1.Evaluation of the navigation system for orbital wall reconstruction in unilateral orbital fractures.

OBJECTIVETo evaluate the efficiency of navigation system for orbital wall reconstruction in unilateral orbital fractures.
METHODSFifteen patients (7 male and 8 female) with unilateral orbital fracture underwent orbital reconstruction with the help of intraoperative navigation system. The average age was 34.3 ± 9.5 years. All patients underwent spiral CT scanning preoperatively, and the CT data was imported to the BrainLab navigation system (Germany, BrainLab company). The orbit of the intact side was mirrored to the opposite side as the reference for pre-operative planning. The titanium mesh was mounted on the resin template made by rapid prototyping machine based on the mirrored CT data. When the injury was limited, the hydroxyapatite sheet was used for the orbital wall reconstruction. During the operation, the real-time navigation helped to ensure precise placement. The re-establishing result was assessed based on the postoperative CT data with the following four variables: the volumetric difference between the bilateral orbit, the volume of the herniated soft tissue, the global projection and the discrepancy between the simulated and the achieved position of the reconstructed orbital wall. The reconstructive discrepancy was measured only in the titanium plate grafting cases.
RESULTSThere were no serious complications such as infection, graft rejection and optic nerve injury in any case. Preoperatively, the average degree of enophthalmos was (3.5 ± 1.6) mm, the average volumetric difference between the injured and the unaffected orbit was (4.5 ± 1.8) ml, and the average volume of the herniated orbital soft tissue was (2.1 ± 0.7) ml. Postoperatively, the three values were respectively reduced to (1.3 ± 0.6) mm, (1.8 ± 0.9) ml and (0.7 ± 0.3) ml. The discrepancy of the medial and inferior wall were (2.5 ± 0.6) mm and (2.1 ± 0.4) mm.
CONCLUSIONSThe intraoperative use of navigation system for the orbital wall reconstruction in unilateral orbital fractures can provide reliable accuracy and achieve satisfactory results.

2.Retrospective study of reverse dorsal metacarpal flap and compound flap: a review of 122 cases.

OBJECTIVETo evaluate the clinical application and discuss the operative indication of the reverse dorsal metacarpal flap and its compound flap on the skin defects of hand.
METHODSFrom 1990 to 2003, we applied the reverse dorsal metacarpal flap and its compound flap to repair soft tissue defects of fingers in 122 cases, which included 90 cases of the reverse metacarpal flap and 32 cases of its compound flaps with tendon grafts, nerve grafts or bone grafts. Based on the follow-up observations, we analyzed the indications of the reverse metacarpal flap and its compound flaps, the postoperative contours, flap colors and textures in comparison to contralateral fingers retrospectively.
RESULTSIn the series of 122 cases, flaps survived and the donor site defects were closed directly. The follow-up period ranged from 1-12 years. The postoperative contours, colors and textures of the flaps and its compound flaps were similar to those of normal fingers, although linear scar remained. According to standards of sense recovery (British Medical Research Council, BMRC), the sense function of the flaps resumed S3 after operation for 1 year. In 10 cases with the tendon defects treated by the flap with tendon grafts, function of flexion-extension of fingers resumed 50%-75% in comparison to the contralateral fingers using the method of measurement of total active motion. In 7 cases with the phalangeal nonunion or bone defects treated by the flap with bone grafts, union occurred after operation for 3 months.
CONCLUSIONSTo soft tissue defects on fingers with bone or tendon exposure, the reverse metacarpal flap and its compound flap are a better choice for repairing. The range of repairing is up to the distal interphalangeal joint of fingers. The second dorsal metacarpal artery is more consistent and larger as the choice of vascular pedicle, in comparison with other dorsal metacarpal arteries. Postoperative flap color and texture are similar to normal fingers.

