1. 99mTc-MIBI myocardial perfusion image in predicting therapeutic effect of percutaneous coronary intervention

Objective: To observe the 99mTc-MIBI myocardial perfusion images in patients with myocardial infarction before/after percutaneous coronary intervention (PCI), so as to screen for the reliable predictors for the therapeutic effect of PCI. Methods: Forty-eight patients with myocardial infarction receiving PCI were included in this study. The rest, stress and nitroglycerin myocardial perfusion imaging tests were performed before PCI. The stress and rest myocardial perfusion imaging tests were repeated 1-2 weeks after PCI. The value of (rest 99mTc-MIBI imaging in myocardial perfusion scores after PCI-stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI)/stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI was taken as the curative effect of PCI; the value of(rest 99mTc-MIBI imaging in myocardial perfusion scores before PCI-stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI)/stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI was taken as the functional viable myocardium; the value of (stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI-nitroglycer 99mTc-MIBI imaging in myocardial perfusion scores before PCI)/stress 99mTc-MIBI imaging in myocardial perfusion scores before PCI was taken as potential functional viable myocardium; and the value of(rest 99mTc-MIBI imaging in myocardial perfusion scores before PCI-nitroglycer 99mTc-MIBI imaging in myocardial perfusion scores before PCI)/rest 99mTc-MIBI imaging in myocardial perfusion scores before PCI was taken as drug relative viable myocardium. To screen the proper predictors for therapeutic effects, linear correlation analysis was performed between the curative effect of PCI and the latter 3 factors. Results: Stress and rest myocardial perfusion defects were significantly improved after PCI (P<0.01). Correlation analysis showed that the curative effect of PCI was correlated with the latter 3 factors, with the correlation coefficients being r1=0.63, P<0.01, r2=0.94, P<0.000 1,and r3=0.92, P<0.000 1, respectively,indicating that the potential functional viable myocardium had the best correlationship with PCI. Conclusion: It is necessary to perform the rest, stress and nitroglycerin myocardial perfusion imaging tests in patients with cardiac infarction. The functional viable myocardium is a reliable index in screening the patients suitable for PCI and in predication of the therapeutic effect.

Stroke is one of the most important causes of disability throughout the world.Epide- miological and clinical studies have demonstrated that statins significantly decrease the incidence of ischemic stroke.In addition to lipid-lowering effect,statins may also have a number of other acting mechanisms that are not dependent on reducing cholesterol levels.Moreover,statins inhibit cerebral vasospasm caused by subarachnoid hemorrhage.For most of the individuals,the safety and tolerability of statins are well.
3.Clinical significance of subclavian artery angiography in arterial embolization for hemoptysis

Objective To study the subclavian artery angiography for hemoptysis and its clinical value.Methods Thirty-eight cases of hemoptysis undertook subclavian artery angiography after embolization of bronchial arteries and branches of thoracic aorta. Group A: 4 recurrent cases underwent subclavian artery angiography (unilateral: n =2, bilateral: n =2). Group B: 16 cases underwent subclavian artery angiography according to the manifestation on X-ray and CT (right: n =10, left: n =6). Group C: 18 cases underwent bilateral subclavian artery angiography.Results of subclavian artery angiography were divided into three classes: normal(-),chaotic and hyperplasia of small branchi vessels(+),obviously bleeding (++).Some of the obviously bleeding cases were embolized and analysed for clinical efficacy and complications. Results Bleeding cases accounted for 63.2%(24/38)and bleeding subclavian arteries accounted for 50%(29/58), including(++):37.9%(22/58) and(+): 12.1%(7/58). Positive rate of chronic fibro-cavitary pulmonary tuberculosis was the highest. Bleeding sites were coincided with lung lesions. Twelve cases were embolized with the immediate cessation rate of hemoptysis reaching 100%. Eight cases with long-term follow-up showed 5 cured, 1 with significant effect and 2 recurrent. The complications occurred with fever, vomiting, chest pain, hiccup and dyspnoea.Conclusions Subclavian artery angiography has important clinical value in artery embolization for hemoptysis, especially for lesions in the upper lobes of lungs and with more fruitful result for chronic fibro-cavitary pulmonary tuberculosis.

