3.The effect of “bulk-fill”flowable resin in filling deep wedge-shaped defects:A randomized controlled clini-cal evaluation

Objective:To observe the effect of “bulk-fill”flowable resin in filling deep wedge-shaped defects.Methods:50 pa-tients with totally 85 pairs of premolars with deep wedge-shaped defects(depth >2 mm)were included.Each pair of the defects was randomly divided into the test group and the control group,the defects were treated by “bulk-fill”flowable resin with single cure tech-nique and conventional flowable resin with incremental layering technique respectively,the tooth sensitive rate 1 week after filling,the filling failure rate,the edge coloring and secondary caries rate 2 years after filling of the 2 groups were compared.Results:There was no significant difference between the 2 groups in the sensitive rate 1 week after filling,the failure rate and the edge coloring and sec-ondary caries rate 2 years after filling(P >0.05).Conclusion:The “bulk-filled”flowable resin using single cure technique can ac-quire the same clinical effect as conventional flowable resin using incremental layering technique in filling deep wedge-shaped defects.
4.Short-segment posterior pedicle screw combined with vertebral arch bone grafting for thoracolumbar burst fractures.

OBJECTIVETo analyze the clinical effects of short-segment posterior pedicle screw combined with transpedicle vertebral bone grafting in treating thoracolumbar burst fractures.
METHODSFrom March 2008 to March 2013, 62 patients with thoracolumbar burst fractures were treated with short-segment posterior pedicle screw combined with transpedicle vertebral bone grafting. Including 40 males and 22 females, the age from 17 to 65 years old with an average of 38 years. According to AO classification, 34 cases were type A3.1, 7 cases were type A3.2 and 21 cases were type A3.3. Load-sharing scores were from 4 to 6 points with an average of 5.4 points. According to ASIA grade, 2 cases were grade C, 5 cases were grade D and 55 cases were grade E. Preoperative, postoperative at 3 d and final follow-up, the Cobb angle, the relative height of anterior vertebral body and the encroachment rate of spinal canal were measured by X-ray films and computed tomography (CT) scan, meanwhile, the information of bone healing and spinal nerves recovery were observed.
RESULTSAll patients were followed up from 11 to 14 months with an average of 12.2 months. The duration of removing internal fixation were from 9 to 13 months (averaged, 11.5 months). One suffered from infection and was cured by debridement. Two cases had mild pain of back. At 6 months after operation, according to ASIA grade to evaluate never function, 1 case was grade C, 3 cases were grade D and 58 cases were grade E. X-ray and CT showed the fractures obtained good union at final follow-up. The Cobb angle, the relative height of anterior vertebral body and the encroachment rate of spinal canal had obviously improved at 3 days after operation (P<0.05); but there was no significant differences between postoperative at 3 d and final follow-up (P>0.05).
CONCLUSIONShort-segment posterior pedicle screw combined with transpedicle vertebral bone grafting is an effective method to treat thoracolumbar burst fractures. It can reduce the loss of postoperative correction and prevent the internal fixation failure.

·AIM:To report a case of entry of antibiotic ointment into the anterior chamber following sutureless clear corneal incisions phacoemulsification.·METHODS:A 62-year-old man had uneventful phaco-emulsification with clear corneal incisions with intraocular lens(IOL) implantation. Postoperative medication included antibiotic/steroid ointment; the eye was firmly patched at the end of the procedure. On the first postoperative day, the patient was seen an oily droplet floating in the anterior chamber. Immediate surgical removal of the ointment without IOL exchange was performed.·RESULTS:During follow-up for six months after irrigation/aspiration of the anterior chamber and removal of the oily droplet, visual acuity and intraocular pressure (IOP) were unchanged and anterior segment was formed and quiet.·CONCLUSION:Ophthalmic ointment may ingress into the anterior chamber through unstable clear corneal incisions. The case highlights the importance of appropriate wound construction and integrity, as well as the risk of tight eye patching following placement of ointment.
7.Association between prodromal headache and propensity of rebleeding after aneurysmal subarachnoid hemorrhage