Ticks are obligate ectoparasites and vectors of arboviruses, vickettsiate, spirochetes and parasitil protozoa of humans and domestic animals. Immunological protection of mammalian hosts against tick infestation has been proposed as the most sustainable alternative tick control method to the current use of acaricides. The success of this method is dependent on the identification of key molecules for use as tick vaccine antigens. Proteolytic enzymes are involved in a wide range of cellular processes, thus they can be considered as good target antigens for a tick vaccine. In the present study, we used rapid amplification of cDNA ends protocol and primers that were designed based on the consensus amino acid motifs flanking present in all papain-like cysteine proteinases, to amplify, sequence and characterize two Rhipicephalus haemaphysaloides haemaphysaloides cathepsin L-like cysteine proteinases, named as cysA and cysB. The full length of cysA is 1168bp, encoding a 332 amino acid residue polypeptide with 36.33kD predicted molecular mass; the full length of cysB is 1153bp, encoding a 335 amino acid residue polypeptide with 37.56kD predicted molecular mass. The consensus amino acid motifs flanking presence in both deduced amino acid sequences. And both genes show high sequence homology to other tick cathepsin L-like cysteine proteinase, so they were identified as members of the cysteine proteinase gene family. Expression analysis by RT-PCR revealed that cysA and cysB were expressed differently in different periods of tick development.

4.Long Noncoding RNA PVT1 Promotes Stemness and Temozolomide Resistance through miR-365/ELF4/SOX2 Axis in Glioma

Long non-coding RNA (lncRNA) are a class of non-coding RNAs demonstrated to play pivotal roles in regulating tumor progression. Therefore, deciphering the regulatory role of lncRNA in the development of glioma may offer a promising therapeutic target for treatment of glioma. We performed RT-qPCR analysis on the expression of lncRNA plasmacytoma variant translocation 1 (PVT1) and miR-365 in glioma tissues and cell lines. Cell proliferation and viability was assessed with CCK8 assay. Cell migration was assessed by wound healing assay. Transwell assay was used to assess cell invasion capacity. Expression of CD133+ cells was detected by flow cytometry. Western blot assay was used to detection the expression of ELF4 and stemness-related protein SOX2, Oct4 and Nanog. Bioinformatics and dual-luciferase assay were used to predict and validate the interaction between PVT1 and miR-365. Elevated PVT1 expression was observed in glioma tissues and cells. Knockdown of PVT1 and overexpression of miR-365 inhibited proliferation, migration, invasion and promoted stemness and Temozolomide (TMZ) resistance of glioma cells. PVT1 regulated ELF4 expression by competitively binds to miR-365. PVT1 regulated the stemness and sensitivity of TMZ of glioma cells through miR-365/ELF4/ SOX2 axis. This study identified that PVT1 promoted glioma stemness through miR-365/ELF4/SOX2 axis.
5.A prevalence survey on the mental health of left behind adolescents in Anhui province

Objective By exploring the mental health situation of the unattended adolescents in Anhui province. This study aimed at investigating the status of mental health and related influential factors to provide evidence for effective strategies and intervention in physical and mental health improvement of this population. Methods By using stratified cluster sampling method, we conducted an investigation on 3421 subjects,in which 682 were unattended adolescents,from 7 middle schools in both south and north parts of Anhui province. MHT was used to assess the general condition and status of mental health in the adolescents. Results In the population aged 10 to 18 in Anhui province,19.94% of them were unattended adolescents and were found with poor mental health and more prone to loneliness, sensitive and stronger fear as compared with those with parents around, which showed statistical difference (P<0.05). As for the mental health status of unattended adolescents in different family types, the subjects seemed worse in the north than in the south Anhui regarding the total scores of MHT, self-accusation, physical symptoms and phobic anxiety disorder (P<0.05). Girl students were also inferior to boys concerning the total scores of MHT, learning anxiety, interpersonal anxiety, self-accusation, sensitivity, physical symptoms and impulsive behavior (P<0.05). The students in Grade 7 scored lower than those of the upperclassmen on MHT (P<0.05), and the subjects from township were superior to those of rural areas regarding interpersonal anxiety and loneliness (P<0.05). Children with siblings appeared worse than their counterparts on the total scores of MHT, including the items of interpersonal anxiety, learning anxiety, self-accusation, sensitivity,physical symptoms, strong fear and impulsive behavior (P<0.05). Conclusion Generally, the mental health status of unattended adolescents in Anhui province appeared poorer than their counterparts, and relatively worse psychological status was seen in girls, high school students,children with siblings and subjects living in north Anhui or rural areas.
6.Analysis on physical and mental health and related influential factors among those ‘left behind ’ adolescents in Anhui province

