1.Comparative studies of continuons-wave Doppler echocardiography and cardiac catheterization in evaluating the systolic function of left ventricular in patients with mitral incompetence

Objective To evaluate the accuracy and feasibility of estimation of left ventricular dp/dtmax by continuous-wave Doppler mitral regurgitation velocity curve versus cardiac catheterization in patients with mitral incompetence. Methods One hundred and five patients with mitral incompetence were enrolled in this study. The Doppler spectrum of mitral regurgitation velocity was recorded with a speed of 100mm/s. The velocities and time of two points on the ascending slope of the spectrum were calculated directly in order to estimate left ventricular dp/dtmax. In 40 patients, left ventricular pressure and dp/dtmax were measured with cardiac catheterization. To investigate the relationship between transcription factor and the change of protein expression levels in heart failure. Methods Bioinformatic method was used to analyze the data of binding-sites on the 5 ' flaking regions of four genes whose mRNA level changed in failing heart from three databases about nucleic acid-EMBL, transcriptional regulation factor-TRANSFAC and protein-SWISS-PORT.The expression level of selected transcription factor was determined by immunohischemical method.Results Nine transcription factors were inferred to influence the proteins' levels in occurrence and development of heart failure.Serum response factor (SRF) was selected from the nine factors and assayed. The results showed that there was a higher level of SRF in healthy group than in chronic heart failure (CHF), and the level was associated with the degree of CHF. It was also found that there was a relative higher level of SRF in the acute myocardial infarction (AMI) than that in CHF, but which was lower than the healthy. Conclusion It showed that SRF had a quantitative change in the development of heart failure, and suggested SRF might play an important regulative role in heart failure. The expression changes of proteins related to myocardial function might be regulated by the quantitative change of transcription factor(s).

5.Controlled continuous curvilinear capsulorhexis in short axial length and shallow anterior chamber eyes

AIM:To investigate the efficacy of controlled continuous curvilinear capsulorhexis(CCC) technique in short axial length and shallow anterior chamber eyes.METHODS:Sixty-eight patients(68 eyes) with short axial length and shallow anterior chamber were included.The routine CCC technique was used in 32 cases (32 eyes) and controlled CCC technique was used in 36 cases (36 eyes).The success rate and complication were compared between two groups. RESULTS:The success rate of the routine technique group and controlled technique group was 53. 13% and 86.11% respectively. Incomplete CCC leading to posterior capsule tears was 9.38% and zero in two groups respectively.CONCLUSION: Controlled CCC technique can increase the success rate and reduce complications in short axial length and shallow anterior chamber eyes.KEYWORDS:phacoemulsification; continuous curvilinear capsulorhexis; complication

8.Relationship between changes of serum response factor and heart heart failure

Objective To investigate the relation ship between trans-acting factor and the change of protein expression levels in heart failure. Methods Bioinformationak methods were used to analyze the bindingsites in the 5' regulation regions of four genes of the protein expression levels change in heart failure. The serum response factor (SRF) level was determined by innuno-hischemical method. Results Nine trans-transcription factors were inferred to influence these protein expression levels in occurrence and development of heart failure. SRF was selected from the nine factors and assayed. The results showed a higher level of SRF in healthy people, the percentage of anti-SRF positive cardiocytes was(91.6±10.05)%. A lower percentage of SRF in congestive heart failure (CHF) was (28.73±12.79)%. And the level of SRF was associated with the functional class of the New York Heart Association. A relative higher level of SRF was found in the acute myocardial infarction (AMI) (4533±13.30)% than that in CHF and in the healthy. Conclusion It was suggested that the level of SRF plays a regulative role in CHF.
9.Changes and significance of plasma B-type natriuretic peptide and cardiac troponin I in patients with sepsis

Objective To evaluate the relationship between changes in B-type natriuretic peptide(BNP) and cardiac troponin I(cTnI)levels and prognosis of critically ill patients with sepsis. Methods This study retrospectively reviewed the clinical data of 75 patients with severe sepsis and septic shock admitted into Emergency Intensive Care Unit(EICU)of the First Affiliated Hospital of Wenzhou Medical University in Zhejiang Province. According to the severity of the cases,they were divided into two groups:severe sepsis group(34 cases)and septic shock group(41 cases),and based on the difference in prognosis,they were divide into survivor group(32 cases) and non-survivor group(43 cases). Electrocardiogram(ECG)was performed within 24 hours after admission in all the patients. Acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score and biochemical markers showing organ dysfunctions as BNP, cTnI, creatine kinase (CK), creatine kinase MB mass(CK-MB), and lactate were compared between severe sepsis and septic shock groups and between survivor and non-survivor groups. Results The septic shock group had significantly higher baseline BNP,cTnI,lactate and APACHE Ⅱscore and mortality rate than those in severe sepsis group〔BNP(μg/L):1.90(1.08,2.79)vs. 0.41(0.31,0.75),cTnI (μg/L):1.15(0.92,1.28)vs. 0.58(0.40,0.79),lactate(mmol/L):6.63±3.72 vs. 3.28±1.66,APACHEⅡscore:26.00(24.00,28.00)vs. 21.50(20.00,29.25),mortality rate:70.73%vs. 41.18%,P<0.05 or P<0.01〕. Compared with survivor group,the ages of non-survivor group were older with more males and higher BNP,cTnI,lactate and APACHEⅡscore〔males(cases):30 vs. 13,age(years old):66.49±14.97 vs. 58.19±17.05,BNP:1.60(0.62, 2.51)vs. 0.57(0.37,1.79),lactate:4.10(3.00,9.00)vs. 3.10(2.13,4.18),cTnI:1.02±0.49 vs. 0.62±0.37, APACHE Ⅱ score:28.00(25.00,30.00)vs. 21.00(20.00,25.75),P<0.05 or P<0.01〕. However,there were no statistically significant differences in the levels of CK and CK-MB between the above compared groups(both P>0.05). The patients' ECGs had no obvious changes. Conclusions High plasma BNP and cTnI levels in patients with sepsis may suggest myocardial damage and relatively bad prognosis. The examination of BNP and cTnI levels may help clinicians to early detect the high-risk patients with septic cardiac dysfunction and assess their prognoses.
