In order to evaluate the diagnostic value of the PACS, the PACS has been applied to Dept.of Radiology in Renji Hospital in Shanghai for nearly two and a half years. Clinical practice shows that the doctors can work more efficiently and accurately by using the diagnostic workstation of PACS. PACS is an inevitable development trend of image diagnosis and is playing a more and more important role in clinical applications.

Objective To explore the diagnosis and therapy experience of vascular ring combined with tracheal compression in infants and neonates.Methods Sixteen cases(including 7 boys and 9 girls,aged 1 day to 12 months)with vascular ring combined with tracheal compression hospitalized in Guangdong General Hospital from Jun.2004 to Dec.2009 were enrolled.In these 16 children,13 cases had congenital heart malformations.All children underwent X-ray,echocardiography and spiral computed tomography examination.Nine cases received bronchoscopy study.Fifteen cases performed surgical division of vascular ring with cardiopulmonary bypass and 1 case underwent vascular ring division and tracheoplasty.Eleven cases received management of congenital heart defect simultaneously.Results Vascular ring anomalies included pulmonary artery sling in 5 children,right aortic arch-left ligmentum/aberrant left subclavian artery in 8 cases,double aortic arch in 1 case,innominate artery compression in 1 case,and pulmonary sling combined with right aortic arch-aberrant left subclavian artery in 1 case.There were 2 ring-sling complex cases in this study.The diagnosis of vascular ring were correctly made by echocardiography in 7 children and made by spiral computed tomography in all 16 cases.Two cases combined with tracheal ring died.In the follow-up study of 11 cases,5 cases were still vulnerable to wheezing.Conclusions The common presentation of tracheal compression in infants and neonates associated with vascular ring are tachypea,stridor,and dyspnea.Multi-slices spiral computed tomography is an important imaging modality.Surgical divisions of vascular ring are safe procedure in most cases and tracheal compression can be relieved by this operation.In patients with severe tracheal stenosis,tracheoplasty should be recommended.

Objective To observe the microanatomy of the bridging veins emptying into the superior sagittal sinus (SSS) for preservation of the bridging veins in surgeries through the anterior transcallosal approach. Methods Blue latex was injected into the SSS and internal jugular veins in 20 cadaver heads (40 sides), in which the bridging veins of the frontal zone and central zone were dissociated and their positions relative to the body surface were determined. Such indexes of the lateral veins in each zone as the caliber, the number of bridging veins, and convergence angle were determined. The opposite hemisphere was manipulated in an identical manner to measure the indexes of the sagittal sinus. Results in an area posterior to the frontal region of the SSS, a "safe zone" was identified where no bridging veins drained into the SSS, covering the area 32.6 nun anterior and 7.5 mm posterior to the coronal suture. After complete dissociation of the bridging veins near the longitudinal fissure in the "safe zone", the fissure allowed an opening width of 4.48～10.86 mm. Conclusion Thorough knowledge of the venous anatomy can help avoid the bridging veins in the anterior transcallosal approach. Total dissociation of the sticking segment and arachnoid segment of the bridging veins can broaden the opening width of the longitudinal fissure without increasing the tension of the bridging veins to better preserve the bridging veins during surgery.

Objective To investigate the application and significance of aerodynamic parameters in voice function assessment. Methods The phonatory aerodynamic system (PAS) was used to collect aerodynamic parameters from subjects with normal voice,vocal fold polyp,vocal fold cyst,and vocal fold immobility.Multivariate statistical analysis was used to compare measurements across groups. Results Phonation threshold flow ( PTF),mean flow rate (MFR),maximum phonation time ( MPT),and glottal resistance (GR) in one hundred normal subjects were significantly affected by sex ( P ＜ 0.05 ),while phonation threshold pressure (PTP),subglottal pressure (SGP),and vocal efficiency (VE) were not (P ＞0.05). PTP,PTF,MFR,SGP,and MPT were significantly different between normal voice and voice disorders (P ＜ 0.01 ),and there were no significant differences among the three disorders ( P ＞ 0.05 ).Receiver operating characteristic (ROC) analysis found that PTP,PTF,SGP,MFR,MPT,and VE in one hundred thirteen voice dis orders had similar diagnostic utility (P ＜0.01 ),with PTP exhibiting the highest area under the curve.The aerodynamic parameters of the three degrees of voice dysfunction due to vocal cord polyps were compared and found to have no significant differences(P ＞0.05).PTP,PTF,MFR,SGP and MPT in forty one patients with vocal polyps were significantly different after surgical resection of vocal cord polyps (P ＜ 0.01 ).Conclusion The aerodynamic parameters can objectively and effectively evaluate the variations of vocal function,and have good auxiliarv diagnostic value.