Objective To explore the physical and mental health status on the ‘left behind ’ students in Anhui province and the related influential factors.Methods Stratified cluster sampling method was used to investigate the 682 out of 3421 students from 7 middle schools in both south and north Anhui province,using multidimensional sub-health questionnaire of adolescents (MSQA) and mental health test (MHT),for assessment of the general condition,physical and mental health status.Results 14.22% of the ‘left behind’ students were in sub-health condition compared to 11.28% among the normal students,and the mental sub-health status represented 13.64% for the former and 10.84% for the latter.In addition,the ‘left behind’ students appeared less content with their life than those of staying with their parents (P<0.05).MHT scores in the subjects were positively correlated with the scores on MSQA and showed significant difference (P<0.01).Results from the multiple linear regression analysis showed that the mental health condition among the left behind students was in linear regression relationship with the decreased physical activity (β =1.456,P=0.001 ),emotional problems (β =1.096,P=0.000 ) and problems of social adaptation (β=1.011,P=0.006).The status of physical sub-health in the Anhui province showed that there was no significant difference in each of the study group (P>0.05).Conclusion Decreased physical activity and emotion aporia as well as difficult social adaptation were major factors leading to degenerated mental health levels in the‘left-behind’ adolescents.The findings suggested that the mental health status could be improved inthis population through better management on their body-health condition.
7.Clinical microanatomy of the bridging veins draining into the superior saggital sinus in the anterior transcallosal approach

Objective To observe the microanatomy of the bridging veins emptying into the superior sagittal sinus (SSS) for preservation of the bridging veins in surgeries through the anterior transcallosal approach. Methods Blue latex was injected into the SSS and internal jugular veins in 20 cadaver heads (40 sides), in which the bridging veins of the frontal zone and central zone were dissociated and their positions relative to the body surface were determined. Such indexes of the lateral veins in each zone as the caliber, the number of bridging veins, and convergence angle were determined. The opposite hemisphere was manipulated in an identical manner to measure the indexes of the sagittal sinus. Results in an area posterior to the frontal region of the SSS, a "safe zone" was identified where no bridging veins drained into the SSS, covering the area 32.6 nun anterior and 7.5 mm posterior to the coronal suture. After complete dissociation of the bridging veins near the longitudinal fissure in the "safe zone", the fissure allowed an opening width of 4.48~10.86 mm. Conclusion Thorough knowledge of the venous anatomy can help avoid the bridging veins in the anterior transcallosal approach. Total dissociation of the sticking segment and arachnoid segment of the bridging veins can broaden the opening width of the longitudinal fissure without increasing the tension of the bridging veins to better preserve the bridging veins during surgery.
8.Effects of chemotherapy on circulating angiogenic factor levels in patients with breast cancer.