Objective To investigate the association between prodromal headache and the propensity of rebleeding after aneurysmal subarachnoid hemorrhage.Method Prodromal headache was defined as a sudden attack of severe headache lasting at least one hour with or without accompanying symptoms,not leading to a diagnosis of SAH 4 weeks before the confirmation of subarachnoid hemorrhage.Two hundred and nine patients with aneurymal subarachnoid hemorrhage were enrolled in this research.Age,gender,Hunt-Hess grade,D-dimer, (FBG),site and number of aneurysms and prodromal headache were recorded.Patients were divided into rebleeding group and non-rebleeding group.Comparing the above survey components between two groups was carried out by using case control study method.Age,aneurysm number,D-dimer and fibrinogen of the two groups were analyzed by t test.Prodromal headache,gender,site of aneurysms were analyzed by chi square test.Hunt- Hess grade was analyzed by rank test.Results Twenty-two(10.5%)patients had rebleeding.In the rebleeding group,10 patients(45.5%)had prodromal headache,while in the non-rebleeding group(n=187),21 patients (11.2%)had prodromal headache.Patients with prodromal headache had greater propensity of rebleeding compared with those without prodromal headache(P

The present study aims to predict the action targets of antidepressant active ingredients of Xiaoyaosan to understand the "multi-components, multi-targets and multi-pathways" mechanism. Using network pharmacology, the reported antidepressant active ingredients in Xiaoyaosan (saikosaponin A, saikosaponin C, saikosaponin D, ferulic acid, Z-ligustilide, atractylenolide I, atractylenolide II, atractylenolide III, paeoniflorin, albiflorin, liquiritin, glycyrrhizic acid and pachymic acid), were used to predict the targets of main active ingredients of Xiaoyaosan according to reversed pharmacophore matching method. The prediction was made via screening of the antidepressive drug targets approved by FDA in the DrugBank database and annotating the information of targets with the aid of MAS 3.0 biological molecular function software. The Cytoscape software was used to construct the Xiaoyaosan ingredients-targets-pathways network. The network analysis indicates that the active ingredients in Xiaoyaosan involve 25 targets in the energy metabolism-immune-signal transmutation relevant biological processes. The antidepressant effect of Xiaoyaosan reflects the features of traditional Chinese medicine in multi-components, multi-targets and multi-pathways. This research provides a scientific basis for elucidation of the antidepressant pharmacological mechanism of Xiaoyaosan.

9.Research progress of p70 ribosomal protein S6 kinase inhibitors.

p70 ribosomal protein S6 kinase (p70S6K), an important member of AGC family, is a kind of multifunctional Ser/Thr kinases, which plays an important role in mTOR signaling cascade. The p70 ribosomal protein S6 kinase is closely associated with diverse cellular processes such as protein synthesis, mRNA processing, glucose homeostasis, cell growth and apoptosis. Recent studies have highlighted the important role of S6K in cancer, which arose interests of scientific researchers for the design and discovery of anti-cancer agents. Herein, the mechanisms of S6K and available inhibitors are reviewed.

10.Three years retrospective clinical evaluation of endodontically treated premolars restored by cast ceramic onlays.

OBJECTIVEThis study aimed to evaluate the success and survival rate of endodontically treated premolars restored by ceramic onlays by comparing restored by quartz fiber posts and metal ceramic crowns.
METHODSNinety-four patients with 126 endodontically treated premolars were enrolled in this study and divided into 4 groups according to the remaining axial walls and restorative methods. The observation time was 36 months. The success and survival rates of the restorations, as well as causes of failures, were analyzed.
RESULTSAt the final follow-up, the success and survival rates of the mild defect endodontically treated premolars were restored by quartz fiber posts and the crowns were at 96.3% and 98.1%; the success and survival rates of the severe defect premolars restored by quartz fiber posts and crowns were at 88.5% and 96.2%. The success and survival rates of the mild defect premolars restored by cast ceramic onlays were both at 96.6%, the success and survival rates of the severe defect premolars restored by cast ceramic onlays were at 94.1% and 100%, respectively. The success and survival rates of the different groups were no significant difference (P>0.05).
CONCLUSIONBased on the results and within the limits of this study, cast ceramic onlays is a very reliable method to restore endodontically treated premolars.