7.Construction of bio-micro-frontier based on theory of biotechnology supremacy

Biotechnology supremacy is a newly-advanced power theory. It is a superior dominance of military biotechnological application based on the microcosm of life structure within a certain period of time. The advancement of biotechnology supremacy and modern biotechnology has created the concept of bio-micro-frontier, which involves information and defense resources of all living ultra-micro-organisms with national and regional characteristics. Being feasible both in theory and practice, the implementation of bio-micro-frontier system is strategically important. This article explores the implementation of bio-micro-frontier in terms of strategy and tactics, which will add a unique dimension to future military transformation and active defense.
8.Inhibition of Hepatitis B Virus Replication by Rheum palmatum L. Ethanol Extract in a Stable HBV-producing Cell

Hepatitis B virus(HBV) infection is a severe health problem in the world.However,there is still not a satisfactory therapeutic strategy for the HBV infection.To search for new anti-HBV agents with higher efficacy and less side-effects,the inhibitory activities of traditional Chinese medicine Rheum palmatum L.ethanol extract(RPE) against HBV replication were investigated in this study.Quantitative real-time polymerase chain reaction(PCR) was employed to analyze the inhibitory activity of RPE against HBV-DNA replication in a stable HBV-producing cell line HepAD38; the expression levels of HBV surface antigen(HBsAg) and e antigen(HBeAg) were also determined by enzyme linked immunosorbent assay(ELISA) after RPE treatment.RPE could dose-dependently inhibit the production of HBV-DNA and HBsAg.The concentration of 50% inhibition(IC50) was calculated at 209.63,252.53 μg/mL,respectively.However,its inhibitory activity against HBeAg expression was slight even at high concentrations.RPE had a weak cytotoxic effect on HepAD38 cells(CC50 = 1 640 μg/mL) and the selectivity index(SI) was calculated at 7.82.Compared with two anthraquinone derivatives emodin and rhein,RPE showed higher ability of anti-HBV and weaker cytotoxicity.So Rheum palmatum L.might possess other functional agents which could effectively inhibit HBV-DNA replication and HBsAg expression.Further purification of the active agents,identification and modification of their structures to improve the efficacy and decrease the cytotoxicity are required.
9.Application value of Provider-Initiated HIV Testing and Counseling in dermatology.

OBJECTIVETo explore the clinical application value of Provider-Initiated HIV Testing and Counseling (PITC) by analyzing the positive rate of HIV tests for people in need of PITC and that of routine HIV tests.
METHODSWe retrospectively analyzed the demographic and epidemiologic data about the patients seeking PITC services or undergoing routine HIV tests in Nanjing Drum Tower Hospital between January and December 2013.
RESULTSThe positive rate of initial HIV screening was 1.98% in the PITC group and 0.24% in the routine test group, while that of confirmed HIV was 0. 40% in the former and 0.07% in the latter, both with statistically significant differences between the two groups (P < 0.01). The positive rate of HIV was markedly higher in males than in females, particularly in the PITC group.
CONCLUSIONPITC has a high clinical value in HIV detection for targeted subjects and therefore deserves general application in dermatology.

10.Dynamic changes and clinical significance of serum Thl/Th2 cytokines in hemorrhagic fever with renal syndrome

Objective To investigate the changes and pathogenic significance of serum interleukin-12p70 (IL-12), intefferon-γ,(IFN-γ) and IL-4 in the course of hemorrhagic fever with renal syndrome(HFRS). Methods Twenty five eases were divided into mild group (14 eases) and severe group (11 cases) according to the severity of illness. Blood samples were collected in various stages(fever, hypotensian and oliguria,diuresis stage). Serum IL-12 and IFN-γ levels were determined by enzyme-linked immunoserbent assay(ELISA), IL-4 by radioimmunoassay (RIA), blood urea nitrogen (BUN) and platelet by automatic biochemical analyzer and blood analyzer. Results Serum IL-12 levels in mild and severe groups were significantly different during various stages of HFRS (F=5.765, P<0.01). The IL-12 level of both patient groups significantly increased(P<0.01) in fever[ (0.87±0.38), (1.08± 0.77)μg/L], hypotension and oliguria [ (0.77±0.21), (2.11±2.13)μg/L] ,and diuresis stage [ (1.42±1.10), (1.20±0.88)μg/L], compared with control group [(0.56±0.10)μg/L]. In various stages, IFN-γ levels of both case groups were respectively (8.04±13.05), (5.94±8.24), (15.95±18.05), (4.41±4.10), (1.09±1.24), (1.38±1.74), (1.12±1.26), (0.19±1.29)μg/L, and the difference was statistically significant compared with control [ (0.27±0.15)rig/L]. K,-4 levels did not change significantly in the stages(F=0.682, P0.05), while the ratios of IFN-γ and IL-4 contents in mild and severe cases were significantly higher than control [(0.36±0.26) μg/L] in fever[ (2.46±3.52), (16.92±22.77)p.g/L], hypotension and oliguria[(2.52±2.72), (1.77±2.06) μg/L],diuresis stage [(1.45±2.28), (2.32±3.98)μg/L], the difference had statically significant (P<0.05 or 0.01).The curve of IL-12 was similar to that of BUN, but was contrary to blood platelet count. Conclusions The elevated levels of IL-12 and IFN-γ, with the imbalance of Th1/Th2 might be the main cause of systemic inflammatoryresponse and involved in the pathogenesis of HFRS.