OBJECTIVETo study the changes in circulating VEGF and endostatin (ES) levels during chemotherapy for patients with breast cancer, and their correlation with efficacy of chemotherapy.
METHODS40 breast cancer patients with metastases were included in this study. They received TAC/TEC, CAF/CEF, NP, CAP, CMF, TFP, TA or TC regime chemotherapy, respectively. Totally 120 serum samples were collected from the patients at three time points: before chemotherapy, the end of 1 and 5-6 chemotherapy cycles, and analyzed for VEGF and ES levels using ELISA. Tumor agiogenesis activity was evaluated by serum soluble vascular cell adhesion molecule (VCAM - 1) measured by ELISA as a surrogate marker.
RESULTS(1) Before chemotherapy, the median level of VEGF in patients with breast cancer was 496.6 pg/ml, 4.7 times higher than that of healthy controls (P <0.001). The median level of ES was 95.5 ng/ml, 18.3% lower than that of healthy controls (P = 0.183). VCAM-1 was 1077.1 ng/ml and higher than that of controls (P <0.001). The serum VEGF levels correlated with VCAM-1 levels, tumor staging and metastatic sites (P <0.05). (2) At the end of 1 cycle of chemotherapy, the serum VEGF level (median 524.8 pg/ml) was higher than the pretreatment values (P = 0.047), whereas the levels of ES and VCAM-1 were not significantly altered (110.5 ng/ml, P = 0.055; and 975.6 ng/ml, P = 0.27). (3) At the end of 5-6 cycles, the changes in VEGF correlated with the response to chemotherapy. Serum VEGF levels in 27 patients with chemotherapy-responsive and stable disease showed a significant decrease (median 287.4 pg/ml) , but not observed in 13 patients with progressive disease. VCAM-1 also showed a treatment-related change like VEGF. However, chemotherapy might only have a minor effect on ES, because there was no significant difference in the ES levels among 5-6 cycle patients, 1 cycle patients and healthy controls, and neither between therapy-responsive patients.
CONCLUSIONIntensive chemotherapy for breast cancer results in a significant decrease of serum VEGF level, which might be an indicator of the controlled disease status, and following the treatment-induced response or stabilization, the tumor angiogenesis seems to change into an anti-angiogenesis direction.

10.The efficiency and safety of laparoscopic surgery for colorectal carcinoma: a systematic review.

OBJECTIVETo evaluate and compare the efficiency and safety of laparoscopic surgery (LS) and open surgery (OS) in the treatment of colorectal carcinoma.
METHODSRandomized controlled trials on laparoscopic surgery and open surgery for colorectal carcinoma from January 2000 to October 2010 were searched in the databases of EMbase, PubMed, Cochrane Library, Sciencedirect, Springer, VIP, CNKI, CBMdisc. The methodological quality was assessed according to the standard of Cochrane systematic review. For homogeneous studies, RevMan5.0 software was used for meta-analysis.
RESULTSA total of 13 RCTs involving 4603 patients were included in this study, and among those 6 were multi-center randomized controlled trials. The meta-analysis showed that: the operation time of the LS group was longer than that of the OS group (WMD = 38.91, 95%CI: 33.89 - 43.93, P < 0.001), the blood loss (WMD = -138.14, 95%CI: -195.79 - -80.50, P < 0.001) and the length of hospital stay (WMD = 2.91, 95%CI: -4.65 - -1.17, P = 0.001) of the LS group was less than those in OS group. There was no significant differences between the two groups in the number of dissected lymph nodes (WMD = -0.62, 95%CI: -1.47 - 0.23, P = 0.150). There was no significant differences between the two groups in terms of the postoperative complications (30 days) (RR = 0.78, 95%CI: 0.59 - 1.01, P = 0.06). There was no significant differences between the two groups in 3-year overall survival (RR = 1.00, 95%CI: 0.96 - 1.04, P = 0.970). There was no significant differences between the two groups in 5-year overall survival (RR = 1.03, 95%CI: 0.99 - 1.08, P = 0.140). There was no significant differences between the two groups in 5-year overall recurrence (RR = 0.89, 95%CI: 0.74 - 1.07, P = 0.200).
CONCLUSIONSLaparoscopic surgery for colorectal carcinoma is a safe and effective therapy as open surgery in the short term or long term outcomes. It could be an acceptable alternative to open surgery for colorectal carcinoma.
