-RAR alpha fusion gene was detected by RT-PCR in 33 APL patients. The optical density of three bands including E5 (+) E6 (+) 636 bp (no deletion), E5 (-) E6 (+) 492 bp (exon 5 deleted) and E5 (-) E6 (-) 232 bp (both exon 5 and exon 6 deleted) was measured by a UVP analysis system and their relative proportions were calculated. The relative expression level of each splicing band, initial WBC count and age were statistically (for single and multi-factor analysis) analyzed with prognosis.
RESULTSThe relative expressions of E5 (-) E6 (+) and E5 (-) E6 (-) in death group were obviously different from that in first complete remission (CR1) group (P < 0.01), but do not for E5 (+) E6 (+) (P > 0.05). The relative expression levels of E5 (-) E6 (+), E5 (-) E6 (-) and E5 (+) E6 (+) were 0.23 +/- 0.12, 0.58 +/- 0.18, 0.20 +/- 0.09 in death group, and 0.45 +/- 0.16, 0.23 +/- 0.12, 0.31 +/- 0.16 in CR1 group, respectively. Initial WBC count and age was no difference between the two groups (P > 0.05). Logistic regression analysis showed that the expression of E5 (-) E6 (+) had no effect on the prognosis (B = 3.475, P = 0.492), but the expression of E5 (-) E6 (-) showed a negative correlation with prognosis (B = -19.660, P = 0.046).
CONCLUSIONSThe high expression of E5 (-) E6 (-) is correlated with the poor prognosis for patients with APL.

7.Activation of nuclear factor kappaB and induction of inducible nitric oxide synthase by lipid-associated membrane proteins isolated from Mycoplasma penetrans.

BACKGROUNDThis study was designed to investigate the potential pathogenicity of Mycoplasma penetrans (M. penetrans) and its molecular mechanisms responsible for the induction of iNOS gene expression in mouse macrophages stimulated by lipid-associated membrane proteins (LAMPs) prepared from M. penetrans.
METHODSMouse macrophages were stimulated with M. penetrans LAMPs to assay the production of nitric oxide (NO). The expression of inducible nitric oxide synthase (iNOS) was detected by RT-PCR and Western blotting. The activity of nuclear factor kappaB (NF-kappaB) and the effects of pyrrolidine dithiocarbamate (PDTC), an inhibitor of NF-kappaB, on the production of nitric oxide and the expression of iNOS were also assessed in mouse macrophages treated with M. penetrans LAMPs by indirect immunofluorescence and Western blotting.
RESULTSM. penetrans LAMPs stimulated mouse macrophages to produce nitric oxide in a dose- and time-dependent manner. The mRNA and protein levels of iNOS were also upregulated in response to LAMP stimulation and inhibited by PDTC treatment. M. penetrans LAMPs were found to trigger NF-kappaB activation, a possible mechanism for the induction of iNOS expression.
CONCLUSIONThis study demonstrated that M. penetrans may be an important etiological factor of certain diseases due to the ability of M. penetrans LAMPs to stimulate the expression of iNOS, which is probably mediated through the activation of NF-kappaB.

8.Clinical observation of target-controlled remifentanil infusion combined with propofol and doxapram in painless artificial abortion.

OBJECTIVETo observe the impacts of doxapram on anesthetic efficacy and respiratory and circulatory functions during anesthesia with remifentanil given by target-controlled infusion (TCI) combined with propofol in painless artificial abortion.
METHODSA total of 120 ASA I women requiring voluntarily painless artificial abortion were randomized into two equal groups. One group was given remifentanil by TCI at 2 ng/ml and propofol 1mg/kg (group I), and the other given remifentanil by TCI, propofol, and doxapram 0.6 mg/kg (group II). The anesthetic efficacy and respiratory and circulatory suppression during anesthesia were observed.
RESULTSBoth of the two groups showed satisfactory anesthetic efficacy with comparable the BIS values (P>0.05). No significant difference was noted in the induction time and awaking time between the two groups (P>0.05), but group I had more serious respiratory and circulatory depression than group II (P<0.05).
CONCLUSIONDoxapram may attenuate respiratory and circulatory depression during anesthesia with remifentanil given by TCI combined with propofol in painless artificial abortion, and provide comparable anesthetic efficacy.

9.The relationship between ApoE gene polymorphism and lipid parameters.

OBJECTIVETo investigate the relationship between apolipoprotein E (ApoE) gene polymorphism and serum lipid profile.
METHODSPolymerase chain reaction-restricted fragments length polymorphism (PCR-RFLP) was used to determine ApoE genotype on 1452 subjects including 1101 cases with cardio cerebrovascular disease including 379 cases with cerebral infarction, 313 cases with cerebral hemorrhage, 257 cases with coronary heart disease, and 152 cases with other types and on 351 healthy controls.
RESULTSAfter adjusting for age, sex and BMI, the subjects with ApoE4 carriers had significantly higher levels of total cholesterol (TC), low density lipoprotein-cholesterol (LDL-C) and ApoB than those with ApoE2 carriers and ApoE3/3 (P < 0.05), and higher level of triglyceride(TG) than those with ApoE3/3 (P < 0.05), while the subjects with ApoE2 carriers had significantly higher levels of high density lipoprotein-cholesterol (HDL-C) than those with ApoE4 (P < 0.05). The effects of ApoE polymorphism exhibited similarity in different sex and age of subjects. Linear regression analysis showed that unlike ApoE3/3, the HDL-C level in ApoE2 carriers tend upward with age (beta = 0.178, P = 0.015), significantly higher than ApoE4 carriers and ApoE3/3 in the cohort of 65-74 years (P < 0.05). The level of TC and TG in ApoE4 carriers had a tendency of downward with age (p = -0.179, P = 0.009; beta = -0.147, P = 0.032).
CONCLUSIONApoE gene polymorphism affected profile of blood lipids and the effects were found in different sex and age. The degrees of effects related to ApoE2 carriers and ApoE4 carriers to blood lipid level seemed to be related to age.
