4.Single-stage revision total knee arthroplasty for infection resulting from methicillin-resistant Staphylococcus aureus in rabbits

ObjectiveTo compare the efficacy of single-stage and two-stage revision prosthesis-relative chronic infection causing by methicillin-resistant Staphylococcus aureus(MRSA) after total knee arthroplasty(TKA) in rabbits, and evaluate the clinical feasibility of single-stage revision TKA. MethodsA new kind of prosthesis was implanted into the right knee joints of 48 New Zealand white rabbits following proper anesthesia. After 4 weeks, the dose of 5×105 colony forming unit MRSA was inoculated into every knee to establish prosthesis joint infection model. The rabbits were divided into 2 groups randomly: experimental and control group. Four weeks after inoculation, the treatments of the experimental and control group were singlestage and two-stage revision respectively. The levels of serum C-reaction protein (CRP) and Erythrocyte sedimentation rate (ESR) were monitored in ten phase, i.e. prior to primary arthroplasty and revision, at 1, 3, 5, 7days, and 2, 4, 6, 12 weeks after revision. Twelve weeks after revision, animals were sacrificed and joint samples were collected for bacterial culture. The positive results were judged as reinfection, and the negative results were judged as successful healing. ResultsFive rabbits were excluded out of the group for some reasons. The recurrence rates of infection in the experimental group and control group were 22.7% (5/22) and 14.3%(3/21) respectively after revision. The difference between them was statistically insignificant(χ2=0.102,P=0.750). The levels of serum CRP of the two groups raised, and reached their peaks at 3 days, then dropped into the normal level prior to primary arthroplasty at 4 weeks after revision. The difference between them was statistically insignificant (F=0.157, P=0.694). The ESR levels of the two groups elevated after revision, and reached their peaks at 5 days, then declined slowly into the original level prior to primary arthroplasty at 12 weeks. The difference between them was statistically insignificant (F=0.936,P=0.339). ConclusionThough the prosthesis-relative chronic infection caused by high virulence organism after TKA, the short-term efficacy of single-stage revision is similar to that of two-stage if the stain of pathogenic bacteria and its spectrum are obtained.
5.Study on effective substance basis and molecular mechanism of Qigui Tongfeng tablet using network pharmacology method.

Qigui Tongfeng tablet (QLTFT) is a traditional Chinese medicine with good effect for treating gout. Here, network pharmacology method and molecular similarity analysis were utilized to study the effective substance basis and molecular mechanism of the QLTFT on the gout. The similarity to the medicinal compounds is reflected in the Tanimoto coefficient that gives the structural similarity of two compounds. Operationally, similar modifiers were described as pairs of concepts with a similarity score of 0. 500. The results of the molecular similarity analysis suggested that the flavonoids in QLTFT could be new leads for gout. Furthermore, complex biological systems may be represented and analyzed as computable networks. Two important properties of a network were degree and betweenness. Nodes with high degree or high betweenness may play important roles in the overall composition of a network. And the results of network analysis showed that dongbeinine, verticinone-N-oxide, verticine N-oxide, peimine, peiminine, isobaimonidine, dongbeirine, peimisine and simi-arenol which with high degree acted on xanthine dehydrogenase/oxidase, matrix metalloproteinase-9, an arachidonate 5-lipoxygenase-activating protein, tyrosine-protein kinase and etc. Inhibition of these targets can prevent the formation of uric acid, reduce inflammation by uric acid and regulate the body's immune response. Thus, these compounds may be the main effective substance basis. The research results not only reveals its molecular mechanism, but also provide a theoretical basis for the quality control of drugs and clinical application.

6.Jiawei shentong zhuyu decoction prevented the occurrence of failed back surgery syndrome and its effect on serum TNF-alpha a clinical study.

OBJECTIVETo explore the clinical roles of Jiawei Shentong Zhuyu Decoction (JSZD) in preventing the occurrence of failed back surgery syndrome (FBSS), and to observe its effect on serum tumor necrosis factor-alpha (TNF-alpha).
METHODSTotally 100 patients prepared for surgical operation due to lumbar intervertebral disc herniation were randomly assigned to the treatment group and the control group according to random number table, 50 cases in each group. Patients in the treatment group additionally took JSZD, one dose per day, taken in two portions, once in the morning and once in the evening. Those in the control group took Celecoxib Capsule (200 mg each time, once per day) and Mecobalamin Tablet (0.5 mg each time, 3 times per day). They only took Mecobalamin Tablet from the 11th day. All patients were treated for 30 days. Japanese Orthopaedic Association (JOA) score was performed before treatment, at week 1, after treatment, at 6 months of followed-ups, and at 12 months of followed-ups. And the levels of TNF-alpha in the peripheral blood were observed before treatment and at one month after treatment.
RESULTSTotally 93 patients completed the followed-up study. The JOA scores were improved after treatment, at 6 and 12 months of followed-ups (P < 0.05, P < 0.01). The JOA score at 6 months of followed-ups was superior in the treatment group to that of the control group (P < 0.05). Five patients (accounting for 10.6%) suffered from FBSS in the treatment group, while 9 (accounting for 19.6%) suffered from FBSS in the control group. The treatment group was superior to the control group (P < 0.05). The TNFalpha level was improved after treatment in the two groups. Of them, the improvement of TNF-alpha in the treatment group was better than that of the control group (P < 0.05).
CONCLUSIONThe application of JSZD was effective for preventing the occurrence of FBSS, and improved the serum TNF-alpha level.

