1.Endoscopic study and evaluation of anatomy related tarsal tunnel syndrome

Objective To explore the anatomy and the adjacent of the medial malleolus,the operative approach of soluting the tibial nerve in endoscopy and the significance of making use of it in operation. Methods The five cadaver feet were dissected,the tibial nerve and the fiber diaphragm in the middle of me- dial and lateral plantar tunnels in the ten cadaver feet were soluted in endoscopic.Results The operative approach was designed on the basis of the anatomy data (the three-portal approach:The perpendicular bisetor which was the medial malleolus pointed to the medial margin of tendon calcaneus insertion was 4.5cm;The midpoint of the medial malleolus pointed to the calcaneus tubercle;The midpoint of the calcaneus tubercle to the medial margin of the first metatarsophalangeal joint).It was succeed that soluted the tibial nerve and the fiber diaphragm in the middle of medial and lateral plantar tunnels.Conclusion The operative approach on the basis of the anatomy data is design,and testfy the feasibility of exploring curved tunnel in endoscopic.
2.Effects of hypercapnia on oxygen free radicals in acute lung injury models

Objective To investigate the protective effects of hypereapnia on acute lung injury(ALI)in an model of rabbits in vivo,and to observe its effect on oxygen free radicals in the lung tissue in order to uncover the potential mechanisms.Method In the laboratory of pharmacology,China Medical Univereity,twenty-two healthy New Zealand white rabbits were randomly assigned to control group(Group C,n=6)with the injection of normal saline(0.1 ml/kg),and sixteen rabbits were injected with oleic acid(0.1ml/kg)intravenously,and then were randomly dirided into normocapnia group(Group N,n=8)and hypercapnia group(Group H,n=8,FiCO2=8%).Then tracheostomy was performed,and the experimental animals were ventilated for 3 hours after oleic acid or sterile normal saline administration.Lung mechanics,hemodynamics,blood-gas analysis were monitored.The rabbits were exsangninated.and the lungs and heart were taken out from the thorax.The concentration of superoxide dismutase(SOD)and malondialdehyde(MDA)in the lung tissue were assayed.Lung tissue wet/dry ratio and pulmonary permeability index were measured and histologic damage was assessed after three hours'mechanical ventilation.Results Peak airway pressure in Group H was significantly lower than that in Group N and the dynamic lung comphance Was significantly higher than that in Group N(P<0.05).PaO2 in Group H was significantly higher than that in Group N(P<0.05).The concentration of MDA in the lung tissue in group H was significantly lower than that in Group N(P<0.05),and SOD in group H was significantly higher than that in Group N(P<O.05).Lung tissue wet/dry ratio and pulmonary permeability in group H were significantly lower than that in Group N(P<0.05).Histological tissus damage in Group N wassignificantly severer than that in Group H.Conclusions Hypercapnia induced by inhalation of high concentration of carbon dioxide(8%)plays protective role in this in vivo model of ALI.The mechanisms may be associated with enhanced SOD activity and the attenuation of lipid peroxidation in the lung tissue.

AIM: To investigate whether hypercapnia is protective against acute lung injury (ALI) in a rabbit model, and study it's potential mechanisms. 4.Breeding of Bacillus tkuringiensis (Bt) with High Insecticidal Activity to Spodoptera exigua

By the breeding model of physics mutagenests (60Co,UV light) and re-strong in the body of insects, We obtained a Bt strain Bt99985 with high insecticidal activity. The shake flask and 40t tank fermentation tests showed that, this strain showed special activity against Spodoptera exjgua, Comparing with original Bt strain: 93005, CK strain: HD-1-580 and GC-91, the value of potency increased respectively by 429%, 655% and 114% in shake flask test. Comparing with GC-91, the value of potency increased by 180% in 40t tank fermentation test.
5.Testicular masses associated with congenital adrenal hyperplasia (a case report and review of the literature)

Objective To improve the diagnosis and treatment of testicular masses associated with congenital adrenal hyperplasia(CAH).Methods A 25-year-old man presented with bilateral testicular nodules.Physical examination revealed palpable multiple nodules in the bilateral testes and epididymides as well as enlarged and hard right testis with uneven surface.Biochemical examination showed testosterone(T) of 18.7 nmol/L,17?-(OH)P>20.0 ng/ml,positive results of rapid ACTH stimulation test and dexametha- sone suppression test.B-ultrasound demonstrated diffuse testicular lesions with nodules in parts of the bilater- al testes.Bilateral testicular nodules were biopsied,and pathological diagnosis was interstitial cell hyperplasi- a.Positive staining for?-inhibin was observed immunohistochemically.The clinical characteristics of testicular masses associated with CAH were reviewed in combination with the literature.Results Based on medical history,imaging,laboratory and pathologic examinations,the patient was diagnosed with testicular masses as- sociated with CAH.He received oral cortisol at dose of 40 mg daily for 2 weeks,and then at dose of 30 mg daily as maintenance therapy.After treatment of 8 months,B-ultrasound showed complete regression of the testicular tumors.Follow-up of 24 months showed no recurrence of the tumor.Conclusions For patient with bilateral testicular masses,endocrinological evaluation and clinical history taking are indicated to exclude testicular interstitial cell hyperplasia associated with CAH.By oral steroid replacement therapy,hormone-sen- sitive testicular masses can regress.
6.Regulatory effects of cyclosporin A and tacrolimus on the immunological gene expressions in renal transplant recipients.