Objective To investigate the expression level of IL-32 in serum and its correlation with serum biochemical indices of liver function test and HBV DNA load in patients with HBV-related liver failure.Methods Fifty-five patients with HBV-related liver failure (severe hepatitis group) and twenty normal cases (control group) were enrolled in the study.Total RNA in PBMCs was extracted by using TRIzol.IL-32 mRNA level was assayed by using Real-time PCR.IL-32 protein level in serum was detected by ELSIA method.The correlation between IL-32 and ALT,AST,TBIL,HBV DNA load was analyzed using pearson's correlation analysis,respectively.Results Serum IL-32 expression level in severe hepatitis group was higher than that of control group.Moreover,the difference between them was statistically significant (P ＜0.05).Serum IL-32 level was positively correlated with serum ALT,AST,TBIL,respectively (P ＜ 0.05),but was not correlated with HBV DNA load (P ＞ 0.05).Conclusion Serum IL-32 expression level was increased in patients with HBV-related liver failure and was associated with the severity of inflammation.We,therefore,believe that IL-32 might be involved in the pathogenesis of HBV-related liver failure.

Objective To explore the clinical characteristics and treatment scheme of periprosthetic joint infection(PJI)caused by Cutibacterium avidum(C.avidum).Methods The diagnosis and treatment process of a patient with PJI caused by C.avidum was summarized,and relevant literatures in the database were retrieved for review.Results A 65-year-old female patient with body mass index(BMI)of 31.1 kg/m2 underwent left humeral head prosthesis replacement surgery following a left proximal humerus fracture.Ten months after the surgery,the pa-tient exhibited poor wound healing and oozing,along with limited movement of the left shoulder joint,and was diag-nosed infection following shoulder arthroplasty.Patient underwent debridement of the infected lesion and removal of the prosthesis.The tissue,bone cement and prosthesis were cultured for C.avidum.Four literatures were re-trieved and screened,a total of 30 patients with PJI(28 cases hip joint infection and 2 cases shoulder joint infection)caused by C.avidum were reported through literature retrieval,and 78.6％(n=22)total hip arthroplasty(THA)surgeries were performed using direct anterior approach(DAA).The positive rate of preoperative joint fluid culture was 71.4％,29 cases underwent surgical combined with sensitive antimicrobials treatment.Except for one patient who had repeated infection and underwent three surgeries,other patients had a good prognosis.Conclusion PJI caused by C.avidum is mostly seen in THA patients who are obese and undergo DAA,with a few cases reported after shoulder arthroplasty.The high sensitivity of preoperative joint fluid culture provides an important basis for the development of surgical strategies and anti-infection protocols.

OBJECTIVESTo explore the clinical, pathological features and prognosis of patients with chromophobe renal cell carcinoma.

METHODSFrom January 1998 to January 2008, clinical data of 29 patients with chromophobe renal cell carcinoma including clinical manifestations, imaging examinations, treatment models, pTNM stages and follow-up results, were summarized to investigate its features and prognosis.

RESULTSAll cases had no obvious clinical and preoperative imaging presentation. There were 23 patients underwent radical nephrectomy, and 6 cases underwent nephron sparing surgery. Postoperative pathological findings confirmed the diagnosis of chromophobe renal cell carcinoma. Macroscopically, the cut surface of the tumors were generally beige in color. Histologically, it showed polygonal chromophobe cells and small round eosinophilic cells with eccentric hyaline degeneration. These tumor cells had a clear and sharp membrane, lightly stained abundant cytoplasm with a fine reticular translucent pattern and irregular nuclei. And a perinuclear halo was often seen in these cells. Histochemically, the tumor cells generally show a diffuse and strong reaction for CK-8 with a negative expression of Vimentin. The pTNM stages of the tumor were as follows, pT1N0M0 in 11 cases, pT2N0M0 in 8 cases, pT3aN0M0 in 5 cases, pT1N1M0 in 3 cases, pT2N1M0 in 2 cases. Twenty-six cases of patients were followed up (24 to 144 months, with an average of 90 months), 3 cases died of cardio-cerebrovascular disease, and local recurrence involved in 6 cases with reoperation in 4 cases, as well as distant metastasis in 1 case. Twenty-one cases survived with tumor-free. The statistical results indicated that the survival rates of the patients with chromophobe renal cell carcinoma in five years and ten years were 83.9%, 77.9%, respectively, compared with renal cell carcinoma of the same stage 63.8% and 49.9% at the same periods, and there is no difference in the survival rate of five years (P > 0.05) but significant difference in that of ten years (P < 0.01).

CONCLUSIONSChromophobe renal cell carcinoma is a morphologically uncommon subtype of renal cell carcinoma with the good prognosis. Definite diagnosis depends on its typical pathological feature. Radical nephrectomy is the first choice for the treatment of chromophobe renal cell carcinoma.