7.Short and long-term efficacy of focused ultrasound therapy for vulva dystrophy

8.Monitor on influence of quality standard improvement upon Guizhi Fuling capsules efficacy.

In 2012, the preparation process and quality standard for Guizhi Fuling capsule were improved. To compare the effects and differences of capsules before (2011) and after(2012-2014) the improvement, evaluation models for intrinsic dysmenorrhea, pelvic inflammation and hysteromyoma were applied in rats. Models were induced by oxytocin, liqiud bacteria mixture and estrogen loading, respectively. The capsules (12 batchs/year, 48 bathcs in all), sampled randomly in 2011-2014, the effects were assessed using the three models. In anti-dysmenorrhea models, remarked reduction of writhing frequency, ET-1 and PGF2α content in uterus could be detected, as well as extension of writhing latency. In pelvic inflammation rats, depression of TNF-α and raise of IL-2 were induced by earh batch of capsules. In hysteromyoma model, uterine weight and smooth muscle proliferation, including E2 and P level in plasma, were lowered obviously by all batchs of capsules. Secondly, Guizhi Fuling capsules produced in 2012-2014 revealed better effectiveness than the ones manufactured in 2011. Moreover, pharmacodynamics indexes of the samples made in 2011 differed significantly between groups, which could not be observed in the ones ot 2012-2014. After tne preparation process and quality standard improvement, the effectiveness and homogeneity of Guizhi Fuling capsules were enhanced.

9.Repair of occipital and nuchal wounds with inferior trapezius myocutaneous flaps in patients after high voltage electrical burn.

OBJECTIVETo explore the methods and effects of repair of occipital and nuchal wounds with inferior trapezius myocutaneous flap after deep electrical bum.
METHODSTwelve patients with high-voltage electrical burn in occipital and nuchal regions were hospitalized to our ward from March 2003 to September 2007. They were repaired with improved inferior trapezius myocutaneous flaps after debridement. Flaps were of two types: (1) blood supply from cutaneous and perforator branches of the original segment of the superficial descending branch of transverse cervical artery. (2) combined blood supply from both superficial and deep descending branches of transverse cervical artery C, i.e., dorsal scapular artery). All flaps carried segmental and limited trapezius muscle cuff surrounding the vascular pedicle of the flap similar to a perforator flap.
RESULTSFlaps survived completely primarily in eight cases. In two patients, infection developed in flaps adjacent to wounds with lignification; they healed after dress change. Necrosis appeared in distal end of flap (one case), it healed after re-operation. One patient with surviving flaps died of sepsis and multiple organ failure 21 days after operation. The flaps which survived were not swollen ; the donor sites at scapular region looked normal without pterygoid or pendulous scapula deformities.
CONCLUSIONInferior trapezius myocutaneous flaps can be used to repair occipital and nuchal wounds, with the advantages of constant blood vessels, reliable blood supply, convenience for application.

10.Effect of peptide-conjugated quantum dots on the tumorigenicity and lymph node metastasis of human tongue squamous cell carcinoma cell line Tca8113 and mouse uterine cervix carcinoma U14 in vivo.

OBJECTIVETo observe the effect of peptide-conjugated quantum dots with a maximal emission of 655 nm (QD655) on growth, proliferation, apoptosis and lymphatic metastasis of human tongue squamous cell carcinoma cell line Tca8113 and mouse uterine cervix carcinoma U14 in vivo.
METHODSTca8113 and U14 cells were labeled by QD655 (Tca8113-QD655, U14-QD655). Tca8113-QD655 and U14-QD655 were inoculated subcutaneously into nude mice and Kunming mice. The tumor formation of Tca8113-QD655 and Tca8113, U14-QD655 and U14 was observed and compared in vivo. The proliferation and apoptosis of Tca8113-QD655 and Tca8113, U14-QD655 and U14 cells from tumors formed in vivo were analyzed by flow cytometry. U14-QD655 and U14 were inoculated into the buccal mucosa of Kunming mice to establish the cervical lymph node metastasis model of buccal cancer. The cervical lymph node metastatic ability of U14-QD655 and U14 was compared.
RESULTSThe tumor weight and volume of Tca8113-QD655 and Tca8113, U14-QD655 and U14 in vivo were not significantly different (P>0.05), the cell proliferation index and apoptosis index of Tca8113-QD655 and Tca8113, U14-QD655 and U14 in vivo were not significantly different (P>0.05). The cervical lymph node metastasis rate of U14-QD655 and U14 buccal cancer were not significantly different (P>0.05).
CONCLUSIONSQD showed no effects on tumorigenicity and lymph node metastasis of Tca8113 and U14 cells. These results provide the scientific basis for noninvasive imaging and long-term tracing study.