OBJECTIVETo observe the change of Th immunological gene in renal transplant recipients after the treatment of cyclosporine (CsA) and tacrolimus (FK506).
METHODSThe peripheral blood lymphacytes just before and 24 hours after CsA and FK506 treatment were isolated. The total RNA of them were reverse-transcripted and examined by real-time quantity PCR array. The results were analyzed by bioinformatic methods.
RESULTSThe TLR4, CEBPB, IL4R, IL1R1,IL18R1,and IL1R2 genes were remarkably upregulated, whereas IL-2, CCL5, CD27, CCR5, CCR4, CD4, RPL13A, TGFB3, CD86, CCR3, STAT1, NFATC2IP, IL23A, IL15, IRF4, and TFCP2 were downregulated 24 hours after CsA treatment. The IL18, IL7, PTPRC, TNFSF4, SPP1, GFI1, TLR4, IL13RA1, TNF, INHBA, LAG3, IL13, IL1R1, SOCS5, IL10, YY1, TBX21, FASLG, IL18R1, and IL1R2 genes were remarkably upregulated, whereas IL-2, IL-3, IL-4, IL-6,CCR5, CD4, CD27, CD40LG, IL15, CCR3, CD86, CCR4, and IRF4 were obviously downregulated 24 hours after FK506 treatment.
CONCLUSIONCsA and FK506 exert their therapeutic effectiveness by regulating the expressions of a series of target genes.

7.Expressions of receptor tyrosine kinases mRNA and protein in carcinoma of bladder.

OBJECTIVETo detect the expressions of receptor tyrosine kinases (RTKs) mRNA and protein and to explore potentially promising tumor markers and conceivable drug target in bladder cancer.
METHODSThe expressions of RTKs mRNA and protein in tissue from invasive urothelial carcinoma of the bladder were examined by real-time quantitative PCR array and cytokine antibody array, with normal bladder tissue as control. The Results were analyzed using bioinformatic approaches.
RESULTSThe expressions of TGFA, STAB1, SERPINE1, ANGPT2, SPINK5, ANGPTL1, PROK1, MDK, CXCL9, GRN, RUNX1, VEGFA, and TGFB1 were obviously upregulated in bladder cancer tissue, while those of EDIL3, PTN, CCL2, PDGFD, FGF13, KITLG, FGF2, SERPINF1, and TNF were downregulated. ALK, Btk, EphB2, ErbB4, PDGFR-α, ROS, Tie-2, Tyk2, and VEGFR3 were over-expressed in bladder cancer, while FRK, Fyn, IGF-IR, Insulin R, Itk, JAK1, JAK3, and LCK were low-expressed.
CONCLUSIONVascular endothelial growth factor/platelet-derived growth factor-targeted therapies may play an active role in treating carcinoma of bladder.

8.Minimization or avoidance of calcineurin inhibitors after renal transplantation.

Calcineurin inhibitors (CNIs) are playing an important role in preventing acute rejection in renal transplantation; however, their nephrotoxicity may impact long-term renal allograft survival. Several CNI-sparing regimens (such as CNI-avoidance or CNI-minimization) have shown at least comparable efficacy with standard-dose CNI regimens. Research continues to achieve the "best" balance between efficacy and toxicity of available immunosuppressive regimens.

10.Living donor kidney transplantation.

Along with the wide application of cadaveric donor kidney transplantation (CDKT), most transplantation centers face the shortage of cadaveric donors. Living donor kidney transplantation (LDKT) has been successfully applied with remarkable advantages over CDKT. Human leukocyte antigen matching generally is not an important problem in non-related LDKT. Even with 6-locus mismatch, the effectiveness of LDKT is still superior to non-mismatch CDKT. Although LDKT donor has extremely low mortality and incidence of complications, the safety of LDKT donor is becoming a research highlight. In addition, many social problems are involved in LDKT, which should be deliberately considered during clinical practice.
